序論
PLGAナノ粒子は、薬物送達、組織および細胞のイメージングを含めた多様な適用のために、ならびに免疫の活性化または寛容の誘導を助ける自己または外来タンパク質を送達するために利用されている。(Sah et al.,「Concepts and practices used to develop functional PLGA-based nanoparticulate systems」, International Journal of Medicine, 2013:8 747-765を参照されたい)。ここで、本発明者らは、官能基化PLGA粒子を生成するための技術を開発し、かつ免疫応答を改変するためのそれらの潜在力を確立した。
本発明者らは、初めに、アレルギー疾患を処置することへの治療的手法として、細胞に連結した抗原寛容に関する本発明者らの研究からこのエリアに着手し(Smarr et al.,「Antigen-fixed leukocytes tolerize Th2 responses in mouse models of allergy」, The Journal of Immunology, 11/2011; 187(1):5090-8を参照されたい)、それによってアレルギー性タンパク質を、EDCカルボジイミド架橋化学を用いて自己細胞に付着させ、マウスに注入して戻し、かつ寛容(すなわち、免疫学的非応答の状態)が生じる。これを患者に適用することは、細胞の使用によって複雑化するため、本発明者らは、代用としてPLGAナノ粒子の潜在的使用を検討し始めた;しかしながら、本発明者らの知見は、PLGAナノ粒子内にカプセル化された抗原が、異なる応答(寛容よりもむしろ脱感作、そのため反応性はしばらくして回復される)を誘導することを示した。マクロファージは、それらの産生を通じた、インターロイキン-10(IL-10)と呼ばれる主要な免疫媒介物質の寛容を含めた、免疫応答において重要であると考えられる1種の免疫細胞であるため、本発明者らは、EDC-細胞体対PLGAナノ粒子の効果を、IL-10産生に対するそれらの効果についてスクリーニングすることへのインビトロ手法を開発した。図1に示されるように、本発明者らは、EDC-細胞が、刺激(リポ多糖(LPS))後にIL-10を増強し、一方でPLGA粒子はそうではないことを観察した。
食物アレルギーは、食物に対する有害な免疫反応として規定され得、そして蕁麻疹および致死的なアナフィラキシーを含み得る。反応の重症度は、摂取された食物の量、食物の形態(例えば、生の、調理された、または加工された)、ならびに年齢、感作の程度、および他の併存状況などの危険要因を含めた、いくつかの要因に依存し得る。食物アレルギーは、古典的にIgE媒介性であるとして知られるが、生理学的な応答および症状においては不均一であり得る。(Sicherer and Sampson, J Allergy Clin. Rev. Mol. Bio. (2008) Mar;9(3):231-41、および図8を参照されたい)。寛容の発生に関与し得るタンパク質、脂質、糖脂質、および炭水化物を含めた、化合物が細胞表面に存在する。
背景
1型糖尿病(T1D)は、膵臓β細胞の自己反応性T細胞媒介性破壊によって引き起こされる自己免疫障害であり、外因性インスリン療法を要する高血糖症をもたらす。T1Dを発症する高い危険性を有する個体は、現在、ヒト白血球抗原に関する遺伝子型判定と膵島細胞自己抗体の一団に関する血清学的試験との組み合わせで同定され得る1。この危険性の高い集団では、臨床的糖尿病の急性発症の前または間、進行中のβ細胞指向性自己免疫が有効かつ永久に阻害され得る場合に、残りのβ細胞が正常血糖を回復させ得るような、実質的なβ細胞質量がなおも存在し得る2,3。制御性T細胞(Treg)は、末梢寛容を維持することにおいて重要な役割を果たし、かつそれらの欠損は、T1Dを含めた制御不良の自己免疫と関連している4。それゆえ、Tregを直接的または間接的に増殖させる免疫療法は、有望な治療的手法として見られてきた5〜7。最近の第1相臨床試験は、エクスビボ増殖の実現可能性、およびT1D患者におけるポリクローナルTregの養子移入の安全性を実証した7;しかしながら、エクスビボ増殖されたTregを用いたそのような養子免疫療法の効力は、依然として判定されていない。他方で、抗原特異的Tregは、T1Dにおける自己免疫を抑制することにおいて、ポリクローナルTregよりも強力であると考えられる8〜10。しかしながら、ヒト療法のための抗原特異的Tregのエクスビボ増殖は、関係するβ細胞自己抗原が、そのような個体において拡散しているよく認識されたエピトープを与えられた移動標的であることは言うまでもなく、非常に労働集約的でありかつ重要な規制およびライセンス義務を負う11。その結果として、内因性Tregのインビボ増殖を目的とする免疫療法が、より実現可能性が高く、かつ所与の個体に存在する自己抗原の特異的セットに合わせて調整された所望の抗原特異的阻害を達成する可能性がより高い。T1Dにおける免疫療法の最も有望な抗原候補は、インスリンそれ自体およびその誘導体である12。理想的には、関連するインスリン由来自己抗原を用いて、他のβ細胞自己抗原に拡散する有効な感染寛容13を誘導し得る。
中心的仮説:図9に概略的に示されるように、本発明者らは、T1Dにおける寛容療法のためのAg-NP送達システムの効力が、(1)CD209およびMer二重シグナル伝達のためのNP上の標的リガンド(LNFPIIIおよびGAS6)を同時に操作すること;ならびに(2)感染寛容を誘導するための初回の疾患関連自己抗原としてのインスリンの脱アミド化形態(INS(Q→E))の送達、によって有意に増強され得ると仮説を立てる。
抗原特異的寛容療法は、とくにT1Dに対する膵島移植の背景において、Luo研究室の主要な焦点になっている14。本発明者らの一次的手法は、関心対象の(ドナー)抗原を、カルボキシル活性化剤の1-エチル-3-(3-ジメチルアミノプロピル)カルボジイミド(ECDI)の存在下で、アミド結合形成によりそれらを脾細胞(Ag-SP)の表面に連結することによって送達することである20。この手法は、自己免疫の動物モデルにおける寛容誘導のために、本発明者らの同僚によって初めに実験された28。Luo研究室における先駆的仕事は、この寛容手法の堅牢な効力を同種および異種移植にさらに広げており21,23,29、一方でBryce研究室は、喘息およびアレルギー疾患においてこの手法の効力を実証するのに成功している19。Ag-SPを製造するための多数の患者細胞を加工する必要性を回避するために、本発明者らは、最近、そのような寛容原性Ag-NPワクチンの有望な効力を実証する、寛容原性抗原送達のための生物工学により作られたNPを用いた本発明者らの先駆的仕事を公開した25〜27。臨床的移植についてのはっきりとした見解を持って、この適用において、本発明者らは、(1)CD209およびMer二重シグナル伝達リガンドをNPの表面に結合させ;かつ(2)感染寛容を誘導するための初回の自己抗原として脱アミド化プロインスリンを送達することによって、T1Dにおける寛容送達のための非常に効果的なAg-NPワクチンを開発することに本発明者らの調査の焦点を合わせる予定である。
Ag-SP細胞を介した抗原送達は、欠失およびアネルギーによりTreg細胞の有意な増殖およびTeff細胞の寛容をもたらす。BALB/c→B6同種膵島移植モデルにおいて、B6レシピエントにおける(0日目のBALB/c膵島移植に対して)-7日目および+1日目のECDI固定されたドナー(BALB/c)脾細胞(Ag-SP)の注射は、無期限の膵島同種移植片生存をもたらす20。Ag-SP注射は、レシピエントの脾臓、流入リンパ節(dLN)、および移植された同種移植片におけるCD4+Foxp3+ Tregの有意な増殖をもたらす(図10A)。したがって、Ag-SP注射の時点のTregの枯渇がその寛容効力を完全に破棄したように20、Ag-SPによって誘導された寛容は、Ag-SPによるTregの増殖に依存している。Ag-SPによるTreg増殖は、最近、本発明者ら自身の仕事(非公開データ)によるおよび他者の公開データによる24、非ヒト霊長類膵島同種移植モデルにおいて立証されている。Treg増殖に付随して、Teff細胞は、2つの異なるメカニズム:(1)間接的特異性を有するT細胞の欠失;および(2)直接的特異性を有するT細胞のアネルギーによって寛容化される23。図10Bに示されるように、脾臓およびdLNにおいて、間接的ドナー特異性を有するT細胞(TEa TCRトランスジェニックT細胞の養子移入によって詮索される46)は、堅牢な初期増殖(-4日目)、それに続く急速な収縮および枯渇を受け(0日目、7日目)、それにより、7日目までに膵島同種移植片に浸潤するそのようなT細胞はほとんどない。対照的に、図10Cに示されるように、直接的ドナー特異性を有するT細胞(4C TCRトランスジェニックT細胞の養子移入によって詮索される47)は、未処理のBALB/c SPの注射に対するそれらの増殖と比較して、1回目のAg-SP注射に対して有意に損なわれた増殖を受ける。より重要なことには、残りの4C T細胞は、2回目のECDI-SP注射に対するそれらの応答の欠乏に現れるように(右のドットプロット)、ドナー刺激にもはや応答せず、それらが有効にアネルギー化されることを示す。まとめると、これらのデータは、Ag-SPがTregを堅牢に増殖させ、一方でTeffを欠失させかつ/またはアネルギー化することを示唆する。

目的1.LNFPIIIおよびGAS6装飾されたNPを開発すること
目的1A.LNFPIII-GAS6-NPの設計および製造。本発明者らは、最初に、Bryantらによって以前に記載された単一エマルション技法25を用いて、直径およそ500nmのポリ(ラクチド-コ-グリコリド(1:1))(PLG)ナノ粒子を作り上げる。ナノ粒子の表面を0.05または0.1MのNaOHで部分的に加水分解して、粒子の表面を官能基化しかつ抗原を連結するのに利用可能なカルボキシル基の密度を増加させる。NPゼータ電位を測定することによって、ならびにトルイジンブルーを用いてカルボキシル含量を定量化することによって、修飾をモニターする50。NP上のカルボキシル基を、カルボジイミド化学(ECDI)を用いて活性化し、かつ(N--マレイミドプロピオン酸ヒドラジド(BMPH)と反応させて、「クリック」化学において利用されるチオール基に反応性である、NPの表面にマレイミド基を提供する51。リガンドLNFPIIIおよびGAS6をシステインで誘導体化して、マレイミド官能基化されたNPへのそれらの共有結合性連結を可能にするチオール基を提供する。LNFPIII-Cysは、LNFPIIIとCysとの間の還元的アミノ化により合成される52。末端Cysを有するGAS6は、以前に記載されるように36、HEK 293T細胞においてHis6タグを用いた組換えDNA技術により合成され、かつNi-NTAビーズを用いたアフィニティークロマトグラフィー、それに続くHiTrap Q FFイオン交換カラム(GE Healthcare)での精製により単離される。
(図14C)、脱アミド化プロインスリン全体、またはMIN6(β細胞株)細胞溶解物全体を負荷する。本発明者らは、スクリーニング「陽性」LNFPIII-GAS6-NP種への抗原負荷のための2つの方法を試験する。第一の方法は、本発明者らが以前に記載したように25、ECDI化学を用いて、ナノ粒子の表面にペプチド/タンパク質抗原を連結することである。粒子に連結されたペプチド/タンパク質の量を、連結反応の前および後の上清中の抗原を定量化することによって判定する。連結効率またはRAW264.7 MFとのそれらの相互作用のいずれかが准最適である場合、本発明者らは、粒子内への抗原のカプセル化がより効率的であるかどうかも試験する。カプセル化されたペプチドとともに形成されたPLG粒子は、自己免疫性脳炎のモデルにおいて有効である54。単一エマルション過程と同様の直径および電荷(500nm、ζ電位=-60mV)を有する粒子を創出することを目的とする二重エマルション過程により、抗原をPLGまたはPPCN粒子内にカプセル化する。カプセル化のためのポリマー組成および平均分子量(固有の粘度によって特徴付けされる)を実験する、というのもこれらの特性はNPの安定性に影響し、それゆえカプセル化された抗原の細胞内在化および放出の両方に影響するであろうためである55。粒子サイズおよびゼータ電位の分布をゼータサイザーで測定する。粒子内にカプセル化されたペプチド/タンパク質の量を、CBQCAアッセイを用いた後続の解析のために、抗原負荷されたNPをDMSOに溶解することによって定量化する56。
目的2A:寛容原性INS(Q→E)-LNFPIII-GAS6-NPワクチンによる、NODにおける糖尿病の予防および処置。マウスモデル。本発明者らは、2種のNODモデルを用いる。第一のモデル(「予防」モデル)において、本発明者らは、2つの年齢コホート:5週齢および9週齢の雌NODマウスを処理する。両方の年齢群において、膵臓における炎症応答は、炎症誘発性免疫細胞浸潤の存在によって実証されるようにすでに始まっているが、血中グルコースレベルは、なおも正常範囲内にある。ゆえに、それらは前糖尿病である。本発明者らは、INS(Q→E)-LNFPIII-GAS6-NP処理(目的1による)について最もよく同定された式を適用して、本発明者らが、これらの前糖尿病NODマウスが糖尿病を発症するのを予防し得るかどうかを判定する。INS(Q→E)-LNFPIII-GAS6-NP処理後、マウスを血中グルコースレベルについて30週齢までモニターする。第二のモデル(「処置」モデル)において、本発明者らは、12週齢から始まる週2回のスクリーニングによって同定された、高血糖症にまさになっている急性糖尿病(12〜30週齢)NODマウスを用いる。本発明者らは、高血糖症の急性発症の3〜5日以内にINS(Q→E)-LNFPIII-GAS6-NP処理を適用する。この段階で、免疫療法による自己免疫の有効な制御が、残りのβ細胞の機能の回復、およびその結果として糖尿病の反転につながり得るような58、相当のβ細胞質量がこれらのNODマウスになおも残る。本発明者らは、急性糖尿病NODマウスにINS(Q→E)-LNFPIII-GAS6-NP処理を適用し、かつ本発明者らが、これらのマウスにおいて糖尿病を反転させ得るかどうかを判定する。INS(Q→E)-LNFPIII-GAS6-NP処理後、マウスを血中グルコースレベルについて合計60日間モニターして、糖尿病反転を判定する。

単独のものと比較して、より良好な寛容効力を呈するかどうかを判定する。(b)β細胞溶解物全体:本発明者らは、それらのβ細胞においてSV40のラージT抗原を発現するトランスジェニックマウスに由来するインスリノーマ細胞株MIN6からβ細胞溶解物全体を調製する60。β細胞溶解物全体を、ECDI架橋により表面に付着させる(本発明者らが、移植抗原に関してドナー細胞溶解物を用いて以前に行っているように25)、またはLNFPIII-GAS6-NP内にカプセル化する。架橋とカプセル化との間での選出を、目的1Cで記載される抗原負荷効率に基づいて同様に判定する。β細胞溶解物-LNFPIII-GAS6-NPを、前糖尿病および急性糖尿病の雌NODマウスに投与し、かつマウスをそれぞれ糖尿病予防および糖尿病反転についてモニターする。
に対する特異性を有する抗原特異的CD4+Foxp3+ Tregの誘導または増殖について検討する:(a)膵臓DLNおよび脾臓を、INS(Q→E)-LNFPIII-GAS6-NP処理後の一連の時点におけるCD4+Foxp3+ Tregの総数について(FACSによって)検討する;(b)膵臓DLNまたは脾臓由来の精製された総CD4+ T細胞(Tregおよび非Treg)を、

Claims (15)
- 炭水化物で修飾された粒子であって、抗原が該粒子によってカプセル化されており、0.01〜500μmの有効平均直径を有する生分解性ポリマー基材と、前記粒子の表面に共有結合的に付着された免疫調節物質である炭水化物部分とを含み、抗原に対する寛容および/または抗炎症応答を誘導する、前記粒子。

- ポリマー基材が、ポリ乳酸(PLA)とポリグリコール酸(PGA)とのコポリマー(すなわち、PLGA)を含む、請求項1に記載の粒子。
- 前記炭水化物部分が、リンカーを介して前記粒子の表面に共有結合的に付着しており、任意で、前記リンカーが、(1)前記炭水化物部分の遊離ヒドロキシル基と反応する求電子種;および(2)ポリマー基材の遊離カルボキシル基と反応する求核種を含む、請求項1に記載の粒子。
- 前記炭水化物部分が、カルボジイミド架橋を介して前記粒子の表面に共有結合的に付着している、請求項4に記載の粒子。
- 前記炭水化物部分以外に付加的な免疫調節物質をさらに含む、請求項1に記載の粒子。
- 免疫調節物質が脱感作もしくは寛容を誘導し、かつ/または免疫調節物質が抗炎症応答を誘導する、請求項1に記載の粒子。
- 付加的な免疫調節物質が、自己免疫疾患または障害と関連した抗原であり、任意で、前記抗原が、インスリンに由来する抗原である、請求項7に記載の粒子。
- 適切な担体、賦形剤、または希釈剤と一緒に請求項1〜8のいずれか一項に記載の粒子を含む、薬学的組成物。
- その必要がある対象における疾患または障害の処置における使用のための請求項1〜7のいずれか一項に記載の粒子を含む薬学的組成物であって、任意で、前記対象が、免疫疾患もしくは障害を有するか、または免疫疾患もしくは障害を発症する危険性がある、前記薬学的組成物。
- 免疫疾患または障害がアレルギー反応であり、かつ前記薬学的組成物が前記対象において寛容を誘導する、請求項10に記載の薬学的組成物。
- 免疫疾患または障害が自己免疫疾患または障害である、請求項10に記載の薬学的組成物。
- 免疫疾患または障害が1型糖尿病である、請求項12に記載の薬学的組成物。
- 以下の段階のうちの1つまたは複数を含む、請求項1に記載の粒子を調製するための方法:
(a)炭水化物部分のライブラリーと免疫細胞とを接触させて、免疫細胞を刺激することに対する前記ライブラリーの効果を測定することによって、免疫調節物質活性について前記ライブラリーをスクリーニングする段階;
(b)免疫細胞を刺激することに対するその効果に基づいて炭水化物部分を選択する段階;ならびに
(c)ポリマー基材から形成された粒子に前記炭水化物部分を付着させる段階であって、任意で、前記炭水化物部分を付着させることが、ポリマー基材から形成された粒子に共有結合的に付着させることである、段階。 - 免疫細胞を刺激することに対する前記ライブラリーの効果を測定することが、サイトカイン産生を測定することを含み、任意で、前記サイトカイン産生を測定することが、ベースラインを上回るIL-10産生を測定することと、ベースラインを上回るIL-6産生を測定することとを含み、かつ免疫細胞を刺激することに対するその効果に基づいて炭水化物部分を選択することが、IL-6分泌を変化させないが、またはIL-6分泌を減少させるが、ベースラインを上回ってIL-10分泌を増加させる炭水化物部分を選択することを含む、請求項14に記載の方法。
