Description
本発明は、例えば、以下を提供する。
(項目1)
血流低減アセンブリであって、
カテーテルシャフトと、
前記カテーテルシャフトと共に組み立てられる拡張可能閉塞部材であって、前記拡張可能閉塞部材は、相互接続支柱を備え、前記支柱の少なくともいくつかの遠位端は、折畳可能突出部を有し、前記折畳可能突出部は、前記折畳可能突出部が、1つまたはそれを上回る方向に前記支柱の前記少なくともいくつかのまわりで枢動するように、ヒンジ部材によって前記遠位端に接続される、拡張可能閉塞部材と、
前記折畳可能突出部に接続される1つまたはそれを上回る接続リンクであって、前記1つまたはそれを上回る接続リンクは、ハンドルまで延在し、前記ハンドルの操作は、前記1つまたはそれを上回る接続リンクおよび前記折畳可能突出部を移動させ、前記折畳可能突出部の閉塞能力を改変する、接続リンクと、
を備える、血流低減アセンブリ。
(項目2)
前記折畳可能突出部を被覆する被覆材をさらに備え、前記被覆材は、血流に不浸透性である、項目1に記載の血流低減アセンブリ。
(項目3)
前記被覆材はまた、前記支柱の遠位端の一部および前記ヒンジ部材の一部を被覆する、項目2に記載の血流低減アセンブリ。
(項目4)
前記折畳可能突出部が内向きに移動されたとき、オリフィスと呼ばれる空間は、前記被覆材内で開放されたままにされる、項目2に記載の血流低減アセンブリ。
(項目5)
前記折畳可能突出部の閉鎖度を示すように構成される前記ハンドル上のインジケータをさらに備える、項目1に記載の血流低減アセンブリ。
(項目6)
相互により近づく前記折畳可能突出部の移動は、前記折畳可能突出部の閉塞能力を増加させ、相互からより離れる前記折畳可能突出部の移動は、前記折畳可能突出部の閉塞能力を減少させる、項目1に記載の血流低減アセンブリ。
(項目7)
前記ヒンジ部材は、前記支柱の一部または全部の遠位端のまわりで円周方向に分散される、項目1に記載の血流低減アセンブリ。
(項目8)
前記折畳可能突出部が相互により近づけられたとき、オリフィスと呼ばれる空間は、前記被覆材内で開放されたままにされる、項目2に記載の血流低減アセンブリ。
(項目9)
前記折畳可能突出部の閉鎖度を示すように構成される前記ハンドル上のインジケータをさらに備える、項目1に記載の血流低減アセンブリ。
(項目10)
血流低減アセンブリであって、
カテーテルシャフトと、
前記カテーテルシャフトと共に組み立てられる拡張可能閉塞部材であって、前記拡張可能閉塞部材は、ループを含む、拡張可能閉塞部材と
を備え、
前記カテーテルシャフトは、外側シャフト内で摺動するように配列される内側シャフトを備える伸縮自在シャフトを備え、前記ループは、前記閉塞部材の近位端と遠位端との間に位置付けられる螺旋ループを備え、前記閉塞部材の近位端は、前記内側シャフトおよび外側シャフトの一方に固着され、前記閉塞部材の遠位端は、前記内側シャフトおよび外側シャフトの他方に固着され、前記近位端と遠位端との間の距離の減少は、前記閉塞部材のループをともに寄せ集め、前記拡張可能閉塞部材の閉塞能力を増加させる、血流低減アセンブリ。
(項目11)
前記拡張可能閉塞部材は、前記ループの中に形成される部分を伴う、バルーンを備える、項目10に記載の血流低減アセンブリ。
(項目12)
前記拡張可能閉塞部材は、自己拡張式である、項目1に記載の血流低減アセンブリ。
(項目13)
前記拡張可能閉塞部材は、流動的に拡張可能である、項目1に記載の血流低減アセンブリ。
腎静脈の入口の後端にある、下大静脈内への本デバイスの設置は、その面積内により低い圧力のゾーンを生成し、それによって、腎臓における圧力勾配を増加させ、腎機能を改良する。
Claims (8)
- 患者の体腔の中に設置されるように構成された血流低減アセンブリ(10)であって、
カテーテルシャフト(12)と、
前記カテーテルシャフト(12)と共に組み立てられる拡張可能閉塞部材(14)であって、前記拡張可能閉塞部材(14)は、相互接続支柱(16)を備え、前記支柱(16)のうちの少なくともいくつかの遠位端は、突出部(20)を有し、前記突出部(20)は、前記突出部(20)が前記支柱(16)の前記少なくともいくつかのまわりで枢動するように、ヒンジ部材(22)によって前記遠位端に接続される、拡張可能閉塞部材(14)と、
前記突出部(20)に接続される1つまたはそれを上回る接続リンク(26)であって、前記1つまたはそれを上回る接続リンク(26)は、ハンドル(28)まで延在し、前記ハンドル(28)の操作は、前記1つまたはそれを上回る接続リンク(26)および前記突出部(20)を移動させ、前記突出部(20)の閉塞能力を改変し、前記1つまたはそれを上回る接続リンク(26)を移動させることは、前記1つまたはそれを上回る接続リンク(26)を引動することを含み、これにより、前記突出部(20)を半径方向内向きに折畳し、前記突出部(20)の下流の血流を低減する、接続リンク(26)と、

を備え、血流は、オリフィス(34)を通って流動することによって前記アセンブリ(10)から退出し、前記オリフィス(34)は、前記突出部(20)が内向きに完全には折畳されない場合の中心を通る、前記被覆材(24)内に開放されたままにされる空間を通して形成され、
前記オリフィス(34)の直径は、前記拡張可能閉塞部材(14)の直径および前記患者の前記体腔の直径よりも小さく、これにより、血液は、前記拡張可能閉塞部材(14)の比較的大径から前記オリフィス(34)の比較的小径を通って、次いで、前記体腔の比較的大径に戻るように流動し、
