Description
本願は2012年8月23日に出願された米国仮出願第61/692,331号(その全体が引用することにより本明細書の一部をなすものとする)の優先権の利益を主張する。
細胞培養。
これらの研究で使用した全ての細胞株を単一のタンデムリピート分析により確認した。BCK4細胞は、胸水に由来する乳がん細胞株である。BCK4細胞及びMCF7細胞をMEM、5 % FBS、NEAA、インスリン及びペニシリン/ストレプトマイシン中で成長させた。ZR75細胞をHEPES及びL-グルタミンを添加した同じ培地中で成長させた。T47D細胞を10 % FBS、L-グルタミン、ペニシリン/ストレプトマイシンを添加したDMEM中で成長させた。LNCaP細胞をRPMI、5 % FBS及びペニシリン/ストレプトマイシン中で成長させた。全ての細胞を5 % CO2を含む37℃のインキュベーターにおいて成長させた。MDA-MB-453及びMDA-kb2(AR依存性MMTV-ルシフェラーゼレポーター遺伝子コンストラクトを安定的に発現するMDA-MB-453の誘導体、ATCC)を、10 % FBS(Gibco)及びペニシリン/ストレプトマイシンを含有するLeibovitz's L-15培地(Invitrogen)中で培養した。チミジンキナーゼ、GFP及びルシフェラーゼのトリプルフュージョンをコードするレトロウイルスSFG-NES-TGLベクターの安定的発現によりMCF7-TGL細胞を作出した。融合タンパク質を発現する細胞をGFPについて分類した。Identifiler Kit(Applied Biosystems)を使用して、DNAプロファイリングにより全ての細胞株の同一性を確認した。
MCF7細胞又はBCK4細胞(それぞれ、1ウェル当たり1000細胞及び10000細胞)を、96ウェルプレート内のチャコール処理血清(CSS)を含有するフェノールレッド不含培地にて平板培養した。平板培養から24時間後に細胞をビヒクル対照(エタノール+DMSO)、10 nMエストラジオール(E2、Sigma)、10 nMジヒドロテストステロン(DHT, Sigma)、1 μMビカルタミド(ChemPacific)、10 μMエンザルタミド(Medivation)又は上記の組合せで処理した。細胞を3日目に再処理した。MTSアッセイ(製造業者の指示書により、Roche)を使用して増殖を評価した。全ての値を処理の日(0日目)に対する倍率変化として報告した。
齧歯動物の餌で輸送されたエンザルタミド(すなわち、MDV3100)を用いるMCF7実験を行った。エンザルタミドを経口強制飼養により輸送したMCF7実験及びMDA-MB-453実験を行った。簡潔には、IVIS画像化の目的で、チミジンキナーゼ、GLP及びルシフェラーゼのトリプルフュージョン(SFG-NES-TGLレトロウイルスベクター)を安定に発現する106個のMCF7-TGL細胞をMatrigel(BDBiosciences)と混合し、雌の卵巣を摘出した胸腺欠損nu/nuマウス又は非肥満糖尿病の(NOD)/SCIDマウス(Taconic)の第4鼠径部乳房脂肪体に注入した。腫瘍を注入する際、E2ペレット(60日放出、1.5 mg/ペレット、InnovativeResearch of America)又はDHT(8 mg/ペレット、サイラスティック管に充填して密封した)を首の後ろの皮下(SQ)に埋め込んだ。腫瘍量をin vivo画像化システム(IVIS)又はノギスによる計測を使用して評価した。一旦腫瘍が定着すると、IVIS又はノギスで計測した全腫瘍量に基づいてマウスを群に対応させた。タモキシフェンを受ける群は、皮下に埋め込まれたペレット(90日放出、5 mg/ペレット、InnovativeResearch of America)を有した。マウスに、餌にエンザルタミドを混ぜて(およそ50 mg/kgの1日量)投与するか、又は経口強制飼養(10 mg/kg/日又は25 mg/kg/日、Medivation Inc)により投与した。エンザルタミドを、餌1グラム当たり0.43 mgで粉に挽いたマウスの餌(Cat # AIN-76、ニュージャージー州ニューブルンスウィックのResearchDiets Inc)と混合した。食餌に放射線照射を行い、使用まで4℃で保管した。対照群のマウスには、エンザルタミドを含まない粉に挽いた同じマウスの餌が与えられた。全ての動物は、調査の全期間に亘り、1日当たり平均3.5 gの食物摂取量で餌又は対照餌と混合したMDV3100を自由に利用できた。動物ケージの食餌を1週間に2回替えた。水及び食餌は自由摂取で調製した。屠殺の2時間前に、マウスに50 mg/kg BrDU(Sigma-Aldrich)を腹腔内注射した。マウスをCO2窒息の後に頸椎脱臼により安楽死させ、血液、腫瘍、結腸、子宮及び乳腺を採取した。
Graphpad Prism 5.0ソフトウェア及びSAS統計ソフトウェア(バージョン9.1)を使用して統計を行った。AR染色とER染色との間の相関について検定するため、スピアマン相関を使用した。2つの群を比較した場合、ステューデントのt検定(正規分布データに対し)又はウィルコクソン順位和(非正規分布データに対し)を使用した。対応のあるt検定又はウィルコクソン符号順位検定を使用して、対応のあるデータを比較した。複数の群の比較には、ボンフェローニの多重比較検定補正(正規分布したデータ)を用いるANOVA、又はダンの多重比較検定補正(非正規分布データ)を用いるクラスカル−ウォリス検定を使用した。各時間点について算出された倍率変化が独立した計測であったin vitroデータに対し、異なる日及び治療群に関する平均倍率変化を比較する二要因母数ANOVAを使用した。上記モデルにて一定期間(fixed terms)の間の相互作用を検定した。ボンフェローニt検定を使用する事後検定を行って、どの群が互いと有意に異なるかを判定した。統計学的検定は両側性であり、0.05未満のp値を統計学的に有意とした。
組入れ基準及び治験計画は他に記載される(8、11)。簡潔には、ER+乳がんを伴う女性を無作為化第II相臨床研究に登録し、エキセメスタン単独(毎日25 mg)又はタモキシフェンと組み合せたエキセメスタン(毎日20 mg)を手術の前4カ月に亘り与えた。上記治験に組み入れられた女性は、新たに診断されたステージII/III、T2〜3のがんを伴い、閉経後であった。治療前にコア針生検材料を取り、最後の切除手術に由来する腫瘍片を分析用に採取した。「応答者」の基準は、やや有効(minor response)から完全緩解(complete response)の範囲であり、一方、「非応答者」は安定しているか、又は進行性の疾患を有した。
この研究は、1977年〜1993年にマサチューセッツ総合病院(MGH)にて乳がんと診断され、アジュバントのタモキシフェンを用いて治療されて1998年までMGHにおいて追跡された221名の女性患者のサブセットを含む。診断時の患者の年齢、腫瘍サイズ、腫瘍グレード及び結節の状態、並びに患者が治療開始から60カ月以内にタモキシフェン治療に失敗したかどうかに基づいて、このデータセットの保管資料のホルマリン固定パラフィン包埋腫瘍を対応させた。タモキシフェン治療に関するデータが完全ではなかったため、治療開始の60カ月以内に失敗の記録を有しなかった患者を失敗しなかったとし、60カ月以内に失敗の記録のある患者を失敗と分類した。ペアリングが完了すると、一部の残っているホルマリン固定パラフィン包埋腫瘍スライドは、カットスライドが古いためAR染色について評価できないと判定された(合計10症例、各群5症例)。上記症例を排除したことにより、分析用の各群に合計38の症例が残った。したがって、これは部分一致を表し、症例対照研究として分析した。
一連のキシレン及びエタノール中でスライドを脱パラフィン化し、10mMクエン酸バッファー(pH 6.0)(BrdU、Ki67)又はpH 9.0の10mMTris/1mM EDTAバッファー(AR、ER、カスパーゼ3)のいずれかにおいて熱により抗原を回復した。BrdU用の組織を2 NのHCl中でインキュベートした後、抗原回復の後0.1 Mのホウ酸ナトリウムでインキュベートした。使用した抗体は、ARクローン441、及びERクローン1D5(Dakocytomation)、切断型カスパーゼ3(Cell Signaling Technology)、Ki67(Santa Cruz sc-15402)及びBrdD(BD Biosciences)であった。Envision-HRP(Dakocytomation)を抗体の検出に使用した。製造業者の指示書により、ApopTagPlus Peroxidase In SituApoptosis Detection Kit(Millipore)を使用してアポトーシスに対してTUNEL染色を行った。AR及びER染色は病理学者により評価され、スコアは強度に陽性細胞パーセントを掛けたものとして報告されるか、又はタモキシフェン治療コホートの場合は細胞陽性パーセントに基づくKM曲線であるが、結果は同様であり、強度に陽性パーセントで掛けた場合であっても有意であった。異種移植片研究におけるBrdU及びTUNEL染色のため、SPOTInsight Mosaic 4.2カメラ及びソフトウェア(ミシガン州スターリングハイツのDiagnosticInstruments, Inc.,)を備えたOlympus BX40顕微鏡(ペンシルベニア州センターバレー)を使用して、各異種移植片腫瘍の3つの別々の200倍視野を撮影した。各画像について色彩の閾値(核の陽性染色に対するRGB、及び全核に対するHSB)をImageJ(米国国立衛生研究所)を使用して手動で調整し、その閾値により生じた粒子を全領域について分析した。RGB領域をHSB領域で除し、各画像について100倍した。核アンドロゲン受容体の分析用に、切断型カスパーゼ3及びKi67のスライドをAperio Scan ScanScope XTにおいて20倍でスキャンした。乳腺腫瘍組織を、Aperio's Scanscopeソフトウェアのネガティブペンツールを使用して、各腫瘍及び摘除した腫瘍の壊死領域について別々にトレースした。核アルゴリズムを利用して、Ki-67及びアンドロゲン受容体染色スライドに対する陽性細胞パーセントを計測し、データをエクスポートした。切断型カスパーゼ3染色スライドを変型Positive Pixel Countアルゴリズムを使用して分析した。
全細胞タンパク質抽出物(50 μg)を変性し、SDS PAGEゲル上で分離してPVDF膜に転写した。TBS-T中の3% BSAにおいてブロッキングした後、4℃で一晩、膜をプローブした。利用した一次抗体は、ERα(Neomarkers Ab-16、1:500希釈)、AR(UpstatePG-21、1:500希釈、又はSanta Cruz N-20、1:1000希釈)、GAPDH(SantaCruz V-18、1:1000希釈)、Topo 1(Santa Cruz C-21、1:1000希釈)及びα−チューブリン(SigmaによるクローンB-5-1-2、1:15000希釈)を含む。適切な二次抗体とのインキュベーションの後、Western Lightning Chemiluminescence Reagent Plus(Perkin Elmer)を使用して結果を検出した。
MDA-MB-453に対し、Current Protocols in Cell Biology(57)に記載されるように細胞分画分析を行った。簡潔には、MDA-kb2細胞を氷冷ダルベッコリン酸緩衝生理食塩水(DPBS)(pH 7.4)で洗浄し、遠心分離を使用してペレット化して2容量の氷冷NSB(10 mM Tris·Cl(pH7.4)、10 mM NaCl、2 mM MgCl2、1×プロテアーゼ阻害剤)中に再懸濁した。初期容量の15倍まで氷冷NSBを用いて容量を調整し、氷上で30分間インキュベートした。最終濃度0.3 %までNP-40を添加することにより、細胞質画分を得た。0.4 mmのクリアランスダウンス型ホモジナイザーを使用して核及び細胞質を分離した。遠心分離後、細胞質画分を含有する上清を回収した。核画分を含有するペレットを10 mM MgCl2を含有する250 mMショ糖溶液に再懸濁し、その後、核画分のもとで5 mM MgCl2を含有する880 mMのショ糖に対して1容量を添加した。その後、ショ糖クッションによる遠心分離によって核を精製した。MCF7には、製造業者の指示書によりNE-PER Nuclear and Cytoplasmic Extraction Kitを使用して細胞分画を行った。
光学マイクロプレート内の5 % CSSを添加したLeibovitz's L-15培地中に2×103細胞/cm2でMDA-kb2細胞を播種した。培養の3日後、細胞をEnza(1 μM又は10 μM)で2時間に亘り前処理し、その後、Enzaの存在下で(合計3時間のEnzaによる処理)1 nMのDHTで1時間に亘り共処理を行った。細胞をリン酸緩衝生理食塩水(PBS)で洗浄し、室温にて30分間4 %ホルムアルデヒドで固定し、0.2 %のtriton X-100で透過処理した。その後、1時間に亘り5 % BSAで試料をブロックし、PBS(0.1 % triton)中のARに対する抗体(AR [N20] Santa Cruz sc-815 1:100)と共に一晩インキュベートした。二次抗体である抗ウサギAlexa Fluor 488(1:1000)とのインキュベーションを周囲温度で2時間、2.5 % BSA中で行った。核を30分間DAPI(1 μg/ml)で染色した。60倍対物レンズ及び黄色蛍光タンパク質(YFP)フィルタ(Chroma U-N31040)を使用するOlympus X71蛍光顕微鏡に連結されたQimagingデジタルカメラにより細胞を可視化した。ImageJソフトウェアを使用して、最低48個の細胞でARの核分布(核ARシグナル/全ARシグナルの比)を定量した。
M-Mulv逆転写酵素(Promega)を使用して、1 μgの全RNAからcDNAを合成した。FASN、PRLR及びGCDFP-15には、以下のプライマー:

を使用してSYBRグリーン定量的遺伝子発現分析を行った。PR及びSDF-1には、AppliedBiosystemsからの検証されたプライマー/プローブセット(アッセイID:PR Hs01556702_m1、SDF-1 Hs00171022_m1、18S Hs99999901_s1)を使用してtaqmanリアルタイムPCRを行った。比較Ct法を使用して相対遺伝子発現を算出し、18Sに対して値を正規化した。
MDA-kb2細胞を96ウェルのルミネセンスプレート中に1ウェル当たり5×103細胞で平板培養し、一晩インキュベートした。DMSO中に調製されたEnza(10 μM、1 μM、0.1 μM)及びDHT(10 nM、1 nM、0.1 nM、0.01 nM、0.001 nM)の10倍段階希釈で細胞を処理した。24時間のインキュベーションの後、ルシフェラーゼアッセイシステム(Promega)を用いて発光レベルを求めた。3つの独立した実験を行い、発光値を相対単位(R.U.)として求め、ビヒクルに対して正規化した。値を平均倍率誘導±標準誤差(SE)として表した。
Ricera Biosciences, LLCにより放射性リガンド結合アッセイが行われた。インキュベーションバッファー(10 mM Tris-HCl、0.1 % BSA、10 %グリセロール、1 mM DTT)中、25℃で2時間に亘り、0.1 μM〜100 μMの範囲の濃度で0.5 nM [3H]エストラジオール及び非標識化MDV3100(エンザルタミド)と共にERα及びERβをインキュベートした。MathIQTM(英国のID Business Solutions Ltd.)を使用して非線形最小二乗回帰分析によりIC50値及び%阻害を求めた。
ネオアジュバント内分泌療法に応答性の腫瘍における正の相関。
ネオアジュバントAI療法に応答性の管腔乳がんにおいてAR発現は減少したが、応答しなかった腫瘍においては維持された(8)。ERに対するARの比と臨床応答との間に関係があるかどうかを判定するため、ネオアジュバントAI療法により治療された患者のコホートを調査した。AI応答性腫瘍において、治療前生検材料でARとERとのタンパク質発現に強い正の相関があった(p=0.006、図1A、左)。対照的に、応答しなかった腫瘍では、ARとERとの発現に有意な相関はなかった(p=0.59、図1B、左)。応答性腫瘍におけるAR及びER染色の代表的な画像は、低(上)、中(真ん中)、又は高(下)に関わらず、同様の量の2つの受容体(図1A、右)を実証し、一方、非応答性腫瘍はERより高いレベルのARを有する傾向がある(図1B、右)。応答性及び非応答性の両方の腫瘍からの周辺正常上皮も調査し、AR及びERについて採点した。「周辺正常上皮」の用語は、乳腺腫瘍部位周辺の非腫瘍細胞を指す。
ERと比較して多量のARタンパク質がERを対象とする治療法に対する失敗を予測し得るかどうかを判定するため、転帰データを伴うタモキシフェン治療患者のコホートを調査した。この研究は、1977年〜1993年の間にマサチューセッツ総合病院(MGH)で乳がんと診断され、アジュバントのタモキシフェンを用いて治療されて1998年までMGHにおいて追跡された221名の女性患者のサブセットを含むものであった。診断時の患者の年齢、腫瘍サイズ、腫瘍グレード及び結節の状態に基づいて、タモキシフェンに対して失敗した患者及び失敗しなかった患者を、最初に対応させた。しかしながら、一部の保管資料組織は分解し、分析から排除した(合計10症例、各群5症例)。最適以下の症例のマッチング及び排除の後、分析のため各群に合計38症例が残った。試料選択は部分一致の結果であったため、研究を症例対照研究として大きく分析し、治療失敗と非失敗との間の患者の相違及び腫瘍の特性における相違を判定した。1.3のAR:ER比は、予後良好と予後不良とを最もよく分離する点と判定した。カプラン−マイヤー分析を行って、AR:ER比の状態による失敗率の差を調査した。失敗までの平均時間45+/-23カ月を有するAR:ERが1.3未満の群と比べて、より高いAR:ER比の群は、失敗までの平均時間29+/-4.3カ月とより短い無病生存期間を有した。
この研究は、エンザルタミドが、in vitroでAR+乳がん細胞株(ER+及びER-の両方)のアンドロゲン誘導性増殖を遮断し得るかどうか、またin vivoで腫瘍成長を遮断し得るかどうかを判断するため計画された。データは、ERの状態に関わらず、エンザルタミドがAR+乳がんのアンドロゲン媒介腫瘍成長を阻害するのみならず、ER+/AR+乳がんの前臨床モデルでエストロゲン刺激性腫瘍成長も阻害し得ることを示す。
E2がER+腫瘍において主要なマイトジェンであることから、エンザルタミドがER+/AR+乳がん細胞においてE2媒介増殖に影響を及ぼすかどうかを判定するため研究を行った。エンザルタミドがARに対して高い親和性結合を有する一方、in vitro放射性リガンド結合アッセイは、エンザルタミドがERα又はERβのいずれにも有意に結合しなかったことを示した。しかしながら、エンザルタミドは、in vitroでMCF7細胞及びBCK4細胞の両方のE2誘導性増殖を有意に阻害した。また、エンザルタミドは、2つのエストロゲン応答性遺伝子である、PR及びストロマ細胞由来因子1(SDF-1)(CXCL12としても知られる)のE2誘導性の上方制御を阻害した。他の抗アンドロゲンもE2媒介増殖を阻害するかどうかを判定するため、in vitroでE2媒介増殖に対するビカルタミドの効果を試験した。ビカルタミドは予想通り、MCF7細胞におけるDHT媒介増殖を阻害したが、エンザルタミドとは対照的に、E2媒介増殖を有意に増加した。このE2媒介作用の誘導は遺伝子発現レベルでも検出され、ビカルタミドがPR及びSDF-1 mRNAのE2媒介誘導を増加した。
本明細書で記載される実験は、MCF-7細胞及び胸水から新たに得られた乳がん細胞株(BCK4)の両方において、アンドロゲンがER-/AR+乳がんの増殖を誘導することができるという、以前の報告(31、37〜39)と一致する観察結果を実証する。さらに、実験は、ARアンタゴニストであるエンザルタミドが、in vivoでER+/AR+及びER-/AR+の両方の乳腺腫瘍のアンドロゲン誘導性成長を阻害することを示した。また、ER+乳がんの大多数はAR+(84 %〜91 %)であり(5、40、41)、ARとER及びPRとを共発現する腫瘍を伴う患者は、おそらくかかる腫瘍がより高分化されていることから、3つ全ての受容体に陰性の腫瘍の患者よりも長い無病生存期間を有する(40)。しかしながら、ARは腋窩転移の独立した予測因子であり(41)、リンパ節陽性状態と相関する(42)。どのようにしてリガンド結合ARがER+乳がんの増殖及び成長に影響を及ぼすのか(43)、またこれが、循環エストロゲン量が異なる場合に閉経前の女性と閉経後の女性と、又はタモキシフェン若しくはAIを用いて治療されている乳がんを伴う女性で異なるのかについては、いまだ議論の的となっている。本明細書で開示されるように、ERタンパク質に対するARタンパク質のより高い比率は、ネオアジュバントAI治療に対する応答を欠き、またタモキシフェンを用いて治療された患者におけるより短い無病生存期間の指標である。これらの知見は、AR:ER比が従来のエストロゲン経路を対象とする内分泌療法に対する応答の新規な独立した予測因子であることを示す。また、本発明は、タモキシフェン又はAIを受けている間に再発する患者が、ARを対象とする治療法の良好な候補であることを開示する。

Claims (14)
- 乳がん患者の内分泌療法に陽性に応答する可能性を予測する方法であって、
前記患者の組織試料中に存在するアンドロゲン受容体(AR)及びエストロゲン受容体(ER)の間の比を求めることと、
前記組織試料中のAR及びERの量を比較することと、
を含み、
ARとERとの間の正の相関が抗エストロゲン内分泌療法に対する陽性の応答の可能性の指標である、乳がん患者の内分泌療法に陽性に応答する可能性を予測する方法。 - ARとERとの間の正の相関が、1.3未満のERに対するARの比を有することを含む、請求項1に記載の方法。
- ARとERとの間の正の相関が、0.45〜0.85の範囲のERに対するARの比を有することを含む、請求項2に記載の方法。
- ARとERとの間の正の相関が、0.55〜0.75の範囲のERに対するARの比を有することを含む、請求項2に記載の方法。
- ARとERとの間の正でない相関が、抗アンドロゲン療法に対する陽性の応答の可能性の指標である、請求項1に記載の方法。
- 乳がんの治療を必要とする対象者において乳がんを治療するのに化学療法剤を選択する方法であって、
前記対象者の組織試料中に存在するアンドロゲン受容体(AR)及びエストロゲン受容体(ER)の間の比を求めることと、
前記組織試料中のARとERとの間に正の相関が存在する場合にERモジュレータを選択するか、又は前記組織試料中のARとERとの間に正の相関が存在しない場合にARモジュレータを選択することと、
を含む、乳がんの治療を必要とする対象者においてかかる治療のための化学療法剤を選択する方法。 - ARモジュレータが、AR阻害剤又はアンドロゲン合成の阻害剤を含む、請求項6に記載の方法。
- ARとERとの間の正の相関が、0.5〜1.0の範囲のERに対するARの比を有することを含む、請求項6に記載の方法。
- ARとERとの間の正でない相関が、1.3より大きいERに対するARの比を有する、請求項6に記載の方法。
- 前記ERモジュレータが、タモキシフェン、アロマターゼ阻害剤、又はそれらの組合せを含む、請求項6に記載の方法。
- 前記組織試料が、乳がん組織、周辺上皮細胞、又はそれらの組合せからなる群から選択される、請求項1又は6に記載の方法。
- 前記組織試料中のAR及びERの間の比を求める前記工程が、該組織試料を免疫染色することを含む、請求項1又は6に記載の方法。
- 前記組織試料中のAR及びERの間の比を求める前記工程が、免疫染色により染色される細胞の割合を求めることを含む、請求項12に記載の方法。
- 前記組織試料中のAR及びERの間の比を求める前記工程が、免疫染色により染色される細胞の染色強度を求めることを更に含む、請求項13に記載の方法。
