Description
癌の治療におけるNACの分子効果を記載した背景技術は、T. Parasassi, et al. Cell Death and Differentiation (2005), Vol. 12, No. 10, pages 1285-1296;E. K. Krasnowska et al. Free Radicals Biology and Medicine 2008, 45(11):1566-72;A. C. Gustafsson et al. BMC Cancer (2005), 5:75を含む。子宮内膜症の治療におけるNACはPittaluga等のMore than an antioxidant: N-acetyl-L-Cysteine in a murine model of endometriosis. Fertil & Steril 2010; 94(7): 2905-8に記載される。さらに、C.H. Kim等は、インスリン感受性を改善するためのN−アセチル−システインによる治療を記載する(ESHREの第22回年次大会の要約、プラハ、チェコ共和国、6月18−21日、2006年、P-463)。N−アセチル−L−システイン(NAC)をそれ自体で、または(ii)セレノメチオニンの形態としてのセレン、および/または(iii)メラトニン、および/または生理学的に許容されるそれらの塩と共に含有する調製物が、欧州特許出願第12710062.6号明細書に記載される。そのような調製物は、IVFの治療にも使用され得る。
IVFの開始時から既に、IVF治療における臨床結果が低いことが問題であった。IVFの費用は高く、治療は通常、治療周期当たりわずかに20−30%の生児出生を与えるのみなので、より高い生児出生率を与えるために、多くの異なる治療法が試験されてきている。好ましくはIVF治療の数日前におけるNACの投与が、より高い生児出生率を与えることが、思いがけず、今、わかってきた。さらに、多くの前のIVF治療が成功しなかった場合において、新しいIVF治療に関連したNACの事前投与が生児出生につながった。
NAC胚毒性の最終的な発生が予備的に試験された。NACの胚毒性を検証するために、2つの胚毒性試験(ウサギ胚バイオアッセイおよびヒト精子運動性バイオアッセイ)の適用が使用された(Pierzynski et al., 2007a参照)。どちらも、治療血液濃度の50倍の濃度まで、NACの胚毒性効果を検出できなかった。NACは、1細胞ウサギ胚の生存に対して影響を及ぼさず、また、孵化されたウサギ胚盤胞の割合を減少させないことが示された。ヒト精子に実行された試験もまた、悪影響は示さなかった。
精子運動性および速度を含めた良好な精子のパラメータを有する3人の健康なドナーから採取されたばかりのサンプルに対して、ヒト精子運動性の評価が実行された。ヒト精子調製媒体上での泳動または"泳ぎ上がる"ことが、運動型の精子を選択するために適用された。選択後に、各精液サンプルは3つのアリコートに分割され、エッペンドルフチューブに移された。1つの対照チューブに加えて、2つのチューブは、100および1,000nMの濃度で加えられたNACを有した。これらのチューブは、5%CO2環境において、一定の温度および湿度条件で24時間インキュベートされた。精子運動性は、NACへの暴露の1時間後および24時間後に、位相差顕微鏡を用いて倍率400倍で評価された。評価は、2分という固定された時間間隔で実行された。9つのサンプルが、36回の測定(各サンプル当たり、1時間および24時間において2回測定)により、精子運動性および速度について評価された。
着床の間、母体胎児境界において、非常に制御された様式でインテグリンが発現される。しかしながら、子宮内膜へのインテグリン媒介された胚付着の間の細胞外基質(ECM)配位子の重要性は、十分には理解されていない。従って、ラットの子宮および胚盤胞におけるフィブロネクチンの分布が、着床の時点において研究された。フィブロネクチンは子宮管腔上皮細胞内には無かったが、栄養膜細胞および内細胞塊中には強く発現された。これは、胚盤胞から分泌されたフィブロネクチンが、母体胎児境界において発現されたインテグリンに対する架橋配位子である可能性があることを示唆する。Arg−Gly−Asp(RGD)ペプチドが、インテグリン上のRGD認識部位をブロックするために使用され、イシカワ細胞へのラット胚盤胞付着に対する効果が調査された。胚盤胞またはイシカワ細胞のどちらかがRGDブロックペプチドによって事前インキュベートされた場合、胚盤胞付着における著しい低減があった。従って、子宮内膜への胚の付着が成功するには、子宮内膜および胚盤胞の両者にあるインテグリンが、フィブロネクチンのようなECM配位子のRGD配列と相互作用する必要がある。RGDペプチドによる胚盤胞およびイシカワ細胞両者の前処理はまた、胚盤胞付着を抑制したが、完全ではなかった。これは、RGD配列を含まないECM架橋配位子もまた胚の付着に関与することを示唆する(Kaneko Y, Murphy CR, Day ML. Extracellular matrix proteins secreted from both the endometrium and the embryo are required for attachment: A study using a co-culture model of rat blastocysts and Ishikawa cells. J Morphol. 2013;274(1):63-72)。
本実験は、インテグリン/フィブロネクチンを含む結合(接着)タンパク質にNAC治療が影響を及ぼすかどうかを解明するために実行された。
7μg/mlのヒトフィブロネクチン(Sigma Aldrich Chem Co)が2%ウシ血清アルブミン(BSA)によって30分間、37℃において飽和され、リン酸緩衝生理食塩水(PBS)によって2回洗浄された。その後ジャーカット細胞は37℃で2時間ウェル上に播種され、次いで非接着性細胞が吸引され、ウェルはPBSですすぎ洗いされた。接着性細胞は、2%ホルムアルデヒドによって一晩固定され、エオシンYで30分間染色された。エオシンYは、その後、1%の氷酢酸および50%のエタノールの混合物の添加によって抽出され、540nmにおいて吸光度が測定された。
モノクローナル抗−VLA−4(クローンHP1/7)による間接免疫蛍光法およびフローサイトメトリーによって、ジャーカット細胞におけるVLA−4抗原発現が検出された。細胞の染色は標準的な手順に従って実行され、フローサイトメトリー解析は、FACS-Caliburサイトメトリー(Becton Dickinson)を用いて実行された。
NACは、フィブロネクチンへのジャーカット細胞の接着性を増大させる。対照またはNAC処置された(5mM、2時間)ジャーカット細胞を用いて接着アッセイが実行された。エオシンYによって染色した後、細胞の写真が撮られた。NAC治療後における、抗−VLA−4抗体による細胞のインキュベーションは、接着を抑制した。その後、細胞接着は、結合タンパク質コートされたプレートに対して付着された細胞をエオシンYで染色し、マイクロプレートリーダーの吸光度を読み取ることにより、定量化された。図1に示されるように、NAC前処理は、細胞接着をほぼ35%増大させた。抗−VLA−4により無効化された効果であった。
本実験は、インテグリンおよびフィブロネクチン遺伝子発現にNAC治療が影響を及ぼすかどうかを、Affymetrix GeneChipプラットフォームを使用した遺伝子発現解析によって解明するために実行された。
マウスにおいて、NACは胚着床を支援することが確認された。動物モデルにおいては、マウスにオキシトシンが与えられた場合、胚着床率は減少した。NACは、投与量に依存して、オキシトシン治療されたマウスにおける着床率を回復させた。これは、胚着床におけるオキシトシンの関与に対する証拠を与えた。
NACが子宮の収縮を減少させることを示す動物モデル
実験計画
動物達は22℃に保たれ、各ケージ当たり3匹収容され、自由に餌を与えられた。日常の膣スミアの調査によって決定された、発情期にある未交尾のウィスター系ラット(200−280g)から単離された子宮が使用された。
硫酸プロタミン(PS)およびNACが純水中に溶解された。
全てのラットは頸椎脱臼によって犠牲とした。子宮角が切除され、注意深くきれいにされ、De Jalon液(NaCl 154mM、KCl 5.6mM、CaCl2×2H2O 0.41mM、NaHCO2 5.9mM、および、グルコース 2.8mM)を含み、37℃において95%の酸素および5%の二酸化炭素で通気された、体積10mlのオルガンバス中に1gの張力下で垂直に取り付けられた。(45分間の)平衡期間の後、子宮が(自発的またはカルシウムイオンにより誘起された)安定な収縮を示した時、インドメタシン(1μg/ml)がPSの前に加えられた。10分後、収縮の完全な停止が観察されるまで、増加する濃度のPSが加えられた。
子宮収縮に対する治療の複数の効果が、対照、非処置、および収縮条件に対するパーセントとして計算された。各データ値は、平均±SEMとして表されている。複数のグループ間の差は、2要因の分散分析によって解析された。EC50値が、1要因の分散分析を使用して比較された。
PSは、投与量に依存した、自発的に活動する子宮の弛緩を引き起こした。NAC前処理(1μg/ml)は、PSにより誘起された弛緩を増大させた。PSはまた、投与量に依存した、カルシウムイオンにより誘起された子宮収縮の弛緩を引き起こした。NAC前処理は、子宮収縮のPSにより誘起された弛緩を調節し、PSにより誘起された弛緩の曲線を変化させた。
N−アセチル−L−システイン(NAC)は、良く知られた低分子量の調合薬であり、次の化学式を有する。
本発明者達は、最近、N−アセチル−L−システイン(NAC)が、上皮性起源の癌細胞に対して著しい抗増殖効果を持つことを見出した(Cell Death and Differentiation 2005, 12(10): 1285-1296)。
腺がん細胞株および原発性正常角化細胞に対するNAC治療の研究(Cell Death and Differentiation 2005, 12(10): 1285-1296)から、抗増殖−分化効果の誘起に対して効果的なNACの投与量は変化すること、および、細胞のタイプに依存することが結論された。従って、原発組織は増殖の完全な阻止を観察するために必要とされる効果的なNAC濃度に影響を及ぼし、それぞれの組織に対して決定されねばならない。さらに、NACの投与量はまた、細胞悪性度にも関連した。詳細に言うと、正常細胞は、増殖することを停止し、分化を開始するために低い投与量を必要としたのに対し、特徴的な予後不良を有する癌細胞は、より高いNAC濃度を必要とした。
1)その他の現在の臨床治療に従ったものであり、望ましくない副作用が無いと考えられる一日当たりのNACの投与量。
2)長期にわたる治療後のNAC血漿レベルにおける減少が報告されている(Pendyala L, Creaven PJ. Cancer Epidemiol Biomarkers Prev. 1995; 4:245-51)ことを前提として、2ヶ月またはそれよりも長い間の治療における最適な生物学的反応に対して、各週の約半分に及ぶ治療の停止が考慮された。
本発明に従った医薬組成物は、薬学分野の当業者にはそれ自体が知られている態様で調製されてよい。組成物は、本発明に従った有効量のNAC、並びに、活性成分に対する賦形剤または媒体として働く適切な担体または賦形剤を含んでよい。そのような担体または賦形剤は、当該分野で知られたものである。医薬組成物は、好ましくは静脈内投与用である。錠剤、カプセル、座薬、懸濁液、シロップ、等のようなその他の形態も考えられる。本発明は、有効な投与量を得るために、NAC調製の薬学的品質の厳密な評価を必要とする。従って、ブランドのまたは保証されたジェネリック調製物が使用されねばならない。NACは安定な分子ではなく、その活性なチオール残基が酸素、光、およびその他の照射によって容易に酸化され得る。そうすると有効な投与量に到達しないであろう。従って調製物は、好ましくは、溶解の間に水から酸素を部分的に除去するのに役立つ炭酸水素ナトリウムとともに、溶解性の錠剤中で光から保護される。
IVF治療に関連した投与
胚移植前のNACの臨床適用のまさに最初のケースが、最近文書に記録された。総数8個の良好な品質の胚が関与する5回の胚移植を以前に受けた42歳の患者に対して、50mg/kgの濃度でNACが投与された。胚移植の直前に、超音波によって、増大した子宮収縮活動の存在が確認された。50mg/kg体重のNACが、60分間にわたる静脈への点滴で投与された。60分間にわたる50mg/kg体重のNACの投与の終了から60分経って、胚移植が実行された。点滴を接続する前および胚移植の直前の両方において、経膣サジタルスキャン記録が実行された。これにより、3分間当り10回の収縮から3分間当り5回の収縮へと子宮収縮が減少し、且つ、それらの振幅の明らかな減少を記録したことが確認できた。治療の成功は、健康な娘の出産によって確認された。
以下は、15人の患者に対してNACのパルス型投与を与えた結果であり、そのうち8人が妊娠した。
一実施形態において本発明は、IVF治療に関連して、1−3日間にわたる1日1回の50−150mg/kg体重の間のNACの静脈投与用に、N−アセチル−L−システイン(NAC)を含む医薬組成物を提供する。
(項目1)
N−アセチル−L−システインをそれ自体で含む、または、少なくとも(ii)セレノメチオニンの形態のセレン、(iii)メラトニン、および、生理学的に許容されるそれらの塩の少なくとも一つと共に含む医薬組成物を投与する段階を備える、IVFに関連した哺乳類の治療方法。
(項目2)
上記哺乳類に対し、N−アセチル−L−システインを含む医薬組成物を、50から150mg/kg/日の間のN−アセチル−L−システインの投与量で1−3日間にわたり静脈内投与する段階を備える、項目1に記載の哺乳類の治療方法。
(項目3)
哺乳類に対し、N−アセチル−L−システインを含む医薬組成物を、30から45mg/kg/日の間のN−アセチル−L−システイン投与量で1−3日間にわたり経口で投与する段階を備える、IVFに関連した哺乳類の治療方法。
(項目4)
IVF治療に先立って、3ヶ月またはそれ以上の期間にわたり、哺乳類に対し、N−アセチル−L−システインを含む医薬組成物を、600mgの投与量で3日間連続して経口で投与し、続いて4日間治療無しとする、経口で投与する段階を備える、哺乳類の治療方法。
(項目5)
上記医薬組成物は、50mg/kgの投与量で1日に投与される、項目2に記載の方法。
(項目6)
上記医薬組成物は、IVF治療が実行される日の朝に投与される、項目5に記載の方法。
Claims (7)
- N−アセチル−L−システイン(NAC)をそれ自体で含み、または、少なくとも(ii)セレノメチオニンの形態のセレン、(iii)メラトニン、および、生理学的に許容されるそれらの塩の少なくとも一つと共に含み、哺乳類の雌の体外受精(IVF)治療において使用される医薬組成物であって、
1−3日間にわたり、50−150mg/kg体重の投与量のNACが1日1回静脈内投与されるように、または、30−45mg/kg体重の投与量のNACが1日1回経口で投与されるように、あるいは、総継続期間3ヶ月またはそれ以上にわたり、600mgのNACが1週間のうち3日間連続で1日3回経口で投与され、続いて4日間薬剤投与されないように、IVF治療に先立って用いられる、
医薬組成物。 - 1−3日間にわたり、50−150mg/kg体重の投与量で1日1回静脈内に投与されるように、または、30−45mg/kg体重の投与量で1日1回経口で投与されるように用いられる、N−アセチル−L−システインを含み哺乳類のIVF治療において使用される請求項1に記載の医薬組成物。
- 前記医薬組成物は、IVF治療に関連して、1−3日間にわたり、1日1回、N−アセチル−L−システインの投与量が50−150mg/kg体重の間である静脈投与用である、N−アセチル−L−システインを含み哺乳類のIVFにおいて使用される請求項1に記載の医薬組成物。
- 3ヶ月またはそれ以上にわたり、600mgの投与量のN−アセチル−L−システインが1週間のうち3日間連続で経口で投与され、続いて4日間薬剤投与されないように、IVF治療に先立って用いられる、N−アセチル−L−システインを含み哺乳類のIVFにおいて使用される請求項1に記載の医薬組成物。
- 1−3日間にわたる経口投与用の投与量が30−45mg/kgであるように用いられる、N−アセチル−L−システインを含む請求項1に記載の医薬組成物。
- IVF治療と同じ日に投与されるように用いられる、50mg/kgの投与量のN−アセチル−L−システインを含む請求項1に記載の医薬組成物。
- IVF治療の1時間前に投与されるように用いられる、請求項6に記載の医薬組成物。
