Description
熱プローブの冷却カテーテルが全処置区間(full treatment interval)で損傷なしに必要なレーザレベルに対処可能であることを実証する実験例がインビトロ(in vitro)で行われた。また、カテーテルの近くの温度を検出しこの温度に従ってパワーを調節するためのフィードバックシステムを用いて、カテーテルの破損又は処置された患者への悪影響をもたらす可能性のある炭化を防止し得ることが実証された。
Claims (13)
- 腫瘍の少なくとも一部を覆う所定の処置領域の温熱療法のための装置であって、
光ファイバと冷却カテーテルとを備える加熱プローブであって、前記光ファイバは前記冷却カテーテル内に挿入され、前記加熱プローブは発光領域を有し、前記加熱プローブは前記所定の処置領域内へと間質的に挿入可能であり且つ前記カテーテル内で循環している流体により内部で冷却可能であるとともに、前記流体を循環させるために少なくとも2本のチューブでポンプユニットに接続されている加熱プローブと、
前記所定の処置領域を加熱するために前記光ファイバに接続可能な光源と、
少なくとも1つのセンサ領域を有し、第1の温度をモニタリング又は推定するために前記所定の処置領域の境界から隔てた距離で位置することが可能な少なくとも第1の熱センサ部材と、
測定された前記第1の温度に基づき、前記処置領域と周囲組織の間の境界での第2の温度が所定の温度になるように前記光源のパワー出力を制御し、もって前記所定の処置領域を取得して維持するための制御ユニットと、を備える装置。 - 前記第1の熱センサ部材は前記第1の温度を測定するために前記境界の外側2〜7mmの距離で位置することが可能に構成され、前記制御ユニットは前記第1の温度が44〜48℃の範囲内になるよう前記パワー出力を制御するように構成される請求項1に記載の装置。
- 第2の熱センサ部材が前記加熱プローブ近傍の距離で位置することが可能であり、前記第2の熱センサ部材は、第3の温度を測定し且つ前記制御ユニットを用いて前記第3の温度が200℃未満になるよう前記パワー出力を制御するように構成される請求項1又は2に記載の装置。
- 前記第2の熱センサ部材は前記加熱プローブから5mm未満の距離で位置することが可能な請求項3に記載の装置。
- 前記第2の熱センサ部材が前記加熱プローブ内に位置する請求項3又は4に記載の装置。
- 前記制御ユニットは、前記境界の外側での推定温度が44〜48℃の範囲内になるよう前記パワー出力を制御するように構成される請求項5に記載の装置。
- 前記制御ユニットは、前記第3の温度の所定の最高温度の場合に警告を発するように構成される請求項3又は4に記載の装置。
- 前記制御ユニットは、所定の時間範囲外に、前記第2の温度の所定の目標温度及び/又は前記第1の温度の所定の目標温度及び/又は前記境界の外側での推定温度の所定の目標温度に達した場合に警告を発するように構成される請求項1〜7のいずれかに記載の装置。
- 前記制御ユニットは、前記所定の時間範囲外に前記所定の最高温度及び/又は前記所定の目標温度に達した場合に前記光源を非動作にするように構成される請求項8に記載の装置。
- 前記所定の時間範囲は5〜15分の間である請求項8又は9に記載の装置。
- 危険性のある臓器に隣接して少なくとも1つの保護用熱センサ部材が位置することが可能であり、
前記熱センサ部材と前記熱プローブとを位置決めし保持するためのテンプレートを備え、及び/又は
前記発光領域は放射された光を拡散するように構成され、及び/又は
前記第1及び第2の熱センサ部材のセンサ領域の少なくとも1つは前記熱プローブの前記発光領域の深さで位置することが可能であり、及び/又は
前記第1の温度センサ部材は互いに離間する複数のセンサ領域を有する請求項1〜10のいずれかに記載の装置。 - 請求項1〜11のいずれかに記載の装置の前記制御ユニット等のコンピュータによって処理するためのコンピュータプログラムが具現化されたコンピュータ可読媒体であって、
前記コンピュータプログラムは、
所定の処置領域の境界の外側に位置する第1の熱センサ部材からの第1の温度を取得することと、
前記所定の処置領域内に間質的に挿入されている前記加熱プローブに接続された光源のパワー出力を、前記第1の温度が目標温度に到達するように、ウォームアップ期間内に制御することと、
前記第1の温度を前記目標温度に維持するために、かつ前記所定の処置領域のサイズを制御するために、前記パワー出力を処置期間内に制御することと、のための複数のコードセグメントを備えるコンピュータ可読媒体。 - 前記ウォームアップ期間外に前記目標温度に達した場合に警告及び/又は前記パワー出力のスイッチングを提供するための更なるコードセグメントを備える請求項12に記載のコンピュータ可読媒体。
