Description
本願は、2012年6月29日に出願された米国仮特許出願第61/666,174号および2013年3月7日に出願された米国非仮特許出願第13/788,696号の出願日の利得を主張するものであり、これらの開示内容は、参照することによって、ここに含まれるものとする。
本発明は、心臓弁置換に関し、特に、折畳み可能な人工心臓弁に関する。さらに詳細には、本発明は、より小さくかつより一貫性のある直径を有する折畳み可能な人工心臓弁に関する。
両交連特徴部116とステント102の近位端119との間に位置する底側環状セル列113および第2の環状セル列115の全てを覆っているとよいが、両交連特徴部間の環状領域の一部の面積しか覆わないようになっているとよい。換言すれば、カフ106の遠位端120は、図1に示されているように、ストラット114a,114b,114e,114f,114g,114hに実質的に沿って配置されているとよい。ストラット114gは、その一端がストラット114hに接続されており、その他端がストラット114b,114cの交点に接続されているとよい。ストラット114hは、その一端がストラット114gに接続され、その他端がストラット114d,114e間の交点に接続されているとよい。ストラット114c,114d,114g、114hは、一緒になって単一のセル112を形成している。
Claims (19)
- 人工心臓弁を組み立てる方法であって、
弁輪区域および大動脈区域を有する折畳み可能および拡張可能なステントを準備するステップであって、前記弁輪区域は、緩和状態における第1の弁輪直径および前記第1の弁輪直径よりも小さい折畳み状態における第2の弁輪直径を有している、ステップと、
前記弁輪区域を前記第1の弁輪直径と前記第2の弁輪直径との間の所定の弁輪直径に拘束するために、拘束体を前記ステントに取り付けるステップと、
人工心臓弁を形成するために、カフまたは複数の弁尖の少なくとも一方を前記拘束された弁輪区域に組み付けるステップと、
前記組み付けるステップの後、前記拘束体を前記ステントから取り外すステップと、
を含んでいる方法。 - 前記大動脈区域は、前記緩和状態における第1の大動脈直径および前記第1の大動脈直径よりも小さい前記折畳み状態における第2の大動脈直径を有しており、前記拘束体を前記ステントに取り付ける前記ステップは、前記大動脈区域を前記第1の大動脈直径と前記第2の大動脈直径との間の所定の大動脈直径にさらに拘束するようになっている、請求項1に記載の方法。
- 前記拘束体は、前記弁輪区域を前記所定の弁輪直径に拘束するために前記ステントの前記弁輪区域に縛り付けられる第1の縫合糸と、前記大動脈区域を前記所定の大動脈直径に拘束するために前記ステントの前記大動脈区域に縛り付けられる第2の縫合糸と、を備えている、請求項2に記載の方法。
- 前記所定の弁輪直径は、前記第1の弁輪直径の約85%から約95%の間にある、請求項1に記載の方法。
- 前記拘束体は、前記弁輪区域を前記所定の弁輪直径に拘束する少なくとも1本の引締め紐を備えている、請求項1に記載の方法。
- 前記少なくとも1本の引締め紐は、前記弁輪区域を前記所定の弁輪直径に拘束するために前記ステントの前記弁輪区域に縛り付けられる第1の縫合糸を含んでいる、請求項5に記載の方法。
- 前記取り外すステップは、前記弁輪区域を解放し、前記所定の弁輪直径と前記第1の弁輪直径との間の未拘束弁輪直径をもたらすために、前記第1の縫合糸を切断することを含んでいる、請求項6に記載の方法。
- 前記拘束体は、前記ステントの周囲に配置される少なくとも1つのリングを備えている、請求項1に記載の方法。
- 前記拘束体は、前記ステントの前記弁輪区域の周りに配置される第1のリングと、前記ステントの前記大動脈区域の周りに配置される第2のリングと、を備えている、請求項2に記載の方法。
- 前記第1のリングおよび第2のリングは、螺旋部によって互いに連結されている、請求項9に記載の方法。
- 前記組み付けるステップを容易にするために、前記ステントに対して前記拘束体を回転させることをさらに含んでいる、請求項10に記載の方法。
- 前記拘束体は、円筒体を備えており、前記円筒体は、近位開口と、遠位開口と、前記近位開口と前記遠位開口との間に画定された内腔と、前記円筒体の少なくとも1つの窓であって、前記窓を通して前記人工心臓弁の組立を可能にするように構成され、かつ配置されている、少なくとも1つの窓と、を備えている、請求項1に記載の方法。
- 前記少なくとも1つの窓は、前記拘束体の周囲に均等に離間した3つの三角窓を含んでいる、請求項12に記載の方法。
- 前記拘束体は、前記遠位開口に円錐部をさらに備えている、請求項12に記載の方法。
- 前記心臓弁の組立を容易にするために互いに離間した切抜きを前記円筒体にさらに備えている、請求項12に記載の方法。
- 前記拘束体は、少なくとも2つのリングおよび前記少なくとも2つのリングに連結された複数の傾斜したストラットをさらに備えており、前記複数の傾斜したストラットは、複数の三角形を賦形している、請求項1に記載の方法。
- 人工心臓弁を組み立てる方法であって、
弁輪区域および大動脈区域を有する折畳み可能および拡張可能なステントを準備するステップであって、前記弁輪区域は、緩和状態における第1の弁輪直径および前記第1の弁輪直径よりも小さい折畳み状態における第2の弁輪直径を有している、ステップと、
前記ステントを、前記弁輪区域の直径が前記第1および第2の弁輪直径間の所定の弁輪直径に減少する所定温度未満の温度に冷却するステップと、
人工心臓弁を形成するために、カフまたは複数の弁尖の少なくとも一方を前記冷却したステントに組み付けるステップと、
を含んでいる方法。 - 前記ステントを冷却する前記ステップは、前記ステントを冷却された空気箱内に配置することを含んでいる、請求項17に記載の方法。
- 前記所定温度は、オーステナイト遷移温度未満である、請求項17に記載の方法。
