前記サイトカインの1つ以上の発現レベルが基準値よりも増加していることが、患者が高度に曝露された持続的血清陰性患者であることを示す方法に関する。
本明細書で使用される「AIDS」という用語はHIV感染症の症候性期を指し、後天性免疫不全症候群(一般的にはAIDSとして公知である)および「ARC」または「AIDS関連症候群」の両方を含む。Adler M,et al.,Brit.Med.J.1987;294:1145−1147参照。AIDSの免疫学的および臨床的な徴候は当技術分野で周知であり、例えば、免疫不全に起因する日和見感染および癌が挙げられる。
1.研究群
単一施設(Hospital Universitari Germans Trias i Pujol,Badalona,Spain)において、コントローラ群および非コントローラ群を含むHIV血清陽性患者、ならびに高度に曝露された持続的血清陰性(HEPS)被験体を募集した。HIVコントローラは、抗レトロウイルス治療を行わない場合のウイルス負荷が2,000コピー未満/mLであり、少なくとも過去3年間のCD4+数が細胞400個超/mm3である被験体と定義した。非コントローラは、血漿RNAが50,000HIV RNAコピー超/mLであり、CD4+数が細胞300個未満/mm3である未治療個体と定義した。HIV感染急性期に同定された高ウイルス負荷の被験体が非コントローラ群に含まれていたことを確認するために、十分に発展したウエスタンブロット分析を実施した。
密度勾配遠心分離(Leucosep tubes)によって、抗凝固処理した新鮮全血から末梢血単核細胞(PBMC)を分離した。
RPMI培地+10%ウシ胎児血清(FBS)+抗生物質を含む48ウェルフラットプレート中で、単離した新鮮PBMC(細胞500,000個/ウェル)をサイトカイン細胞内染色の場合には6時間培養し、またはサイトカイン上清検出の場合には5日間培養した。各配列について最終濃度2mg/mLのp24トグルペプチドで細胞を刺激し、インキュベート(37℃、5%CO2)した。抗CD3+抗CD28磁気ビーズを陽性対照として使用し、RPMIを陰性対照として使用した。
トグルペプチド、抗CD3/CD28磁気ビーズまたはPBS1×によって新鮮PBMC(細胞500,000個/ウェル)を刺激し、GolgiStop(Becton−Dickinson Labware Inc.,Franklin Lakes,NJ,US)と共にCD49dおよびCD8抗体(Becton−Dickinson Labware Inc.,Franklin Lakes,NJ,US)を使用して6時間同時刺激した。この期間中に抗CD107aも追加した。6時間のインキュベーション後、培養物をサイトカイン細胞内染色まで冷蔵庫内で一晩維持した。ルーチン分析のために、フローサイトメトリによって、CD3、CD4、CD8およびTh1、Th2およびTh17サイトカインの発現について、透過処理した固定細胞(Invitrogen Corp.,Carlsbad,CA,US)を評価した。ペプチド刺激したT細胞の複数の機能を評価するために、以下の抗体を様々な組み合わせで使用した:CD3−PE、CD4−APC、CD8−V500、CD14−V450、CD19−V450、LiveDead、CD107a−PeCy5、IFN−c−FITC、IL−2−FITC、TNFa−FITC、MIP1b−FITC、IL4−PECy7、IL10−PECy7およびIL17−PECy7(Becton−Dickinson Labware Inc.,Franklin Lakes,NJ,US)。8色パラメータを検出するように設定されたLSR II機器(Becton−Dickinson Labware Inc.,Franklin Lakes,NJ,US)で細胞を収集し、FACSDivaソフトウェアを使用して分析を実施した。ゲーティングした後、あらゆる特定の機能組み合わせについて、陰性対照チューブで検出されたバックグラウンド応答を、刺激した試料で検出された応答から差し引いた。
以前に記載されているように、IFN−γELISpotアッセイを実施した。Harlow E,Lane D,“Antibodies:A Laboratory Manual”(Cold Spring Harbor Laboratory Press,Cold Spring Harbor,NY,US,1988)参照。簡潔に言えば、抗ヒトIFN−γmAbを用いて、ELISpotプレート(Millipore Corp.,Bedford,MA,US)を4℃で一晩コーティングした。1%FCSを補充した100μLのPBSを用いて、プレートをブロッキングした。PBMC(細胞100,000個/100μL/ウェル)を0.2mg/mLのSL9およびGL9の刺激と共に16時間インキュベートした。PHA(5μg/mL)を陽性対照として含めた。ビオチンコンジュゲート抗ヒトIFN−γと共に室温で1時間インキュベートすることによって、捕捉IFN−γを分泌部位で検出した。PBSで6回洗浄した後、ALPコンジュゲートストレプトアビジンを室温で45分間インキュベートした。PBSでよく洗浄した後、NBTおよび基質を含むTRIS塩化物を追加した。30分後、Tweenを用いて反応を停止させた。
Hospital Universitari Germans Trias i Pujol,BadalonaおよびBCN Checkpoint community center,Barcelona,Spainにおいて、コントローラ(n=10)および非コントローラ(7)を含むHIV血清陽性被験体(n=17)、高度に曝露された持続的血清陰性(HESN)被験体(MSM、男性と性交渉を持った男性、n=8;血清不一致パートナを持つ異性愛者、n=3)およびHIV非曝露個体(n=4)を募集した。HIVコントローラは、抗レトロウイルス治療を行わない場合のウイルス負荷が2,000コピー未満/mLであり、少なくとも過去3年間のCD4+数が細胞400個超/mm3である被験体と定義した。非コントローラは、血漿RNAが50,000HIV RNAコピー超/mlであり、CD4+数が細胞300個未満/mm3であるHAART未治療個体と定義し、十分に発展したウエスタンブロットによって、これらが、HIV感染急性期に同定された高ウイルス負荷の被験体ではないことを確認した(表I)。男性と性交渉を持った男性(MSM)の高リスクコホートからHESNを同定し、Checkpoint(バルセロナのMSMコミュニティセンター)で月3回観察した。この研究は地方研究倫理委員会によって承認されており、すべての参加者が書面によるインフォームドコンセントを提出した。
すべてのアッセイについて、単離した新鮮PBMCを静脈穿刺の4時間以内に使用した。フローサイトメトリ研究およびFlowCytomix研究のために、RPMI培地+10%FBS+抗生物質を含む48ウェルフラットプレートに、1ウェル当たり500,000個の細胞をサイトカイン細胞内染色(ブーストフローサイトメトリのセクション参照)の場合には6時間にわたって追加し、またはサイトカイン上清検出(FlowCytomixのセクション参照)の場合には5日間にわたって追加した。各配列について最終濃度14mg/mlの個々のp24トグルペプチドで細胞を刺激し、対応するアッセイに使用するまで37℃、5%CO2でインキュベートした。抗CD3+抗CD28磁気ビーズ(Invitrogene)またはPMA(またはホルボール12−ミリステート13アセテート)(10ng/ml)+イオノマイシン(Io)(1uM)を陽性対照として使用した。ウイルスペプチド刺激のためのさらなる陽性対照として、それぞれCD8T細胞およびCD4T細胞刺激のためのEBV MHCクラスI拘束性ペプチドプール(EBVプール1)およびEBV MHCクラスII拘束性ペプチドプール(EBVプール4)を最終濃度10ug/mlで使用した。
5日後に回収した培養上清中で、製造業者の説明書にしたがってフローベースの手法を使用して、13種のサイトカインを検出した。ヒトTh1/Th2/Th9/Th17/Th22 13−plex Kit FlowCytomix(eBioscience)は、フローサイトメトリによってヒトIFNγ、IL−1β、IL−2、IL−4、IL−5、IL−6、IL−9、IL−10、IL−12p70、IL−13、IL−17A、IL−22およびTNFαを定量的に検出するためのビーズベースの分析物検出系である。簡潔に言えば、検出するべき様々なサイトカインに対して特異的に反応する抗体を用いて、様々なサイズおよびスペクトルアドレスの蛍光ビーズをコーティングする。各サイトカインに対するコーティングビーズミックスを試料と共に室温で2時間インキュベートする。ビオチンコンジュゲート二次抗体を追加し、室温で1時間インキュベートしてから、ストレプトアビジン−フィコエリトリンを追加して、ビーズサイズおよびその発光スペクトルに基づいて様々なサイトカインを検出および定量する。抗原特異的応答のために、全患者にわたる中央レベル(pg/ml)の各サイトカイン非刺激培養物を陽性応答のカットオフとして適用した。
ブーストフローサイトメトリ法は、多機能性研究について以前に記載されている細胞内サイトカインプロトコール(Lamoreaux,L.2006.Nat Protoc.1:1507−1516)をいくつか修正したものに基づいている。簡潔に言えば、GolgiStopおよび抗CD107a抗体(Beckton Dickinson)の存在下で、個々のトグルペプチドならびにCD49dおよびCD28抗体(Becton Dickinson,Mountain View,CA)を用いて、新鮮PBMC(細胞500,000個/ウェル)を6時間刺激した。6時間のインキュベーション後、培養物を細胞内サイトカイン染色まで4℃で一晩維持した。細胞を洗浄した後、生存染色用のアミン反応性バイオレット色素(LIVE/DEAD(登録商標)Fixable Dead Cell Stain Kit,Invitrogene)を追加してから、それぞれCD19−V450およびCD14−V450(Becton Dickinson)を使用してBリンパ球および骨髄細胞を排除するために、T細胞マーカー(Becton Dickinson製のCD3 PE、CD4 APCおよびCD8 V500)について細胞を染色した。細胞内サイトカイン染色のために、固定および透過処理工程(Invitrogen製のFix and Perm kit)を行って、続いてFITCコンジュゲート抗Th1サイトカイン抗体(IFN−γ、IL−2、TNFαおよびMIP1β)およびPe−Cy7コンジュゲート抗Th2/17サイトカイン抗体(IL−4(eBioscience)、IL−10およびIL−17(Biolegend))を用いて染色した。8色パラメータを検出するように設定されたLSR II機器(Becton Dickinson)で細胞を収集し、FACSDivaソフトウェアを使用して分析を実施した。一重項のCD14−およびCD19−生細胞のうち、ヘルパーT細胞(CD3+CD4+)およびCTL(CD3+CD8+)をゲーティングし、サイトカイン分泌パターン(Th1様および/またはTh2/17様)+脱顆粒能(CD107a+)を決定した(補足図5)。ゲーティングした後、あらゆる特定の機能組み合わせについて、陰性対照培養物で検出されたバックグラウンド応答を、刺激した試料で検出された応答から差し引いた。いくつかのサイトカインを同じチャネルに含めることによりシグナルだけでなくバックグラウンドも高まることは、非常に限定的なカットオフの適用を示唆している。Roederer et al(Roederer,M.2011.Cytometry A.doi:10.1002/cyto.a.21015)によって記載されているように、バックグラウンドを差し引いたデータを閾値化した後に検出され得る応答を陽性シグナルとみなした。
単離した新鮮PBMC(PBMC細胞100,000個/ウェル)を140ulのR10 96ウェルポリビニルプレート(Millipore,Bedford,MA)に追加した。以前に記載されているように(Frahm,N.2007.Jour of Immunol)製造業者の説明書にしたがって、IFN−γMabtechキットを使用した。陽性応答の閾値は、1ウェル当たりのスポットが少なくとも5個であり、「陰性対照ウェルの平均スポット数+陰性対照ウェルの3標準偏差」および「陰性対照ウェルの平均の3倍」のいずれか高い方を超える応答と定義した。
Prism Version 4,GraphPad Prismを使用して、統計分析を実施した。ブーストフローアッセイのバックグラウンド補正後、以前に記載されている分析手順(Roederer,M.2011.Cytometry A.doi:10.1002/cyto.a.21015)にしたがってデータを閾値化した。検出された陽性応答の質の間の差異を検出するために、χ二乗検定を適用した。具体的な比較のために、ノンパラメトリックマンホイットニー検定を示されているように適用した。すべての分析において、0.05未満のp値を統計的に有意であるとみなした。マンホイットニー検定を適用した。χ二乗検定を適用した。すべての分析において、0.05未満のp値を統計的に有意であるとみなした。
HIV血清陽性患者(n=18、コントローラおよび非コントローラ)、および高度に曝露された持続的血清陰性被験体(n=8)を上記一般的な手順にしたがって分析した。
PMA+IoまたはEBV由来の抗原を陽性対照として使用するか(図6および7参照)、またはp17のような様々なHIV由来の抗原を使用する以外は実施例1に示されているのと同様の実験を繰り返した。ポリクローナル刺激PMA+Ioを使用することにより、ペプチド特異的刺激よりも高い陽性応答がHEPSで発生する。しかしながら、EBVペプチドのプールを使用することにより、陽性応答を検出することもできる。また、ブーストフローサイトメトリ法を使用することによって、本発明者らは、EBVペプチドのプールで刺激した後に検出された応答と同様に、HIVペプチドのプールに対する応答をHESN個体(血清不一致カップル)で検出することができた。最後に、本発明は、CD4(MHCクラスIIペプチド)およびCD8(MHCクラスIペプチド)における特異的ペプチド応答、ならびにこれまでに観察されなかったHESNにおけるHIVに対する捕獲応答を決定することを可能にする。
Claims (24)
- 被験体における病原体に対して特異的なT細胞応答をアッセイする方法であって、
i)被験体由来のT細胞を含む試料を、病原体由来の抗原を含む組成物と接触させ、
ii)当該試料中のT細胞によって産生される複数のサイトカインのレベルをブーストフローサイトメトリによって決定すること
を含んでなり、
前記ブーストフローサイトメトリが、フローサイトメトリによって同じ蛍光チャネル内のいくつかのサイトカインを同時に検出することを含むものである、方法。 - 前記抗原を含む組成物が、抗原または抗原ライブラリである、請求項1に記載の方法。
- 前記病原体由来の抗原または抗原ライブラリが、病原体のポリペプチド由来のペプチド変異体またはペプチド変異体ライブラリである、請求項2に記載の方法。
- 前記ペプチド変異体ライブラリが、トグルペプチドである、請求項3に記載の方法。
- 前記病原体がHIVまたはEBVである、請求項1〜4のいずれか一項に記載の方法。
- 前記ペプチド変異体が、GagまたはNefポリタンパク質に由来するものである、請求項5に記載の方法。
- 前記ペプチド変異体が、p24またはp17に由来するものである、請求項6に記載の方法。
- 前記複数のサイトカインが、Th1応答に特徴的なサイトカイン、Th2応答に特徴的なサイトカイン、およびその両方からなる群から選択されるものである、請求項1〜7のいずれか一項に記載の方法。
- 前記Th1応答に特徴的なサイトカインが、IFN−γ、TNF−α、MIP1−β、IL−2、IL−12およびIL−1Bからなる群から選択されるものである、請求項8に記載の方法。
- 前記Th2応答に特徴的なサイトカインが、IL−4、IL−5、IL−6、IL−10およびIL−13からなる群から選択されるものである、請求項8に記載の方法。
- CD4+細胞および/またはCD8+細胞において、Th1応答に特徴的なサイトカイン、Th2応答に特徴的なサイトカインまたはその両方を決定する、請求項1〜10のいずれか一項に記載の方法。
- Th9、Th17またはTh22応答に特徴的なサイトカインのレベルを決定することをさらに含んでなる、請求項8〜11のいずれか一項に記載の方法。
- T細胞を含有する試料がPBMC調製物である、請求項1〜12のいずれか一項に記載の方法。
- 高度に曝露された持続的血清陰性患者を同定するための方法であって、
i)前記患者のT細胞を含有する試料を、HIVまたはEBVペプチド組成物と共にインキュベートし、
ii)IL−1b、IL−5、IL−6、IL−9、IL−10、IL−12p70、IL−13、IL−17およびIL−22からなる群から選択される複数のサイトカインのレベルをブーストフローサイトメトリによって決定することを含んでなり、
前記ブーストフローサイトメトリが、フローサイトメトリによって同じ蛍光チャネル内のいくつかのサイトカインを同時に検出することを含むものであり、
前記サイトカインの複数の発現レベルが基準値よりも増加していることが、患者が高度に曝露された持続的血清陰性患者であることを示すとする、方法。 - 前記HIVペプチド組成物が、Gag p24 HIVまたはGag p17 HIVまたはNefタンパク質由来のペプチドを含むものである、請求項14に記載の方法。
- 前記Gag p24 HIVまたはGag p17 HIVまたはNefタンパク質由来のペプチドが、トグルペプチドである、請求項15に記載の方法。
- 前記T細胞を含有する試料が、PBMC調製物である、請求項14〜16のいずれか一項に記載の方法。
- 各サイトカインの基準値が、HIV感染患者由来のT細胞を含有する試料中の前記サイトカインの発現レベルである、請求項14〜17のいずれか一項に記載の方法。
- 高度に曝露された持続的血清陰性患者を同定するための方法であって、
i)T細胞を含む前記患者の試料を、HIVまたはEBVタンパク質由来のペプチド組成物と共にインキュベートし、
ii)IFN−γ、TNF−α、MIP1−βおよびIL−2からなる群から選択される複数のサイトカインのレベルを測定することによって、Th1様応答を決定し、IL−4およびIL−10からなる群から選択される複数のサイトカインのレベルを測定することによって、Th2様応答を決定し、または、Th17(IL−17)のレベルを測定することによって、Th17応答を決定することを含んでなり、
前記複数のサイトカインのレベルがブーストフローサイトメトリによって測定され、前記ブーストフローサイトメトリが、フローサイトメトリによって同じ蛍光チャネル内のいくつかのサイトカインを同時に検出することを含むものであり、
前記細胞におけるTh1様応答、Th2様応答またはTh17応答が基準値よりも増加していることが、患者が高度に曝露された持続的血清陰性患者であることを示すとする、方法。 - CD4およびCD8Tリンパ球において、ブーストフローサイトメトリによって、サイトカインレベルの決定を行う、請求項19に記載の方法。
- 前記HIVタンパク質が、p24またはp17またはNefである、請求項19または20に記載の方法。
- 前記ペプチド組成物が、トグルペプチド混合物である、請求項19〜21のいずれか一項に記載の方法。
