動物及び糖尿病の誘導
雄Sprague-Dawleyラット(〜150g)はCharles River Laboratories (Wilmington, MA)から購入し、そして標準実験室条件で飼育した;水及び餌料は自由摂取とした。すべての実験はThe Pennsylvania State University College of Medicine Institutional Animal Care and Use Committeeのガイドラインに準拠した。
Diabetes, 49:244-252 (2000))。この投薬計画は、72−96h以内に100%の動物でインスリン依存性糖尿病を誘導し;>350mg/dlを示す血糖レベルのラットを糖尿病(DB)と見なした。血糖レベルは、STZの投与前、及び高血糖の誘導後1、4、及び8週にTrue Track(登録商標)Smart System 血糖計測器 (Home Diagnostics, Ft. Lauderdale, FL) を用いて尾静脈からモニターした。当量の滅菌食塩水の注射を受けた対照動物は「正常」とした。すべてのラットの重量を週1回測定した。
ナルトレキソン適用
正常(n=24)及び糖尿病(DB)ラット(n=24)は、STZ又は生理食塩水の注射後8週にランダム化し、そしてNTX(Sigma-Aldrich, Indianapolis, IN)を全身(30mg/kg、0800hにi.p又は皮下(s.c.))又は局所に0700、1200、及び1700hに1日3回投与した。局所適用では、NTXは、Sorensonリン酸緩衝液(SPB)、KYゼリー(KYJ)(K-Y, Personal Products Company, McNeil-PPC, Inc., Skillman, NJ)、Neutrogena保湿クリーム(MCN)(Neutrogena Corporation, Los Angeles, CA)、又はDMSO(VWR, Philadelphia, PA)に溶解した。NTXは、10-4M、10-5M又は10-6Mの投薬量で滅菌水に溶解し、そして各々の担体へと完全混合し(1:1、v/v);0.5mlの生理食塩水はそれぞれの賦形薬対照としての機能を果たす対応する担体に混合した。
DNA合成を受ける細胞を評価するために、ラットに、Euthasol(Virbac, Fort Worth, TX)で安楽死の3及び6h前に、100mg/kgのBrdU(Sigma-Aldrich)をi.p.注射した。皮膚の適切な領域を切除し、10%緩衝ホルマリン中に固定し、そしてパラフィン切片(6μm)を抗BrdU−POD(Invitrogen, Carlsbad, CA)で染色した。上皮基底層中のBrdU染色細胞を、2切片/標本を用いて250X倍率で4つの0.1mmグリッドにおいて計測し;3組織標本を各担体中の各投薬量で解析した。
全層皮膚創傷
実験の第2セットにおいて、正常(n=70)及びDB(n=70)Sprague−Dawleyラットは、12−14週齢でそしてSTZの注射の8週後に創傷を受けた。8ラット/処置投薬計画を3独立実験のコースにわたって評価した。ラットの背部を剃り、そして創傷を常に位置決めするために領域(20mm直径)をマーカーで縁取りした。動物を、ケタミン(60mg/kg)、キシラジン(10mg/kg)、及びアセプロマジン(1mg/kg)のi.p.注射によって麻酔し、そして6mmの全層輪状切除皮膚創傷を、Acupunch(Acuderm Inc., Fort Lauderdale, FL)を用いて皮筋のレベルまでマークのない領域の中心に作った。ラット当り3つの創傷は、第1の創傷が動物の頭蓋の後方で後頭切痕の5cm後方にそして正中線から1cm離れて作るように標準化した。第2の創傷は、動物の頭蓋の後方で後頭切痕の7cm後方に、そして第1の創傷と反対側で正中線から1cm離れて作製した。第3の創傷は、動物の頭蓋基底において後頭切痕の9cm後方にそして第1の創傷と同じ側で正中線から1cm離れて入れた。日周リズムの影響を最小化するために、0800−0900h間に外科処置を行なった。
創傷サイズは、表面から20cmにセットされた三脚に搭載されたデジタルカメラで画像を捉えることによりモニターし;定規は各創傷毎に写真撮影した。動物は写真撮影のために麻酔なしで手で拘束した。写真は、手術直後及び3、5、7、9、及び11日目に取った。各創傷の面積はImage ProPlus 6.2 (Media Cybernetics, Inc., Bethesda, MD)を用いて解析した。百分率残存創傷はすべてのラットに対して各時点で算出した。8日時点で、創傷閉鎖はまた、Pietramaggiori and colleagues (Pietramaggiori et al., J Surg Res 160:169-177 (2010))に従って、元の創傷面積の百分率として収縮、再上皮化、及び開放創を測定することにより定量化した。収縮、再上皮化、及び開放創面積の総計は、元の創傷サイズの100%均等化で規定した(Yannas I., Tissue and Organ Regeneration in Adults. New York: Springer, 2001)。
細胞増殖は、上記のように創傷後5及び7日目にBrdUで評価した。上皮基底層中の標識及び非標識細胞の数は250X倍率で4つの0.1mmグリッドにおいて計測し、並びに隣接のそして0.5mm周囲領域から創傷部位の縁までサンプリングした。1日3回MCN+賦形薬又はMCN+NTXを投与する3正常及び3DBラットにはBrdUを注射した。
体重及びグルコース測定結果はスチューデント両側t検定で解析した。欠陥(例えば、百分率残存創傷、収縮)の面積測定結果、上皮の厚さ、及びDNA標識率は、Newman-Keuls検定を用いて行ったその後の予定比較で分散分析(ANOVA)を用いて評価した。
実験I.ナルトレキソン及び非創傷皮膚
全ラットはSTZの注射後72h以内に高血糖性となり、正常及びDB動物の血糖レベルはそれぞれ119±4及び569±9 mg/dLであった(図1)。DBラットの体重は、STZの注射後1週で開始して実験期間を通して継続した正常コホートから8%〜28%減少した(図1)。DB群中のラットにはインスリンを与えなかった。高血糖で死んだ動物はいなかった。
第1の実験からの標識率データに基づいて、2つの局所送達システム、KYJ及びMCNに適用された10-5MでのNTXを、全層創傷治癒に対するオピオイドアンタゴニスト遮断の影響を調べるその後の実験に選択した。正常及びDBラットは図1と同様の体重及びグルコースパターンを呈した。全ラットは問題なく創傷処置に耐容性であった(例えば、正常な食餌摂取量)。
6mmトレフィンを用いて計算した創傷サイズは28.3mm2で、10%より多く偏位した創傷は解析に含ませなかった。健常及びDBラットでの創傷閉鎖の評価から、DB動物の残存創傷の面積は、健常ラットに対して手術後5、7、及び9日目に24%、44%、及び132%大きかったことを示した(図5A)。
正常及びDBラットの全層創傷の治癒に対するNTXの局所適用の効果を調べるために、NTXの連日適用の数が決定的に重要な意味を持つかを検討する研究を開始した(図4、5)。正常ラットに対してMCN中での10-5MNTXの単回及び3回の連日処方の3日以内に、創傷面積はMCN+賦形薬を受けた動物に対してそれぞれ30%及び11%減少した(図5B)。NTX処置正常動物で3、5、7、及び9日目の創傷サイズはMCN+賦形薬を受けたコホートよりも小さく、1回及び3回の連日処方に対してそれぞれ31−82%及び11−23%の範囲で変動する減少を示した;全データは9日目の1日3回群の値を除いて統計的に有意であった。MCN+NTXの1日1回適用を受けたDB動物はまた賦形薬に露出されたDBラットと比べて創傷サイズの減少を有したが、しかし5及び7日目にそれぞれわずか32%及び42%の減少を記録した。しかしながら、DB動物については、1日3回投与したMCN+NTXは一貫して創傷閉鎖を促進し、この群はMCN+賦形薬で処置したDBラットから13%〜57%減少した平均残存創傷を示した。
手術後20日での創傷領域の全体病理をヘマトキシリン/エオジン染色で評価した(図7)。賦形薬又はNTXを受けた正常及びDBラット間の上皮の全般構造は類似しているようであった(例えば、壊死細胞の不存在)。しかしながら、正常ラット中の24.6±1.6μmの肉芽組織を覆う上皮の幅と比べて、賦形薬を受けたDBラットは44%減少を有した。NTXで処置した正常動物、並びにNTXを受けたDBラットは、賦形薬を受けた正常動物に対して上皮の厚さが異ならなかった。
DBマウスでの創傷治癒に対してNTX作用機構を解明始めるために、そしてNTXは細胞増殖を標的とするという既報(Zagon et al., Journal of Invest Dermatology, 106:490-497 (1996); Zagon et al., Diabetes, 51:3055-3062 (2002); Zagon et al., Journal of Vascular Surgery, 37:636-643 (2003); Blebea et al., Journal of Vascular Surgery, 32:364-373 (2000); Zagon et al., Immunobiology, 216:173-183 (2011); Zagon et al., Immunobiology, 216:579-590 (2011); Wilson et al., Cell Prolif, 33:63-73 (2000))を踏まえて、賦形薬又はMCN中の10-5MNTXを1日3回受けた正常及びDBラットは創傷後5及び10日目にBrdU標識化を評価し(図8);創傷部位に隣接した及びその周囲の皮膚を評価した。MCN+NTXは創傷に隣接した皮膚の基底上皮中のDNA合成を変化させたが、しかしDB及び正常ラットの両方において周囲領域では変化しなかった。DBラットでは、MCN+NTXで処置したラットは創傷に隣接した皮膚において、5及び10日目にそれぞれ標識率において72%及び36%増加を有した。正常動物では、賦形薬を受けたコホートと比べて、正常ラットの創傷に隣接した皮膚に位置する標識細胞において、5及び10日目にそれぞれ30%及び69%の増加があった。
2型糖尿病及び全身肥満モデルにおいて創傷治癒に対するオピオイドアンタゴニストの効果を研究するために、創傷治癒を野生型とLeprdb(db/dbともいわれる)変異マウス間で比較した。糖尿病自然突然変異(Leprdb)にホモ接合性のマウスは、凡そ3〜4週齢の識別可能な肥満となった。血漿インスリンレベルは10〜14日目に上昇し始めるようになり、そして血糖レベルは4〜8週に上昇し始めるようになった。これらのマウスは野生型マウスと比べて創傷治癒遅延を示した。
ラットが高血糖、体重減少、及び創傷治癒不良などの糖尿病のヒトで認められる代表的な生理的特徴を発現したT1Dの良く特徴付けられたモデル(Zagon et al., Diabetes, 51:3055-3062 (2002); Klocek et al., Journal of Ocular Pharmacology and Therapeutics, 23:89-102 (2007); Havel et al., Diabetes, 49:244-252 (2000))を用いての、この研究での主要知見は、オピオイドアンタゴニストNTXとインターフェースする内因性オピオイド−オピオイド受容体の破壊が、T1D動物、並びに正常(非糖尿病)コホートの皮膚の基底上皮細胞におけるDNA合成を増加させ得ることである。細胞増殖に対するNTXの効果は、非毒性で、比較的短時間作用型であり、そして濃度(局所適応された100倍範囲以内で)あるいは投与経路(全身又は局所)に依存しなかった。薬物をもつ担体の下にある細胞はBrdU標識が上昇し、しかしNTX適用から遠い基底上皮細胞はそうではなかったので、局所送達で投与されたNTXは、DNA合成に影響を及ぼすように局在化した。
Claims (14)
- 糖尿病患者である対象の局所的表皮または真皮の創傷への局所的投与により、糖尿病患者である対象における全層皮膚表皮または真皮の創傷を局所的に処置するための、有効量のオピオイドアンタゴニストを含む局所用薬剤であって、
該オピオイドアンタゴニストが、ナルトレキソン、ナロキソン、又はそのプロドラッグ若しくは塩である、薬剤。 - 前記オピオイドアンタゴニストを、投与に先立って製剤を製造するために薬学的に許容される担体と混合する、請求項1に記載の薬剤。
- 担体は液体、半固体、又は固体である、請求項2に記載の薬剤。
- 前記オピオイドアンタゴニストを、少なくとも10-6Mの濃度で液体担体と混合する、請求項2に記載の薬剤。
- 液体担体と混合した前記オピオイドアンタゴニストを更に、投与のための半固体担体中に混入する、請求項4に記載の薬剤。
- 前記製剤を少なくとも1日1回投与する、請求項2に記載の薬剤。
- 前記対象は糖尿病患者である、請求項1に記載の薬剤。
- 糖尿病患者である対象における全層皮膚表皮または真皮の創傷を局所的に処置するための、有効量のオピオイドアンタゴニスト及び薬学的に許容される担体を含んでなる組成物
であって、
該オピオイドアンタゴニストが、ナルトレキソン、ナロキソン、又はそのプロドラッグ若しくは塩である、組成物。 - 前記組成物は溶液として製剤化される、請求項8に記載の組成物。
- 前記オピオイドアンタゴニストは、少なくとも10-6Mの濃度で前記溶液中に存在する、請求項9に記載の組成物。
- 前記組成物はクリーム剤及びゲル剤として製剤化される、請求項8に記載の組成物。
- 前記組成物はパッチ剤及び包帯剤の形態である請求項8に記載の組成物。
- 表皮または真皮の創傷を処置するために少なくとも1つの更なる有効成分を含んでなる、請求項8に記載の組成物。
- オピオイド成長因子受容体(OGFr)が前記表皮または真皮の創傷に隣接した細胞上に存在する、請求項1に記載の薬剤。
