Description
本発明は、プラスミノーゲン活性化因子阻害剤1(PAI−1)およびトロンビン活性化可能な線溶阻害剤(TAFI)の二重阻害により、脳卒中および血栓塞栓症などの血栓性障害の治療および予防に関する。二重阻害は、両方の標的に結合する二重特異性抗体誘導体によって媒介され得る。
現在では、急性心血管病の発症後、閉塞された血管を再疎通するために、利用可能な唯一の治療手段は、高用量のプラスミノーゲン活性化因子の全身送達である。プラスミノーゲン活性化因子は、発症後すぐに投与された場合に有効であるが、頭蓋内出血および神経毒性などの衰弱性副作用を引き起こす。また、高用量のプラスミノーゲン活性化因子を投与しても、再疎通率が低く且つ再閉塞率が高いため、血流の正常回復が保証できない(Saver JL. et al. (2011) J Thromb Haemost. 9 Suppl 1, 333-343)。したがって、当技術分野では、例えば、線維素溶解または血栓溶解を促進することによって、閉塞した血管を有効に治療する方法が必要である。
本明細書は、脳卒中および血栓塞栓症などの血栓性障害を治療するために使用されるダイアボディDb−TCK26D6×33H1F7を開示する。Db−TCK26D6×33H1F7は、血栓障害の発症前または発症後に投与されてもよく、tPAなどのプラスミノーゲン活性化因子の存在または不存在下で投与されてもよい。
本開示のさらなる局面は患者の急性血栓性障害の治療に使用される二重特異性抗体誘導体に関する。この二重特異性抗体誘導体は、トロンビン活性化可能な線溶阻害剤(TAFI)に結合し、配列番号13と少なくとも80%の同一性を有するアミノ酸配列により表されるVH領域と、配列番号14と少なくとも80%の同一性を有するアミノ酸配列により表されるVL領域とを含む第1標的ドメインと、プラスミノーゲン活性化因子阻害剤1(PAI−1)に結合し、配列番号15と少なくとも80%の同一性を有するアミノ酸配列により表されるVH領域と、配列番号16と少なくとも80%の同一性を有するアミノ酸配列により表されるVL領域とを含む第2標的ドメインとを含む。この二重特異性抗体誘導体は、急性血栓性障害の発症後に投与される。いくつかの実施形態において、患者の急性血栓性障害の治療に使用される二重特異性抗体誘導体は、配列番号17と少なくとも80%の同一性を有するアミノ酸配列によって表される第1ドメインと、配列番号18と少なくとも80%の同一性を有するアミノ酸配列によって表される第2ドメインとを含む。この二重特異性抗体誘導体は、急性血栓性障害の発症後に投与される。
特定の実施形態において、これらの二重特異性抗体は、急性血栓性障害に起因する脳病変の治療または予防に使用される。
本発明の方法は、TAFIおよびPAI−1に対するダイアボディを使用するため、tPAに比べて、頭蓋内出血および神経毒性を引き起こすリスクが少ない。本発明のダイアボディは、脳病変を患っている患者の病変サイズを減少するために使用される。脳病変は、脳卒中、急性虚血性脳卒中(AIS)および/または中大脳動脈閉塞(MCAO)などの血栓性障害によって引起され得る。
本発明は、血栓性障害の治療に使用される二重特異性抗体誘導体に関する。二重特異性抗体誘導体は、二重標的化ストラトジーに従って、TAFIおよびPAI−1を標的し、両方のタンパク質を阻害する。いくつかの実施形態において、二重特異性抗体誘導体は、Db−TCK26D6×33H1F7として知られているダイアボディ(diabody)であり、急性血栓性障害を治療するために使用することができる。いくつかの実施形態において、二重特異性抗体誘導体は、急性血栓性障害の発症後に投与される。特定の実施形態において、二重特異性抗体誘導体は、急性血栓性障害または慢性血栓性障害のいずれかである血栓性障害を発症するリスクのある患者に投与される。
「血栓溶解」は、特に凝血塊を形成する線維素の糸および他の構成要素を崩壊することによって、凝血塊の分解を意味する。
二重特異性抗体誘導体は、2つのIgGから由来する場合、二重特異性を達成するために、凝血塊に効率的に進入することができる現在利用可能な最小の構成を表す。二重特異性は、同一の時間、同一の位置および同一の濃度でTAFIおよびPAI−1を阻害するため、類似の薬物動態プロファイルおよび生体分布をもたらす。いくつかの実施形態において、血栓性障害の治療に使用された二重特異性抗体誘導体は、モノクローナル抗体(MA)に基づいている。例えば、TAFIを標的にするMAの例示として、MA−RT36A3F5およびMA−TCK26D6[6,7](上記に引用されたHillmayer; 上記に引用されたVercauteren (2011))が挙げられ、PAI−1を標的とするMAの例示として、MA−33H1F7およびMA−MP2D2(上記に引用されたDe BrockおよびVan De Craen)が挙げられる。二重特異性抗体誘導体の1つの例示として、DbTCK26D6×33H1F7(Wyseure T et al. (2013) J Thromb Haemost. 11, 2069-2071)が挙げられる。特定の実施形態において、二重特異性抗体誘導体の効力は、いずれかのMAを単独で投与する時の効力を超える。
血栓溶解治療のタイミングが重要である。例えば、急性心筋梗塞、急性虚血性脳卒中または急性大規模肺塞栓症などの急性血栓性障害の場合、一般的に、血栓を崩壊させるために、脳卒中発症後15時間以内に、脳卒中症状の発症後できるだけ早く、例えば0〜6時間または4.5時間に、tPAを投与する。同様に、いくつかの実施形態において、本明細書に開示された二重特異性抗体誘導体は、血栓性障害症状の発症後0〜15時間の間に投与される。血栓性障害は、例えば、脳卒中、AISまたはMCAOなどの急性血栓性障害であってもよい。特定の実施形態において、二重特異性抗体は、血栓性障害症状の発症後0.5時間、1時間、1.5時間、3時間、4時間、4.5時間、またはそれ以上の時間に投与され、例えば、症状の発症後4.5時間に投与される。いくつかの実施形態において、二重特異性抗体誘導体は、血栓性障害症状の発症後12時間に投与される。二重特異性抗体誘導体は、静脈投与されてもよく、または凝血塊に直接投与されてもよい。
血栓性障害は、特に高齢者集団および/または過去に血栓性障害を患った患者において多発である。血栓性障害は、急性血栓性障害であってもよく、慢性血栓性障害であってもよい。AISおよび/またはMCAOなどの脳卒中の追加の危険因子は、高齢、アルコールの摂取、アテローム性動脈硬化症、心房細動、経口避妊薬の使用、糖尿病、不良飲食、脳卒中の家族歴、線維筋性異形成、高血圧、高コレステロール、(遺伝性または後天性の)血液凝固亢進、炎症、低出生体重、偏頭痛、肥満症、卵円孔開存、運動不足、閉経後ホルモン治療、脳卒中前歴、特定の人種/民族、鎌状赤血球症、睡眠時無呼吸、一過性脳虚血発作、喫煙などを含む。肺塞栓症を頻繁に誘発する深部静脈血栓症(DVT)などの静脈血栓症の危険因子は、高齢、大手術、整形外科手術、癌、運動抑制、妊娠、抗リン脂質症候群、外傷、足軽傷、静脈血栓症前歴、経口避妊薬の使用、ホルモン補充療法、中心静脈カテーテル、炎症性疾患または自己免疫疾患、ネフローゼ症候群、肥満症、感染症、HIV、真性多血症、および化学療法、ならびに遺伝性危険因子を含むがこれらに限定されない。遺伝性危険因子としては、抗トロンビン欠乏症、プロテインC欠乏症、プロテインS欠乏症、第V因子ライデン、プロトロンビンG20210A、異常線維素原血症、非O血液型を含むがこれらに限定されない。追加の危険因子としては、低レベルのプロテインS、活性化プロテインC抵抗性、高レベルの第VIII因子、高ホモシステイン血症、および/または高レベルの線維素原、第IX因子および/または第XI因子を含むがこれらに限定されない。

A.CDRグラフト化による安定な可変ドメインの設計
scFv−RT36A3F5が過去の研究において産生されたが、細菌によって産生されたものではないため、対応のDb−RT36A3F5×33H1F7が不安定であり、凝血塊を溶解する作用が低いことが分かった。したがって、相補性決定領域(CDR)を安定なscFv−4D5骨格(図1A)にグラフト化することによって、MA−RT36A3F5の可変ドメインを最適化した(Jung S. & Pluckthun A. (1997) Protein Eng. 10, 959-966)。この論文は、CDRグラフト化に使用された4D5ヒト化抗体を開示した。2つの手法で行われた。具体的には、(i)scFv−RT36A3F5およびscFv−4D5の構造整列に基づくストラトジー(structure alignment-based strategy)を用いて、scFv−RT36A3F5−4D5DMを産生した(Marc Demaeyer教授との共同研究)。(ii)証例に基づくストラトジー(evidence-based strategy)を用いて、scFv−RT36A3F5−4D5を産生した(Ewert S. et al. (2004) Methods. 34, 184-199)。後者の手法は、scFv−T12D11(de Develter et al. 2008 J Thromb Haemost. 6, 1884-1889に開示されたscFv−RT36A3F5−T12D11を産出する抗TAFI抗体)の安定な骨格上で行われた。ウエスタンブロット解析は、単独のscFv−RT36A3F5−4D5が正しく発現および分泌される(図1B、レーン1)ことを明らかにした。したがって、これらのCDRグラフト化した可変ドメインは、対応する(sc)Db構造物に使用された。
MA−RT36A3F5およびMA−33H1F7から、4つの二重特異性阻害剤、すなわち、Db−RT36A3F5×33H1F7(Db)、Db−RT36A3F5−4D5×33H1F7(CDRグラフト化Db)、scDb−33H1F7×RT36A3F5(MA−RT36A3F5の可変ドメイン間に追加の可撓性リンカを備えたscDb)、およびscDb−33H1F7×RT36A3F5−4D5(CDRグラフト化scDb)を作り出した(図2を参照)。CDRグラフト化によって、DbおよびscDbの細菌および真核細胞による発現は、それぞれ増加した(CDRグラフト化Dbの場合、Dbに比べて、約1.5mg/Lの培地では2倍に増加した。CDRグラフト化scDbの場合、scDbに比べて、11±1mg/Lの培地では10倍に増加した)。
機能アッセイによって確認されたように、Db、CDRグラフト化Db、およびCDRグラフト化scDbは、親抗体の阻害特性を保留した。scDbの産出が不足のため、その阻害特性を評価することができなかった。
すべての構造物のうち、CDRグラフト化scDbのみは、ダイアボディの対照物Db−T12D11×33H1F7と同様の安定性を示した(37℃で3時間後、88±13%の残基結合活性、図3A)。また、CDRグラフト化scDbは、MA−RT36A3F5に比べて、0.81±0.23という相対的な線維素溶解促進作用を有するため、最も強力な構造物であった(図3B)。PAI−1の基準血漿濃度が低いため、血漿に基づくアッセイにおいて、PAI−1阻害部分の作用寄与を評価することができなかった。
A.Db−TCK26D6×33H1F7の産出
各々の親抗体(MA−TCK26D6とMA−33H1F7)の阻害能力を保留した安定なscFvの成功産出に基づいて、Db−TCK26D6×33H1F7を産出した。図2の左下に示すように、このダイアボディは、2つのポリペプチド鎖を含む。1つ目は、抗TAFI抗体のVH鎖と抗PAI−1抗体のVL鎖との融合タンパク質である。2つ目は、抗TAFI抗体のVL鎖と抗PAI−1抗体のVH鎖との融合タンパク質である。
機能アッセイによって確認されるように、ヒトおよびマウスのTAFIおよびPAI−1に対する親抗体の阻害特性は、Db−TCK26D6×33H1F7に保留された。また、Db−TCK26D6×33H1F7は、ヒト血漿、マウス血漿およびラット血漿に37℃で8時間インキュベーションされた後、安定であった。
TAFIおよびPAI−1の阻害による線維素溶解促進作用を評価するために、4つのDb−TCK26D6×33H1F7をヒト全血にインキュベートし、その作用を血栓粘弾性測定法(thromboelastometry)によって解析した(Wyseure T. et al. (2013) J. Thromb. Haemost. 11, 2069-2071)。両方のMAの複合添加およびダイアボディの添加は、線維素溶解を非常に有意義な程度(P<0.001)に促進した。一方、単一のMAの添加は、わずかな効果しか引き起こさなかった(図4A)。
1.全身血栓誘発後、TAFI/PAI−1の二重阻害による補完効果
すべての循環抗原を標的とする用量のMA−TCK26D6またはMA−33H1F7で前処理したマウスは、トロンボプラスチンの全身投与によって、血栓塞栓症に罹病させた。肺内の線維素の沈着のみが、TAFI阻害剤(図5)の投与によって、基準線レベルに低減された。PAI−1の濃度がマウスにおいて非常に低いため、PAI−1の阻害効果が検出されなかった。
i.モノフィラメント媒介MCAO
一過性閉塞は、モノフィラメントをMCAに入れることによって達成された。このモデルは、脳虚血/再灌流損傷に対するTAFIおよびPAI−1の阻害効果を評価するために使用された。一般的に、このモデルは、測定可能な神経/運動欠陥を有する未処理マウスに大きな病変体積を引き起こす。脳卒中の前臨床評価に最も重要なのは、病変サイズに加えて、神経パラメータを評価することである。興味深いことに、このモデルにおいて、tPAは、局所脳虚血後、神経損傷を悪化させることによって有害な作用を有することが明確に記載されている(Wang YF, et al. (1998) Nat Med. 4, 228-231)。25mg/kgのMA−TCK26D6または6mg/kgのMA−33H1F7による治療は、閉塞後の24時間に、脳病変を減少し(図7Aおよび7D)、神経および運動の同時回復を引き起こした(図7B〜Cおよび7E〜F)。また、処理されたマウスは、対照マウス(図7G)に比べて、脳の同側の線維素(原)の含量がより少なった。対照物としてのIgGは、図7ではMA−T30E5であり、図8ではMA−NB27B3であった。1mg/kgのいずれか1つの親抗体による治療は、閉塞後の24時間に、神経および運動の回復を引き起こさなかった(図8A〜C)。しかしながら、抗体の併用投与は、脳病変を実質的に1.9倍に減少した(図8A)。さらに、同様用量のダイアボディは、病変サイズを同様に減少(2.3倍)した上、神経および運動スコアを同時に改善した(図8A〜C)。病変サイズは、媒介物の場合、76±11mm3であり、対照物IgGの場合、81±11mm3であり、MAと併用する場合、43±8mm3であり、ダイアボディの場合、35±8mm3であった。さらに、ウエスタンブロット解析は、親抗体またはダイアボディの併用が、再灌流損傷によって誘発された大規模な線維素沈着を少なくとも2倍に効果的に減少したことを明らかにした(図8D、p<0.05、n=3〜4匹マウス/群)。
トロンビン注射により得られた血栓塞栓性脳卒中モデルは、使用された。このモデルは、血栓に線維素が豊富であるため、tPAによる血栓溶解を受けやすい。ダイアボディの効力は、現行の血栓溶解剤としてのtPAと比較した。tPAの循環半減期が短いため(約5分)、血栓溶解の臨床手順を模擬するために、最初にtPA容積の10%をボーラスとして投与し、残りの90%を40分間以内に点滴により投与した(Chandler WL et al. (1997) Circulation. 96, 761-768)。閉塞の24時間後、媒介物グループ(血管造影中央値スコア=2、図9B)を含むすべてのグループに、動脈内腔の完全再疎通が見られた。同一時点のスペックル造影は、媒介物グループ(図9C)の閉塞の遠位端に位置するMCA領域において、40%の組織灌流の減少を示した。興味深いことに、単独のダイアボディまたはtPAは、脳灌流を著しく増加しなかったが、ダイアボディおよびtPAの併用は、脳灌流を実質的に回復した(図9C、媒介物に対してp<0.05、n=6〜8匹マウス/群)。tPAの投与によって、病変体積が減少されたが、この減少は、統計的に有意ではなかった(図9A、26±12mm3に対して37±13mm3であり、p=0.203、n=6〜8匹マウス/群)。これとは対照物的に、閉塞後20分でダイアボディの早期投与(0.8mg/kg)は、tPAの同時投与に関係なく、24時間後病変体積を実質的に縮小した(図9A、ダイアボディの場合、15±4mm3であり、ダイアボディ+tPAの場合、15±8mm3である。媒介物に対する各々のp<0.01およびp<0.05、n=6〜8匹マウス/群)。図9Aおよび9Dは、同様の条件を示している。また、処置は、臨床により関連する時点、例えば閉塞後の90分(中間時点)に引き延ばされ(Hacke W. et al. (2004) Lancet 363, 768-774)、閉塞後の24時間で、すべての治療群において完全再疎通が観察された(血管造影中央値スコア=2)。中間時点まで引き延ばしたダイアボディの投与またはtPAの投与は、病変体積に有益な効果を示した(図9E、媒介物の25±3mm3に比べて、tPAは、24±3mm3であり、Dbは、21±4mm3である。n=9〜10匹マウス/群)。しかしながら、同一の処理時点で、tPA点滴の前に、ダイアボディの投与は、病変体積を有意に減少した(図9E、15±2mm3(Db+tPA)、媒介物に対してp<0.05、n=10匹マウス/群)。
血小板豊富な凝血塊は、tPAによる処置に対してより抵抗性がある(Kim EY et al. (2006) Neurology 67, 1846-1848)。したがって、FeCl3により誘発した血小板豊富なMCAOモデルを使用して、臨床に関連する問題を模擬した。予想の通りに、tPAは、(i)閉塞後の1時間にCBFの増加(レーザドップラ追跡、図10A)、(ii)閉塞後の24時間に血管造影スコアの改善(図10C)、(iii)閉塞後の24時間に病変体積の減少、または(iv)閉塞後の24時間に脳灌流の増加(スペックル造影、図10D)には、効果がなかった。1.6mg/kgのダイアボディの投与は、閉塞後の1時間にCBFを著しく増加し(図10A、媒介物に対してp<0.05、n=8〜15匹マウス/群)且つ閉塞後の24時間に血管造影スコアを著しく増加したが(図10C、媒介物に対してp<0.05、n=8〜15匹マウス/群)、脳灌流または病変体積を改善しなかった(図10Bおよび10D)。これとは対照物的に、3.6mgmg/kgのダイアボディの投与は、閉塞後の1時間にCBFを著しく増加(図10A、媒介物に対してp<0.05、n=8〜15匹マウス/群)した。これによって、閉塞後の24時間に、血管造影スコアを著しく増加し(図10C、p<0.05、n=8〜15匹マウス/群)、病変体積を縮小し(図10B、媒介物に対してp<0.05、n=8〜15匹マウス/群)、および脳灌流を増加した(図10D、媒介物に対してp<0.05、n=8〜14匹マウス/群)。
scDbは、真核細胞中で効率的に産生することができるため、インビボで発現された。TAFIおよびPAI−1に対する以下の構造物、具体的にscDb−33H1F7×RT36A3F5、scDb−TCK26D6×33H1F7およびscDb−TCK26D6×MP2D2は、産生され、pcDNA3.1にクローンされた。HEK293T細胞におけるインビトロ発現レベルは、0.6〜1.1μg/mlの範囲(図11A)に変動した。また、scDb−33H1F7×RT36A3F5−4D5にCDRグラフト化することによって、scDb−33H1F7×RT36A3F5をさらに最適化した。これによって、(37℃3時間のインキュベーションの後)、発現レベルを10倍に増加し、血漿安定性を7倍に増加した。マウスにおいて、遺伝子導入後3日目のscDb−33H1F7×RT36A3F5−4D5および遺伝子導入後6日目のscDb−TCK26D6×33H1F7の血漿濃度ピーク値は、各々584±79ng/ml(n=6)および188±19ng/ml(n=4)であり、scDb−TCK26D6×MP2D2の発現は、検出されなかった(図11C)。得られた血漿濃度が薬理評価に低すぎるため、循環半減期を延長するようにこれらのscDbの薬物動態を変更した。この目的のために、親和性を改変したアルブミン結合ドメイン(Jonsson A et al. (2008) Protein Eng Des Sel. 21, 515-527)を37℃で三日のインキュベーション後(図11B)、血漿中で安定な構造物scDb−TCK26D6×33H1F7およびscDb−TCK26D6×MP2D2のC末端に融合した。アルブミン結合構造物は、scDb−TCK26D6×33H1F7×ABDHとscDb−TCK26D6×MP2D2×ABDHとして命名した。残念ながら、これらの構造物のインビトロ発現の2倍〜4倍の減少は、観察された(図11A)。しかしながら、scDb−TCK26D6×33H1F7のアルブミン結合変種のインビボ発現濃度は、9日目に2倍に増加(287±28ng/ml、n=5)し、scDb−TCK26D6×MP2D2のアルブミン結合変種の発現は、検出されなかった(図11C)。
tPAのIV注射による効果を比較するために、2つの異なる用量、すなわち、ヒトの臨床治療に使用される用量(1mg/kg)と同等の用量および一般的にマウスに使用される用量(10mg/kg)と同等の用量で、追加の尾部出血実験を行った。0.8mg/kgおよび3.6mg/kgのダイアボディ(Db−TCK26D6×33H1F7)を注射した。尾部インキュベーションの60分後、最大3.6mg/kgまでのダイアボディのIV投与は、尾部出血時間またはヘモグロビン累積損失を改変しなかったことに対し、両用量のtPAは、尾部出血時間を延長し、ヘモグロビン損失を増加した(図12Aおよび図12B、n=9〜16匹マウス/群)。トロンビン媒介MCAOモデルに試験した治療計画であったダイアボディ(0.8mg/kg)とtPA(10mg/kg)との同時投与は、tPAの単独投与に比べて、尾部出血時間およびヘモグロビン損失をさらに増加しなかった。
ダイアボディ(Db)および一本鎖ダイアボディ(scDb)の産出
抗体の誘導体は、モノクローナル抗体を産出するハイブリドーマ細胞株の可変ドメイン(VH、VL)をクローニングすることによって作製された。(周辺質分泌から)細菌および(細胞外分泌から)真核細胞を産生するために、DbまたはscDbを含むDNA断片をそれぞれ設計した。DNA断片は、合成方法により作製され、大腸菌RV308からDbを産出するために、pSKID2にクローニングし、HEK293T細胞を用いてscDbを産出するために、pcDNA3.1にクローニングした。Ni++カラムを用いて、His6で標記した抗体の誘導体を精製し、インビボ評価の前に、陰イオン交換クロマトグラフィーによって、内毒素を除去した。
PAI−1およびTAFIに対する同時結合に基づいて、ELISAによって(sc)Dbを定量化した。簡単に言えば、マウスPAI−1で被覆した微量定量プレートを用いて、(sc)Dbを結合し、その後、マウスTAFIと共にインキュベーションを行い、続いて、TAFIに対するMA−TCK32G12−HRPを加え、発色基質としてのo−フェニレンジアミンを用いて顕色した後、検出を行った。
TAFIに対する抗体誘導体の阻害能力は、発色アッセイを用いて残存TAFIaの活性を測定することによって定量化され、親モノクローナル抗体(MA)の阻害特性と比較した。TAFIは、MAの動作メカニズムに応じて、トロンビン/トロンボモジュリンまたはプラスミンにより活性化される前または後に、TAFIに対して0.06〜8倍範囲のモル比濃度のMAまたはsc(Db)と共にインキュベートされた。残留TAFIa活性は、基質としてヒプリル−アルギニンを使用し、比色反応によって、決定された。
活性PAI−1に対する抗体誘導体の阻害能力は、プラスミノーゲン結合発色法を用いて定量化され、親MAの阻害特性と比較した。PAI−1は、PAI−1に対して0.06〜32倍範囲のモル比濃度のMAまたはsc(Db)と共に、(室温で2時間)予備インキュベートされた。引き続きtPAと共に(37℃で15分)インキュベーションした後、プラスミノーゲンを加え、プラスミンへの変換量が発色基質によって定量された。
蓄積されたラットのクエン酸血漿は、MAまたはsc(Db)と共に、(37℃で10分)予備インキュベートされた後、CaCl2およびt−PAを加えた。その後、微量定量プレート読み取り器を用いて、濁度(OD405nm)を測定することによって、凝血塊の溶解を経時的に監視した。線維素溶解の度合いは、180分の時間フレームに亘る曲線下方の面積として表記された。取得されたデータは、(sc(Db)の結合部位の数と同様に、それぞれの抗原と等価する数の結合部位に相当する濃度の)MAの存在下で得られた値に正規化した。
4人の健康ドナーからまたは(実験開始の6時間前に、LPS(0.5mg/kg)の腹腔内注射によって健康にしたまたは内毒素を除去した)マウスからのクエン酸化全血は、(それぞれの抗原と等価する数の結合部位を生成する濃度の)MAまたはDbと共に、予備インキュベートされた。凝固およびその後の線維素溶解は、トロンボプラスチン、CaCl2およびtPAによって引き起こした。ヒト血液の場合、線維素の溶解は、初期凝固後45分の振幅の減少と最大振幅との比率して、決定された(L(%)=[(Amax−A45)/Amax]×100)。各実験では、溶解の基準線(LWO)は、MAまたはDbを含まない同様の血液試料を用いて測定した(LWOは、12%を超えることがなかった)。その後、特定の阻害剤で強化した溶解は、ΔL(%)=L阻害剤−LWOとして決定した。マウス血液の場合、特定の阻害剤で強化した線溶は、(凝固時間〜凝固時間+120分の間に)生理食塩水(AUCwo)の曲線下面積(AUC)および処置条件(AUC阻害剤)の曲線下面積(AUC)の差分と、生理食塩水(AUCwo)の曲線下面積(AUC)との比率とし、て決定された(相対ΔAUC=[(AUCwo−AUC阻害剤)/AUCwo]×100)。
血栓塞栓症モデル
MA、ダイアボディまたは生理食塩水(0.9%のNaCl)は、一晩絶食させられた未麻酔SWISSマウス(実験開始の3時間前に、LPS(0.5mg/kg)の腹腔内注射によって健康にしたまたは内毒素を除去した)に静脈(IV)注射した。5分後、トロンボプラスチンをIV注射することによって、血栓塞栓症を誘発した。マウスをペントバルビタール(60mg/kgの腹腔内投与)により麻酔し、血栓誘発後の15分に、10 IU/mlのヘパリンで肺を灌流した。その後、肺を単離し、均質化した。その後、(左肺)から洗い出したホモジネートを2μMのマイクロプラスミンと共にインキュベートすることによって、線維素を可溶化線維素分解物に変換した。これらの線維素分解物は、マウス線維素原と交差反応するELISAを用いて、定量化される。肺内の線維素含有量は、線維素原当量濃度(μg/ml)で表した。
(2%イソフルラン/酸素混合物の吸入によって)麻酔されたSWISSマウスは、脳定位装置上に配置され、頭蓋骨切除術によって右中大脳動脈(MCA)を露出させた。レーザドップラ流速計測法によって確認されたように、現場での閉塞は、MCAにネズミ科α−トロンビンを微量注入することによって(Orset C et al. (2007) Stroke. 38, 2771-2778)、または20%のFeCl3で飽和した濾紙を適用することによって(Karatas H et al. (2011) J Cereb Blood Flow Metab. 231, 1452-1460)、実行された。血栓発症後15分に、DbまたはPBS(媒介物)を静脈注射した。5分後、尾静脈カテーテルを介して、tPA(10mg/kg)または生理食塩水を投与した(10%をボーラスとして投与し、残りの90%を40分の間に点滴した)。最初の閉塞後の24時間に、頭蓋骨をスペックル造影装置に露出させることによって、脳同側の血流および反対側の血流をマッピングした(Moor FLPI-2、Moor機器)。反対側に比べて、脳同側血流の減少は、観察された。脳病変体積は、T2重み付きMRIによって決定され、MCAの血管造影スコアは、MR血管造影によって決定された(0=閉塞、1=部分的な再疎通、2=完全再疎通)。T2*重み付きMRIを使用して、出血の発生を排除した。
(2%イソフルラン/酸素混合物の吸入によって)麻酔されたC57BL/6マウスの頸正中皮膚を切開した後、近位の総頸動脈および外頸動脈を結紮した。右内頸動脈から、標準化シリコンゴムにより被覆した6.0のナイロンモノフィラメントを挿入することによって、右MCAの端部開口を閉塞した。閉塞してから60分後、管腔内モノフィラメントを取り出し、5分間灌流後、MAまたはPBSを静脈注射した。最初の閉塞の24時間後、マウスの神経機能および運動機能をそれぞれ評価するために、改良Bederson検査および握力検査を行った。その後、マウスを安楽死させ、脳を回収して、(2,3,5−トリフェニルテトラゾリウムクロライド染色によって)病変体積を決定した。
(MCAOモデル)閉塞後の24時間に、マウス全体の神経機能および運動機能を評価するために、マウスに改良Bederson検査および握力検査をそれぞれ実行した。改良Bederson検査は、以下のスコアシステム、すなわち、0=不足、1=前肢屈曲、2=横方向プッシュの抵抗減少、3=単方向旋回、4=縦方向回転、5=動き無しを使用している。
(MCAOモデル)閉塞後の24時間に、マウスを安楽死させた。脳を直ちに回収し、マウス脳切片マトリックスを用いて、厚さ2mmの冠状切片にスライスした。脳出血の有無は、目視で評価した。PBSに溶解した2%の2,3,5−トリフェニルテトラゾリウムクロライド(Sigma-Aldrich社、St. Louis, MO)を用いて、切片を染色することによって、健康組織を染色されない梗塞から識別する。Nikon(登録商標)D70デジタルカメラを用いて、染色された切片を撮影した。梗塞領域(白)は、Image J software(国立衛生研究所、Bethesda, MD)を使用して盲目的に測定した。
ホルマリン固定のTTC染色脳切片から、皮質および大脳基底核を含む虚血組織を切離し、前述のように、0.1%のSDSおよび0.25%のプロテアーゼ阻害剤の混合物(Roche)を含有するRIPA緩衝液(25mmol/Lのトリス(pH=7.4)、150mmol/LのNaCl、1%のNP40)を微調整したRIPA緩衝液に均質化した。39個のサンプルは、CLI12ミキサーを用いて均質化し、4℃で20分間インキュベートし、その後、氷上で超音波処理した。次いで、4℃で組織溶解物を15,000×gで20分間遠心分離し、上清液を以下のようにウエスタンブロット解析に供した。30μgの総タンパク質を装入し、SDSポリアクリルアミドゲル上で電気泳動し、ニトロセルロース膜に移した。ブロッキング緩衝液(5%脱脂粉乳、50mmol/Lのトリス−HCl(pH=7.5)、150mmol/LのNaCl、0.05%のTween(登録商標))−20で1時間ブロッキングした後、膜を抗線維素原ポリクローナル抗体(AP00766PU−N、Acris社、1:500倍希釈)または抗アクチンMA(MAB1501、Millipore社、1:500倍希釈)と共にそれぞれ4℃で一晩または1時間インキュベートした。次いで、膜を洗浄して、HRP結合ヤギ抗ウサギIgG(線維素原)(Jackson ImmunoResearch、1:14000倍希釈)または抗マウスIgG(アクチン)(Dako、1:2000倍希釈)と共に室温で60分間インキュベートした。SuperSignal West Pico化学発光基質(Thermo Scientific社)を用いて、ブロットを顕色し、LAS−4000ミニ撮像装置(GE Healthcare社)を用いて、信号を検出した。
前述のように、マウス尾静脈の出血時間は、尾部切断アッセイを用いて決定した。近位尾静脈注射を介して、PBS、ダイアボディ、tPAを単独投与としてまたはtPA投与の5分前にダイアボディを共投与として、マウスに投与した。注射後5分に、尾部先端から3mmの部分を切断し、切断尾部を直ちに37℃で0.9%の等張食塩水に浸漬した。出血時間は、出血の初期停止(すなわち、30秒以内に再出血しない)まで監視された。実験は、治療に対して盲目に行った。尾部切断後の60分間にわたって、累積ヘモグロビン損失を測定した。遠心分離(2000×gで10分間)の後、血液細胞を1mLの等張食塩水に再懸濁し、Cell-Dyn 3500R計数器(Abbott, Diegem, Belgium)を用いて、ヘモグロビン含量を測定した。
尾静脈注射を介して、Db−TCK26D6×33H1F7(0.8mg/kg)をマウス(n=6)に投与した。実験前に、血液は、0.38%のクエン酸三ナトリウム(=プレサンプル)から回収した。注射後、血液は、5分、45分、3時間、6時間および24時間のいくつかの時点で、0.38%のクエン酸三ナトリウムから回収した。対応する血漿サンプル中のダイアボディ濃度は、ELISAに従って、PAI−1およびTAFIと同時結合するダイアボディに基づいて決定された。ポリスチレン微量定量プレートをウェルに入れ、PBSに溶解した200μlの組換えマウスPAI−1(4μg/ml、pH=7.4)と共に、4℃で72時間インキュベートし、ウェルを空にし、1%(m/v)ウシ血清アルブミンを追加したPBSを用いて、2時間処理した。洗浄後、血漿サンプルの連続2倍希釈液(180μl)をウェルに添加し、4℃で一晩インキュベートした。その後、ウェルを洗浄し、170μlのマウスTAFI(0.1μg/ml)と共に室温で2時間インキュベートした。その後、プレートを洗浄し、(TAFIに対する)160μlのHRP結合MA−TCK32G12をプレートに添加し、室温で2時間インキュベートした。すべての洗浄は、Tween80(0.002%)を含有するPBS中で行い、希釈は、Tween80(0.002%)およびウシ血清アルブミン(0.1%m/v)を含有するPBS中で行った。ELISA顕色は、300μg/mlのo−フェニレンジアミンおよび0.01%の過酸化水素を含有する0.1mol/Lのクエン酸:0.2mol/Lのリン酸ナトリウム緩衝液150μL(pH=5.0)を用いて行った。室温で30分後、50μlのH2SO4(4mol/L)を用いて、ペルオキシダーゼ反応を停止した。492nmで吸光度を測定した。キャリブレーターとしてDb−TCK26D6×33H1F7を使用した。
(2%イソフルラン/酸素混合物の吸入によって)麻酔されたSWISSマウスは、事前に採血した。プラスミドDNA(scDbを含むpcDNA3.1)注射の3時間前に、両方の大腿四頭筋にヒアルロニダーゼを注射し、その後、電気穿孔した。DNA注射後15日までの発現レベル(cfr.3.2)を決定するために、後眼窩穿刺によってマウスを失血させることによって、クエン酸血漿を準備した。
神経細胞培地は、スイスマウス胚(胎生期14日)から用意された。皮質を切開し、DMEMに分散し、ポリ−D−リジン(0.1mg/ml)およびラミニン(0.02mg/ml)でコーティングした24−ウェルプレートにプレーティングした。5%ウシ胎児血清、5%ウマ血清(両方ともInvitrogen社、Cergy Pontoise, France)および2mMのグルタミンを補充したDMEMに、細胞を培養した。培養物を、加湿した5%CO2雰囲気に37℃で保持した。グリア増殖を抑制するために、インビトロ培養(DIV)3日後に、シトシンβ−D−アラビノシド(10μM)を皮質培養物に添加した。
すべての定量データは、平均値および平均値標準誤差(SEM)として提供された。時間に対してプロットした血漿濃度を非線形フィッティングすること(グラフパッドプリズムバーション5、グラフパッドソフトウェア、San Diego, CA, USA)によって、ダイアボディの循環半減期を取得した。統計分析は、グラフパッドプリズムバーション5(グラフパッドソフトウェア)を用いて行った。(Export toolから得られた)血栓粘弾性測定法からの曲線は、グラフパッドプリズム5に統合された。異なる処置グループからの血管造影スコアを比較するために、カイ二乗検定を実施した。グラブス検定を行うことによって、外れ値を除外した。統計分析の前に、D'Agostino and Pearson正規性検定を用いて、データ分布を確認した。Bonferroni多重比較検定と共に一元配置分散分析(One-way ANOVA)を用いて、FeCl3誘発MCAO後の病変容積およびスペックル造影データを統計的に比較した。独立スチューデントt検定を用いて、機械的tMCAO後の病変体積を統計的に比較した。Dunn多重比較検定と共に、Kruskal−Wallis分散分析(ANOVA)を用いて、(i)溶解、ΔLおよび相対ΔAUCの血栓粘弾性測定パラメータ、(ii)静脈血栓塞栓症モデルにおける肺線維素原等価物、(iii)IIa誘発性MCAOモデルにおける病変容積およびスペック造影データ、(iv)FeCl3誘発性MCAOモデルにおけるレーザドップラデータ、および(v)尾部出血時間およびヘモグロビン含量を統計的に比較した。神経/運動データ、機械tMCAO後の線維素原濃度、およびインビトロ神経毒性データを統計分析するために、Mann-Whitney検定を実施した。0.05未満のP値が有意であると見なした。
下線付き部分は、CDR配列を示す。配列番号17および18のN末端に位置するMetおよびAla残基は、pelBシグナルペプチドからのものである。太字は、合成リンカーおよび標記物を表す。
Claims (11)
- 患者の急性血栓性障害を治療または予防するための医薬品であって、二重特異性抗体を含み、
前記抗体は、
a)トロンビン活性化可能な線溶阻害剤(TAFI)に特異的に結合し、アミノ酸配列の配列番号1を有するCDR1H、配列番号2を有するCDR2H、配列番号3を有するCDR3H、配列番号4を有するCDR1L、配列番号5を有するCDR2L、および配列番号6を有するCDR3Lにより表される相補性決定領域(CDR)を含む第1標的ドメインと、
b)プラスミノーゲン活性化因子阻害剤1(PAI−1)に特異的に結合し、アミノ酸配列の配列番号7を有するCDR1H、配列番号8を有するCDR2H、配列番号9を有するCDR3H、配列番号10を有するCDR1L、配列番号11を有するCDR2L、および配列番号12を有するCDR3Lにより表される相補性決定領域(CDR)を含む第2標的ドメインとを含む、患者の急性血栓性障害を治療または予防するための医薬品。 - a)トロンビン活性化可能な線溶阻害剤(TAFI)に結合する前記第1標的ドメインは、配列番号13と少なくとも80%の同一性を有するアミノ酸配列により表されるVH領域と、配列番号14と少なくとも80%の同一性を有するアミノ酸配列により表されるVL領域とを含み、および
アミノ酸配列の配列番号1を有するCDR1H、配列番号2を有するCDR2H、配列番号3を有するCDR3H、配列番号4を有するCDR1L、配列番号5を有するCDR2L、および配列番号6を有するCDR3Lにより表される相補性決定領域(CDR)を含み、
b)プラスミノーゲン活性化因子阻害剤1(PAI−1)に結合する前記第2標的ドメインは、配列番号15と少なくとも80%の同一性を有するアミノ酸配列により表されるVH領域と、配列番号16と少なくとも80%の同一性を有するアミノ酸配列により表されるVL領域とを含み、および
アミノ酸配列の配列番号7を有するCDR1H、配列番号8を有するCDR2H、配列番号9を有するCDR3H、配列番号10を有するCDR1L、配列番号11を有するCDR2L、および配列番号12を有するCDR3Lにより表される相補性決定領域(CDR)を含む第2標的ドメインとを含む、請求項1に記載の患者の急性血栓性障害を治療または予防するための医薬品。 - a)トロンビン活性化可能な線溶阻害剤(TAFI)に結合する前記第1標的ドメインは、配列番号13と少なくとも90%の同一性を有するアミノ酸配列により表されるVH領域と、配列番号14と少なくとも90%の同一性を有するアミノ酸配列により表されるVL領域とを含み、
b)プラスミノーゲン活性化因子阻害剤1(PAI−1)に結合する前記第2標的ドメインは、配列番号15と少なくとも90%の同一性を有するアミノ酸配列により表されるVH領域と、配列番号16と少なくとも90%の同一性を有するアミノ酸配列により表されるVL領域とを含む、請求項1または2に記載の患者の急性血栓性障害を治療または予防するための医薬品。 - a)トロンビン活性化可能な線溶阻害剤(TAFI)に結合する前記第1標的ドメインは、配列番号13と少なくとも95%の同一性を有するアミノ酸配列により表されるVH領域と、配列番号14と少なくとも95%の同一性を有するアミノ酸配列により表されるVL領域とを含み、
b)プラスミノーゲン活性化因子阻害剤1(PAI−1)に結合する前記第2標的ドメインは、配列番号15と少なくとも95%の同一性を有するアミノ酸配列により表されるVH領域と、配列番号16と少なくとも95%の同一性を有するアミノ酸配列により表されるVL領域とを含む、請求項1〜3のいずれか1項に記載の患者の急性血栓性障害を治療または予防するための医薬品。 - a)トロンビン活性化可能な線溶阻害剤(TAFI)に結合する前記第1標的ドメインは、配列番号13と少なくとも98%の同一性を有するアミノ酸配列により表されるVH領域と、配列番号14と少なくとも98%の同一性を有するアミノ酸配列により表されるVL領域とを含み、
b)プラスミノーゲン活性化因子阻害剤1(PAI−1)に結合する前記第2標的ドメインは、配列番号15と少なくとも98%の同一性を有するアミノ酸配列により表されるVH領域と、配列番号16と少なくとも98%の同一性を有するアミノ酸配列により表されるVL領域とを含む、請求項1〜4のいずれか1項に記載の患者の急性血栓性障害を治療または予防するための医薬品。 - 前記二重特異性抗体は、ヒト化抗体である、請求項1〜5のいずれか1項に記載の患者の急性血栓性障害を治療または予防するための医薬品。
- 急性血栓性障害に起因する脳病変を治療または予防するための請求項1〜6のいずれか1項に記載の医薬品。
- 前記急性血栓性障害は、急性末梢動脈閉塞、中大脳動脈閉塞(MCAO)、および深部静脈血栓塞栓症、肺塞栓症などの血栓塞栓症からなる群から選択される、請求項1〜7のいずれか1項に記載の患者の急性血栓性障害を治療または予防するための医薬品。
- tPA(組織プラスミノーゲン活性化因子)をさらに含む、請求項1〜8のいずれか1項に記載の患者の急性血栓性障害を治療または予防するための医薬品。
- 前記治療または予防は、前記二重特異性抗体の投与前、投与中または投与後に、tPAの投与なしで行われる、請求項1〜8のいずれか1項に記載の患者の急性血栓性障害を治療または予防するための医薬品。
- 前記急性血栓障害は、血小板豊富な凝血塊の出現によって特徴付けられる、請求項1〜10のいずれか1項に記載の患者の急性血栓性障害を治療または予防するための医薬品。
