Description
The present invention relates to a blood purification apparatus for purifying a patient's blood while circulating it extracorporeally, such as dialysis treatment using a dialyzer.
Generally, at the time of dialysis treatment, a blood circuit for circulating the collected patient's blood extracorporeally and returning it to the body is used. Such a blood circuit is, for example, a dialyzer (blood purification means) having a hollow fiber membrane. It is mainly composed of an arterial blood circuit and a venous blood circuit that can be connected to each other. An arterial puncture needle and a venous puncture needle are attached to the tips of the arterial blood circuit and the venous blood circuit, respectively, and are punctured by the patient to perform extracorporeal circulation of blood in dialysis treatment.
Among these, the arterial blood circuit is provided with an iron-type blood pump, and the blood pump is driven in a state in which the patient is punctured with the arterial puncture needle and the venous puncture needle. The blood collected from can be sent to the dialyzer while circulating outside the body. Furthermore, an arterial air trap chamber and a venous air trap chamber are connected to the arterial blood circuit and the venous blood circuit, respectively, and after the extracorporeally circulated blood is defoamed (removal of bubbles), the patient's body. It is configured to be returned to.
By the way, when filling only the blood channel with the priming solution, if the filling amount is increased, there is a possibility that the dialyzer may leak to the dialysate channel side by its own weight through the blood purification membrane of the dialyzer. However, when a physiological saline solution is used as the priming solution, it is necessary to use a physiological saline solution that is more expensive than the dialysis solution on the dialysis fluid flow path side. There may be a shortage. Moreover, there is a possibility that it takes time to fill the blood channel with the priming solution by the amount leaked to the dialysate channel side. Even when dialysate is used as the priming solution, there is a similar concern.
Thus, in the conventional blood purification apparatus, as disclosed in
The conventional blood purification apparatus has the following problems.
Blood purification means used at the time of blood purification treatment are usually a wet type in which a blood flow path through which blood flows and a dialysate flow path in which dialysate flows are pre-filled with a filling type, and a dry type in which the filling liquid is not filled. In addition, there are dialyzers that are generally distributed, and doctors and the like can arbitrarily select one of the dialysers in consideration of various circumstances.
However, in the case where the filling process is uniformly performed regardless of the type of the dialyzer, as in the conventional blood purification apparatus, when the wet type dialyzer is used, the filling process of the soot that is originally unnecessary However, there is a problem that it takes priming time. That is, since the wet type dialyzer is filled with the filling liquid in advance in the blood flow path and the dialysate flow path, it leaks out to the dialysate flow path side in the process of filling only the blood flow path with the priming liquid. Therefore, it is not necessary to perform a filling step as a method for solving the problem.
In addition, when performing the filling process, it is necessary to introduce the dialysate into the dialysate flow path from the dialysate introduction line. Even when a wet type dialyzer is used, the dialyzer and the dialyzer body are introduced into the dialysate. It is necessary to connect via a line and to operate a liquid feed pump or the like on the dialyzer body side. Therefore, when a wet type dialyzer is used, if a filling process that is not necessary is performed, it cannot be used for other functions such as a self-diagnosis function using the piping part of the dialysis apparatus, and there is a problem that it takes time.
The present invention has been made in view of such circumstances, and the priming is performed by selectively performing the filling step depending on whether the wet type blood purification unit or the dry type blood purification unit is used. It is an object of the present invention to provide a blood purification device that can efficiently perform the treatment in a short time.

In addition, a
More specifically, as shown in FIGS. 10 and 11, the
The
Then, a hematocrit value indicating the blood concentration is obtained based on the received light voltage generated by the
The

This unit includes a housing portion including a lid portion H1 and a main body portion H2, a groove H2a formed in the main body portion H2, a bubble detector (D1, D2) disposed in the main body portion H2, and a
The bubble detectors (D1, D2) are composed of sensors capable of detecting bubbles (air) flowing through the flexible tube fitted in the groove H2a. As shown in FIG. 14, for example, ultrasonic vibration composed of a piezoelectric element is used. An element α2 and an ultrasonic receiving element α1 made of a piezoelectric element are provided. Then, ultrasonic waves can be irradiated from the ultrasonic vibration element α2 toward the flexible tube constituting the
That is, since the attenuation rate of ultrasonic waves is higher than that of blood or replacement liquid, the voltage (detected by the ultrasonic receiving element α1) exceeds a predetermined threshold value, so that the bubbles (gas) flowed. It is detected. Such bubble detectors (D1, D2) are capable of detecting gas or liquid (air or filling liquid) in a determination step described later by changing or adding a threshold value.
The
The light receiving element β2 is configured such that the voltage changes according to the amount of light received, and configured to be able to determine the presence or absence of blood flowing through the
Furthermore, the
The bubble detector D3 is made of an accessory called a clip type, and can be placed at an arbitrary position (between the

It consists of a flow path such as a flexible tube. The arterial pressure detection sensor P1 can detect the pressure in the upper part (air layer side) of the arterial air trap chamber 5, thereby detecting the pressure of blood flowing into the dialyzer 3 (inlet pressure of the dialyzer 3). It is possible.

The open line L10 is composed of a flow path such as a flexible tube whose proximal end is connected to the arterial air circulation line L8 and the venous air circulation line L9 and whose distal end is open to the atmosphere. The liquid level adjusting pump 11 comprising a squeezing pump is disposed. The liquid level adjusting pump 11 has a squeezing portion that can be driven forward and backward, and the squeezing portion squeezes a flexible tube that forms the open line L10 in the longitudinal direction thereof, thereby causing arterial air. Note that the liquid level adjustment pump 11 indicates normal rotation drive when rotated clockwise in the figure, and reverse rotation drive when rotated counterclockwise (similarly, the
Thus, when the liquid level adjustment pump 11 is driven to rotate forward, air is sucked from the tip of the open line L10. Therefore, when the clamp means V9 is in the open state, the arterial air trap via the arterial air circulation line L8. When air is introduced into the chamber 5 to lower the liquid level and the liquid level adjustment pump 11 is driven in reverse, air is discharged from the tip of the open line L10. Therefore, when the clamp means V9 is in the open state, the artery side Air is discharged from the artery side air trap chamber 5 through the air circulation line L8 to raise the liquid level.
Similarly, when the liquid level adjustment pump 11 is driven to rotate forward, air is sucked from the tip of the open line L10. Therefore, when the clamp means V10 is in the open state, the venous air trap passes through the venous air circulation line L9. When air is introduced into the

Hereinafter, the control content by the control means C according to the present embodiment will be specifically described.
After the blood circuit is connected to the dialyzer main body B, before the blood purification treatment (before dialysis treatment), as shown in FIG. 2, a self-diagnosis step S1, a discrimination step S2, and a priming step S3 are sequentially performed. . The self-diagnosis step S1 forms a closed circuit including the connection position of the pressure detection means (pressure detection sensors P1 to P4) in the blood circuit, and changes the pressure detection means ( This is a step of diagnosing the quality of the connection state of the blood circuit based on the detection values of the pressure detection sensors P1 to P4).
Specifically, the self-diagnosis process includes a process as shown in FIG. That is, as shown in FIG. 6, the clamp means V2, V4, V5, V11 are closed while the
Then, the detection value of the pressure detection means (pressure detection sensors P1 to P4) that changes by pressurizing the inside of the closed circuit is monitored, and it is determined whether or not the predetermined pressure is reached within a predetermined time t1. (S1B). If the predetermined pressure is reached within the predetermined time t1, it can be estimated that there is no leakage of the pressurized air at least in the closed circuit, so that the connection of the blood circuit is determined to be appropriate (pass) and the predetermined pressure within the predetermined time t1. If the pressure does not reach, it can be estimated that the pressurized air is leaking at least in the closed circuit, so that the connection of the blood circuit is determined to be inappropriate (failed). In addition, when it is judged as a failure, it is preferable to issue a warning or the like indicating that the connection of the blood circuit is inappropriate.
If it is determined that the self-diagnosis process is acceptable, a determination process is performed. This discriminating step is a step capable of discriminating whether the
Then, it is determined whether or not the detection means (bubble detector D3) has detected a liquid (S2B). When the liquid is detected, it is determined that the
Further, instead of the above embodiment, as shown in FIG. 8, the clamp means V4, V5, V9, V10, V11 are closed while the liquid level adjustment pump 11 is stopped, and then the
The priming step is for filling the blood circuit with a dialysis solution as a priming solution, and has a step as shown in FIG. That is, as shown in FIG. 9, by connecting the connector a and the connector b, the distal end of the
Thereafter, it is determined whether or not the
It is determined whether or not the overflow process (S3D) has passed a predetermined time (S3E). When the predetermined time has passed, the clamp means V11 and Va are closed, the clamp means V1 and V2 are opened, and the
Here, in the present embodiment, whether or not to perform the filling process can be determined by the control means C according to the type of the
When performing the filling step, for example, the
Then, after the circulation step, it is determined whether or not bubbles are detected by the bubble detectors V1, V2, etc. (S3G). When bubbles are detected, the process returns to S3D, and if bubbles are not detected, Proceeding to S3H, a gas purge step (S3H) is performed. The gas purge step (S3H) is a step of filling the dialysate flow path of the
Thereafter, it is determined whether or not a predetermined time has elapsed (S3I), and when the predetermined time has elapsed, the clamp means V1, V2, V11 and Va are opened to drive the
According to the above embodiment, the control means C can determine whether or not to perform the filling process according to the type of the
That is, when the dry-
In particular, the control means C according to the present embodiment causes a determination step to determine whether the
Further, the blood circuit is connected to detection means capable of detecting gas or liquid in the flow path, and the discrimination step drives the liquid level adjustment means 11 or the
Still further, the artery side air trap chamber 5 connected to the artery
In addition, the liquid level adjusting means A includes a liquid level adjusting pump 11 that can be driven forward and backward, and the control means C drives the liquid level adjusting pump 11 at an arbitrary timing during priming. Since air can be introduced into or discharged from the upper portion of the air trap chamber 5, the type of the
The control means C can adjust the amount of air discharged from the arterial air trap chamber 5 by the liquid level adjustment means A according to the type of the
Next, a blood purification apparatus according to a second embodiment of the present invention will be described.
As in the first embodiment, the blood purification apparatus according to the present embodiment is composed of a dialysis apparatus for performing dialysis treatment. As shown in FIG. 16, the blood is composed of an
Here, the
Then, it is determined whether or not the detection means (
In this way, the control means C performs a determination step that can determine whether the
Next, a blood purification apparatus according to a third embodiment of the present invention will be described.
The blood purification apparatus according to the present embodiment includes a dialysis apparatus for performing dialysis treatment, as in the first and second embodiments, and includes an
Here, as shown in FIG. 6, the control means C according to the present embodiment forms a closed circuit including the connection positions of the pressure detection means (pressure detection sensors P1 to P4) in the blood circuit. A self-diagnosis process for diagnosing the quality of the connection state of the blood circuit based on the detection value of the pressure detection means (pressure detection sensors P1 to P4) that changes when the pressure is applied to the pressure sensor. The determination step is performed based on a change in the detected value by the pressure detection means (pressure detection sensors P1 to P4) obtained in the diagnosis step.
Below, the self-diagnosis process and discrimination | determination process which concern on this embodiment are demonstrated based on the flowchart of FIG.
First, after forming a closed circuit as shown by a bold line portion in FIG. 6, the liquid level adjusting pump 11 or the
(S3)。 If the predetermined pressure is reached within the predetermined time t1, it can be estimated that there is no leakage of the pressurized air at least in the closed circuit, so that the connection of the blood circuit is determined to be appropriate (pass) and the predetermined pressure within the predetermined time t1. If the pressure does not reach, it can be estimated that the pressurized air is leaking at least in the closed circuit, so that the connection of the blood circuit is determined to be inappropriate (failed). The process proceeds to S3 on the condition that the self-diagnosis process is determined to be acceptable, and the detected value of the pressure detection means (pressure detection sensors P1 to P4) monitored in S2 is within a predetermined time t2 (where t1> t2). In step S3, it is determined whether a predetermined pressure has been reached.
If the predetermined pressure is reached within the predetermined time t2, it is determined that the
On the other hand, due to the difference in properties between a compressible gas such as air and an incompressible liquid such as a filling liquid, the dry-
Thereafter, the process proceeds to a priming process, and it is possible to determine whether or not to perform the filling process according to the type of the
Next, a blood purification apparatus according to a second embodiment of the present invention will be described.
As in the previous embodiment, the blood purification apparatus according to the present embodiment is composed of a dialysis apparatus for performing dialysis treatment, and as shown in FIG. 18, a blood circuit composed of an
The priming liquid supply line L12 is composed of a flexible tube having one end connected to the
In the blood purification apparatus according to the present embodiment, the self-diagnosis process, the determination process, and the priming process are sequentially performed as in the previous embodiment (see FIG. 2), and the dialyzer determined in the determination process in the priming process. Depending on the
Thus, in the present embodiment, the physiological saline is used as the priming liquid. Therefore, when the
In particular, in the present embodiment, a physiological saline solution is used as the priming solution, so that it is not necessary to introduce the dialysis solution from the dialyzer body B during priming. Therefore, when the
Although the present embodiment has been described above, the present invention is not limited thereto. For example, in the discrimination process according to the present embodiment, the gas or liquid in the blood flow path of the
In the present embodiment, the control unit C performs the determination process, and it is possible to determine whether or not to perform the filling process according to the type of the
Furthermore, the liquid level adjusting means A may be provided in the dialyzer main body B, or may be provided in a position different from the dialyzer main body B, and further the liquid level adjusting means. A that does not include A may be used. In this embodiment, the present invention is applied to a dialysis apparatus used at the time of hemodialysis treatment, but other apparatuses that can purify the patient's blood while circulating it outside the body (for example, blood filtration dialysis, blood filtration, AFBF) You may apply to the blood purification apparatus used, a plasma adsorption apparatus, etc.).
The control means can determine whether or not to perform the filling process according to the type of the blood purification means connected to the blood circuit at the time of priming. If the type of the blood purification means is a dry type, the control means is allowed to perform the filling process. In addition, when the type of blood purification means is a wet type, any blood purification device that does not perform the filling process can be applied to devices having other functions.
