Description
アデノウイルスは、中程度のゲノムサイズ、操作の利便性、高い力価、広範な標的細胞および優れた感染力により、遺伝子導入ベクターとして広く使用されている。ゲノムの両末端は、100〜200bpの逆位末端反復配列(ITR)を含み、これはDNA複製およびDNAパッケージングに必須のシスエレメントである。ゲノムのE1領域(E1AおよびE1B)はウイルスDNA複製に関与するタンパク質をコードしている。
レトロウイルスは、自身の遺伝子を宿主のゲノムに挿入でき、大量の外来性遺伝物質を運搬でき、感染可能な広範囲の細胞を有し得るため、遺伝子導入ベクターとして使用される。
アデノ随伴ウイルス(AAV)は、非分裂細胞に感染でき、様々な種類の細胞に感染する能力を持つため、本発明の遺伝子送達システムに適している。AAVベクターの調製および使用の詳細な説明は、米国特許第5,139,941号および第4,797,368号に詳細に開示されている。
他のウイルスベクターもまた、本発明の遺伝子送達システムとして使用され得る。ワクシニアウイルス(Puhlmann M, et al., Human gene therapy 10: 649-657 (1999); Ridgeway, "Mammalian expression vectors," In: Vectors: A survey of molecular cloning vectors and their uses. リポソームは、水中に分散するリン脂質によって自動的に生成する。外来DNA分子をリポソームに送達するのに成功した例は、NicolauおよびSene, Biochim. Biophys. Acta, 721: 185-190 (1982)ならびにNicolau, et al., Methods Enzymol., 149:157-176(1987)に記述されている。一方、リポフェクタミン(Gibco BRL)は、リポソームを用いた動物細胞の形質転換に最も広く使用されている試薬である。CCN5タンパク質遺伝子および送達される標的ヌクレオチド配列を封入したリポソームは、エンドサイトーシス、細胞表面への吸着、形質細胞の膜との融合などの機構を介して細胞と相互作用し、CCN5タンパク質遺伝子および送達される標的ヌクレオチド配列を細胞中に送達する。
(A)本発明は、心筋線維化または心筋線維化を伴う心疾患を処置する医薬組成物を提供する。
(B)本発明の医薬組成物は、心筋線維化における筋線維芽細胞を選択的に死滅させることによって線維性基質を溶解する効果、および線維性基質を正常な組織へ回復させる効果を有するため、不可逆的な疾患進行とみなされ、現在まで治療薬が開発されていない心筋線維化および心筋線維化を伴う心疾患を処置する医薬組成物として効果的に使用され得る。
大動脈縮窄術(TAC)による圧負荷の動物モデル(心不全モデル)の作成
ジャクソン研究所からの8週齢〜10週齢のC57BL/6雄性マウス(体重25〜30g)を本研究に用いた。マウスを、95mg/kgケタミンおよび5mg/kgキシラジンで構成された溶液を用いて腹腔内注射によって麻酔した。マウスを、酸素呼吸器を用いて0.2の1日の呼吸率および毎分11呼吸の呼吸速度で呼吸させた。大動脈弓を観察するため、近位胸骨周囲の領域を縦方向に2〜3mm切開した。27ゲージ針を無名動脈と左総頸動脈との間に配置し、大動脈弓を横方向に接続した。針をすぐに除去し、切開部位を覆った。
C57BL/10ScSn(Dmdmdx/Utrntm1Jrs)をジャクソン研究所(Bar Harbor, ME)から購入した。9月齢以上の雄性MDX/UTRN(+/−)マウスを実験に用い、退行性筋ジストロフィーの心疾患を確実に誘導した。この雄性マウスは、ヒトX染色体に関連する先天性遺伝障害であるデュシェンヌ型筋ジストロフィーとの臨床的関連性が示されているモデルである。
ラットにおいて圧負荷および虚血再灌流を同時に行った、複合心不全モデルを使用した。体重180〜200グラムのSprague-Dawleyラットを用いた。第二肋間を開胸術で開き、上行大動脈を外径が0.965mmであるPE−50チューブなどの4−0積層部分(4-0 laminated portion)で包み、PE−50チューブを引き抜いた。この手法を用いて、心臓に圧負荷を加えた。2月後、左冠動脈前下行枝を30分間結び、再灌流した。詳細には、血管を左心の末端より4mm下の点まで6−0縫合で完全に結び、30分後に解放した。さらに1月後、上行大動脈を結んだ縫合を完全に解放した。この複合心不全モデルは、大動脈バンディング虚血再灌流デバンディング(AID)と呼ばれ、4月かけて作成される。
実験動物であるモデルマウスを、100μg/gの濃度のケタミンの腹腔内注射を介して麻酔した。二次元画像およびMモードの記録のために、15.0MHzのコンバーター(GE Vivid 7 Vision)を用いて、心室内径収縮率および程度を決定し、左心室乳頭筋の収縮を報告した。ラットを、気管切開術でチューブを挿入した後、イソフルランで麻酔した。心臓を、7つのソノマイクロメーター結晶(Sonometrics, London, Ontario, Canada)を用いて、胸部切開によって左室心外膜における結び目に連結した。Millar SP−671圧力コンバーターを、左心室の前壁を介して挿入した。データを市販のソフトウェアSonoLabを用いて収集し、血行動態測定を、下大動脈および大動脈の一時的な閉鎖の後の前負荷の圧力および後負荷の圧力の範囲で行った。心臓機能のデータを、MATLAB(v 7.0, The MathWorks, Natick, MA)で作成した一連のアルゴリズムを用いて分析した。心エコー実験を実験期間全体を通して同一の実験者で進行させ、実験の正確性を向上させた。550bpmの最小心拍数が実験の正確性を改善するために必要であったが、最小化された心臓機能の徐脈に関連する過小評価を伴う構造的および機能的評価を含むように実験を行った。各マウスについて測定を3回行い、平均値を計算し、数値として表現した。インビボでの血行動態分析を、1.2Fr圧−容積(PV)コンダクタンスカテーテル(Scisense, Ontario, Canada)を用いて行った。マウスを、ウレタン(1mg/g)、エトミデート(10μg/g)およびモルヒネ(1μg/g)の複合溶液を用いて腹腔内注射によって麻酔し、気管支切開を介してチューブを挿入することによって、人工呼吸器を用いて毎分125の呼吸および7μl/gの呼吸率に設定した。PVカテーテルを頭頂の手法(vertex approach)で左心室内部に配置し、PVデータをIOX2ソフトウェア(EMKA technologies)を用いて分析した。心臓負荷を、生理食塩水中のドブタミンを1μg/ml、10μg/ml、100μg/mlおよび1mg/mlの増加させた濃度で注射することによって(具体的には、血行動態のパラメータを正規化するために、右頸静脈に挿入された中心静脈カテーテルを介して10分の時間間隔で)測定した。
自己相補的なAAV(血清型9)を作成するために、ヒトCCN5遺伝子をpds−AAV2−EGFPベクター中にクローン化した。ウイルスパッケージングの阻害を防ぐため、eGFP配列をAAVベクター作成のために調製した。組換えAAVを293T細胞を用いて作成した。細胞培養溶液中のAAV粒子を回収し、硫酸アンモニウムで沈殿させ、その後、イオジキサノール勾配を用いて超遠心分離法を介して精製した。AAV粒子を、多数の希釈を介して濃縮し、イオジキサノールを乳酸リンゲル液で置換した。AAVの濃度を、定量RT−PCRおよびSDS−PAGEを用いて定量化した。AAV−VLPおよびAAV−CCN5を、5×1010〜1×1011個のウイルスゲノムでモデルマウスおよびモデルラットの尾静脈に注射した。
CCN5タンパク質を生成するために、pcDNA3.1−CCN5HAプラスミドを用いた。HEK293細胞を60mmペトリ皿に5×105個播種し、細胞を1日間安定化させた。その後、pcDNA3.1−CCN5HAをリポフェクチンを用いて感染させた。4時間後、リポフェクチンを除去するため、血漿を除去した条件培地(CM)で置換した。次いで、24時間の培養後に得られた、分泌されたCCN5をCM−CCN5と名付け、これを実験に用いた。CCN5タンパク質の発現をウエスタンブロット法で確認し、CCN5で標識された抗HA抗体を用いて確認した。
8週齢の雄性マウス(Damul Science)に、イソフルランを用いた吸入麻酔(respiratory anesthesia)を行い、心臓を取り出した。大動脈カニューレを大動脈に挿入した後、ラットをすぐにランゲルドルフ装置に取り付け、心臓に付着した不要な組織を除去した。一定速度の灌流、生理液(Tyroid緩衝液;組成:125mM/L NaCl、5mM/L KCl、12.5mM/L HEPES、11mM/Lグルコース、10mM/L BDM、5タウリン、2.4 MgCl2)を用いた。生理液を用いて、血液を十分に除去した後、37℃で100%に飽和した酵素溶液(300μg/mL II型コラーゲン分解酵素、80μg/mLヒアルロン酸)を、50分間灌流し、心臓組織を分解した。この過程を通して心臓組織が分解した後、ピペッティングを数回行い、次いで完全に分解していない組織および単細胞を70μmの細胞フィルターを介して分離した。心筋細胞と、心臓を構成する他の細胞(線維芽細胞、内皮細胞、および平滑筋細胞)との最大の違いは細胞のサイズである。この特徴を利用して、濾過した細胞溶液を25gで3分間遠心し、心筋細胞を分離した。上清を取り出し、250gで10分間遠心し、線維芽細胞、内皮細胞、および平滑筋細胞を分離した。したがって、得られた細胞をDMEMを含む培養培地(低グルコース/10%FBS/1%抗生物質)中に混合し、培養皿に播種し、37℃、5%CO2インキュベーター中で、4時間インキュベートした。線維芽細胞は、短時間で培養皿に接着するという安定化の特徴を有し、この特徴は他の細胞とは異なる。したがって、培地を4時間後に置換し、培養皿に接着することによって安定化した細胞である線維芽細胞以外の他の細胞を除去した。翌日に培地を交換した後、線維芽細胞を、2日置きに培地を交換しながら培養した(Skrzypiec-Spring, et al. J. Pharmacol. Toxicol. Methods 55: 113-126 (2007))。
ラットの心臓から単離した常在性線維芽細胞を、10ng/mL TGF−β(Peprotech)で処理し、37℃、5%CO2インキュベーター中で、2日間培養した。筋線維芽細胞への分化を確認するため、マーカータンパク質であるα−SMAを免疫化学を用いて確認した(Kovacic J.C., et al. Circulation, Apr 10; 125(14):1795-1808 (2012))。
線維芽細胞および内皮細胞(ヒト冠動脈内皮細胞、HCAEC)を16mmカバーガラス上に播種し、安定化のために一晩培養し、次いで10ng/mLのTGF−βおよびCCN5で処理し、48時間培養した。4%パラホルムアルデヒド溶液での固定化後、細胞膜の透過処理を0.5%トリトンX−100溶液を用いて行い、次いで5%BSA溶液でブロックした。抗VE−カドヘリン(Cell Signaling Technology)、抗ビメンチン(Santa Cruz)、または抗α−SMA(Sigma)抗体を反応のために使用し、Alexa Fluor488またはAlexa Fluor594(Invitrogen)を二次抗体として用いた。核をDAPIを用いて染色した。免疫化学を行った細胞を、蛍光顕微鏡(Olympus)を用いて分析した(Okayama K., et al. Hypertension 59:958-965 (2012))。
コラーゲンゲル収縮アッセイにおいて、1×106細胞/mLの線維芽細胞および1.2mg/mLコラーゲン溶液(Invitrogen)を混合し、1N NaOH(約20μL)を添加してコラーゲン溶液のpHを中和した。ピペッティングによって単純に混合した後、500mLの各コラーゲンゲル懸濁液を、24ウェルプレートに配置し、37℃、5%CO2インキュベーター中で、30分間重合した。コラーゲンゲルを形成した後、対照群およびTGF−β単独での処理群またはCCN5を伴うTGF−βでの処理群を、それぞれDMEM培地で培養した。48時間後、コラーゲンゲル収縮の程度を、Image Jソフトウェアを用いて観察および分析した(Dobaczewski M., et al. Circ. Res. 107: 418-428 (2010))。
EndMTを、ヒト冠動脈内皮細胞(HCAEC)を用いて誘導した。HCAECをLonzaを介して購入し、EGM−2(Lonza)培養溶液を用いて、5%CO2インキュベーター中で培養した。ヒト由来細胞であるHCAECは、購入時に継代培養数3の細胞であり、継代培養数5〜7の細胞を実験に用いた。HCAECを用いてEndMTを誘導するために、培養中、培養溶液の交換中などの細胞密度を介して、細胞の状態をよく維持することが重要である。培養皿上にHCAECを播種した後、約12時間安定化し、10ng/mL TGF−βで処理し、3日間培養した。EndMTを誘導している間、培養溶液は交換せず、3日後に免疫細胞化学および定量PCRを行い、HCAECの間葉細胞への分化転換が起こったか否かを確認した。Tie2およびVE−カドヘリン(CD31)をHCAECのマーカー遺伝子として用い、ビメンチン、α−SMA、I型Cコラーゲン、III型コラーゲンおよびFSP−1を間葉細胞のマーカー遺伝子として用いた(Medici D., et al. Biochem. J 437:515-520 (2012))および(Zeisberg E.M., et al. Nat Med. 13: 952-961 (2007))。
スクラッチ分析を行い、細胞遊走能を測定した。HCAECを12ウェルプレートに配置し、細胞を一晩安定化させた。翌日、培養培地を交換し、200μLのチップを用いて、細胞上にスクラッチを作った。スクラッチされた細胞を生理食塩水で洗浄し、次いで10ng/mL TGF−βで処理されたCM−ConまたはCM−CCN5と共に培養した。48時間後、細胞をDAPIで染色し、蛍光顕微鏡で分析した。細胞の移動距離を、MetaMorphソフトウェア(Widyantoro B, et al. Circulation 121:2407-2418(2010))を用いて解析した。
ラットの線維芽細胞および筋線維芽細胞を、6ウェルプレート中に3×105細胞/ウェルで配置し、リポフェクタミン2000(Invitrogen)を用いて、NF−Bレポータープラスミド(pBIIx)、pRenillaおよび他のプラスミド(空のpcDNA3ベクターおよびpcDNA−hCCN5)を遺伝子導入した。48時間後、細胞を受動溶解緩衝液(passive lysis buffer)で溶解し、14,000rpm、4℃で遠心し、細胞片を除去した。ルシフェラーゼ活性を、二重ルシフェラーゼレポーターアッセイシステム(Promega)を用いて測定した。
培養した心筋細胞、線維芽細胞、筋線維芽細胞をカバーガラス上に広げ、DeadEnd Fluorometric TUNELキット(Promega)を用いて染色した。末端デオキシヌクレオチドトランスフェラーゼ(TdT)を用いて、DNAの分節化がアポトーシスによって生じたことが確認された。TUNEL蛍光を確認するため、Fluoview FV 1000球状集束顕微鏡(spherical focus microscope)を用いた(Oberhaus S.M. Methods Mol Biol.218:85-96(2003))。
マウスの心臓組織を、OCTコンパウンド(Tissue-Tek)を用いて凍結保存し、厚さ6μmの切片を作成した。組織の固定化後、アセトン−メタノールを用いて−20℃で20分間反応させ、透過性膜とすることを可能にした。組織を、PBSを用いて再水和し、5%BSA溶液で1時間ブロックした。その後、反応を、抗α−SMA、抗GFP、および抗ビメンチン抗体を用いて、4℃で12時間行った。一次抗体と反応させた組織を生理食塩水で洗浄し、蛍光性Alexa546またはAlexa488を連結した二次抗体と、室温で1時間反応させた。蛍光免疫化学で調べた心臓組織について、結果をFluoview FV 1000共焦点顕微鏡を用いて確認した。
モデル動物から心筋を得た後、適切な切削温度で包埋溶液(Fisher Healthcare (Pittsburgh, PA)から購入)に即座に浸し、次いで新鮮なまま冷凍し、厚さ8μmの切片を作成した。線維化の程度を決定するため、薄片となった試料を、マッソン−トリクローム(Abcam)染色後、光学顕微鏡を用いて観察した。
別々の線維芽細胞の層を、0.2%ウシ胎児血漿(FBS)を含む生理食塩水で洗浄し、2.5%ホルムアルデヒド溶液で固定した。細胞膜を0.5%トリトンX−100溶液で透過処理した後、反応を、FITCを連結した抗α−SMA抗体(Abcam)を用いて行った。培養した線維芽細胞および筋線維芽細胞を、0.2%FBS溶液で洗浄し、PEを連結した抗アネキシン抗体(eBioscience)を用いて反応させた。染色した細胞を、Guava easyCye HT (Millipore)を用いて分析した(Saada J.I., et al. J. Immunol. 177:5968-79(2006))。
細胞および組織の均一溶液について、タンパク質をSDS−PAGEゲルを用いてサイズによって分離し、PVDF膜(Millipore)に移行した。タンパク質発現の確認を、基本的な実験プロトコル(Kawaki, et al., 2011)に従って行った。調製した膜を、抗CCN2、抗CCN5(Lifespan Biosciences)、抗SERCA2a(21st Century Biochemicals)、抗Smad4、抗p−Smad2、抗Smad2/3(Cell signaling)、抗Smad7(Lifespan Biosciences)、抗LOX(Abcam)、抗α−アクチニン、抗α−SMA(Sigma)、抗ビメンチン、抗Bcl−2(Santa Cruz)、および抗カスパーゼ抗体キット(Cell signaling)と共にインキュベートした。
リアルタイムPCRを、QuantiTect SYRB GreenリアルタイムPCRキット(Qiagen Ltd.)を用いて行い、転写レベルをこのキットで分析した。トリゾール(gibco BRL)を用いて、心臓組織からRNAを分離し、cDNA中に合成した。種々の定量リアルタイムPCR条件は、37サイクル:94℃で10分間、57℃で15分間および72℃で5分間であった。本実験で用いたプライマーの情報を、以下の表1に示す。
データを、平均値±SDの値で表す。群の平均値を、スチューデントのt検定またはボンフェローニの事後検定と共に、one−way ANOVA(Statview, V5.0, SAS)を用いて比較した。P<0.05を統計的に有意とみなした。
A.大動脈縮窄術(TAC)による圧負荷動物モデル
心不全の心臓組織におけるCCN5タンパク質発現の減少
心臓移植手術中に提供された心不全患者の心臓組織から得たタンパク質を、正常な人間の心臓組織(表1)とウエスタンブロット法によって比較した結果、心不全患者のCCN5タンパク質の発現レベルは、約24%減少していた(図1A)。圧負荷によって誘導した心不全のマウスモデルにおけるCCN5の発現量は、約90%減少していた(図1B)。本実験の結果、心不全状態の心臓組織におけるCCN5の発現量の減少はヒトおよびマウスにおいて同様に示され、これはヒトとマウスとの間の臨床相関を示す。15μgのCCN5タンパク質抽出物を電気泳動に用い、抗CCN5および抗GAPDH抗体で確認した。(**P<0.01)
作成したAAV9−CCN5および対照のウイルスベクターAAV9−VLPをマウスの尾静脈に2種類の濃度のAAV−CCN5で注射し、4週間後に、有効な濃度をウエスタンブロット法で決定した。注射したAAV−CCNが心臓に特異的に発現したことを確認することによって、心筋線維化の病理活性と筋線維芽細胞との関係を調べた(n=6、エラーバー=S.D.、*P<0.05、図2)。TACで誘導された圧負荷心疾患モデルにおいて、心筋線維化がTAC手術の8週間後に生じたことを確認した。AAV−CCN5および対照群であるAAV−VLPを、心筋線維化を誘導した実験ラットの尾静脈を介して、ウイルスゲノムの濃度を5×1010として、それぞれ注射した(n=16、図3A)。CCN5遺伝子導入の8週間後よりも後に、心臓組織を試料採取し、組織線維化の程度をマッソン−トリクローム染色を介して定量化し、心筋線維化の治療効果を確認した。さらに、各実験群の心臓を分離し、線維芽細胞をランゲンドルフ等速灌流システムを用いて分離し、α−SMAを発現している細胞の程度をフローサイトメトリーで測定した。
相関分析を行い、心筋線維化の回復が筋線維芽細胞のアポトーシスによって起こるのか否かを決定した。圧負荷モデルマウスの誘導と共に、AAV9−CCN5およびAAV9−VLPを注射し、2週間後、心臓組織切片をTUNEL染色(赤)および線維芽細胞特異的なタンパク質である抗α−SMA抗体(緑)で同時に染色し、線維芽細胞のアポトーシスが生じているか否かを確認した(図4B)。結果として、プラシーボ手術群において、2つの染色法に応答する細胞(アポトーシスを起こした筋線維芽細胞)はほとんど存在せず、AAV9−VLPを注射した対照群において疾患群の約9%であり、AAV9−CCN5群において約72%であった(n=3)。非常に多くの細胞が、AAV−CCN5を注射した疾患モデル群において存在していた(図4Bおよび4C)。したがって、心筋線維化の悪化および筋線維芽細胞の増殖は比例しており、心筋線維化が進行している動物モデルにおいて注射されたAAV9−CCN5の可逆的な治療効果は、筋線維芽細胞のアポトーシスに起因し、CCN5遺伝子注射による線維化の治療効果およびアポトーシスによる筋線維芽細胞の減少が直接的に関連していることが確認された。
CM−ConまたはCM−CCN5を、TGF−βの条件下で線維芽細胞から分化した筋線維芽細胞に添加し、1日または2日間培養し、次いでタンパク質を細胞から分離し、ウエスタンブロット法を行った(図5A)。使用した抗体は、抗Bcl2、抗BAX、抗プロカスパーゼ3、抗切断型カスパーゼ3、抗プロカスパーゼ7、抗切断型カスパーゼ7、抗プロカスパーゼ8、抗切断型カスパーゼ8、抗プロカスパーゼ9、抗切断型カスパーゼ9および抗GAPDH抗体であり、これらはアポトーシスに関連するタンパク質である。CCN5タンパク質で処理した群において、アポトーシスを防ぐタンパク質であるBCL−2の発現量は著しく減少したが、アポトーシスを促進するタンパク質であるBAXならびにカスパーゼ3、7および9の発現量は時間と共に増加した。
細胞から分泌されるタンパク質としてCCN5を過剰発現させるために、HA標識されたAd−CCN5プラスミドをHEK293細胞に注入し、24時間培養し、次いで分泌されたタンパク質を無血清細胞培養溶液から得た(図6A)。培養培地および細胞を分離し、抗HA抗体を用いて、CCN5の発現量および分泌量を決定し、細胞質標的タンパク質である抗GAPDH抗体によって、培養培地に分泌されたCCN5が細胞質に混入していなかったことを確認した。
ウエスタンブロットを行い、ラットの心筋細胞(Myo)、線維芽細胞(FB)およびTGF−βのそれぞれの存在下において、分化した筋線維芽細胞(MyoFB)がよく分離および分化したことを確認し、これは抗α−アクチニン、抗ビメンチン、抗α−SMA、および抗GAPDH抗体を用いて確認された。線維芽細胞の筋線維芽細胞への分化転換後でさえ、線維芽細胞のマーカータンパク質であるビメンチンは消失しなかった。したがって、ビメンチンおよびα−SMAの両方が筋線維芽細胞から発現されたことを確認することで、筋線維芽細胞が分離および分化したことを確認し、これにより本実験の信頼性は向上した(図7)。
プラシーボ手術または大動脈交差狭窄手術(TAC)の8週間後に、AAV9−VLPまたはAAV9−CCN5(マウス毎に5×1010ウイルス遺伝子担体)をマウスモデルに注射し、さらに8週間後に、線維化のシグナル伝達に関連するタンパク質の発現量を、屠殺したモデルの心臓においてウエスタンブロットを用いて確認した(n=3、図8Aおよび8B)。心臓組織から得たタンパク質を50μgの濃度で用い、抗SERCA2a、抗CCN5、抗Smad4、抗p−Smad2、抗Smad7、抗CCN2、抗LOX、および抗GAPDH抗体を用いて確認した。結果として、対照群では、TGF−βのシグナル伝達の活性化に関与するSmad2はリン酸化されていたが、CCN5注射群では、p−Smad2の発現が阻害されていた。さらに、TGF−βシグナル伝達の阻害に関与するSmad7は、CCN5群でのみ発現していた。線維化の進行を阻害する重要な薬物標的であるリジルオキシダーゼ(LOX)は、線維化の過程において分泌されるコラーゲンを架橋および重合する酵素であり、組織の外部環境を治癒するのに重要な役割を担っている。LOXは対照群において増加したが、CCN5注射群において大幅に減少したことが確認された。さらに、心臓の収縮力の増加による血液循環のポンプ機能を担うSERCA2aタンパク質の増加の結果によって、心臓の収縮力による収縮期の心臓機能改善効果が予測され得る。
最近の研究の結果、心筋細胞の過程で増殖する線維芽細胞は、内皮細胞からの分化転換(EndoMT)によって一部が形成され、筋線維芽細胞の形成を促進していることがわかった。CCN5がそのような分化転換を阻害するか否かを、Scl−Cre−ERT;R26RstopYFP二重トランスジェニックマウスモデルを用いて確認した。
以下の実験を行い、CCN5が線維芽細胞から筋線維芽細胞への分化を阻害するか否かを確認した。8週齢のマウスモデルにAAV9−VLPまたはAAV9−CCN5(マウス毎に5×1010ウイルス遺伝子担体)を注射し、同時にプラシーボ手術または大動脈縮窄(TAV)手術を行い、8週間後に、心臓組織を用いて免疫化学を行った(図10A)。心臓組織の断面を抗ビメンチン(赤)および抗α−SMA(緑)抗体で染色した。ビメンチンおよびα−SMAを同時に発現する細胞は、線維芽細胞への転換を起こした細胞であり得る(図10B)。ビメンチンを発現する細胞のうち、同時にα−SMAを発現する細胞を分析し、グラフに示した(n=3、エラーバー=S.D.、**P<0.01)。結果として、AAV9−VLP注射群では、約17%の細胞がα−SMAおよびビメンチンを同時に発現したが、AAV9−CCN5注射群では、約4%の細胞がα−SMAおよびビメンチンを同時に発現した。この結果は、心臓におけるCCN5の発現量の増加が、線維化を引き起こす線維芽細胞を筋線維芽細胞に分化させ、増殖させる病理学的機構を効果的に抑制することを示す(n=3、エラーバー=S.D.、**P<0.01、図10C)。
DMDモデルにおける心筋線維化に対するAAV−CCN5の治療効果
デュシェンヌ型筋ジストロフィーモデルマウスである老齢MDX/UTRN(+/−)マウスにおけるAAV−CCN5の治療効果を、図11Aにおける計画に従って検査した。線維化の程度を、マッソン−トリクローム染色を用いて、心筋の低温切断法を介して確認した(図11B)。心筋線維化の範囲を測定した結果、線維化の範囲は、AAV−VLP注射群(10.14%+/−5.11)と比較して、AAV−CCN5注射群(3.39%+/−0.58)において、平均で2.6倍減少した(図11B)。AAV−CCN5を注射した心臓における間質性線維化の蓄積の抑制は、構造リモデリングの効果の改善および心筋組織の性質を正規化する治療効果を示した(n=3〜4、p<0.001)。
心エコー法を、AAV−VLPおよびAAV−CCN5の注射の8週間後に行った。心室短縮率を測定した結果、CCN5による心室短縮の治療効果が、正常なシャム手術をしたWT(+/−)群、AAV−VLP群、およびAAV−CCN5群において、それぞれ58.70%±1.05(n=3)、45.75%±1.29(n=7)および53.88±1.21(n=6)として示された(P<0.005、図12A)。さらに、心臓の血行動態機能を測定した結果、AAV−CCN5注射群の平均の収縮末期圧−容積関係(ESPVR)は3.54±2.49(n=7)であり、AAV−CCN5注射群の平均のESPVRは6.10±2.25(n=6)であった。この結果は、心臓の収縮末期の弾力性(terminal systolic elasticity)および収縮力を改善する、過剰発現したCCN5タンパク質の治療効果を示す(p<0.05、図12B)。
DMDモデル動物において、AAV−VLPおよびAAV−CCN5、心筋の収縮に関連するSERCA2a;線維化に関連する遺伝子である、Col1A2、線維芽細胞活性化タンパク質(FAP)、p−SMADおよびα−SMA;ならびにCCN5タンパク質を注射した、正常なマウスおよび老齢MDX/UTRN(+/−)マウスに対して、ウエスタンブロットを行った。AAV−CCN5注射群において、心収縮に重要なSERCA2aタンパク質の発現量は増加し、線維化マーカータンパク質であるCol1A2、線維芽細胞活性化タンパク質(FAP)、p−SMADおよびα−SMAの発現量は減少した(図13)。遺伝子レベルでの発現量をqRT−PCRを用いて決定した結果、AAV−CCN5注射群において、Col1A2およびTGF−β1の発現量が減少し、抗炎症性サイトカインであるIL−10の発現量は増加した(図14)。したがって、AAV−CCN5注射群において、過剰発現したCCN5タンパク質が心筋線維化の処置に有効であることが、タンパク質および遺伝子発現実験を介して見出された。
CCN5遺伝子導入による、心筋線維化に対する治療効果
本発明で用いられる拡張期心不全モデルは、大動脈バンディング虚血再灌流デバンディング(AID)外科方法を介して、ラットに対して作成したモデルである。このモデルは、心不全の研究に通常用いられる圧負荷モデルよりも、心不全患者の状況をよく反映する。すなわち、多くの心不全患者は大抵、高血圧などによる圧負荷、および狭心症、心筋梗塞などによる虚血を同時に経験する。しかしながら、圧負荷を外科的な方法および薬物処置によって取り除き、冠動脈の灌流を達成しても、心臓の症状は改善せず、心不全に進行する(図15A)。本発明者は、心筋線維化が、AIDモデルの心臓から重篤に進行し、それによって心臓の拡張機能が著しく減少したことを発見した。一方、心臓の収縮機能における減少は、統計的有意性を有するほど大きくなかった。したがって、AIDモデルは心不全患者の状況をよりよく表し、特に駆出率が保存された心不全の特徴を有する。AID心不全ラットモデルを対照群と処置群に分けた後、AAV9−CCN5(マウス毎に1×1011ウイルスゲノム)を尾静脈に注射し、2月後に分析した。心臓の切片をマッソン−トリクローム染色し、顕微鏡下で観察した。心筋線維化において分泌されたコラーゲンは、マッソン−トリクローム染色によって青色染色された。
CCN5を、AAV−CCN5を用いて心筋細胞に過剰発現させた場合、AIDモデルにおける心エコー分析で得られた短縮率は、実験群の間で有意差を示さなかったが、正常に近い機能を示した(図16A)。CCN5の効果を、AID心不全モデルにおいて血行動態分析を介して確認した。この分析において、拡張末期圧−容積関係(ESPVR)の値は心臓の収縮機能(収縮力)に比例し、(EDPVR)の値は心臓の拡張機能(伸展性)に反比例する。実験結果は、AIDラットのAAV−CCN5注射群において、ESPVRのわずかな減少およびEDPVRの著しい増加を示した(n=6、P<0.05、図16Bおよび16C)。これは、AIDラットが、正常なマウスと比較して、著しく減少した収縮力を有するのではなく、著しく減少した拡張機能を有することを意味する。この拡張機能の減少は、AAV9−CCN5を注射したラットにおいて観察されなかった。これらの結果は、心臓の拡張機能の減少が、CCN5タンパク質を過剰発現させたAIDモデルにおいて回復し得ることを意味する。
不可逆的な疾患である線維性疾患には、疾患発生後に診断されるという問題がある。現在まで、いかなる治療に成功した治療薬も開発されていない。線維性疾患の特徴として、病態遂行細胞である線維芽細胞は、組織および臓器の種類にかかわらず、疾患の発生および進行に中心的な役割を果たしている。筋線維芽細胞は、通常の創傷を治癒する過程において一時的に誘導され、消失する特徴を有する細胞であるが、線維性疾患状態において、筋線維芽細胞はアポトーシスを防ぐ機構を獲得することにより、持続的に増殖する機能および活性を有する。線維芽細胞のそのような持続的活性は、線維化が進行する疾患に一般的な疾患の経路において示される。病原性筋線維芽細胞は、線維性細胞外マトリックスを過剰生産し、蓄積させる分泌性細胞の機能、炎症性免疫細胞の流入および炎症性物質の自己分泌における炎症性細胞の機能、ならびに周囲の細胞および周囲の分泌物質との異常な接続によって構造組成細胞の機能を破壊する、シグナル伝達の細胞機能を有する。結果的に、筋線維芽細胞の持続的な増殖および活性化は、組織のリモデリングを生じる線維化の原因であるだけでなく、炎症性細胞の機能によって反応性線維化の過程を促進する役割を果たしている。

- デュシェンヌ型筋ジストロフィーを治療するための医薬組成物であって、CCN5タンパク質をコードするヌクレオチド配列を含むアデノ随伴ウイルスを有効成分として含む医薬組成物。
- 筋線維芽細胞特異的なアポトーシスを誘導する、請求項1に記載の医薬組成物。
- CCN5タンパク質が配列番号1に示されるアミノ酸配列を含む、請求項1または2に記載の医薬組成物。
- CCN5タンパク質をコードするヌクレオチド配列が、配列番号2に示されるヌクレオチド配列からなる、請求項1〜3のいずれかに記載の医薬組成物。
- アデノ随伴ウイルスが、アデノ随伴ウイルス血清型1、アデノ随伴ウイルス血清型6、アデノ随伴ウイルス血清型8、およびアデノ随伴ウイルス血清型9からなる群から選択される、請求項1〜4のいずれかに記載の医薬組成物。
- 筋線維芽細胞特異的なアポトーシスを誘導し、または線維芽細胞の筋線維芽細胞への分化を抑制する、請求項1〜5のいずれかに記載の医薬組成物。
