Description
酢酸ウリプリスタル(UPA)は、プロゲステロン標的組織中のプロゲステロン受容体に有効に結合し、これを阻害するプロゲステロン受容体モジュレーターである。
以前はCDB-2914として知られていたUPAは、式I:
本発明者らは、酢酸ウリプリスタル(UPA)またはその代謝物の何れかを投与することを含む、患者において、乳房腫瘍を予防または処置する方法を提案する。
このため、患者において乳房腫瘍を予防または処置するのに使用するための、酢酸ウリプリスタル(UPA)またはその代謝物の何れかを提供する。
特に好ましい態様において、酢酸ウリプリスタル(UPA)またはその代謝物の何れかは、BRCA1および/またはBRCA2遺伝子に変異を有する患者において、乳房腫瘍を予防するのに使用される。
他の好ましい態様において、酢酸ウリプリスタル(UPA)またはその代謝物の何れかは、患者において、好ましくは乳癌である乳房腫瘍を処置するのに使用される。
“患者”は、本発明の予防的または治療的処置の必要があるあらゆる対象、好ましくは女性の対象を意味する。しかし、男性も同様に乳房腫瘍に感受性であり得ることから、男性もまた包含する。
好ましくは、患者は、BRCA1および/またはBRCA2遺伝子に変異を有すると診断されている。
BRCA1またはBRCA2位置の変異事象は、コード配列および非コード配列の欠損、挿入および点変異を含み得る。欠損は、全遺伝子であっても遺伝子の一部のみであってもよい。点変異は、停止コドン、フレームシフト変異またはアミノ酸置換をもたらし得る。体細胞変異は、特定の組織、例えば腫瘍組織にのみ起こるものであって、生殖系列に遺伝されないものである。生殖細胞系列変異は、身体のあらゆる組織または細胞で見られ、遺伝する。単一対立遺伝子のみが変異したならば、乳癌の素因が示される。
BRCA1またはBRCA2素因対立遺伝子は野生型対立遺伝子に対して劣性である、すなわち、少なくとも1つの野生型BRCA1またはBRCA2対立遺伝子を含む細胞は癌性ではないと考えられている。しかし、1つの野生型BRCA1またはBRCA2対立遺伝子および1つの素因対立遺伝子を含む細胞は、時折、ランダム変異または細胞分裂中の染色体喪失の何れかによって、野生型対立遺伝子の喪失を経験し得る。このような変異細胞の後代は全て、BRCA1またはBRCA2の野生型機能を失い、腫瘍に発達し得る。従って、BRCA1またはBRCA2の素因対立遺伝子は、癌に感受性であり、その易罹患性は優性に遺伝する。
Myriad社の最新BRCA診断テストであるBRACAnalysis(登録商標)は、BRCA1およびBRAC2遺伝子における乳癌の著しいリスクに関連する変異を識別するために、2つの伝統的な方法、すなわちサンガー配列決定およびPCRの組み合わせを使用する。
また、BRCA1遺伝子の変異を検出するタンパク質をベースとする系を含む他の方法が、開示されている(米国特許第5,965,377号;第6,514,713号)。
特定の態様において、患者は遺伝性乳癌・卵巣癌(HBOC)と診断されている患者である。
“予防”または“予防する”は、無症状の患者または乳房腫瘍を示さない患者に、UPAまたはその代謝物の何れかを投与することを意味する。より具体的には、患者は、例えば、家族歴またはBRCA1および/またはBRCA2遺伝子状態を考慮して乳房腫瘍を発症するリスクがあり得る。このような予防は、乳癌の発症リスクを減らすことを意図する。
予防において、処置は数年維持され得る。
治療的処置において、UPAは、数日間から数ヶ月間または数年間、例えば少なくとも3ヶ月間〜約5年間投与され得る。
材料および方法
ステロイド
プロゲステロン受容体アンタゴニスト酢酸ウリプリスタル(UPA)およびそのモノ−N4デメチレーテッド代謝物CDB-3877(CDB)は、HRA-Pharma (Paris, France)から恵与された。17β−エストラジオール(E2)、プロゲステロン(P)、デキサメタゾン(DEX)およびミフェプリストン/RU-486(RU)は、Sigma (St Quentin Fallavier, France)から購入した。
T−47D細胞株およびMCF−7細胞株を、それぞれ10% ウシ胎児血清(PAA Laboratory, Les Mureaux, France)を加えたRPMI 1640培地およびDMEM培地で維持した。T−47D細胞株はヒト乳管癌由来であり、大量のPR(プロゲステロン受容体)およびER(エストラジオール受容体)を構成的に発現した。正常ヒト乳房上皮細胞(HBE)の初代培養物を20人の女性(年齢17〜50歳)から得た。HBE細胞の培養に用いられる手順は、Gompelらによって詳細に記載されている(Gompel, et al., 1986, J Clin Endocrinol Metab 63(5): 1174-1180)。HBE細胞を、ヒドロコルチゾン(5ng/ml)、トリヨード−L−チロニン(6.5ng/ml)、コレラ毒素(10ng/ml)、トランスフェリン(5mg/ml)、インスリン(0.016U/ml)、上皮増殖因子(10ng/ml)(Sigma, St Quentin Fallavier, France)および5% ヒト血清(Etablissement Francais du Sang)を含むHAM F10培地(PAA Laboratory, Les Mureaux, France)で維持した。HBE初代培養物は、上皮マーカー、ならびに低レベルのエストラジオール受容体(ER)およびエストラジオール誘発PRを発現する(Malet, et al., 1991. J Clin Endocrinol Metab 73(1): 8-17; Courtin, et al., 2011, Breast Cancer Res Treat.)。
播種後、細胞を血清およびフェノールレッドを含まない培地中で24時間培養した。次いで、5% デキストラン−チャコール処理済み血清を含み、フェノールレッドを含まない培地中で、処理を行った。PまたはDEX単独(100nM)、またはそのUPA、RU(1nM〜1μM)またはCDB-3877(100nM)との組み合わせで、細胞を処理した。コントロール細胞を、1:1000最終エタノール濃度のビークルとしてエタノールのみで、またはE2(10nM)で処理した。
細胞を、グルココルチコイドとプロゲステロン応答エレメント(GRE/PRE)を含むレポーター遺伝子プラスミドでトランスフェクトした。1) MMTV-Lucは、pFC31ベクターにおいて、蛍ルシフェラーゼ遺伝子上流の1GRE/PRE回文構造および3GRE/PREヘミ回文構造を含むマウス乳房腫瘍ウイルス長鎖末端反復プロモーターである。2) GRELucは、pBLベクターにおいて、蛍ルシフェラーゼ遺伝子上流GRE/PRE回文構造の6コピーを含む。示された時、HBE細胞を、POP3ベクターにおいて構築されたヒトPRアイソフォームhPR−AおよびhPR−B発現プラスミドでトランスフェクトした。β−ガラクトシダーゼ上流ラウス肉腫ウイルスプロモーター(pRSV-β-Gal)を、コントロールとして各々の実験にトランスフェクトした。トランスフェクションは、製造者の説明書に従って、それぞれ乳癌細胞株またはHBE細胞のためのリポフェクタミン試薬またはリポフェクタミンLTX試薬(Invitrogen, Cergy-Pontoise, France)を用いて行った。トランスフェクションの24時間後、乳癌細胞およびHBE細胞をそれぞれ24時間または48時間処理した。実験の最後に、細胞を溶解し、ルシフェラーゼ活性を、ルシフェラーゼアッセイ系(Promega, Charbonnieres-les-bains, France)を用いて決定した。ルシフェラーゼ活性データを標準化するために、β−ガラクトシダーゼ活性をGalacto Star kit (Applied Biosystems, Courtaboeuf, France)を用いて評価した。
全RNAをTriZOL試薬(Invitrogen, Cergy-Pontoise, France)を用いて抽出した。
2μgの全RNAについて、ランダムプライマーを用いて、37℃で1時間逆転写(RT)を行った。2μlのRT産物を希釈し(1:10)、配列特異的プライマー(300nM)およびBrillant SYBR GREEN QPCR master mix (Fermentas, Saint-Remy-les-Chevreuse, France)を用いて、Mx3000P装置(Agilent Technologies, Massy, France)で、定量的PCRを行った。条件は、95℃で10分間を1サイクル、続いて95℃で30秒間、60℃で1分間および72℃で30秒間を40サイクルであった。遺伝子発現値を、ハウスキーピング遺伝子36B4に対して標準化した。ステロイド処理時間を、示された遺伝子に最適な刺激となるよう選択した。処理の6時間後にALPLおよびG0S8のmRNAを分析した。処理の24時間後にIEX−1、FASNおよびBCL2を分析した。GR応答について24時間後、またはPR応答について48時間後に、サイクリンAのmRNAを分析した。
ホルモン処理の24時間後、[メチル−1 3H]チミジン(Perkin Elmer, Courtaboeuf, France)と共に、37℃で、HBEまたは癌細胞についてそれぞれ48時間または20時間、細胞をインキュベートした。インキュベーション後、細胞を、PBS 1Xで2回、5% トリクロロ酢酸(TCA)で1回洗浄した。細胞を、5% TCA中で、4℃で15分間インキュベートし、0.1NのNaOHで37℃で30分間溶解させた。全細胞ライセートを、5mlのEcolite scintillation liquid (MP biomedical, Illkirch, France) に加え、放射活性を、β−カウンター HIDEX 300SL(ScienceTec, Courtaboeuf, France)で計数した。
HBE細胞またはMCF−7細胞およびT−47D細胞についてそれぞれホルモン処理の96時間後または48時間後、細胞をPBSで洗浄し、マトリックスを、accutase酵素(PAA laboratory, Les Mureaux, France)で分離し、1350rpmで5分間遠心分離した。細胞を固定し、70% エタノール中で−20℃で凍結した。分析の前に、細胞をPBSで洗浄し、PBS中10μg/mlのヨウ化プロピジウム(0.835U/mlのRNAアーゼAを含む)(Sigma, St Quentin Fallavier, France)で染色した。各サンプルについて、少なくとも10,000細胞を、BD LSR II フローサイトメトリー(BD Biosciences, Le Pont de Claix, France)で計数した。ダブレットとデブリを排除後、サイクル分布を ModFit LT software (Verity Software House,USA)を用いて分析した。
結果を平均値±SEMで表した。処理の統計的有意性を決定するために、一元配置ANOVAおよびテューキー・クレーマー多重比較検定を行い、Instat 3 software (GraphPad, USA)で各処理の相対的有効性を比較した。2つの処理のみを比較したときは、独立t検定を行った。p<0.05を有意と考えた。
UPAはPR遺伝子トランス活性化に作用する
PR誘発遺伝子トランス活性化に対するUPAのアンタゴニスト性を分析するために、正常ヒト乳房上皮細胞(HBE)およびT−47D乳癌細胞株を、MMTV-Lucレポーター遺伝子でトランスフェクトした(図1)。患者の中で少量および可変量のPRを発現したHBE細胞において(Malet, et al., 1991. J Clin Endocrinol Metab 73(1): 8-17)、プロゲステロン(P)は、著しくルシフェラーゼ発現を誘発した(コントロールに対する誘発倍率=1.38±0.11, p<0.05)(図1A)。PRに対するUPA投与の効果をより良く試験するために、HBE細胞を、hPR−AおよびhPR−B発現プラスミドでコ−トランスフェクトした(図1B)。UPAおよびRU−486(RU)は、1,000〜10nMで用量依存でP誘発MMTV11 Luc トランス活性化を阻害した。次いで、PRアイソフォーム間でのUPA作用を区別するために、HBE細胞をhPR−AのみまたはhPR−Bのみでトランスフェクトした。UPAアンタゴニスト作用について、各PRアイソフォームのレベル上昇に差異は見られなかった(図1C、1D)。T−47D細胞株において、UPAおよびRUは、1,000〜10nMで、同様の強力なPRアンタゴニスト作用を示した(図1E)。また、部分アンタゴニスト応答が1nMで検出された(図1E)。HBE細胞およびT−47D細胞の両方において、UPAおよびRUは、レポーター遺伝子転写に対するプロゲステロンアゴニスト性を示さなかった(図1B〜E)。これらの結果は、UPAが、正常および癌性乳房細胞において、強力なPアンタゴニストとして作用することを示した。
100nMは、HBE細胞において完全アンタゴニスト活性を発揮するのに必要な最も低いUPA濃度であった。そのため、この濃度が、HBE細胞およびT−47D細胞において特異的P標的遺伝子に対するUPAの作用をさらに調べるために選択された。以前に報告された通りPR発現を増加させるために、エストラジオール(E2)をHBE細胞に加えた。
この試験は、以前に報告された通りに(Courtin, et al.; 2011, Breast Cancer Res Treat.)、多量のGRを発現したHBE細胞およびMCF−7細胞で行った。細胞をGRE-Lucレポーター遺伝子でトランスフェクトさせ、デキサメタゾン(DEX)、UPAおよびその近位モノデメチレーテッド代謝物であるCDB-3877(CDB)(UPAより抗グルココルチコイド活性が低いことが知られている (Attardi, et al., 2004, J Steroid Biochem Mol Biol 88(3): 277-288.))で処理した。UPAは41.3±5.8%までDEX誘発ルシフェラーゼトランス活性化を阻害するが、この代謝物はHBE細胞においてDEX活性を阻害しなかった(18.1±11.8%)(図3A)ことから、我々は、この代謝物の低下した抗グルココルチコイド活性を確認した。しかし、MCF−7細胞では、UPAおよびCDBの両方が、それぞれ59±3.4%および26.5±9.1%までDEX誘発ルシフェラーゼトランス活性化に顕著に拮抗した(図3B)。これらの結果は、UPAの抗グルココルチコイド効力が、HBE細胞よりもMCF−7乳癌細胞で強いことを示唆している。
HBE細胞およびMCF−7細胞におけるUPA抗グルココルチコイド作用をより良く定義するために、様々なグルココルチコイド応答性遺伝子のmRNA発現を、DEXおよびUPA処理後に分析した。最初期応答3(IEX−1)およびGタンパク質シグナリング2レギュレーター(G0S8)は、それぞれ、ストレス条件下での細胞生存およびGタンパク質シグナリングに関与する。IEX−1およびG0S8は、グルココルチコイド応答性遺伝子として特性決定された。図4Aに示された通り、IEX−1のmRNA発現は、DEXによって下方制御され、UPAは、HBE細胞およびMCF−7細胞において、この応答に拮抗しなかった。他方、UPAは、G0S8のmRNA上方制御に対して、僅かに、有意でない拮抗作用を奏したが、その抗グルココルチコイド活性は、MCF−7細胞において強力であった(p<0.001)(図4B)。サイクリンAおよびBCL2遺伝子は、以前に、インビトロで、骨芽細胞および神経芽腫においてそれぞれDEXによって制御されることが示された。このため、HBE細胞およびMCF−7細胞において、サイクリンAおよびBCL2のmRNA発現に対するDEXおよびUPAの作用を分析した。mRNA発現がHBE細胞中で誘発され、MCF−7細胞で抑制されるため、サイクリンAは、2種の細胞で、DEXによって異なって制御された(図4C)(Courtin, et al.; 2011, Breast Cancer Res Treat.)。両方の細胞モデルにおいて、UPAは、サイクリンAのmRNA発現に対するDEXの作用を一部阻害した。BCL2のmRNAは、HBE細胞およびMCF−7細胞において、DEXによって下方制御された(図4D)。しかし、UPAは、HBE細胞において、BCL2のmRNAのDEX下方制御を逆転しなかったが、UPAは、MCF−7細胞においてこの効果に一部拮抗した。UPAは、これらの遺伝子に対してグルココルチコイドアゴニスト活性を全く示さなかった(図4)。
Pおよびグルココルチコイドは、正常および腫瘍性乳房細胞に対する異なる増殖作用および生存作用を誘発できる。これらの細胞事象に対するUPAの役割を評価するために、増殖およびアポトーシスを、3つの細胞モデルで、トリチウム化チミジン取り込み(図5)およびフローサイトメトリー(図6)によってそれぞれ測定した。PRに対するUPAの作用を、HBEおよびT−47Dで試験したが、我々は以前にMCF−7細胞株でPR機能性がないことを証明したことから(Courtin, et al.; 2011, Breast Cancer Res Treat.)、MCF−7細胞では試験しなかった。本明細書で用いられるT−47D細胞はGRを発現しないため(Courtin, et al.; 2011, Breast Cancer Res Treat.)、GRに対するUPAの作用は、HBE細胞およびMCF−7細胞でのみ試験した。HBE細胞において、UPAは、Pによって誘発される抗増殖作用を阻害しなかった(図5A)。逆に、UPAは、DEXによって誘発される強い増殖活性を阻害した(図5A)。T−47D細胞において、UPAは、Pの抗増殖活性を逆転させた(図5B)。MCF−7細胞において、UPAは、DEXの弱い抗増殖作用を妨げる傾向があった(図5C)。UPA処理のみでは、何れのタイプの細胞においても、増殖に対して作用を有さなかった(図5)。アポトーシスは、フローサイトメトリー分析による細胞のsub−G1パーセンテージの定量化によって測定した。Pは、HBE細胞において、抗アポトーシス性を表すDEXとは対照的に、強いアポトーシス促進作用を奏した(図6A)。UPAと組み合わせたとき、両方のホルモンの活性を逆転させた。同様に、Pはsub−G1細胞のパーセンテージを増加させ、UPAはT−47D細胞においてこの効果を阻害する傾向があった(図6B)。MCF−7において、DEXは、アポトーシス促進性を示すが、これはUPAによって完全に阻害された(図6C)。UPAはそれ自身ではアポトーシス性または生存性はないが、HBE細胞、T−47D細胞およびMCF−7細胞において、PおよびDEX作用の大部分を相殺する。
材料および方法
患者
乳房組織サンプルを、臨床試験に関するフランスの法律に従ってインフォームドコンセントを得た乳房縮小術のための手術を行う6人の女性(29〜42歳)から得た。患者には乳房の疾患の病歴はなく、ヘマトキシリン−フロキシン−サフラン(HPS)染色を含む免疫組織学的試験は、正常乳房組織のみを示した。
4週齢の卵巣切除したメスのNMRInu/nu 20胸腺欠損マウスを、Janvier laboratory (Le Genest Saint Isle, France)から購入した。乳房組織サンプルを6人の女性(29〜42齢)から得た。ヒト乳腺組織を2×2×2mmの断片に切断し、4つの断片を群当たり4匹のマウスの背中に皮下で異種移植した。4つの処置群とした:コントロール、E2、E2+P、および、E2+P+UPA。処置は、各マウスの首にステロイドペレットを移植することによって投与した。実験条件を、最初に、ペレットにコレステロールと混合したホルモンの投与量範囲を用いることによって決定した。血液サンプルアッセイを処置2週後および4週後に行い、血漿ホルモン濃度を測定した。最後に、期待される血液濃度を提供するように、0.3mgのE2および20mgのPおよびUPAの投与量を用いた。コントロール群について、コレステロールのみを含むペレットを用いた。月経周期条件を再現するために、実験初日に、マウスに、コレステロール、E2および/またはUPAを含むペレットを移植し、14日目に、コレステロールまたはプロゲステロンを含むペレットを、コントロール群およびE2群に、または、E+P群およびE2+P+UPA群にそれぞれ移植した。実験開始後28日目にマウスを屠殺した。各マウスの血液を集め、血清をホルモン濃縮まで−20℃で凍結した。
乳房組織断片を集め、免疫組織化学的分析のために、すぐにパラホルムアルデヒド溶液で固定化した。全ての実験プロトコルおよび動物の部屋の環境条件は、the French Ethic committee for the care and use of laboratory animals Charles Darwinによって承認されたものである。
臨床アッセイエストラジオール−2(Sorin Biomedica Diagnostics SpA, Saluggia, Italia)を用いた放射イムノアッセイによって、エストラジオールを測定した。プロゲステロンレベルを、Acquity UPLC および Quattro Premier XE (Waters, Milford MA, USA)を用いるUPLC−MSMSによって評価した。
UPA濃度を、MPI Research (State College, Pennsylvania, USA)によって、LC−MS/MS法を用いて測定した。
Ki67抗体を用いて有糸分裂指数を計算し、マウスに移植した各乳房組織について決定した。免疫組織化学的分析は、BOND-MAX workstation (Leica, Nanterre, France)を用いて行った。乳房組織異種移植片のパラフィン切片を脱蝋して30分間再水和した後、Ki67抗体についてクエン酸回収溶液(pH 6.0)またはPRおよびERα抗体についてEDTA回収溶液(pH 9.0)を用いて抗原を回収した。次いで、切片を、1:100のKi67(Novocastra, NCL5 L-Ki67-MM1)、1:80のPR(Biogenex, MU-328-UC)または1:300のER(Novocastra, NCL-L6 ER-6F11)モノクローナル抗体と共にインキュベートした。シグナル検出のために、the Kit Bond Polymer Refire Detectionを用いた。試薬を Menarini-Diagnostic (Rungis, France)から購入した。ネガティブコントロール(第1抗体を除く)を各セットに含めた。それぞれのマーカーについて、各マウスに移植された4つの乳房組織断片中の総量1,000個の小葉細胞および1,000個の乳管細胞において、陽性細胞の割合を決定した。各実験において、各処置の最終パーセントは、群当たり4匹のマウスで得られたパーセンテージの平均値であった。
乳房組織増殖に対するUPA作用
発明者らは、乳房組織へのUPAの長期間投与を調べるために、インビボモデルを開発した。ヒト正常乳房組織サンプルを、E2またはE2+PまたはE2+P+UPA、またはコレステロール(コントロール)で処置された胸腺欠損マウスに異種移植した(実験手順を参照のこと)。ヒトの女性の月経周期で生じるE2およびP分泌の時系列を再現するために、E2ペレットを実験の最初に移植し、一方、Pペレットを14日目に移植した。慢性処置を模倣するために、UPAペレットを実験の最初に移植した。マウスの血清中のE2、PおよびUPA濃度を測定し、処置方法を確認した。マウスの血清中の平均E2濃度は、卵胞期に報告された生理的E2レベルの低い範囲に対応して、36.88±4.25pg/mlであった(平均±SEM)。Pレベルは、黄体中期の女性の平均P血清レベルと同じ13.05±1.14ng/mlであった(平均値±SEM)。UPA濃度は、臨床的使用で見られる範囲と同じ範囲で、63.49±10.46ng/ml(平均値±SEM)であった。ホルモンレベルは、コントロールマウスで検出できなかった(E2<0.8pg/ml;P<0.4ng/ml;UPA<0.5ng/ml)。図7Aに示された通り、エストラジオール受容体(ER)およびPR発現は、移植前の元の乳房組織と比較して、実験終了時の処置され移植された乳房組織断片で維持された。我々は、処置による腺性小葉および乳管における乳房組織の増殖活性を決定するために、有糸分裂Ki67発現マーカーを分析した(図7B、C)。移植された組織のコントロール群において、有糸分裂細胞の割合は低く均一であり、小葉で1.7±0.4%、乳管で1.8±0.6%であった。増殖活性は、コントロール群と比較したとき、E2処置群の小葉において、僅かに、有意でない程度に増加した(図7B、C)。しかし、乳管において、有糸分裂指数は、E2処置群で著しく上昇した(コントロール群に対して3.1±0.7誘発倍率, p<0.05)。小葉構造および乳管構造のE2処置と比較して、E2+P群およびE2+P+UPA群で有意な差異は観察されなかった(図7B、C)。これらの結果は、乳房組織の増殖活性が主にE2によって媒介されることを強く示唆している。また、我々は、UPAが正常上皮乳房細胞の増殖速度にあまり影響しないことを示した。
材料および方法
変異乳房組織サンプルをBRCA1に変異を有する5人の女性(36〜57歳)から得た。
異種移植を上記(実施例2)の通り行った。通常、1人のBRCA1変異を有さない患者由来の4つの乳房組織断片と、1人のBRCA1変異を有する患者由来の4つの乳房組織断片を、同じマウスの脊椎左側および右側にそれぞれ移植した。同じ群を形成した。
発明者らは、BRCA1変異を有する患者で集めたヒト乳房組織の増殖に対するホルモンの作用を調べるために、上の章で記載したものと同じモデルを用いた。移植前には、ERα発現に関する高変動性が、BRCA1変異を有する患者の小葉構造と乳管構造の両方で観察された。PRの基礎発現レベルは、野生型対立遺伝子を有する患者の小葉構造と比較して、BRCA1変異を有する患者で低かった。対照的に、BRCA1状態は乳管構造でPR発現レベルに影響しなかった。Ki67の発現による増殖状態の測定値は、BRCA1患者由来の小葉細胞で高いが、乳管細胞では野生型の患者よりも低い。
増殖がE2+P処置の組み合わせによって刺激された各場合において、UPAは、抗増殖活性を示した(図8および図9参照)。
材料と方法
この試験に用いた異種移植片モデルは、HBCx−34であった。HBCx−34は、野生型P53を有し、HER2の過剰発現がなく、PRとERαの過剰発現がある乳管癌である。腫瘍は、アドリアマイシン/シクロホスファミドに高応答性であり、ドセタキセルおよびカペシタビンに応答性であった。HBCx−34は、悪液質性を示さなかった。
HBCx−34腫瘍(P14.0.0/2)を、5〜10匹のマウス(ドナーマウス, 継代(n−1), メスの胸腺欠損ヌードマウス(Hsd:Athymic Nude-Fox1nu), 6〜9週齢, Harlan Laboratories (Gannat, France))に皮下移植した。これらの腫瘍が1000〜2000mm3に達したとき(60〜78日間)、ドナーマウスを頸部脱臼によって屠殺し、腫瘍を無菌的に切除し、解剖した。壊死領域を除いた後、腫瘍を約20mm3に測って断片に切断し、さらなる成分を全く含まない滅菌DMEM/F12培養培地に、移植前に最大10分間移した。
腫瘍体積に従ってマウスを異なる群に振り分け、各処置アームで同じ平均および中間腫瘍体積とした。75〜144mm3の間のHBCx−34腫瘍を有する10マウス/群を、腫瘍体積に従って、実験群にランダム化し、処置(UPA 130mg/kg, 経口, またはコントロールビークル)を、腫瘍移植後33日目に、42日間の全持続時間で開始した。
外見、行動および臨床変化についてマウスを観察した。動物の体重を、全実験期間中、週2回測った。異なる処置の毒性を体重減少として決定した。
腫瘍体積を、全実験期間中、週2回評価した。腫瘍を処置の終了時に回収し、秤量し、分析のために処理した。
UPA処置は耐容性がよく、この試験中で著しい体重減少は記録されなかった。処置関連臨床所見は実験期間中報告されなかった。
処置開始時の平均腫瘍体積(TV)は、コントロール群およびUPA処置群において、それぞれ112.2±7.1mm3および102.4±7.7mm3であった。
UPAは、抗腫瘍活性を示した。平均TVは、UPA処置マウスにおいて、2.17倍増加した(屠殺時の平均TV:332.0±61.0mm3)。処置群 対 コントロール群(T/C)のTV比は53%であった。
Claims (13)
- 酢酸ウリプリスタル、またはCDB-3877、CDB-3963、CDB-3236およびCDB-4183からなる群から選択されるその代謝物の何れかを含む、BRCA1および/またはBRCA2遺伝子に変異を有する患者における乳房腫瘍の予防または処置用薬剤。
- 乳房腫瘍の予防に使用するための、請求項1に記載の薬剤。
- 乳房腫瘍が癌である、請求項1または2に記載の薬剤。
- 経口で投与される、請求項1〜3の何れか1項に記載の薬剤。
- 静脈内に投与される、請求項1〜3の何れか1項に記載の薬剤。
- 膣または子宮内経路によって投与される、請求項1〜3の何れか1項に記載の薬剤。
- それを必要とする患者が女性の患者である、請求項1〜6の何れか1項に記載の薬剤。
- 乳房腫瘍が良性腫瘍である、請求項1〜7の何れか1項に記載の薬剤。
- 有効成分として酢酸ウリプリスタルを含む、請求項1〜8の何れか1項に記載の薬剤。
- 乳房腫瘍細胞の増殖、およびBRCA1/BRCA2変異乳房細胞の異常増殖に拮抗する、請求項1〜9の何れか1項に記載の薬剤。
- 避妊を必要とする女性患者において乳房腫瘍を予防するための、請求項1〜10の何れか1項に記載の薬剤。
- 前記女性患者において避妊薬としても作用する、請求項11に記載の薬剤。
- 酢酸ウリプリスタルまたはその代謝物を5mg〜80mgの量で含む、請求項1〜12の何れか1項に記載の薬剤。
