本明細書で使用する「炎症性疾患または状態」は、個体における炎症の増加を特徴とする疾患または状態を意味する。本明細書で使用する「炎症性疾患または状態」を典型とする感染性疾患または状態も意味する。炎症性疾患または状態は、「慢性炎症性疾患または状態」であってよい。慢性炎症性疾患または状態は、数週間、数カ月間またはそれ以上の後に消散しない炎症性状態である。慢性炎症性状態は、急性炎症性状態に続くことがある、または一部の疾患または状態については、急性炎症性疾患または状態の非存在下で発生することがある。または、炎症性状態は、急性炎症性エピソードの結果の場合がある。本明細書で使用する「急性炎症性エピソード」は、亢進した先天性免疫反応を意味する。急性炎症の症状には、発赤、発熱、腫脹、疼痛および機能喪失、例えば関節運動の喪失が含まれる。例えば、急性炎症性疾患または状態の急性炎症性エピソードは、下記のように慢性炎症性疾患または状態の典型的な症状とは異なる。頻回に、急性炎症反応中には、肝臓は血流中で検出可能な急性期タンパク質または急性期反応物質を合成する。急性期反応物質には、C反応性タンパク質(CRP);α1−アンチトリプシン;α1−アンチキモトリプシン;α2−マクログロブリン;凝固因子、例えばフィブリノゲン、フィブリン、プロトロンビン、トロンビン、第VIII因子およびプラスミノゲン;補体タンパク質および血清アミロイドタンパク質が含まれる。さらに、急性炎症性エピソード中、局所炎症細胞、例えば好中球およびマクロファージは、多数のサイトカイン、最も顕著には、IL−1、IL−6、IL−11、HMGB1およびTNF−αを血流中に分泌する(「サイトカインカスケード」)。CIRP阻害剤は、炎症性状態のこれらの作用物質およびマーカーの一部または全部を阻害する、減少させる、またはさもなければ軽減するために投与されてよい。
CIRP阻害剤の投与経路は、治療対象の状態に左右される。例えば、静脈内注射は、敗血症性ショックなどの全身性障害の治療のために好ましいことがある、および例えば胃潰瘍などの消化器障害を治療するためには経口投与が好ましいことがある。
出血性ショックモデルにおける動物の組織中および血清中で高濃度のCIRPが検出された。
実験動物:雄性Sprague−Dawley系ラット(体重、275〜325g)は、Charles River Laboratories(Wilmington,MA)から購入し、12時間明暗周期で温度調節した室内に収容し、標準Purinaラット用飼料を摂取させた。出血性ショックを誘導する前に、ラットは一晩絶食させたが、水は随意に摂取させた。全ての実験は国立衛生研究所(Bethesda,Maryland)による実験動物使用ガイドラインを順守して実施し、フェインスタイン医学研究所の動物実験委員会(IACUC)により承認された。
出血した動物における組織中および循環中のCIRPのアップレギュレーションおよび放出:実験的失血(出血)を受けたラットは、様々な組織中でCIRP mRNAの有意に増加した発現を示した。CIRP発現は、疑似手術コントロール群に比して肝臓中では約5倍(図2A)および心臓内では約3倍(図2B)増加した。出血ラットにおいてウェスタンブロット分析によりCIRPタンパク質の高循環中濃度が検出された。出血群は、疑似群(図2C)では見いだされなかった明白な免疫反応性CIRPバンドを示した。CIRPタンパク質の発現はさらに、疑似手術ラット群(同等の負荷を保証するためにβ−アクチンが投与された)と比して、出血した動物の心臓においても増加した(図2D)。出血性ショックのラットモデルは、25〜30mmHgの平均動脈圧(MAP)まで動物に出血され、90分間にわたりMAPを維持し、その後に急速輸液を行うことにより使用した。血清CIRPは240分後に検出可能であり、出血したラットにおけるショックの330分後に有意に上昇することが見いだされた。CIRPタンパク質濃度は、肝臓および心臓内で各々150および240分後に増加し始めた。血清CIRP濃度は、標準物質として精製CIRPの連続希釈液を使用して推定した。データは、平均値±標準誤差、n=4〜6/時点、*P 時点0群に比して<0.05(図9Bおよび2C)。それに対応して、CIRP mRNA濃度は、肝臓および心臓内ではショック240分後に各々4.1および2.8倍まで有意に誘導された。これらの濃度は、リアルタイムRT−PCRによって分析した。データは、平均値±標準誤差、n=6/群、*P 疑似群に比して<0.05(図9Cならびに図2AおよびB)。
出血性ショックに罹患しているヒトにおいてCIRPの上昇した血清中濃度が検出された。
ヒト血液検体:血液サンプルは、出血性ショックのために外科集中治療室(ICU)に入院した患者から入手した。血清を分離し、−80℃でアリコートにして貯蔵した。インフォームドコンセントおよびヒト被験者プロトコルは、米ノースショア・ロングアイランド・ユダヤ人ヘルスシステム(North Shore−Long Island Jewish Health System)の施設内治験審査委員会(IRB)により承認された。
ショックを有する外科ICU患者における血清CIRPの検出:臨床状態におけるCIRPの役割を探索するために、外科ICU患者10例由来の血清CIRP濃度(表1)を検査した。5例の女性および5例の男性がいて、平均年齢は71歳であった。集中治療における急性期患者の重症度評価II(APACHE II)は13〜25に及び、平均は19であった。平均血液サンプル採取時間は、能動的出血中または外傷性損傷後いずれかでの臨床的に証明された収縮期血圧<90mmHgにより規定されたショック開始43時間後であった。血清CIRPは、臨床パラメータの差とは無関係に、健常志願者においてはほとんど観察されなかったが、患者全10例において著明に検出された(図9A)。
CIRPは、低酸素状態に曝露された細胞から分泌された。再酸素化後、CIRPは核から細胞質に、次に細胞質から細胞外マトリックスに転位した。
細胞培養:マウスマクロファージ様RAW264.7細胞はATCC(American Type Culture Collection、Manassas,VA)から入手し、10%(体積/体積)のFBS(56℃で30分間にわたり熱不活性化した)、100U/mLのペニシリン、100μg/mLのストレプトマイシンおよび2mMのグルタミンを含有するダルベッコの改変イーグル培地(DMEM、Life Technologies社、Grand Island,NY)中で増殖させた。細胞を培地中に再懸濁させ、6もしくは48ウエル組織培養皿内に入れて一晩、加湿培養器(37℃、5%のCO2)中でインキュベートした。実験では、細胞単層を様々な指示濃度および様々な指示時間にわたり組換えCIRPを用いて、または用いずに刺激した。無細胞上清をTNF−αについてはELISAによって、またはHMGB1についてはウェスタンブロット分析によってアッセイした。
低酸素状態に曝露させたマクロファージからのCIRPの転位および放出:CIRPはショック後のヒトおよびラットの両方の血清中で検出可能であったので、CIRP放出について調査した。マクロファージは、様々な炎症メディエーターの放出の原因となる主要な細胞集団である。マウスマクロファージ様RAW264.7細胞は、出血性ショック中に発生する方法と同様の方法で20時間にわたり正常酸素状態(NM)または低酸素状態(1%のO2)で培養し、CIRPの細胞部位を試験した。低酸素状態に曝露させた細胞を0、2、4、7または24時間にわたり再酸素化した(H/R0、H/R2、H/R4、H/R7またはH/R24)。細胞抽出物を核(N)および細胞質(C)成分に分画化し、次にウェスタンブロッティング法にかけた。各分画の完全性は、抗GAPDHおよび抗ヒストン抗体を使用して検証した。CIRPは正常酸素状態中には主として核内に位置したが、他方細胞質CIRPは生化学分画化によって決定した20時間の低酸素状態からの再酸素化7時間後に検出され、24時間後には顕著に増加した(図9D)。遺伝的アプローチは、さらにCIRPの転位を確証するためにも使用した。RAW264.7細胞を緑色蛍光タンパク質(GFP)発現プラスミドでトランスフェクトしたコントロール実験では、緑色蛍光は正常酸素性および低酸素性状態の両方において細胞全体にわたって観察された(図15)。対照的に、RAW264.7細胞をGFP−CIRP発現プラスミドを用いてトランスフェクトした場合は、正常酸素状態下でのヘキスト核染色からの青色蛍光によってオーバーラップされながら緑色蛍光は細胞の中心でしか観察されなかった(図9E)。しかし、低酸素状態からの再酸素化4時間後には、緑色蛍光は核および細胞質両方に分布していた(図9E)。これらをまとめると、低酸素/再酸素化は、マクロファージ内での核から細胞質へのCIRP転位を誘導した。
炎症性サイトカインおよび臓器障害マーカーの放出によって測定されるように、rCIRPの投与後にはマウスおよびヒト細胞系ならびに健常動物において炎症反応の増加が観察された。
組換えタンパク質(rCIRP):細菌発現系由来のヘキサヒスチジンタグ(His−tag)を用いた組換えタンパク質の発現および精製のための連続法を使用した。cDNAは、以前に記載されたように50UのMuLV逆転写酵素を備える改変オリゴd(T16)プライマーを使用してラット心臓の4μgの全組織RNAを逆転写させることによって調製した(Dwivedi AJ,Wu R,Nguyen E,Higuchi S,Wang H,Krishnasastry K,Marini CP,Ravikumar TS,Wang P:Adrenomedullin and adrenomedullin binding protein−1 prevent acute lung injury after gut ischemia−reperfusion.J Am Coll Surg 205:284−293,2007)。CIRPタンパク質を入手するために、CIRPコーディング配列は、PCRによってCIRP cDNAからプライマーセット:センス5’−CAC CAT GGC ATC AGA TGA AGG−3’(配列番号2)およびアンチセンス5’−CTC GTT GTG TGT AGC ATA GC−3’(配列番号3)を用いて増幅させ、合成し(GenBankによる設計:NM_031147、NCBI)、ラットCIRPクローンを単離するために使用した。次にPCR産物をEcoRVおよびNotIで消化し、C末端ヘキサヒスチジンタグ(His−タグ)(Invitrogen社によって記載されている)でpENTRベクター内にクローン化し、次に結果として生じる発現プラスミドとしての大腸菌(E.coli)BL21(DE3)へ形質転換させた。CIRPの誘導発現を数リットルのBL21(DE3)細胞培養中で実施し、次にCIRPを単離し、製造業者によって記載されたように精製した(Novagen社、Madison,WI)。以前に報告されたように(Ertel W,Morrison MH,Wang P,Ba ZF,Ayala A,Chaudry IH:The complex pattern of cytokines in sepsis.Association between prostaglandins,cachectin,and interleukins.Ann Surg 214:141−148,1991)、故意ではないリポ多糖(LPS)汚染を回避するために、Triton X−114抽出を使用して可能性のある内毒素汚染を除去し、最終LPS含量はアメリカカブトガニ溶解物(Limulus amebocyte lysate:LAL)アッセイ(BioWhittaker社、Walkersville,MD)を使用して決定した。
マウス細胞系における組換えCIRP(rCIRP)誘導性炎症反応:細胞外CIRPが炎症メディエーターとして機能できるかどうかを解決するために、組換えマウスCIRP(rmCIRP)は97%を超える純度を備える細菌発現系を用いて発現させて精製し、ウェスタンブロッティング法により確証した(図16AおよびB)。アメリカカブトガニ溶解物アッセイにより測定して約10pgのLPS/μg(CIRP)の残留物を含む精製rmCIRPからリポ多糖(LPS)を取り出すためにTriton X−114抽出法を適用したが(例えば、Aida,Y.& Pabst,M.J.Removal of endotoxin from protein solutions by phase separation using Triton X−114.J.Immunol.Methods 132,191−195(1990)を参照されたい)、これは他の同定された内因性DAMPsにおいて記載されたものに匹敵している(例えば、Wang,Y.,et al.Identification of stimulating and inhibitory epitopes within the heat shock protein 70 molecule that modulate cytokine production and maturation of dendritic cells.J.Immunol.174,3306−3316(2005);Henderson,B.,et al.The extracellular signaling actions of molecular chaperones are not due to microbial contaminants.Cell Stress Chaperones 15,123−141(2010)を参照されたい)。RAW264.7細胞へのrmCIRPの添加は用量および時間依存法でTNF−α放出を増加させた(例えば、図10Aおよび10Bを参照されたい)。RAW264.7細胞は指示濃度のrmCIRPと4時間(図10A)、または100ng/mLのrmCIRPと指示時間(図10B)にわたりインキュベートしたが、このとき*PはrmCIRPなしまたは時点0群に比して<0.05であった。
健常ラットにrmCIRP(1mg/kg体重)または生理食塩水(ビヒクル)を静脈内投与した。コントロールとしてのrCIRP(1mg/kg体重)またはバッファー溶液(同一容積)の注射後、TNF−αの血清中濃度はrCIRP群ではバッファー(疑似)群より約5倍高く、顕著に増加した(図4A)。TNF−α遺伝子およびタンパク質発現は、rCIRP投与後に肝臓(図4CおよびD)ならびに腸(図4EおよびF)中で増加した。rmCIRPはさらに、また別の炎症促進性サイトカインHMGB1の放出を用量依存性で誘導した(図10C)。RAW264.7細胞を指示濃度のrmCIRPとともに20時間にわたりインキュベートした。馴化培地(CM)中のHMGB1濃度は、ウェスタンブロッティング法により決定した。バンド強度は、デンシトメトリーにより定量した。図4Bは、rCIRP(1mg/kg体重)の投与後の炎症促進性サイトカインであるHMGB1の循環中濃度の増加を示している。rCIRP処置ラットは、疑似群における弱いバンド(2回ずつ)に比して、強度の免疫反応性HMGB1バンド(3回ずつ)を示した。インビトロ作用に加えて、健常ラットへのrmCIRP(1mg/kg体重)の静脈内投与は、生理食塩水(ビヒクル)が投与された場合に比して血清中TNF−α、IL−6およびHMGB1濃度ならびに臓器障害マーカーであるASTおよびALT(図17A〜C)を有意に増加させた。4時間後、血液および組織を分析のために収集した。図17Aは、ELISAによって測定した血清中TNF−α濃度およびウェスタンブロッティング法により測定したHMGB1濃度を示している。図17Bは、リアルタイムRT−PCRおよびELISA各々によって測定された肝臓中のTNF−α mRNAおよびタンパク質濃度を示している。血清中ASTおよびALTをrmCIRP投与の4時間後に測定し、濃度を図17Cに示した。データは、平均値±標準誤差、n=6〜9/群、*P ビヒクル群に比して<0.05。
材料および方法
実験動物:C57BL/6バックグラウンドを備えるCirp−/−マウスは日本の熊本大学により提供された(Sakurai,T.,et al.Cirp protects against tumor necrosis factor−alpha−induced apoptosis via activation of extracellular signal−regulated kinase.Biochim.Biophys.Acta.1763,290−295(2006))。Rage−/−、Tlr2−/−およびTlr4−/−マウスは、Dr.Helena Erlandsson−Harrisから入手し、Feinstein Institute for Medical Researchで飼養された(Yang,H.,et al.A critical cysteine is required for HMGB1 binding to Toll−like receptor 4 and activation of macrophage cytokine release.Proc.Natl.Acad.Sci.USA 107,11942−11947(2010))。C57BL/6野生型マウスは、Jackson Laboratory(Bar Harbor,Maine)から購入した。実験では雄性の年齢を適応させた(10〜12週齢)のマウスを使用した。動物は疑似群、ビヒクルコントロール群または処置群に無作為割り付けした。各群に使用した動物の数は、出血および敗血症の動物モデルに関する以前の刊行物に基づいた。全動物試験は、完全遮蔽法では実施しなかった。動物は、外科手術中に死亡した場合は、分析から除外した。
健常ラットにおける組換えCIRP(rCIRP)誘導性組織損傷:正常動物におけるrCIRPの作用を調査するために、細菌発現系から精製した組換えタンパク質であるrCIRP(1mg/kg体重)を正常健常ラットに投与し、ASTおよびALT(肝障害の指標)の血清中濃度を測定した。rCIRPを用いて処置したラットは、AST(図3A)およびALT(図3B)の有意に上昇した濃度を示した。これらの結果は、rCIRPが炎症性組織損傷を直接的に誘発することを示している。
材料および方法
組換えタンパク質:rmHMGB1は以前に記載されたように生成した(Yang,H.,et al.Reversing established sepsis with antagonists of endogenous high−mobility group box 1.Proc.Natl.Acad.Sci.USA 101,296−301(2004))。組換えラットTNF−αは、Biosource社から入手した。C末端DDKタグを備える組換えヒト(rh)CIRP(アクセッション番号NP_001271;全長)は、ヒトHEK293細胞からトランスフェクトして発現させ、Origene(Rockville,Maryland)から入手した。rhTLR2(アクセッション番号NP_003255;Glu21−Leu590)およびrhTLR4(アクセッション番号O00206;Glu24−Lys631)は、マウス骨髄腫NS0細胞系からトランスフェクトして発現させた。rhMD2(アクセッション番号BAA78717;Glu17−Asn160)は、大腸菌(E.coli)から形質転換させて発現させた。rhTLR4/MD2複合体は、NS0細胞である、各タンパク質にHisタグを備える共発現したrhTLR4(アクセッション番号O00206;Glu24−Lys631)およびrhMD2(アクセッション番号Q9Y6Y9;Glu17−Asn160)から精製した。rhTLR2、rhTLR4およびrhMD2全部をそれらのC末端で10−Hisタグと融合させ、R&D Systems社(Minneapolis,Minnesota)から入手した。C末端6−Hisタグを備えるrhRAGE(アクセッション番号Q15109;Ala23−Ala344)は、ヒトHEK293細胞からトランスフェクトして発現させ、Biovision社(Milpitas,California)から入手した。
CIRPおよびHMGB1がTNF−α放出の刺激に及ぼす相加作用:rmCIRPは、マクロファージ中でのTNF−αおよびHMGB1放出を誘導した(図10a、bおよびc)。HMGB1もまたTNF−α放出を刺激できると報告された(Andersson,U.,et al.High mobility group 1 protein(HMG−1)stimulates proinflammatory cytokine synthesis in human monocytes.J.Exp.Med.192,565−570(2000))。TNF−α放出を刺激することにCIRPおよびHMGB1が影響を及ぼす関係は、各中和抗体を適用することにより分析した。RAW264.7細胞内でのmCIRPによるTNF−α産生を用量依存性方法で効果的に阻害する抗CIRPポリクローナル抗体を生成した。図16Cに示したように、RAW264.7細胞は20μg/mLの抗CIRP抗体またはウサギコントロール(非免疫)IgGを用いて20分間プレインキュベートし、その後に指示濃度でrmCIRPを添加した。4時間後に、馴化培地(CM)中のTNF−α濃度をELISAによってアッセイした。データは、3回の独立実験からの平均値±標準誤差である。*P rmCIRP+コントロールIgG群に比してIgG群に比して0.05。図16Dでは、RAW264.7細胞を20分間にわたり1mg/mLの指示希釈濃度でウサギコントロールIgGまたは抗CIRP抗体を用いてプレインキュベートし、その後に1.0μg/mLのrmCIRPを添加した。4時間後に、CM中のTNF−α濃度をELISAによってアッセイした。データは、3回の独立実験からの平均値±標準誤差である。*P rmCIRP+コントロールIgG群に比して<0.05。THP−1細胞は、4μg/mLの抗CIRP抗体、抗HMGB1抗体または非免疫コントロールIgGとともに1時間インキュベートし、その後に8時間にわたりrmCIRP(0.3μg/mL)、rmHMGB1(0.3μg/mL)またはrmCIRP/rmHMGB1(0.3μg/mL、0.3μg/mL)を添加した。*P rmCIRP群に比して<0.05;#P rmHMGB1群に比して<0.05。抗HMGB1抗体とのプレインキュベーション(Yang,H.,et al.Reversing established sepsis with antagonists of endogenous high−mobility group box 1.Proc.Natl.Acad.Sci.USA 101,296−301(2004))は、THP−1細胞中でのrmCIRPにより誘導されたTNF−α放出の31%減少を生じさせたが(P=0.292)、他方抗CIRP抗体とのプレインキュベーションは有意な70%減少を生じさせた(P<0.05、図10G)。これとは逆に、抗CIRP抗体とのプレインキュベーションは、rmHMGB1により誘導されたTNF−α放出の17%減少しか生じさせなかった(P=0.182、図10G)。さらに、rmCIRP、rmHMGB1およびrmCIRP+rmHMGB1は、各々0.8、0.6および1.6ng/mLのTNF−α濃度を誘導した。これらをまとめると、これらの結果は、CIRPおよびHMGB1がマクロファージからのTNF−α放出を刺激することに相加的に機能したことを示している。全収集CM中のTNF−α濃度をELISAによってアッセイした。データは、3または4回の独立実験からの平均値±標準誤差である。
抗CIRP抗体の投与は、出血の生存率を改善して動物モデルにおける炎症反応を減弱させた。
抗CIRP抗体産生:CIRPに対するポリクローナル抗血清は、標準手順にしたがって、ウサギに精製組換えCIRPを3週間以上の間隔をあけて注射することによって生成した(Covance Research Products社、Denver,PA)。抗CIRP抗体のIgGは、供給業者の取扱説明書(Pierce、Rockford,IL)にしたがって固定化免疫精製タンパク質A/Gカラムを使用することにより血清から親和性精製した。抗体力価は、96ウエルフォーマットの直接ELISAによって(Covance Research Products社、Denver,PAに記載されているように)決定した。LPSは、アメリカカブトガニ溶解物アッセイ(BioWhittaker社)によって測定された精製抗体調製物中では検出できなかった。
抗CIRP抗体は出血後の有意な延命効果を提供した:CIRPが例えば出血などの様々な課題に対する炎症反応における新規なメディエーターであることをさらに確証するために、出血性ラットにCIRPに対する特異的抗体(3mg/kg体重)を投与した。結果は、CIRP遮断が急性失血において有意な延命効果を提供することを証明した。図6に示したように、抗CIRP抗体処置は実験的に出血させた動物の生存率を43%から85%へ増加させた(P<0.05)。
材料および方法
複数菌性敗血症の動物モデル:動物にイソフルラン吸入により麻酔をかけた。正中開腹術を通して盲腸結紮および穿刺(CLP)を実施した。手短には、2cmの正中腹壁切開術を実施した。盲腸を露出させ、腸閉塞を回避するために回盲弁のすぐ遠位で結紮し、18ゲージのニードルを用いて2回穿刺し、少量の糞便を穴に通して流動させるためにわずかに圧搾し、次に腹腔へ戻した。腹部は縫合糸を用いて層状に閉鎖した。疑似手術動物は、盲腸を結紮も穿刺もしなかったことを除き、同一手順を受けさせた。動物には手術直後に3mL/100g体重の生理食塩水を皮下に輸液した。
敗血症性動物中のCIRPのアップレギュレーションおよび放出:高炎症によって誘発されるまた別の臨床状態である敗血症におけるCIRPの炎症促進活性を調査した。複数菌性敗血症の確立された動物モデルである、盲腸結紮および穿刺(CLP)を受けたラットにおけるCIRP発現について調査した(Yang,S.,Zhou,M.,Chaudry,I.H.& Wang,P.Novel approach to prevent the transition from the hyperdynamic phase to the hypodynamic phase of sepsis:role of adrenomedullin and adrenomedullin binding protein−1.Ann.Surg.236,625−633(2002)を参照されたい)。20時間後、血液および肝組織を分析のために収集した。血清中CIRP濃度は、ウェスタンブロッティング法により決定した。バンド強度は、デンシトメトリーにより定量した。CLPの20時間後、CIRPの血清中濃度は疑似群に比して3.4倍増加した(図12A)。肝臓中のCIRPのmRNAおよびタンパク質濃度をリアルタイムRT−PCR法およびウェスタンブロッティング法各々によって評価した。データは、平均値±標準誤差、n=4〜6/群、*P 疑似群に比して<0.05である。肝臓中のCIRPのmRNAおよびタンパク質濃度もまた、各々2.4および4.0倍増加した(図12Bおよび12C)。マクロファージ内のCIRPの発現および放出にLPSが及ぼす作用を評価した。ラットから単離した腹膜マクロファージを平板培養し、LPS(10ng/mL)に曝露させた、または処置しなかった(Non)。6時間後、マクロファージ中のCIRP mRNA濃度をリアルタイムRT−PCRにより分析した。CIRP発現濃度をGAPDHへ正規化した。非処置細胞における数値を比較のために1と指定する。データは、3回の独立実験からの平均値±標準誤差である。*P Non群に比して<0.05。ラット一次腹膜マクロファージ中のCIRPのmRNAおよびタンパク質濃度は、LPSへの各々6および24時間にわたる曝露後に有意に増加した(図12D)。全細胞溶解物および培養培地は、LPSへの曝露の各々24時間後および6時間後に採取し、ウェスタンブロッティング法にかけた。データは、3回の独立実験を表す。CIRPタンパク質はLPSへの6時間の曝露後にさらにCMで検出されたが、非処置細胞からは検出できなかった(図12E)。CIRP放出を誘導する他の炎症メディエーターを調査した。RAW264.7細胞のrmHMGB1(1μg/mL)またはrmTNF−α(30ng/mL)との24時間にわたるインキュベーションはCM中へのCIRP放出を誘発しなかったが、他方CIRPタンパク質はLPS(100ng/mL)へ曝露した細胞から検出することができた。馴化培地をウェスタンブロッティング法にかけ、データqは3回の独立実験を表す(図12F)。細胞外CIRPの有害な活性の妥当性を確認するために、中和抗CIRP抗体を敗血症性動物に投与した。CLPラットに抗CIRP抗体(10mg/kg体重、n=18)または非免疫コントロールIgG(10mg/kg体重、n=18)をCLPの5時間後に投与した。死亡率を10日間監視した。*P コントロールIgG群に比して<0.05。PS レッド、ポンソー(Ponceau)Sレッド染色。敗血症性ラットの10日間の生存率は、抗CIRP抗体を用いた治療後に39%から78%へ有意に増加した(図12G)。そこで、CIRPはさらに敗血症性ショックにおける有害因子でもあった。
材料および方法
表面プラズモン共鳴(SPR)分析:タンパク質−タンパク質およびペプチド−タンパク質相互作用の分析は、BIAcore T200機器(GE Healthcare社)を使用して実施した。結合反応は、0.01%のTween−20を含有する1×PBSバッファー(pH7.4)中で実施した。CM5デキストラン・チップ(フローセル−2)は最初に、89μLの0.1MのN−エチル−N’−[3−ジエチルアミノ−プロピル]−カルボジイミドおよび0.1MのN−ヒドロキシスクシンイミドの注射によって活性化した。10mMの酢酸ナトリウム(pH4.5)中に希釈した200μLの5μg/mLのリガンドのアリコートを固定化のためのCM5チップのフローセル−2内に注入した。次に、135μLの1Mのエタノールアミン(pH8.2)を次に残りの活性部位を遮断するために注入した。非特異的結合を評価するために、リガンドを用いたコーティングを施していないフローセル−1をコントロールとして使用した。結合分析は、25℃で30μL/分の流速で実施した。結合を評価するために、分析物(動態分析のためには62.5nM〜1.0μMの範囲内または合格/不合格結合分析のためには0.5μM)をフローセル−1および−2内に注入し、分析物とリガンドの関連を表面プラズモン共鳴法により各々記録した。ブランクチャネル(フローセル−1)からのシグナルをリガンドでコーティングしたチャネル(フローセル−2)から減じた。データは、BIAcore T200評価ソフトウエアによって分析した。全サンプルについて、バッファー単独を用いたブランク注射を結果として生じた反応表面データから減じた。データは、1:1結合についてのLagmuirモデルに全体的に当てはめた。
CIRPはTLR4を通しての炎症反応を誘導した:細胞外CIRPシグナル伝達を伝播することの原因となる細胞表面受容体を同定した。炎症を媒介することが公知の3つの主要PRR:RAGE、TLR2およびTLR4を試験した((a)Zhang,X.& Mosser,D.M.Macrophage activation by endogenous danger signals.J.Pathol.214,161−178(2008);(b)Beutler,B.A.TLRs and innate immunity.Blood 113,1399−1407(2009);(c)Ward,P.A.The sepsis seesaw:seeking a heart salve.Nat.Med.15,497−498(2009))。WT、Rage−/−、Tlr2−/−またはTlr4−/−マウスから単離した腹膜マクロファージを1.5μg/mLのrmCIRPと4時間インキュベートした。馴化培地CM中のTNF−α濃度をELISAによってアッセイした。データは、3回の独立実験からの平均値±標準誤差である。*P WT群に比して<0.05。野生型マウスとrmCIRPに反応した各受容体を標的とするノックアウトマウスとを比較すると、TLR4−欠損性マクロファージだけがTNF−α誘導に対する反応を消失したが、RAGE−およびTLR2−欠損性マクロファージは野生型マクロファージへの類似の反応を維持した(図13A)。CIRP活性を媒介する際にTLR4の要件を確証するために、野生型およびTLR4ノックアウトマウスにrmCIRP(1または5mg/kg体重)または生理食塩水(ビヒクル)を注射した。4時間後、血清サンプルを採取し、ELISAによるTNF−αおよびIL−6およびウェスタンブロッティング法によるHMGB1ならびに血清中ASTおよびALTについてアッセイした。データは、平均値±標準誤差、n=6〜9/群、*P WT rmCIRPなし群に比して<0.05;#P 5mg/kgでrmCIRPを用いたWT群に比して<0.05である。ラットに類似して、野生型マウスは血清中炎症促進性サイトカイン(TNF−α、IL−6およびHMGB1)ならびに臓器障害マーカー(ASTおよびALT)の増加をrmCIRP注射へ用量依存性方法で示した(図13Bおよび13C)。これとは対照的に、rmCIRPが野生型マウスに及ぼすこれらの有害な作用がTlr4−/−マウスでは減少した。
材料および方法
オリゴペプチドの合成:一団の32個の15マーオリゴペプチドを合成し、Genscript社(Piscataway,New Jersey)での高性能液体クロマトグラフィーによって精製した。各15マーオリゴペプチド配列(配列番号13〜44)はhCIRPに由来する(図1)。
C22およびC23が分化THP−1細胞内のrmCIRP誘導性TNF−α産生に及ぼす作用:図20Aに示したように、CIRP−ペプチド単独ではTNF−α産生、培地に類似する検出可能な量に影響を及ぼさなかった。しかし、用量50ng/mLでのC22(配列番号13)またはC23(配列番号14)を用いた前処置は、rmCIRP 300ng/mL誘導性TNF−α産生を有意に減弱させた。阻害濃度は67〜72%の範囲に及ぶ。2つの分子間のサイズの差を考慮に入れるために、分子濃度に変換した場合はrmCIRPより2倍高い用量のペプチドを使用した。
以下の結果は、ペプチドC22およびC23がTNF−αのrCIRP誘導性分泌に及ぼす驚くべき阻害作用を証明している。
オリゴペプチドC1〜C21およびC24〜C32の合成についての実験プロトコル、THP−1細胞を使用する細胞培養、細胞の処置ならびにサイトカインアッセイは実施例10に記載した。細胞は、刺激のためにCIRP(1000ng/mL)を用いて処置した。ペプチドは刺激前の1時間にわたり10×モル濃度のCIRPとともにインキュベートした。TNF−α濃度は、刺激4時間後にELISAにより細胞培地中でアッセイした。

創傷治癒におけるCIRPの改善を同定するために、皮膚創傷の動物モデルを使用して野生型(WT)とCIRP−ヌルマウスとの間の創傷閉鎖率を比較した。詳細な手順および測定は以下のとおりであった。全層2.0cm径の円形切開創を外科的に、3月齢の雄性CIRP−ヌルおよびWTマウス両方の背部上に作製した。創傷のサイズを創傷作製後第14日まで測定し、NIH画像ソフトウエアによって定量した。また別の2組の動物に第3および第7日に麻酔をかけ、全層皮膚サンプルを組織学的評価およびリアルタイムRT−PCRによる様々な遺伝子の発現の測定のために採取した。
オリゴペプチドC23は、TLR4共受容体MD2に対する最高親和性を備えるオリゴペプチドであると同定された(選択された断片のKDについての匹敵するデータを列挙している図14を参照されたい)。
下記の実験は、オリゴペプチドC23の全体的な有益な作用を証明している敗血症性マウスの生存試験の結果を提供している。
