Description
この出願は、「白内障手術での繊維切断を用いたレンズ分断(又は分裂又は破砕;フラグメンテーション;fragmentation)のための方法とデバイス」と題された2014年9月17日に出願された米国仮出願番号第62/051396号、および「眼の手術での内部からの介入嚢間分裂、修復、および摘出のための方法とデバイス」と題された2015年1月5日に出願された米国仮出願番号第62/099590号から優先権を主張し、全ての目的のためこれら全てが参照することにより本明細書に組み込まれる。
又、テーパー片は、挿入の間切開要素16を拘束するために切開要素16を覆い得る。切開要素16が切開部4を一旦通過する場合、テーパー片は、切開要素16が延在可能である又は破って開けることが可能であるスリットを前部に更に有し得る。
Claims (24)
- 眼(1)の水晶体嚢(6)内のレンズ(8)を切断するための外科用デバイスであって、
ルーメン(14)、ルーメン(14)の開口部および遠位端部を有するシャフト(12);および
第1の挿入形態から第2の捕捉形態に向かうようにシャフト(12)に対して移動可能な切開要素(16)
を有して成り、
ルーメン(14)は、シャフト(12)を通るように規定されており、長手軸を有し、
第2の捕捉形態では、切開要素(16)の一部がルーメン(14)の外側に延在し、
切開要素(16)の少なくとも一部は、切開要素(16)が第1の挿入形態にある時にルーメン(14)内に位置付けられており、第1の挿入形態の切開要素(16)およびシャフト(12)の遠位端部の両方は、切開部(4)を通じた眼(1)の前房内への挿入、および水晶体嚢(6)内のレンズ(8)の前面への配置のためのサイズを有しており、
第2の捕捉形態は全体として切開要素(16)によって形成された略閉ループを含み、該略閉ループは開口領域(46)を含み、開口領域(46)の第1部分は前記開口部より遠位側にあり、開口領域(46)の第2部分は前記開口部より近位側にあり、
アクチュエータ(44)は、切開要素(16)に操作可能に結合されており、
アクチュエータ(44)は、アクチュエータ(44)が第1方向へ移動すると開口領域(46)を拡張し、アクチュエータ(44)が反対の第2方向へ移動すると切開要素(16)に張力をかけて、開口領域(46)を減少させレンズを切断するように構成され、
第1方向へのアクチュエータ(44)の移動は、第2の捕捉形態の切開要素(16)が水晶体嚢(6)内のレンズ(8)の少なくとも一部を取り囲むように、さらに切開要素(16)をルーメン(14)の長手軸に対して回転運動させ、切開要素(16)を水晶体嚢(6)とレンズ(8)との間で動かす、外科用デバイス。 - 切開要素(16)は、第1脚(18)および第2脚(20)を有する、請求項1に記載の外科用デバイス。
- 切開要素(16)の先端部分以外の全てが第1の挿入形態でルーメン(14)内に位置付けられており、
前記先端部分は、第1脚(18)の少なくとも一部および第2脚(20)の少なくとも一部を有して成る、請求項2に記載の外科用デバイス。 - 切開要素(16)は、第2の捕捉形態において、ルーメン(14)の内部に位置付けられる第1脚(18)の一部と第2脚(20)の一部とを有して成る、請求項2に記載のデバイス。
- 第2脚(20)は、第2の捕捉形態において、ルーメン(14)の長手軸に対して120度よりも大きい角度後方に屈曲する、請求項2に記載の外科用デバイス。
- 切開要素(16)の第2の捕捉形態において、ルーメン(14)の外側に位置付けられる切開要素(16)の大部分が、ルーメン(14)の長手軸からずれている、請求項1に記載の外科用デバイス。
- 第1の挿入形態から第2の捕捉形態へと切開要素(16)が遷移する間において、切開要素(16)は、該切開要素(16)がルーメン(14)から出るように延在する結果、形状変化し、
切開要素(16)の第2の捕捉形態が、予め調節された形状となっている、請求項1に記載の外科用デバイス。 - 切開要素(16)が、ニッケル-チタン合金のワイヤーまたはストラップである、請求項1に記載の外科用デバイス。
- 前記略閉ループが平面を規定しており、ルーメン(14)の長手軸が該平面に位置しており、切開要素(16)の回転運動は該平面を回転させる、請求項1に記載の外科用デバイス。
- 外科用デバイスが、シャフト(12)の近位端部に取り付けられたハンドル(42)を更に有して成る、請求項1に記載の外科用デバイス。
- アクチュエータ(44)がスライダーであって、該スライダーが、切開要素(16)とスライダーの直線動作を切開要素(16)の回転運動に変換するカム(50)との両方に結合されている、請求項10に記載の外科用デバイス。
- 前記略閉ループは湾曲しており、かつ非平面状である、請求項1に記載の外科用デバイス。
- 切開要素(16)の第1の挿入形態は、眼の水晶体嚢(6)の前面にある裂嚢(10)を通じて挿入する寸法となっており、
切開要素(16)は、レンズ(8)と水晶体嚢(6)との間で移動するように第1の挿入形態から第2の捕捉形態へと移動可能であり、切開要素(16)が第2の捕捉形態を有する際、切開要素(16)は水晶体嚢(6)内にてレンズ(8)を囲む形状およびサイズを有しており、
レンズ(8)が水晶体嚢(6)内に留まる間、切開要素(16)は、レンズ(8)に切断力を加えるために第2の捕捉形態から第3形態になるように移動可能である、請求項1に記載のデバイス。 - 水晶体嚢(6)に対するダメージに関連する閾量よりも小さい力を水晶体嚢(6)に加える間、切開要素(16)は、レンズ(8)と水晶体嚢(6)との間で移動するように第1の挿入形態から第2の捕捉形態へと移動可能である、請求項1に記載のデバイス。
- 切開要素は、摩擦を低減するために材料で被覆されている、請求項1に記載の外科用デバイス。
- 材料は、疎水性材料を含んで成る、請求項15に記載の外科用デバイス。
- 第2の捕捉形態における略閉ループの径は、2mmから8mmまでの間の水晶体嚢内の裂嚢の径よりも大きい、請求項1に記載の外科用デバイス。
- カムは、閉ループであるカムパスを有するらせん状カムである、請求項11に記載の外科用デバイス。
- カムパスには、カムの回転や移動を抑制するラッチまたはレバーが組み込まれている、請求項18に記載の外科用デバイス。
- 切開要素は、0.001インチから約0.030インチの断面径を有する丸みのあるワイヤである、請求項1に記載の外科用デバイス。
- 切開要素は第1および第2の切開要素を有して成り、第2の捕捉構成における第1および第2の切開要素(16)のそれぞれは、切開要素(16)によって完全に形成された略閉ループを有して成り、該略閉ループは開口領域(46)を有して成り、開口領域(46)の第1部分は開口部の遠位にあり、開口領域(46)の第2部分は開口部の近位にある、請求項1に記載の外科用デバイス。
- アクチュエータは、第1および第2の切開要素に接続されて、単一の摺動部の動作と同期して切開要素を動かす、請求項21に記載の外科用デバイス。
- 切開要素は、開くと同時に回転するように構成されている、請求項22に記載の外科用デバイス。
- 切開要素の回転は、第1切開要素が第2切開要素の前に開くようにずれ調整される、請求項22に記載の外科用デバイス。
