Description
本出願は、参照によりその全内容が本明細書に組み込まれている、2013年11月15日に出願された米国特許仮出願第61/904,659号、マシュー・ブリーン(Matthew Breen)、代理人整理番号NS13005USVの利益を主張するものである。
移行上皮癌(TCC)は、尿路上皮癌(UC)とも称されており、イヌにおいて最もよく見られる尿管新生物である。この形式の癌は、腎臓、輸尿管、膀胱、前立腺、及び尿道を含む1つ以上の解剖学的部位に局在される場合があり、膀胱において最も多くの症例が見られる[1]。膀胱においては、癌は、膀胱の裏地を形成する移行上皮細胞から成長し、膀胱壁及び筋肉層に侵入する。塊が大きくなるにつれ、その結果として、腎臓から膀胱への、或いは膀胱から尿道を通る尿の流れが阻害されることが多くなる。イヌのTCCを病理評価すると、ほとんどが、リンパ節及び他の体内器官(肺、肝臓、その他)に広がる可能性のある高グレード腫瘍であることがわかる。
イヌのTCCの診断においては、イヌの尿路上皮癌の症状が他の様々な尿管症状と共通であるという大きな問題がある。例えば、イヌにおいて、膀胱感染症、膀胱結石、膀胱内の肥厚成長、及び膀胱の炎症は全て、膀胱癌に起因する症状と同様の症状を引き起こしうる。イヌの尿に対する通常の細胞学的評価は、誤認につながる可能性がある。これは、上述の悪性でない症状が異常な外観の細胞を尿内に脱落させる可能性があり、これが悪性と間違えられる可能性がある為である。放射線写真検査や超音波検査などの撮像技術を用いると、尿管内で異常な成長の存在が見つかる可能性があるが、これらは、悪性の場合も悪性でない場合もあり、又、異常な外観の細胞を尿内に存在させる場合もある。現在では、イヌのTCCの確定診断は、病理医が腫瘍の生検試料を評価した後にのみ行われてよい。尿管内で可能性の高い塊の生検試料を採取することは、手術中、膀胱鏡検査中、又は外傷性カテーテル挿入によって行われてよく、これらはそれぞれ介入レベルが下がりつつある。しかしながら、可能性の高い腫瘍の塊をかき乱す処置は、いかなるものであれ、悪性上皮細胞を局部上の別の場所に散らばらせることになり、癌を広げることになるおそれがある。この「散らばらせる」可能性は、臨床管理にとっての懸念である。従って、TCCを連想させる症状を示すイヌにおけるTCCの確定診断は、自由採取尿試料の評価から行うのが望ましいであろう。
「泌尿生殖器悪性腫瘍」は、膀胱の組織又は近隣組織において発生する癌である。泌尿生殖器悪性腫瘍は、本明細書では、移行上皮癌(TCC)を含み、これは尿路上皮癌とも称される。本明細書に記載の方法及び試薬は、扁平上皮癌及び腺癌の検出にも用いられてよい。
多くの場合は、当該技術分野においてよく知られている幾つかの核酸増幅手順のいずれかを用いて核酸配列を増幅することが望ましい。具体的には、核酸増幅は、増幅される核酸配列(鋳型)に対して相補的な配列を含む核酸コピーの化学合成又は酵素合成である。本発明の方法及びキットは、当業者には知られている核酸の増幅又は検出の方法のいずれかを用いてよく、それらは、例えば、米国特許第5,525,462号(タカラダ等(Takarada et al.))、同第6,114,117号(ヘップ等(Hepp et al.))、同第6,127,120号(グラハム等(Graham et al.))、同第6,344,317号(ウルノビッツ(Urnovitz))、同第6,448,001号(オク(Oku))、同第6,528,632号(カタンザリティ等(Catanzariti et al.))、及び国際公開第WO2005/111209号(ナカジマ等(Nakajima et al.))に記載されており、これらは全て、参照によりその全内容が本明細書に組み込まれている。

データは、1対全(即ち、病気対健康)の場合、及び全ペアワイズ(即ち、健康対個々の病気)の場合の両方において、そのバイオマーカ識別能力に関してランク付けされてよい。ランク付けに使用される統計量の1つが、受信者動作特性(ROC)曲線(感度対(1−特異性)のプロット)の下の面積である。複数のデータセットにまたがる信頼性に関してはバイオマーカが評価されるが、独立した試料セット同士は、ROCランク付けの為には結合されない。結果として、各バイオマーカの、関心対象の群を識別する能力に関して、複数の独立した分析が行われ、複数の独立したランク付けが得られる。
試料は、尿試料、組織試料、血液試料、並びに血液、血漿、血清、尿からの無細胞抽出液であってよい。各例において示されている細胞遺伝学的アッセイの場合、細胞は、FISHプローブの為のテンプレートを提供する為に使用される。PCRアッセイの場合、腫瘍DNAは、細胞、又は無細胞血漿/血清/尿から取得されてよい。
本発明は、イヌの泌尿生殖器悪性腫瘍を検出する為の組成物及びキットを提供し、これは、(a)CFA13、CFA19、又はCFA36を特定して検出できる核酸プローブからなる群から選択される少なくとも1つの試薬と、(b)イヌからの生物試料においてCFA13、CFA19、又はCFA36のコピー数を測定し、CFA13又はCFA36のコピー数が正常対照より増加しているか、CFA19のコピー数が正常対照より減少している場合に使用される指示と、を含む。
6.1.実験データ
TCCを有する未治療のイヌから原発腫瘍の生検試料(n=31)が採取され、ホルマリンで固定されるか急速凍結されてから液体窒素中に保存された。更に、ノースカロライナ州立大学獣医学部において、臨床疾患の形跡がないイヌの死検時に非新生物性の膀胱組織が採取された。対照試料が獣医病理学者によって評価され、新生物形成の形跡がなく、病理組織学的に「正常」であることが確定された。
腫瘍からDNAが抽出され、アガロースゲル電気泳動法により、分子量が高いことが確認され、分光光度計の読みが260:230>2及び260:280>1.8であった(ナノドロップ(Nanodrop)のNanodrop−1000)。原発腫瘍の生検試料(検査試料)のそれぞれからDNAが分離され、ゲノムDNA酵素標識キット(Genomic DNA Enzymatic Labeling Kit)(アジレント(Agilent))を使用して蛍光体共役dNTPを取り込むことにより、DNAが標識された(これについては以前に[2]に記載されている)。一方の性に特定した基準DNA試料が雑種イヌから生成され、10匹の健康なオスと10匹の健康なメスからの等モル量のDNAがプールされ、これらは同様に、ただし異なる蛍光体共役dNTPで標識された。蛍光標識された検査試料及び基準試料が、オリゴヌクレオチドアレイcCGH (oaCGH)アレイを特徴とするCanine G3 Sureprint 180,000(アジレント(Agilent)、AMADID 025522) と、65℃、20rpmで40時間にわたってハイブリダイズされた(これについては以前に[3]に記載されている)。アレイは、高分解能マイクロアレイスキャナ(アジレント(Agilent)、G2505C)を使用して3μmでスキャンされ、特徴抽出(Feature Extraction)(v10.9)ソフトウェアを使用してデータが抽出された。スキャンデータのデータ品質の評価が、アジレント(Agilent)の特徴抽出ソフトウェア(v10.5)(アジレント・テクノロジーズ(Agilent Technologies))の「品質測定(Quality Metrics)」レポートによって行われた。コピー数呼び出しを行う為に、隠れマルコフモデル(HMM)ベースのアプローチであるFASST2セグメンテーションアルゴリズムが用いられた。FASST2アルゴリズムは、他の一般的な、コピー数推定の為のHMM方法と異なり、各プローブにおけるコピー数状態を推定しようとするのではなく、多数の状態を用いて、モザイクイベントなどのより大きな可能性をカバーする。その後、これらの状態値を使用して、対数比閾値に基づく呼び出しが行われる。セグメンテーションの有意性閾値は5×10上−6に設定されたが、これには又、セグメント当たり最低3個のプローブが必要であり、セグメントをブレークする前の隣接プローブ間の最大プローブ間隔を1Mbにする必要があった。1つのコピーの増加、及び1つのコピーの減少の対数比閾値は、それぞれ+0.201及び−0.234に設定された。
31件の原発イヌTCCの各ケースから採取された生検試料から抽出されたDNAのゲノム全体のoaCGHデータから、イヌゲノム全体にわたるコピー数異常が明らかになり、これらを組み合わせて使用することにより、TCCの発現と符合する異常の存在を検出する為のアッセイを開発することが可能になった(図1)。

TCCの100%は、3つの異常のうちの1つ以上を有する。
TCCの93.55%は、3つの異常のうちの2つ以上を有する。
TCCの67.74%は、3つの異常を全て有する。
非新生物性であることが病理学的に確認された組織(対照)からの数百個のDNA試料を分析すると、CFA13、CFA19、及びCFA36のいずれもコピー数異常を呈していないことが示された。そこで、少なくとも100個の全面的に陰性である対照の存在に基づいて、3つの領域のそれぞれについて、感受性、特異性、正しく分類された比率(%)、及びAUC値(95%の信頼区間)が計算された。
(最大で3つ全ての領域を一緒に使用して、3つ全ての領域の増加情報及び減少情報が含まれた状態で)多変量モデルの潜在的予測能力を評価する為に、J48アルゴリズムを使用して決定木モデルが構築された。クロスバリデーションの有無にかかわらず、最良の木には変数が1つだけ含まれていて、それは、CFA13のコピー数増加であった。これが最良のモデルであり、更なる変数を追加してもモデルは改善されず、弱体化することもなかった。
イヌ泌尿生殖器癌/TCCの場合、イヌ染色体(CFA)13及び36は、コピー数が増加することになり(n>2)、CFA19は、コピー数が減少することになる(n<2)。同じ細胞において、CFA8はコピー数が平衡する(n=2)。この例では、イヌBACクローンは、パネルCのMb位置でCFA8、13、19、及び36を表すように選択された。4つのBACクローンからのDNAは、蛍光in situハイブリダイゼーション(FISH)分析で使用されるように標識されており、この為には、別々のハイブリダイゼーション箇所として検出することを可能にする、4つのスペクトル的に分解可能な蛍光体の取り込みが、CFA8=金色、CFA13=赤色、CFA19=緑色、CFA36=ピンク(グレイスケールとして表示)のように行われた。図2Aは、健康なイヌから採取された細胞からのイヌ中期標本にハイブリダイズされた4つのFISHプローブを示しており、ここでは、イヌ染色体は、DAPIで対比染色された結果として青色である。図2Bは、パネルAと同じ画像であるが、DAPIバンディングをグレースケールで見せるように処理されている。図2A及び図2Bは、各プローブがゲノムの2つの領域とのみハイブリダイズし、各箇所が染色体8、13、19、及び36の同族体を表すことを示している。図2Cは、パネルBの同族体の各対の一方を拡大し、且つ適切な方向に向けて示すことにより、ハイブリダイゼーション信号の染色体位置を示している。各染色体の横に示されているのは、染色体番号と、選択されたプローブが結合する、その染色体上での厳密な物理位置(単位はMb)である。

Claims (12)
- イヌからの生物試料において泌尿生殖器悪性腫瘍を検出する方法であって、
(a)CFA13、CFA19、及びCFA36のコピー数を測定するステップと、
(b)CFA13又はCFA36の前記コピー数が正常対照に比べて増加しているか、CFA19の前記コピー数が正常対照に比べて減少している場合に、前記イヌの前記泌尿生殖器悪性腫瘍の可能性が高まっていると判定するステップと、
を含む方法。 - 前記コピー数は、蛍光in situハイブリダイゼーション(FISH)によって測定される、請求項1に記載の方法。
- 前記コピー数は、ポリメラーゼ連鎖反応(PCR)によって測定される、請求項1に記載の方法。
- 前記コピー数は、比較ゲノムハイブリダイゼーション(CGH)によって測定される、請求項1に記載の方法。
- 前記コピー数は、次世代配列決定によって測定される、請求項1に記載の方法。

- 泌尿生殖器悪性腫瘍の治療の対象となるイヌを選択する方法であって、イヌからの生物試料においてCFA13、CFA19、及びCFA36のコピー数を測定するステップと、CFA13又はCFA36の前記コピー数が正常対照に比べて増加しているか、CFA19の前記コピー数が正常対照に比べて減少している場合に、前記イヌを泌尿生殖器悪性腫瘍の治療の対象として選択するステップと、を含む方法。
- 前記泌尿生殖器悪性腫瘍の治療は、手術、放射線療法、又は化学療法である、請求項10に記載の方法。
- イヌの泌尿生殖器悪性腫瘍を検出する為のキットであって、
(a)CFA13、CFA19、及びCFA36を特定して検出できる核酸プローブと、
(b)イヌからの生物試料においてCFA13、CFA19、及びCFA36のコピー数を測定し、CFA13又はCFA36の前記コピー数が正常対照に比べて増加しているか、CFA19の前記コピー数が正常対照に比べて減少している場合に使用される指示と、
を備えるキット。
