Description
本明細書において「最小限に処置された」という用語は、ドナー(例えば、ハプロタイプ一致ドナー)から単離されたPBSCのプールが、細胞の関連性のある生物学的特徴を改変する、本発明の方法(subject methods)または手技でないことを意味する。ドナー(例えば、T細胞産物を生成する)から単離されたPBSCのプールからPBSCの一部分を単離する行為は、本明細書に記載されている、あるいはPBSCのプール調製、処理および/または貯蔵における他の慣例的な工程(例えば、限定されないが、密度勾配分離、細胞選択、遠心分離および凍結保存)の結果として、依然として「最小限に処置される」PBSCのプールを生ずる。なぜなら、試料採取では特定の細胞の母集団が富化せず且つ/あるいは枯渇しないからである。特定の態様において、ドナー(例えば、ハプロタイプ一致ドナー)から単離されたPBSCのプールは、対象に投与する前にPBSCのプールからの特定の細胞(例えば、T細胞が挙げられるが、これに限定されるものではない)の母集団を富化/または枯渇させる、本発明の方法または手技ではない。
注入されたドナーのリンパ球の大多数(約80%)は、T細胞(それ以外に注入された細胞は、B細胞およびNK細胞)である。T細胞は、移植後免疫現象(幹細胞の生着、GvHD、GvT、および免疫の再構築)における主要な役目を担っており、B細胞およびNK細胞は支持的な役割に寄与している可能性の高いことを明らかにしてきた(1,7)。T細胞の大多数(約95%)は、αβT細胞(αβT細胞)と呼ばれるアルファ・ベータT細胞受容体(αβ-TCR)を担持している一方、T細胞の極一部分は、ガンマ・デルタT細胞(γσT細胞)と呼ばれる別のT細胞受容体(γσ-TCR)を担持している。
胞が共有する機能によって、悪性細胞を認識できる(8)。
ネズミおよびヒトの両方に関する研究が示唆するところによれば、γσT細胞は、GvHDを発症させる主要因子ではなく、αβT細胞のGvHD活性を調節しているというのが実情でありうる。Drobyski(25)が明らかにしているように、大用量のIL-2によって増殖したγσT細胞を、致死線量照射MHC異種マウス(C57BL/6[H-2b]a BIO.BR[H-2k]およびC57BL/6[H-2b]a B6D2Fl[H-2b/d])に注入しても、GvHDの発症を誘発しない可能性がある。Ellison(29)が注目したように、γσT細胞はGvHD反応によって活性化されるが、γσT細胞を介してGvHDが発症するという証拠は見出されていない。この研究は、Drobyski(25)による以後の研究と一致している。Drobyskiが公表したところによれば、活性化されたγσ細胞および未処置のαβT細胞を同時に注入するとGvHDが増悪したが、αβT細胞の注入を2週間延期すると生存率が改善されるという結果が得られた。ヒトの研究において、Schilbach(30)およびLamb(31)は、インビトロでの同種異系混合リンパ球培養においてγσT細胞が実質的に活性化されないことを見出した。BMT後の幾つかの研究によって、γσT細胞が過渡的に増加することが、明らかにされてきたが、この所見をGvHDと関連付けなかった(32-34)。一方、Tsuji(35)は、T細胞が病変に編成され且つCD4+αβT細胞によって活性化される可能性のあることを見出した。αβTCD移植片を投与された患者の予後を、汎TCD移植片を投与された患者と比較する幾つかの研究はいずれも、αβTCD群ではGvHDの発症率が低いことを明らかにしている。これは、移植片中のγσT細胞を注入しても、患者においてGvHDのリスクが増大しないことを示唆するものである(36,37)が、γσT細胞がGvHDの発症の一因となる可能性が本当に低いかどうかは依然として試験されていない。
本開示は、HSCT方法を提供するものである。一実施形態において、本開示は、エクスビボでのγσT細胞増殖およびαβT細胞枯渇方法(例えば、CLINIMACS(登録商標)システムを使用)とインビボでのT細胞枯渇方法(例えば、移植後シクロホスファミド)との組み合わせを使用して、HSCT方法を提供する。インビボでのT細胞枯渇(例えば、最小限に処置された幹細胞の移植片注入後に、CYを注入すること)によって、この方法を用いない限りGvHDのリスクを助長させかねないアロ反応性T細胞が(インビボで)枯渇する。エキソビボで増殖/活性化されたγσT細胞産物によって、選択的にαβT細胞が枯渇するが、またNK細胞の二次母集団を含むことになる。
αβT細胞枯渇の方法は、当該技術分野において知られており、当該技術分野で公知の任意の方法を使用できる。一実施形態において用いられるαβT細胞枯渇方法は、以下のとおりである。αβT細胞を枯渇させるには、α/β TCR試薬キットおよび他の関連試薬が付属しているCLINIMACS(登録商標)デバイスを使用する。CLINIMACS(登録商標)α/β TCR試薬は、αβ細胞に特異的な滅菌モノクローナル抗体試薬である。αβT細胞の枯渇は、メーカーの指示に従って前述したように実施する(16)。概略説明すると、白血球分離/エクスビボで増殖された産物を磁気粒子に接合された適切な抗体と共にインキュベートし、その後、CLINIMACS(登録商標)デバイス(Miltenyi Biotec)を使用して処理する。CLINIMACS(登録商標) Plus Instrumentは、血液試料(細胞産物)を処理するソフトウェア制御式計器である。CLINIMACS(登録商標)チューブセットは、独自の細胞選択列を含む、1回限りの使い捨て式滅菌チューブセットである。CLINIMACS(登録商標)PBS/EDTA緩衝液は、1mMの等張性滅菌リン酸緩衝EDTA生理食塩水であり、インビトロで血液細胞を調製するための外部洗浄および輸送流体として使用される。
γσT細胞の増殖の方法は、当該技術分野において知られており、当該技術分野で公知の任意の方法を使用できる。一実施形態では、末梢血単核細胞(PBMC)は、末梢血採血または白血球分離を介して得られる。培養物中にPBMC産物を1~2×106/mlの密度で補充し、2mMのZOMETA(登録商標)(Novartis, Inc.;ゾレドロン酸)および100μ/mlのインターロイキン2(IL-2)(ミルテニーバイオテク、本拠:独国ベルギッシュ・グラートバッハ)を添加して、適切なGMPグレードベースの培地培養物または生体化学反応器システム(例えば、組織培養プラスチックまたはPRODIGY(登録商標)生体化学反応器システム(ミルテニーバイオテク)で、単球を付着させるのを可能にする。細胞培養の14日後に、αβT細胞を獲得し且つ枯渇させた。
患者が本研究に登録するための要件は、全ての適格基準を満たしていること、および除外基準により除外の対象とはならないことである。
本研究に対する適格基準は次のとおりである。
(i) 腫瘍性血液学的不全症の患者で、全米総合癌情報ネットワーク(NCCN)もしくは他の下記の標準ガイドラインに準ずる同種異系移植術を適応とすること;(a) 急性リンパ芽球性の白血病[ALL]で、高リスクの特徴もしくは再発性疾患を有すること;(b) ホジキンまたは非ホジキンリンパ腫[HLまたはNHL]:再発性疾患で、寛解持続期間が1年未満であるか、過去の自家移植術後に再発したか、または化学療法を受けても完全奏効(CR)が得られないこと;ならびに(c) 骨髄悪性疾患(急性脊髄性の白血病[AML]で、NCCN基準に準ずる中等度/高リスクの特徴を有するかまたは再発性疾患を伴うこと、あるいは血液学的寛解期もしくは慢性期における慢性脊髄性の白血病[CML]を有すること;
(ii) 骨髄不全(骨髄異形成症候群[MDS]で、中等度/高リスクの特徴を有するか、または難治性疾患もしくは骨髄増殖性不全を有すること;原発性または続発性骨髄不全で、高リスクの特徴もしくは難治性疾患を有すること);
(iii) ふさわしいHLA適合ドナーの不在;
(iv) 年齢基準:19~65歳であること;
(v) 臓器機能の基準:本研究に登録する前の35日以内に、以下の臓器機能試験を実施する必要がある;(a) 心臓:MUGAまたは心エコー図で、左室駆出分画(LVEF)の50%以上であること;(b) 肺:FVC、FEV1およびDLCO(補正後)が、予想値の50%以下である必要がある:(c) 腎臓:血清クレアチニンレベルが2mg/dl未満であるか、または推定クレアチニンクリアランス(CrCl)が40mL/min/1.73m2(コッククロフト・ゴールト式による計算値)以上であること;ならびに(d) 肝臓薬:血清ビリルビンが1.5×正常数の上限(ULN)以下、アスパルテート・トランスアミナーゼ(AST)/アラニン・トランスアミナーゼ(ALT)2.5×ULN以下、且つアルカリホスファターゼが2.5×ULNであること;
(vi) 一般全身状態:カルノフスキー指数(Karnofsky)が70%以上であること;且つ
(vii) 同意:全ての患者が、本研究の治験的性質に関して通知を受け、且つ研究機関のガイドラインおよび連邦ガイドラインに準じた書面によるインフォームドコンセントを得る必要がある。
適用の対象となる除外基準は、以下のとおりである。(i) 服薬が順守されていないこと;(ii) 適切な介護者が同定されないこと;(iii) 制御不能な医学的障害または精神障害によって患者が臨床研究(主治医師の判断)を経るのを妨げられる可能性があること;(iv) 中枢神経系(CNS)に活性な新生物病変が存在すること;(v) 患者がDMSOに対するアレルギーの既往歴を有すること;(vi) HIV1(ヒト免疫不全ウィルス-1)またはHIV2に対して陽性であること;ならびに(vii) 妊娠中または授乳中であること。
予備的な化学療法レジメン
開示されている方法は、様々な病態の治療に使用される。治療の対象となる病態に応じて、以下の前処置レジメンのうちの1つを使用する。

化学療法での用量は、調整後の体重に基づく(但し、実際の体重が理想体重(IBW)を下回る場合には、実際の体重を用いる)。体重は、次式のように算出される。
・ 理想体重(IBW):
o 男性:IBW=50+([身長(cm)×0.39)-60]×2.3)
o 女性:IBW=45.5+([身長(cm)×0.39)-60]×2.3)
・ 調整済の重量=IBW+(実重量-IBW)×0.4
ドナーは、α/βが枯渇したドナーリンパ球輸注を投与される患者に対し、研究機関の手技に従い、同種異系造血幹細胞を提供するうえでの適格性および適合性についてスクリーニングを受ける。移植の前日に、CD34+細胞用量5×106細胞/kg以上の体重の(4×106細胞/kg超)の被移植者を対象とした好適且つ適格なハプロタイプ一致ドナーは、幹細胞採取のための末梢血アフェレーシスを経る。移植産物を4×106CD34+細胞/kgのCD34用量未満に低減することなしに、α/βT細胞を枯渇させるため、最小限1×105γσT細胞/kgを含有している本産物の一部を除去する。処置された画分を低温保存し、好中球の生着が確証されるまで貯蔵する。採取された幹細胞の総数が5×106細胞/kg未満で、且つ/あるいはCD34用量を4×106細胞/kg未満に減らすことなしに、必要なγσT細胞用量を得ることかできない場合、本産物を追加的な処理に備えて分割せずに、患者が本研究から脱退する。本産物を投与される前に本研究から脱退した参加者に対しては、割り当て済の前処置レジメンに従って、引き続く再発に備えて移植後サイトキサンを投与する。
γσT細胞の増殖プロトコールに関しては、cGMP/cGMP製造プロトコールの下で、分類済ISO7スペース内の標準生物学的安全キャビネットで製造を実施する。ドナーのアフェレーシス産物を、自家血清2μMゾレドロネート(Novartis Oncology、本拠:ニュージャージー州イーストハノーバー)+GMPグレードのIL-2(Miltenyi Biotec)50μ/ml有りまたは無しの商用GMPグレードT細胞の増殖培地中に1.0-2.0×106/mlで懸濁させる。培養物を、14日間にわたって元の密度のままに維持し、培養後2日、6日および10日後にIL-2を50μ/ml添加し、pHおよび細胞密度により定量された完全な培地を添加する。培養を開始してから0日目、7日後および14日後に、組成、純度、および生存度をフロー・サイトメトリで定量する。14±3日目に、CLINIMACS(登録商標)、PRODIGY(登録商標)(ミルテニーバイオテク、本拠:カリフォルニア州オーバーン)もしくは他の好適な生体化学反応器/細胞分離システムを使用して、記載されているとおりに、αβT細胞を枯渇させる。最終生存度の定量は、To Pro Iodideの取り込み、または他の細胞生存度染色のフロー・サイトメトリ解析によって得られる。我々は、注入許容範囲であるγσT細胞60%以上、αβT細胞5%以下およびNK細胞25%を、最終産物の放出基準として定めた。注入対象の産物を放出するための生存度を、70%以上として確証する必要がある。産物の生存度が70%未満の場合は、例外的な放出を必要とする。K562細胞に対してインビトロでの細胞毒性を使用して、細胞産物の効力を定量する。
別個に投与される細胞産物の一部が低温保存されている場合、最小限に処置された分画の注射中に、本研究の参加者をジメチルスルホキシド(DMSO)に曝露させる。DMSOの毒性は、低温保存された産物投与の際に考えられる合併症である。副作用および症状は、ヒスタミン放出を伴うのが一般的である。徴候および症状としては、咳嗽、紅潮、発疹、胸部緊張、喘鳴、悪心、嘔吐、および心血管不安定性が挙げられる。DMSOに対する反応のリスクを低減するため、標準幹細胞を注入する際には、予防措置を講じる。これらの予防措置としては、注入速度を減速すること、抗ヒスタミン剤を予め投薬すること、投与中に連続的に監視することが挙げられる。
移植後3日目および4日目に、CY(50mg/kg)の注入を行う。出血性膀胱炎を防ぐため、研究機関のガイドラインに従ってメスナ(MESNA)を投与する。
移植後7日目以降、好中球の生着が確認された後3日目までの任意の時点で、γσT細胞の注入を行う。このタイミングによって、CYが完全にウォッシュアウトされた後に、γσT細胞注入液の注入が可能になる。CYの消失半減期は、3~12時間(LexiComp monograph)であるため、7日目に、γσT細胞の移植片の注入が予測される前に、5半減期(=60時間)後に予期された完全なクリアランスが起こる。生着が予期されるのは、典型的に、約14日目~18日目である。ドナー細胞の移植後注入用に設定されたプログラムの標準順序に従って、産物注入が実行される。7日目に、患者の病態が、例えば高熱、不安定な圧力、または重篤な容量過負荷などのように悪化した場合、主治医師の判断で、T細胞注入液の注入が(9日目まで)2日間保留にされる場合がある。また、患者が腎臓機能不全を発症した場合、移植後のCYに続いて、注入を9日目まで延期して、CYの消去を確実に行った後でγσT細胞を注入する。
GvHDの予防は、移植後のCY(移植後3日目および4日目に50mg/kgの静注)からなる。他のGvHDの予防は、必要に応じて、ミコフェノール酸モフェチル(MMF、CELLCEPT(登録商標))およびカルシニューリン阻害剤、例えば、タクロリムスを含む。一実施形態において、本開示の方法によって、GvHD予防レジメンを低減することが可能になる。
移植後5日目に、ハプロタイプ一致移植患者のケア標準の一環としてG-CSF 5mcg/kgにて開始し、これを好中球が生着するまで継続する。
監視対象となる観察は以下のとおりである。
移植前に必要な観察:(本研究に登録する前の35日以内から、最終の移植評価日まで)
1.既往歴および身体検査(カルノフスキーの一般全身状態スコア)。
2.CBC、BUN、クレアチニン、AST、ALT、総ビリルビン。
3.心エコー図またはMUGA。
4.肺機能試験:FVC、FEV1、DLCO(ヘモグロビンに対し補正後)。
5.片側性骨髄穿刺液および生検(急性の白血病患者が対象)、形態学、ならびに細胞遺伝学。
6.CT走査または全身CT/PET走査によって、適宜に、疾病状態の評価を行う。
7.ALL患者において腰椎穿刺を実施する。予備移植髄腔内治療は、主治医師の判断で実施される場合がある。
移植後に必要とされた観察および経過観察の計画は、以下のとおりである。
疾病状態:疾患タイプおよび部位によって示唆されたように、適切な試験を用いて、移植後の疾病状態を評価する。
臨床的に安定で、且つその時点までの疾患進行が実証されていなかった全ての患者を対象とし、移植後30日目(±7)、100日目(±14)、180日目(±21)、および1年目(±45日)に、形態学的検査および細胞遺伝学的目的で、骨髄穿刺液および生検試料を採取する。加えて、再発の疑いがある場合は常に、片側性骨髄穿刺液を採取する。
臨床的に安定で、且つその時点までの疾患進行が実証されていなかった全ての患者を対象とし、移植後100日目(±14)、180日目(±21)、および1年目(±45日)に、CT走査または全身CT/PET走査を実施する。

シタラビン
シタラビン(1-β-D-アラビノフラノシルシトシン)は、式C9H13N3O5(分子量243.22)の抗悪性腫瘍薬であり、静脈投与、髄腔内投与または皮下投与用の滅菌溶液として使用される。成人および小児科患者の急性非リンパ性の白血病における寛解誘発に対して、シタラビン注射剤と他の認可済の抗癌剤との併用が適応とされる。また、急性非リンパ性の白血病、急性リンパ芽球性の白血病、急性脊髄性の白血病、および慢性骨髄性の白血病芽球期の治療に有用であることも見出されてきた。髄膜白血病の予防および治療においては、シタラビン注射剤(防腐剤を含まない製剤のみ)の髄腔内投与が適応とされる。細胞フェーズの特異性を呈し、一次的な死滅細胞がDNA合成(S-フェーズ)を経て、或る条件の下でG1フェーズからS-フェーズまでの細胞進行をブロックする。シタラビンは、その作用機序が完全には理解されていないが、DNAポリメラーゼを阻害することによって作用するらしい。
シクロホスファミドは、化学的に2-[ビス(2-クロロエチル)アミノ]テトラヒドロ-2H-1,3,2-オキサアザホスホリン2-オキシド一水和物として認識される合成の抗悪性腫瘍薬である。CYの分子式はC7H15Cl2N2O2P・H2Oで、分子量は279.1である。非経口用CYを調製する際には、0.9%塩化ナトリウム溶液(直接注射の場合)または滅菌水(注入の場合)のいずれかを添加する必要がある。水で構成されたCYは低緊張であるので、直接に注入してはならない。CYと塩化ナトリウム溶液とを混合した溶液は、静脈内に、筋内に、腹膜内に、または胸膜内に注射される場合がある。構成されたシクロホスファミドは、室温で24時間静置するかまたは6日間冷凍すれば、物理的におよび化学的に安定である。調製済溶液には、微生物防腐剤がいっさい含有されていないので、溶液が無菌状態かどうかを監視する必要がある。
リン酸フルダラビン(フルダラビン)は、化学名9H-プリン-6-アミン、2-フルオロ-9-(5-0-ホスホノ-0-D-アラビノ-フラノシル)(2-フルオロ-アラ-AMP)の、代謝拮抗物質である。分子式は、C10H13FN5O7P、分子量は365.2である。滅菌水を白色の固形物ケーキに加えて、点滴静注用フルダラビンを調製する。固形物ケーキを滅菌水2mLで再構成して、適切な濃度の25mg/mLリン酸フルダラビンを含む溶液を生成する。研究機関のガイドラインに従って、フルダラビンを更に調製し投与する手技を行う。再構成済の点滴静注用フルダラビン中には抗菌防腐剤が含有されていないため、再構成してから8時間以内に利用する必要がある。相溶性の不明な別の静注用溶液への同時注入は、厳禁である。
ブスルファンは、ジメタンスルホン酸1,4-ブタンジオールとして化学的に公知の二官能性アルキル化剤であり、分子式CH3SO2O(CH2)4OSO2CH3および分子量246g/molである。IVブスルファンは、先に希釈してから、NSまたは5%ブドウ糖液(D5W)のいずれかと使用する必要がある。希釈剤の量を、BUSULFEX(登録商標)容量の10倍とし、ブスルファンの最終濃度をおよそ0.5mg/mLとする必要がある。薬物注入ポンプを使用して、希釈ブスルファン溶液を投与する必要がある。相溶性の不明な別の静脈注射用溶液への同時注入は、厳禁である。警告:点滴静注用ブスルファンの急速注入は、未試験であって推奨されない。ブスルファンの調製および投与は、研究機関のガイドラインに従って行うこと。
タクロリムスは、ストレトサイト・ツクバエンシス(Stretocyces Tsukubaensis)によって産生されるマクロライド免疫抑制剤である。タクロリムスは、実験式C44H69NO12H2Oおよび式量822.05を有する。タクロリムスは、白色の結晶または結晶性粉末の外観をしており、実質的に水に不溶で、エタノール内の溶け易く、メタノールおよびクロロホルムに極めて溶け易い。厳密な作用機序は公知ではないが、タクロリムスはTリンパ球の活性化を阻害する。実験的な証拠は、タクロリムスは細胞内タンパク質であるFKBP-12に結合することが示唆されている。この場合、タクロリムス-FKBP-12とカルシウムとカルモジュリンとカルシニューリンとの複合体が形成され、カルシニューリンのホスファターゼ活性が阻害された。この効果によって活性化されたT細胞(NF-AT)の核因子の脱リン酸化および転座が妨げられる可能性があり、核成分によってリンフォカイン(例えば、インターロイキン2、γインターフェロン)の形成に対する遺伝子転写が開始されると考えられる。最終結果は、Tリンパ球活性化の阻害(すなわち、免疫抑制)である。タクロリムス(PROGRAF(登録商標)注射剤)は、使用前にNSまたは5%ブドウ糖液(D5W)で希釈する必要がある。タクロリムスは、連続注入として投与される。経口製剤は、12時間ごとに空腹時に投与される。
CELLCEPT(登録商標)は、ミコフェノール酸(MPA)の2-モルホリノエチルエステル、免疫抑制剤、イノシン一リン酸デヒドロゲナーゼ(IMPDH)阻害剤である。ミコフェノール酸モフェチル(MMF)の化学名は、2-モルホリノエチル(E)-6-(1,3-ジヒドロ-4-ヒドロキシ-6-メトキシ-7-メチル-3-オキソ-5-イソベンゾフラニル)-4-メチル-4-ヘキサノエートである。C23H31NO7および分子量は433.50の実験式を有する。ミコフェノール酸モフェチルは、灰色がかった白色ないし白色の結晶性粉末であり、水(43μg/ml、pH7.4)に僅かに溶解し、酸性培地(4.27mg/mL、pH3.6)中では可溶性が増大する。アセトンに大量に溶解し、メタノールに溶解し易く、エタノールにはやや難溶である。1-オクタノール/水(pH7.4)緩衝溶液における見かけの分配係数は、238である。ミコフェノール酸モフェチルのpKa値は、モルホリノ基では5.6、フェノール基では8.5である。ミコフェノレートモフェチル塩酸塩は、5%ブドウ糖液(D5W)中に65.8mg/mL溶解する。再構成済溶液のpHは、2.4~4.1である。MPAの吸収に変動が生ずるのを回避するため、経口投薬製剤(錠剤、カプセル、懸濁液)は空腹時に投与すべきである。経口溶液は経鼻胃管(最小8フレンチ、内径1.7mm)を介して投与することが可能であるが、経口懸濁液は、他の薬品と混合すべきでない。遅延放出錠は、圧壊、切断または咀嚼すべきでない。静注用溶液は、少なくとも2時間にわたって(末梢静脈または中心静脈のいずれかで)投与すべきであり;逆に、急速注入またはボーラス注射を介して静注用溶液を投与してはならない。
NEUPOGEN(登録商標)は、組み換えメチオニルヒト顆粒球コロニー刺激因子(r-methHuG-CSF)を表す、フィルグラスチムの商標名である。NEUPOGEN(登録商標)は、大腸菌(Escherichia coli(E. coli))を利用する組み換えDNA技術によって生産される、175アミノ酸タンパク質である。NEUPOGEN(登録商標)は、分子量が18,800ダルトンで、且つ大腸菌(E. coli)における発現に必要なアミノ酸配列(但し、N末端の追加的メチオニンを除く)が天然ヒトDNAのアミノ酸配列に類似している。NEUPOGEN(登録商標)は、点滴静注または皮下注入として投与することが可能である。骨髄注入液後少なくとも24時間、NEUPOGEN(登録商標)を投与することが推奨される。投薬量の調節は、好中球応答により定量される。必要に応じて、NEUPOGEN(登録商標)を5%デキストロースで希釈し、アルブミン(ヒト)を添加することによって、可塑性物質に吸収されるのを防ぐことができる。最終濃度5mcg/mL未満に希釈することは、如何なる時点においても推奨されない。生成物が沈澱する可能性があるので、生理食塩水で希釈しないこと。バイアルまたは予備充填されたシリンジを使用するときには、今後の投与に備えて、未使用の薬物を保存しないこと。全ての未使用部分を処分する。
TBIは、放射線腫瘍医によって実装されたケア手技の標準に従って投与される。5年までに、用量/分画が2Gy×6を超えない思春期後の患者の場合、単独のTBIは、重篤な遅発性毒性のリスク(臓器不全または重大な機能障害)に対して5%未満と優にほとんどの正常な臓器の許容範囲内に収まる。注目すべき例外は、白内障の発症、骨髄抑制、および卵巣および睾丸の機能障害を起こすリスクはである。また、第2の悪性疾患に罹るリスクが僅かにある。最も一般的な急性効果としては、嘔気、嘔吐、下痢、および耳下腺の有痛性腫脹が挙げられる。移植環境(setting)においてTBIを他の療法と併用した場合、食欲低下、口渇、嚥下困難もしくは嚥下痛、頭痛、口内炎(咽喉痛/口腔)、皮膚の完全性の改変、脱毛、腫張、感染症もしくは出血のリスク増大、肺不全の可能性、乾性咳、疲労、苦悶、熱、肝不全の可能性、肺の瘢痕化、視力低下、息切れ、滅菌、胸やけ、膀胱炎、睡眠障害、消化管および尿生殖器作用の改変、神経障害、フィステル、内分泌作用の改変、心膜炎、ならびに第2の癌のリスク増大などの更なる副作用リスクが存在する。概して、上記に概説されているように、放射を他の療法と同時に施したとしても、ほとんどの重大な毒性の発病率は、依然として低く、稀れであり、重篤な副作用の可能性もある。
ABD研究で予期された予後は、急性GvHDの発症が、ABD補充移植片を含まない移植後シクロホスファミドを投与されるハプロタイプ一致移植患者と全然違いのないことである。この予後はまた、EAGD患者に対しても予期されるが、加えて、我々の予測するところでは、移植後の初期期間(100日)に感染性合併症の発病率が低下し、1年、2年および5年目に再発性疾患の発病率が低下する。

Claims (16)
- 同種異系造血幹細胞移植(HSCT)治療レジメンにおいて使用するための、γσT細胞の増殖された集団を含む、組成物であって、

該HSCT治療レジメンは、
0日目に、末梢血幹細胞(PBSC)を含む同種異系造血幹細胞(HSC)が患者へ注入され、
0日目に対して、約7日目~約25日目に、該組成物が患者へ注入され、
インビボでT細胞を枯渇させる薬剤は、患者に該組成物が注入される前に、投与され、及び
0日目より前に予備的な化学療法レジメンが投与されることを含む、HSCT治療レジメンである、
組成物。 - 組成物における少なくとも約70%の細胞が生存している、請求項1に記載の使用のための組成物。
- 0日目に対して、約1日目~10日目に、インビボでT細胞を枯渇させる薬剤が患者に投与される、請求項1又は2に記載の使用のための組成物。
- インビボでT細胞を枯渇させる薬剤が、シクロホスファミド(CY)である、請求項3に記載の使用のための組成物。
- 0日目に対して、約7日目~約9日目に患者に注入される組成物である、請求項1~4のいずれかに記載の使用のための組成物。
- 移植片対宿主病(GvHD)予防的治療レジメンが、CY、ミコフェノール酸モフェチル(MMF)、タクロリムス、又はそれらのいずれかの組み合わせを含み、0日目に対して約5日目から1日以上の投与が開始され、0日目に対して約50日目以上まで継続して、患者に投与される、請求項1~5のいずれかに記載の使用のための組成物。
- 患者の体重の約5×108γσT細胞/kg以下、患者の体重の約1×107γσT細胞/kg以下、又は患者の体重の約5×106γσT細胞/kg以下を含む、請求項1~6のいずれかに記載の使用のための組成物。
- 急性リンパ球性の白血病(ALL);再発性ALL;ホジキンリンパ腫(HL);非ホジキンリンパ腫(NHL);再発性HLまたはNHL;急性骨髄性の白血病(AML);再発性AML;慢性骨髄性の白血病(CML);骨髄異形成症候群(MDS);難治性MDS;星状細胞腫;非定型奇形腫様ラブドイド腫瘍;脳幹部神経膠腫;脈絡叢腫瘍、癌腫および乳頭腫;頭蓋咽頭腫;線維形成性乳児星細胞腫;生殖細胞腫瘍;髄芽細胞腫;神経線維腫;乏突起膠腫;視覚神経膠腫;神経芽細胞腫;ユーイング肉腫;感染;又は原始神経外胚葉性腫瘍の治療のための、請求項1~7のいずれかに記載の使用のための組成物。
- 患者における移植片対宿主病(GvHD)を治療又は最小限にすることにおいて使用するための、γσT細胞の増殖された集団を含む、組成物であって、
該組成物は、αβT細胞が枯渇し、細胞生存度染色を使用したフロー・サイトメトリ解析による測定で、60%を超えるγσT細胞、約5%未満のαβT細胞及び約25%未満のナチュラルキラー(NK)細胞を含み、
0日目に、末梢血幹細胞(PBSC)を含むハプロタイプ一致造血幹細胞(HSC)が患者へ投与され、
0日目に対して、約7日目~約25日目に該組成物が患者へ投与され、
患者に該組成物が注入される前に、インビボでT細胞を枯渇させる薬剤が投与され、及び
0日目より前に予備的な化学療法レジメンが投与される、
組成物。 - 組成物中の細胞の少なくとも約70%が生存している、請求項9に記載の使用のための組成物。
- インビボでT細胞を枯渇させる薬剤が、シクロホスファミド(CY)である、請求項9又は10に記載の使用のための組成物。
- 患者の体重の約5×108γσT細胞/kg以下、患者の体重の約1×107γσT細胞/kg以下、又は患者の体重の約5×106γσT細胞/kg以下を含む、請求項9~11のいずれかに記載の使用のための組成物。

- 同種異系造血幹細胞(HSC)がハプロタイプ一致HSCである、請求項1に記載の組成物。
- 予備的な化学療法レジメンが、骨髄疾患に対するフルダラビン/ブスルファン/全身放射線照射、重大な共存症を有さない40歳未満の患者における急性リンパ球性白血病(ALL)又は侵攻性非ホジキンリンパ腫(NHL)又はホジキンリンパ腫(HL)に対する全身放射線照射/シクロホスファミド(TBI/CY)、及び強度TBI/CYにより非再発死亡率(NRM)が高まる前兆となる重大な共存症を有する40歳を超えるかもしくは任意の年齢の患者におけるALLまたはリンパ腫に対するフルダラビン/全身放射線照射から選択される、請求項1に記載の組成物。
- 同種異系HSCが最小限に処置されている、請求項1に記載の組成物。
