Description
静動脈体外式膜型人工肺(VA ECMO)システムであって:
近位端と、遠位端と、前記近位端と前記遠位端との間で延びる側壁と、を有する第一脱血管と、
前記第一脱血管と同軸上に配置されていて、近位端と、遠位端と、前記近位端と前記遠位端との間で延びる側壁と、を有する第二脱血管と、を有する二腔式脱血カニューレ;
ポンプ入口およびポンプ出口を有し、前記ポンプ出口は前記第一脱血管および第二脱血管と流体連通するように構成された血液ポンプ;
人工肺入口および人工肺出口を有し、前記人工肺入口は前記ポンプ出口と流体連通するように構成された人工肺;および
前記人工肺出口と流体連通するように構成された返血カニューレ、を備え、
前記第一脱血管の遠位端は複数の第一脱血口を有し、前記第二脱血管の遠位端は複数の第二脱血口を有するVA ECMOシステム。
前記第一脱血管および前記第二脱血管は、前記第一脱血管が前記第二脱血管の内側に配設されるように同軸上に配置される第1項に記載のVA ECMOシステム。
前記第一脱血管は前記第二脱血管よりも長い第1項に記載のVA ECMOシステム。
前記第一脱血管および前記第二脱血管を前記ポンプ入口に接続するコネクタをさらに有する第1項に記載のVA ECMOシステム。
前記複数の第一脱血口は前記第一脱血管の外周の周囲に環状パターンで前記第一脱血管の前記側壁を通って延びる第1項に記載のVA ECMOシステム。
前記複数の第二脱血口は前記第二脱血管の外周の周囲に環状パターンで前記第二脱血管の前記側壁を通って延びる第1項に記載のVA ECMOシステム。
前記複数の第一脱血口は前記第一脱血管の長手軸に垂直な方向で前記第一脱血管の前記側壁を通って延びる第1項に記載のVA ECMOシステム。
前記複数の第二脱血口は前記第二脱血管の長手軸に垂直な方向で前記第二脱血管の前記側壁を通って延びる第1項に記載のVA ECMOシステム。
前記複数の第一脱血口は前記第一脱血管の長手軸に対して鋭角もしくは鈍角で前記第一脱血管の前記側壁を通って延びる第1項に記載のVA ECMOシステム。
前記複数の第二脱血口は前記第二脱血管の長手軸に対して鋭角もしくは鈍角で前記第二脱血管の前記側壁を通って延びる第1項に記載のVA ECMOシステム。
前記複数の第一脱血口は前記第一脱血管の長手軸に沿って前記複数の第二脱血口から所定の間隔だけ離れている第1項に記載のVA ECMOシステム。
前記所定の間隔は、患者の年齢、患者の体格、および、所望の流量の内の少なくとも一つに基づいて選択される第11項に記載のVA ECMOシステム。
前記血液ポンプは、遠心ポンプ、軸流ポンプ、または、ローラ式ポンプである第1項に記載のVA ECMOシステム。
前記血液ポンプの作動を制御するためのコントローラをさらに含む第1項に記載のVA ECMOシステム。
記二腔式脱血カニューレは、前記第一脱血管の前記遠位端が患者の右心房内にあり、かつ、前記第二脱血管の前記遠位端が前記患者の肺動脈内にある前記患者の血管系を通して操作するのに適合している第1項に記載のVA ECMOシステム。
本開示の一つの態様によると、VA ECMOシステムは、近位端と、遠位端と、近位端と遠位端との間に延びる側壁と、を備える第一脱血管を有する二腔式脱血カニューレを含むことができる。二腔式カニューレは、さらに、近位端と、遠位端と、近位端と遠位端との間に延びる側壁と、を備える、第一脱血管と同軸上に配置された第二脱血管を有することができる。コネクタは、第一脱血管と第二脱血管とを流体連通するように構成することができる。VA ECMOシステムは、さらに、ポンプ入口およびポンプ出口を備える血液ポンプを有することができる。ポンプ出口は、コネクタと流体連通するように構成することができる。VA ECMOシステムは、さらに、人工肺入口および人工肺出口を備える人工肺を有することができる。人工肺入口はポンプ出口と流体連通するように、人工肺出口は返血カニューレと流体連通するように、構成することができる。
Claims (15)
- 静動脈体外式膜型人工肺(VA ECMO)システムであって:
近位端と、遠位端と、前記近位端と前記遠位端との間で延びる側壁と、を有する第一脱血管と、
前記第一脱血管と同軸上に配置されていて、近位端と、遠位端と、前記近位端と前記遠位端との間で延びる側壁と、を有する第二脱血管と、を有する二腔式脱血カニューレ;
ポンプ入口およびポンプ出口を有し、前記ポンプ出口は前記第一脱血管および第二脱血管と流体連通するように構成された血液ポンプ;
人工肺入口および人工肺出口を有し、前記人工肺入口は前記ポンプ出口と流体連通するように構成された人工肺;および
前記人工肺出口と流体連通するように構成された返血カニューレ、を備え、
前記第一脱血管の遠位端は複数の第一脱血口を有し、前記第二脱血管の遠位端は複数の第二脱血口を有するVA ECMOシステム。 - 前記第一脱血管および前記第二脱血管は、前記第一脱血管が前記第二脱血管の内側に配設されるように同軸上に配置される請求項1に記載のVA ECMOシステム。
- 前記第一脱血管は前記第二脱血管よりも長い請求項1に記載のVA ECMOシステム。
- 前記第一脱血管および前記第二脱血管を前記ポンプ入口に接続するコネクタをさらに有する請求項1に記載のVA ECMOシステム。
- 前記複数の第一脱血口は前記第一脱血管の外周の周囲に環状パターンで前記第一脱血管の前記側壁を通って延びる請求項1に記載のVA ECMOシステム。
- 前記複数の第二脱血口は前記第二脱血管の外周の周囲に環状パターンで前記第二脱血管の前記側壁を通って延びる請求項1に記載のVA ECMOシステム。
- 前記複数の第一脱血口は前記第一脱血管の長手軸に垂直な方向で前記第一脱血管の前記側壁を通って延びる請求項1に記載のVA ECMOシステム。
- 前記複数の第二脱血口は前記第二脱血管の長手軸に垂直な方向で前記第二脱血管の前記側壁を通って延びる請求項1に記載のVA ECMOシステム。
- 前記複数の第一脱血口は前記第一脱血管の長手軸に対して鋭角もしくは鈍角で前記第一脱血管の前記側壁を通って延びる請求項1に記載のVA ECMOシステム。
- 前記複数の第二脱血口は前記第二脱血管の長手軸に対して鋭角もしくは鈍角で前記第二脱血管の前記側壁を通って延びる請求項1に記載のVA ECMOシステム。
- 前記複数の第一脱血口は前記第一脱血管の長手軸に沿って前記複数の第二脱血口から所定の間隔だけ離れている請求項1に記載のVA ECMOシステム。
- 前記所定の間隔は、患者の年齢、患者の体格、および、所望の流量の内の少なくとも一つに基づいて選択される請求項11に記載のVA ECMOシステム。
- 前記血液ポンプは、遠心ポンプ、軸流ポンプ、または、ローラ式ポンプである請求項1に記載のVA ECMOシステム。
- 前記血液ポンプの作動を制御するためのコントローラをさらに含む請求項1に記載のVA ECMOシステム。
- 前記二腔式脱血カニューレは、前記第一脱血管の前記遠位端が患者の右心房内にあり、かつ、前記第二脱血管の前記遠位端が前記患者の肺動脈内にある前記患者の血管系を通して操作するのに適合している請求項1に記載のVA ECMOシステム。
