血漿は、溶解されたタンパク質、脂質、および糖質; 赤血球、白血球、および血小板のような浮遊細胞; イオンおよび他の小分子; ならびに他の可溶性成分を含有する、全血の液体成分である。血清はフィブリノゲンおよび他の凝固因子のない血漿である。
可溶性ウロキナーゼ受容体(suPAR)はELISAにより、糸球体疾患を有する患者の血清中で測定された。移植患者の場合、suPARレベルは、他に指定のない限り、移植前の血清から測定された。suPAR血清レベルは、健常対象と比べた場合にFSGS患者において有意に上昇していることが分かった。対照的に、再発期(RLP)であろうと寛解期(REM)であろうと、微小変化型疾患(MCD)、膜性腎症(MN)、または子癇前症を有する患者においては、suPARの有意なバラツキがなかった。FSGS vs. MNまたは子癇前症それぞれの場合にP < 0.05; FSGS vs. 健常、MCD RLP、MCDまたはREMそれぞれの場合にP < 0.001。FSGSを次いで、3つの異なる亜集団: 原発性FSGS、同種移植片における再発性FSGSおよび移植後の再発なしのFSGSに階層化した。再発なしの移植患者のものと比べて再発性FSGSを後に発症している患者の移植前血清において、有意にいっそう高いレベルの血清suPARが特定された。再発性FSGS vs. 非再発性FSGSまたは非移植原発性FSGSそれぞれの場合にP < 0.01。
有足細胞において、uPARはβ3インテグリンに結合する。さらに、suPARはβ1およびβ2インテグリンと結び付くことが知られている。したがって、suPARが同様にβ3インテグリンに結合できるかどうか調べた。suPARおよびβ3インテグリンの共免疫沈降を用いて、suPARは膜結合型uPARの挙動と同じようにβ3インテグリンと相互作用することが分かった。翻訳開始因子をコードするGFP-E1F1B、およびリボ核タンパク質PTB結合1をコードするFlag-Raverを陰性の結合対照として用いた。
suPARと有足細胞β3インテグリン活性との間の関係をさらに規定するために、正常対象由来の血清とともに、ならびに非再発性FSGS患者および再発性FSGS患者由来の移植前の血清とともにインキュベートされた培養ヒト有足細胞におけるβ3インテグリン活性について(AP5抗体を用いて)FACS分析を行った。有足細胞β3インテグリン活性(AP5)に及ぼすsuPAR含有患者血清の影響をさらに調べるために、分化ヒト有足細胞を正常対象(n=5)由来のプール血清、非再発性FSGS患者(n=10)および再発性FSGS患者(n=15)由来の移植前血清とともにインキュベートした。細胞を次に、β3インテグリン活性(平均蛍光強度MFIによって測定した場合のAP5染色)についてFACS分析によりアッセイした。再発性FSGS移植前血清は、非再発性FSGS血清および正常対象血清と比べて、有意に上昇したβ3インテグリン活性を有することが分かった(再発性FSGS血清 vs. 非再発性FSGS血清またはvs. 正常対象の場合にP < 0.001)。概して、suPARレベルは、A5抗体で視覚化された有足細胞β3インテグリンの活性とよく相関する。
suPARがFSGSの原因または結果であるかを判定するために、異なる3種の腎臓疾患マウスモデルを樹立した: (1) uPARノックアウト(Plaur-/-)マウスへの組み換えsuPARの注射、(2) ハイブリッド移植マウスでの内因性suPAR放出モデル、および(3) 血中suPARを過剰発現する遺伝子操作マウス。
第二に、内因性suPARレベルの増大が野生型マウスにおいて腎臓疾患を引き起こすかどうか判定した。LPSは、単球からの放出を通じてヒト対象の血液中のsuPARを増加させることが示されている。したがって、LPSがまた、マウスにおいてsuPARの血中レベルを増強しうるかどうか判定した。実際に、LPS注射は、対照マウス(n=6)で認められたレベルと比べて5倍までのマウス(n=6)の血清suPARおよび尿suPARの強力な増加を引き起こした。血清suPARレベルはLPS注射後24時間で大幅に増加し、48時間で減少するが、しかし対照と比べてなお有意に高い。LPS注射マウス24時間の時点 vs. PBS対照、およびvs. 0時間の時点の場合にP < 0.001。LPS注射マウス48時間の時点 vs. 0時間の時点の場合にP < 0.01。尿中、LPSによって誘導されたsuPARのレベルは、LPSから48時間後に高く、ピークに達した。LPS注射マウス48時間の時点 vs. 0時間の時点、およびvs. いずれかの時点でのPBS対照の場合にP < 0.001。LPSマウス24時間の時点 vs. 0時間の時点の場合にP < 0.01。野生型マウス由来の一方の腎臓を切除し、その後、Plaur-/-の腎臓を移植した腎臓ハイブリッドマウスを作出した。これらのハイブリッドマウスは、外科手術後14日以内に完全に回復し、正常な腎機能および構造を有していた。次いで、ハイブリッドマウス5匹に単回低用量のLPSを注射して、suPARを刺激した; ハイブリッドマウス3匹に陰性対照としてPBSを注射した。24時間後、EM分析のために腎臓を切除した。陰性対照ハイブリッドマウスにおいて移植片にも自然腎にも正常なろ過障壁が存在していた。uPARの免疫組織化学から、自然野生型の腎臓の糸球体では低レベルのuPAR発現が、しかし移植Plaur-/-腎臓ではそれがないことが示される。対照的に、LPSを注射したハイブリッドマウスは、自然野生型の腎臓でも移植されたPlaur-/-の腎臓でもともに顕著な足突起消失を示す。PBSを注射したハイブリッドマウスは、自然野生型の腎臓の糸球体において低レベルのuPAR発現を示し、移植されたPlaur-/-の腎臓ではそれがないことを示す。しかしながら、LPSを注射したマウスでは、免疫組織化学から、自然野生型の腎臓の糸球体において強力なuPAR発現が示唆される。興味深いことに、移植されたPlaur-/-の腎臓において強力なsuPARシグナルも認められ、腎臓において内因的に誘導されたsuPARの沈着が示唆された。また、野生型腎臓でもPlaur-/-でも顕著な有足細胞の足突起消失が認められた。Plaur-/-マウスはLPS誘導性のタンパク尿および有足細胞消失から概ね保護されるので、Plaur-/-移植片の有足細胞消失は、移植片における過剰な有足細胞β3インテグリン活性化に至る野生型宿主由来の沈着suPARによって最も良く説明される。
第三に、マウスの血清suPARの持続的上昇が進行性糸球体症を引き起こすかどうか探索するために、皮膚からのsuPARの発現を推進する野生型マウスを遺伝子操作した。DIおよびDIIドメインを含有する分泌suPARに対する公知のコード配列に基づいて、マウスsuPARプラスミドを作出した。対照として、β3インテグリン結合欠損suPAR変異体を作出した。この変異体はドメインDIIに点突然変異を有し、これをE134Aと名付けた。プラスミドは、皮膚へのインビボエレクトロポレーションによりマウス皮膚へ送達された。マウスsuPARの両形態はエレクトロポレーション後に同じように十分に発現する。GFP-EIF1Bを陰性の結合対照として用いる。タンパク質の発現はFlagタグ付タンパク質に対してまたはGFPタグ付タンパク質に対して別々の免疫ブロッティングにより確認する。suPARが進行性糸球体疾患を誘導しうるかどうか調べるため、suPARのインビボ遺伝子送達を用いて、エレクトロポレーションされたマウスの血清中のsuPAR発現を増強した。血清および尿中のsuPARレベルはエレクトロポレーションから2日後に上昇し始めたが、これを、分析期間にわたる血中suPARレベルの持続的上昇のために週1回繰り返した。suPARの血清レベルは1週間後にピークに達する。7日目 vs. 0日目(最初のエレクトロポレーションの前)でP < 0.05。比較的、より多くのsuPARが尿中で認められる。7、14および28日目 vs. 0日目の場合にP < 0.001; 28日目 vs. 7日目の場合にP < 0.05。野生型suPARまたは対照となるsuPAR変異体E134Aについて血中または尿中suPARレベルの差異は認められない(各群でn=4)。
本試験から、suPARは、FSGSを引き起こしうる循環血清因子であることが実証される。この結論は、小児FSGS患者および成人FSGS患者の集団での血清suPARレベルの上昇を示すヒト試験に基づいており、遺伝子操作suPARの過剰発現によってFSGSに特有の腎疾患を発症する動物モデルに基づいている。移植前の高い血清suPARレベルは、自然FSGSの存在と関連しており、また、移植後の再発性FSGSの著しく増大したリスクの一因となる。腎臓移植から1年後に、suPARレベルは、FSGSを再発させた患者では著しく上昇したままである。suPARと関連したFSGSによって引き起こされる傷害の機構は、有足細胞の足突起消失およびタンパク尿を起こすのに十分な事象である、有足細胞上のβ3インテグリンの活性化によるものである。suPARによって推進される有足細胞β3インテグリン活性のレベルは、個体の血清suPARの量に依り、おそらくまた、suPARの翻訳後修飾(すなわち、グリコシル化状態)に依り、全血清uPAレベルとは無関係であるように思われるが、これはある形態のがんでのsuPARとuPAとの間の結び付きとは対照的である。このように、循環血からsuPARを除去することによる発病の干渉により、suPARを介した有足細胞傷害から対象を保護することができる。suPARのレベルは、有足細胞β3インテグリン活性を低減することによって疾患発病を停止または緩徐化するのに十分なレベル(絶対量、循環血中の濃度、または別の血清タンパク質と比べた場合の相対量として測定される)まで低減されなければならない。
患者
FSGSを有する患者78人、微小変化型疾患(MCD)を有する患者25人、子癇前症を有する患者7人、膜性腎症(MN)を有する患者16人、および正常対照22人から血清を集めた。2組の一卵性双生児を含めたことに留意されたい。いずれの場合にも、一方が影響を受けなかったが、その双子の兄弟はFSGSを有していた。これらの患者を7箇所の異なる医療センターに登録した。試験は各参加センターの施設内倫理委員会によって承認された。移植FSGS患者の場合、移植時の年齢は非再発性FSGSで28.1±15.7歳、再発性FSGSで26.6±15.7歳であった。男性 vs 女性の比率は非再発性FSGSで16:8および再発性FSGSで15:15であった。全ての移植患者は免疫抑制の誘導および維持、ならびに移植前および移植後のケアを受けた(Renal Transplant Protocols, Royal Infirmary of Edinburgh, 4th Edition, 2007)。再発は、移植後の最初の30日間にスポット尿中タンパクとクレアチニンとの比が3.5 g/g超と定義された。移植の時点で自然腎タンパク尿を有する患者の場合、移植された腎臓にステントを置いて、移植片のタンパク尿を測定し、それから、2〜3週後に外来患者向け診療所でステントを除去した。
β3インテグリン多型PIA2 (Leu33Pro)に対する遺伝子型決定は、既報(Salido et al., J. Am. Soc. Nephrol. 10:2599-2605, 1999)のように腎臓移植患者に対するドナーにおいて行われた。
Plaur-/-マウスへの組み換えsuPARの注射
循環血中suPARが腎臓に沈着し、腎限外ろ過機能に影響を与えうるかどうか判定するため、漸増用量を静脈内注射した: 尾静脈を通じてPlaur-/-マウス(雌性、20±2.0 g)に組み換えマウスsuPAR (R&D Systems) 2 μg、10 μgまたは20 μgを静脈内注射した。Plaur-/-マウスは、もともと、75% C57BL/6および25% 129の混合型の系統にあったが、いずれの使用の前にも10世代にわたりC57BL/6マウスと戻し交配された。アルブミンおよびクレアチニン分析のため、注射前におよび注射後12時間ごとに尿を採取した。注射から24時間後に、Plaur-/-マウスを殺処理し、免疫蛍光のために腎臓を瞬間凍結した。
内因的に誘導されたsuPARが有足細胞傷害を引き起こしうるかどうか判定するため、交差腎臓移植を通じてハイブリッドマウスモデルを樹立した(n= 10)。右側の腎臓をPlaur-/-マウスからひとまとめにして摘出した。このPlaur-/-の腎臓をドナー腎臓と指定し、その一方で生来の右腎臓が切除された野生型マウスをレシピエントとした。それらは、糸球体中にベースラインレベルのuPARおよび血中に低レベルのsuPARを有していた。移植手術は、若干の修正を加えながら既刊のプロトコルにしたがって行われた(Coffman et al., J. Immunol. 151:425-435, 1993; Han et al., Microsurgery 19:272-274, 1999)。手短に言えば、麻酔下で、右側腎臓、尿管を、Plaur-/-マウス(雌性、20±2.0 g)から幅の狭い動脈カフにより腎動脈を、および幅の狭い大動脈カフにより腎静脈を含め、ひとまとめにして摘出した(Han et al., 1999)。グラフトに10 U/mlのヘパリンを含有する冷乳酸リンゲル液0.5 mlを原位置でかん流させた。
製造元のプロトコルにしたがいQuikChange (登録商標) II XL Site-Directed Mutagenesis Kits (Stratagene)を用いて部位特異的突然変異誘発を行った。マウスsuPAR cDNA (GenBankアクセッション番号BC010309)を保有するプラスミドを鋳型として用いた。DII中のsuPAR配列を変異させることを目的とした異なるプライマー対を作出した。部位突然変異誘発の後、新たに作出されたuPAR変異体をHEK細胞に、それぞれ、マウスβ3インテグリンプラスミドと同時にトランスフェクトした。次に共免疫沈降を行って、β3インテグリンとの異なるsuPAR変異体の結合能を調べた。β3インテグリンの結合で欠損が見られたsuPAR変異体(すなわち、E134A)を次いで、さらなる使用のために選出した。
suPARの持続的上昇が進行性糸球体疾患およびFSGSを引き起こしうるかどうか調べるため、マウスsuPARをコードするプラスミドを野生型マウスにおいて発現させた。麻酔下で、suPAR (ドメインDI-DII)をコードするプラスミド(PBS中40 μg)を、後肢へ皮内にかつ両側に注射し、引き続いてDerma Vax (商標) DNA送達システム(Cyto Pulse Sciences)でのインビボエレクトロポレーションを行った。対照のため、上記のように、β3インテグリンの結合で欠損のあったsuPARプラスミドE134Aで変異体マウスを作出した。遺伝子送達を最大6週まで週1回行った。分析のために各遺伝子送達の前後に血液および尿を採取した。毎週マウス4匹を腎臓の検査のために殺処理した。
suPARの影響をさらに確認するため、suPARインビボ遺伝子送達を受けているマウス4匹を無作為に選出し、モノクローナル抗マウスuPAR抗体(R&D Systems, 500 μg/kg)を投与し、その一方で、他のマウス4匹に同量のアイソタイプIgG対照を投与した。初めに、抗uPAR抗体を最初のsuPAR遺伝子送達から1日後に、引き続いて最大4週まで3日に1回投与した。尿を毎日採取し、最初の遺伝子送達から4週後に検査のために腎臓を摘出した。
ヒト対象における循環血中suPARの濃度を、製造元のプロトコルにしたがいQuantikine Human uPAR Immunoassayキット(R&D Systems)によって定量化した。マウスsuPARを自家の酵素免疫測定法(ELISA)によって評価した(Tjwa et al., J. Clin. Invest. 119: 1008-1018, 2009)。血清uPAの濃度をIMUNBIND uPA ELISAキット(American Diagnostic)によって測定した。
ヒト有足細胞に及ぼすβ3インテグリンの活性を定量化するため、フローサイトメトリーをFACScan機器にて行い、Cellquestソフトウェア(Becton Dickinson)で分析した。手短に言えば、完全に分化したヒト有足細胞を24時間ヒト血清(5%)またはsuPAR組み換えタンパク質で処理した。細胞をその後、培養プレートからかき集め、再懸濁した。AP5抗体(50分の1)を加え、室温(RT)で30分間インキュベートした。流動用緩衝液での十分な洗浄の後、細胞を20分間、二次抗体Alexa Fluor (登録商標) 488ヤギ抗マウスIgG (Invitrogen, 500分の1)とともにさらにインキュベートした。細胞を2%パラホルムアルデヒド(PFA)で固定し、FACScan (Becton Dickinson)を用いて分析した。アイソタイプIgG1とともにインキュベートされた細胞を、陰性の染色対照として用いた。
条件的に不死化されたヒト有足細胞は、既述(Saleem et al., J. Am. Soc. Nephrol. 13:630-638, 2002)のように培養された。手短に言えば、有足細胞は、10% FBSおよび1%インスリン・トランスフェリン-セレン(Sigma)を含有するRPMI-1640培地(Invitrogen)中にて33℃で増殖かつ保持された。培養有足細胞をカバースリップ上に播種し、いずれかの処理の前に37℃の増殖制限温度で14日間分化させた。有足細胞に及ぼす異なる血清の効果を調べるため、慣用の有足細胞培地を除去し、4%ヒト血清を含有するRPMI-1640培地へ交換した。組み換えヒトsuPARタンパク質(R&D Systems)を1 mg/mlで用いた。uPAR-β3インテグリン経路の活性化による血清誘導性の細胞効果を調べるため、モノクローナル抗uPAR抗体(R&D Systems、1 mg/ml)およびシクロRGDfV (Biomol、1 mg/ml)、ανβ3インテグリン阻害剤をそれぞれ再発性FSGS血清とともに同時にインキュベートした。処理から24時間後、免疫蛍光標識化の前に4% PFAでヒト有足細胞を固定した。suPARおよびβ3インテグリンの相互作用を調べるため、ヒト胎児腎臓(HEK) 293細胞を、10% FBSを含有するDEME培地中にて37℃で培養した。90%の密集度にて、細胞にGFPタグ付またはFlagタグ付マウス膜結合型uPAR (Genbankアクセッション番号NM_011113)またはsuPAR (Genbankアクセッション番号BC010309)プラスミドを、マウスβ3インテグリン(Genbankアクセッション番号NM_016780)、翻訳開始因子をコードするGFP-E1F1B、およびリボ核タンパク質をコードするFlag-Raverを保有するプラスミドとともに同時にトランスフェクトした。PTB結合1 (PTB-binding 1)も同時にトランスフェクトして、その後の共免疫沈降試験のための陰性の結合対照とした。トランスフェクションを製造元の使用説明書にしたがい、リポフェクトアミン(商標) 2000 (Invitrogen)で行った。トランスフェクションから24時間後に、HEK細胞をさらなる使用のために収集した。
培養ヒト有足細胞に及ぼすβ3インテグリンの活性を分析するため、固定された有足細胞を有するカバースリップをRTで30分間ブロッキング用緩衝液(5%ヤギ血清、5%ロバ血清) (詳細は細胞培養の項を参照のこと)とともに、その後、1時間AP5抗体(GTI, 50分の1)とともにインキュベートした。各3分間PBSでの3回の洗浄後、二次抗体Alexa Fluor(登録商標) 488ヤギ抗マウスIgG (Invitrogen, 1000分の1)を加え、45分間インキュベートした。次にカバースリップをPBSで洗浄し、5分間4',6-ジアミジノ-2-フェニルインドール(DAPI, 3 mM, Invitrogen)でペルオキシダーゼ活性を目的に対比染色した。切片を脱水し、Bio Mount (Bio Optica)中で封入した。抗体標識化の特異性は、一次抗体の代わりにPBSおよび適切な対照免疫グロブリン(Invitrogen)を用いた後の染色の欠如によって実証された。
本発明者らの刊行済みのプロトコルにしたがって(Wei et al., Nature Med. 14:55-63, 2008)共免疫沈降を行い、suPARとβ3インテグリンとの間の相互作用を調べた。手短に言えば、所望のGFPタグ付またはFlagタグ付プラスミドによるトランスフェクションから24時間後に、氷上にて30分間、プロテアーゼ阻害剤カクテル(Roche)を含有するRIPA緩衝液(Boston Bioproducts)中でHEK細胞を溶解させた。20分間14,000 rpmでの遠心分離の後、上清(すなわち、溶解物)を回収し、その総タンパク質濃度を測定した。約500 mgの総タンパク質を保有していた溶解物を次に、4℃で終夜、Flagビーズ、抗Flag M2アフィニティーゲル(Sigma)とともにインキュベートして、Flag融合タンパク質およびそのインタラクトームをプルダウンした。各10分間、RIPA溶解用緩衝液で5回洗浄した後に、Flagビーズを遠心分離によって収集し、NuPAGE LDSサンプル用緩衝液(Invitrogen)中に再懸濁し、10分間70℃でインキュベートした。短時間の遠心分離の後、溶出液(Flag融合タンパク質およびそのインタラクトームを含有する上清)を次いで、ウエスタンブロッティングのために回収した。
TEMはWeiら(2008)によって記述されているように行った。マウス腎臓組織は腎臓病医により既述のスコアリング系にしたがって盲検方式で検査およびスコア化された(Crowley et al., J. Clin. Invest. 119:943-953, 2009)。手短に言えば、固定された腎臓組織をパラフィン中で包埋し、切片にし、ヘマトキシリン・エオシン(H&E)および過ヨウ素酸シッフ(PAS)で染色した。腎臓切片を糸球体、尿細管、血管および間質の異常の存在および重症度に基づいて類別した。半定量的尺度を用いて腎臓の病理学的異常の重症度を類別し、その中で0を異常なしとし、1+、2+、3+、および4+は、それぞれ、軽度、中等度、中重度、および重度の異常とした。各腎臓に対する組織学的スコアは、糸球体、尿細管、および間質に対する個々の評点に血管損傷または動脈狭窄の存在に対しての1点を加えて合計することによって得た。糸球体硬化症の割合は、切片中の糸球体の総数で除された、硬化症の証拠が有る糸球体の数と定義される。
統計分析は一元配置分散分析またはスチューデントの対応のあるもしくは対応のないt検定によって行われた。帰無仮説はP値0.05で拒絶された。特に指定のない限り、値は平均±標準偏差(S.D.)として提示されている。
本試験では、原発性FSGSの病因、治療反応性、および疾患進行に対して血清suPARの関与を評価した。小児および成人を含む、医学的に処置され、生検により確定診断された2つの異なる原発性FSGSコホートにおいて循環血中suPARを分析した。患者70人は北米に基づく無作為化FSGS臨床試験(FSGS-CT)に由来し、患者94人はステロイド抵抗性ネフローゼ症候群の試験に向けた欧州に基づくコンソーシアム(PodoNet)に由来した。
対照の対象110人、FSGS-CTコホート由来の原発性FSGS患者70人、およびPodoNetコホート由来の原発性FSGS患者94人において循環血中suPARを測定した(図5)。対照の対象はPodoNet FSGS患者に年齢を適合させた。群内で性別分布の有意差はなかった。対照の対象と比べて、FSGS患者における血清suPARレベルは、両方のFSGSコホートにおいて著しく増加した(FSGS-CT vs. 対照、PodoNet vs. 対照、FSGS-CT vs. PodoNetの場合にP < 0.001)。異常な濃度を定義するために3000 pg/mlのカットオフ値を用いたところ、ベースラインの循環血中suPARレベルはPodoNetのFSGS患者の56%と比べてFSGS-CTのFSGS患者の84%において上昇した。平均suPARレベルは、PodoNetコホートにおけるよりもFSGS-CTコホートにおいて高かった(4588 pg/ml ± 203 pg/ml vs. 3497 pg/ml ± 195 pg/ml、P < 0.0001)。
ベースライン(W01)時および26週(W26)の処置後に回収した血清から測定された循環血中suPARレベルを、人口統計学的変数および血清クレアチニン、血清アルブミン、推算糸球体ろ過量(eGFR)、またはタンパク尿(Up/c)との相関関係について分析した。分析された変数にはW01時のsuPARレベルについて予測的であると思われるものがなかったが、ベースラインおよび26週の両時点での血清アルブミン、ならびに26週時のUp/cは26週時のsuPARレベルと相関していた(図6)。
PodoNetコホートにおいて、重回帰分析により、循環血中suPARレベルは血清クレアチニン(P < 0.01)およびeGFR (P < 0.05)と相関していたが、しかしタンパク尿と、または疾患発症時の年齢、サンプリング時の年齢と、またはPodoNetコホートにおける性別と相関していなかったことが示された。相当数の家族性の症例またはFSGS患者で規定の遺伝子変異(NPHS2)が有ったので、このコホートを2つの亜群、家族性/遺伝性FSGS vs 非遺伝性FSGSにさらに階層化した(図8)。2つの亜群間で疾患発症時の年齢、サンプリング時の年齢、性的衰弱、eGFR、または血清アルブミンおよびクレアチニンレベルに関する差異はなかった。しかしながら、タンパク尿は家族性/遺伝性の亜群においていっそう高かった。興味深いことに、循環血中suPARレベルは、原発性FSGSの非遺伝性の症例におけるレベルと比べた場合に、家族性または遺伝性FGSSの群において有意に高かった(P < 0.05)。
原発性FSGSにおいてはsuPARレベルの上昇が見られる。suPARは糸球体に侵入し、有足細胞の足底板のβ3インテグリンに結合し、それを活性化して、足突起消失およびタンパク尿を引き起こしうる。本発明者らは、以下と結論を出した: (1) 循環血中suPARレベルは2つの異なるコホートでの原発性FSGSを有する患者の大多数において著しく上昇していた; (2) MMF治療は経時的な血清suPARの減少と関連していた; (3) FSGS-CTでの26週の処置にわたるsuPARレベルの減少は、臨床的に安定した完全寛解と関連していた; および(4) 原発性FSGSを有する患者において、suPARレベルは家族性の症例または規定のポドシン変異を有する患者において有意に高かった。
FSGS臨床試験(FSGS-CT)コホート
FSGS-CTは、サイクロスポリン(CSA)の効果をミコフェノレートモフェチル(MMF)およびデキサメタゾンの組み合わせと比較した無作為化比較対照試験である。主要な試験対象患者基準は、年齢2〜40歳、1.73 m2あたりeGFR > 40 ml/分、生検により確定診断されたFSGS、およびコルチコステロイド治療に対する抵抗性であった。除外基準には、二次性FSGS、肥満、または従前の実験的治療が含まれた。アンギオテンシン変換酵素阻害剤に寛容でなかった者において、全ての対象がリシノプリルまたはロサルタンを投与された。対象は52週間、処置された。主要転帰は、試験薬物による52週の積極的処置後の最初の朝の尿サンプルにおいて尿中タンパク質/クレアチニン比(Up/c) < 0.2と定義されるタンパク尿の正常化であった。主な二次転帰は72週時、つまり試験薬物の中断後6ヶ月のタンパク尿のレベルに基づいていた。対象は処置期間の間、11回受診し、各来院時に、血圧を測定し、血液および尿を採取して、血清クレアチニン、eGFR、アルブミン、コレステロール濃度、およびタンパク尿を測定した(Gipson et al., Kidney Int'l 80:868-878, 2011)。ベースライン(W01)および処置26週(W26)の時点で採取した血清サンプル(各治療群でn = 35)をsuPAR測定のために、NIDDK Biorepositoryから回収した。
PodoNetは、ステロイド抵抗性ネフローゼ症候群(SRNS)の臨床的、遺伝的、および実験的試験のためのコンソーシアムである。試験対象患者基準は、参加医療センターの管理プロトコルに基づいてSRNSを有する小児(年齢0〜18歳)および家族性SRNSを有する成人である。本試験に含まれた患者は、生検により確定診断されたFSGSを有していた(n = 94)。対象の処置は、その主治医によって臨床的に判定され、管理された。
血漿サンプルは、年齢が0〜18歳であった、110人の健常な白人の小児および青年(女性 = 55人)から入手できた。これらの対象はRostockの小学校もしくは高校から、および独国のRostock大学医学部から登用された。小手術の前にまたはてんかんおよび起立性の病状のような非炎症性疾患に続いて鑑別診断のために大学小児病院Rostockに来院していた小児も適格とした。登録時に成長障害、新鮮骨折または栄養不良の既往歴、急性感染症、C反応性タンパク質(5 mg/l以上)またはクレアチニン(2 SD以上)の血清中濃度の上昇を有する小児、ならびに代謝障害、慢性炎症性疾患、および腎疾患または肝疾患を有する小児は除外した。試験は病院倫理委員会(HV-2009-003)により承認され、適切な場合、親および/または参加者からインフォームドコンセントを得た。血清およびEDTA-血漿を分割し、次に、後で分析するために-80℃で貯蔵した。最初の試験には健常な成人対照におけるsuPARレベルを含めた。
血清suPARの測定は、QuantikineヒトuPAR免疫アッセイキット(R&D Systems)を用いて行った。
患者および対照の対象の人口学的および臨床的特徴は、カテゴリー変数の場合にはχ2試験および連続型変数の場合にはスチューデントのt検定を用いて比較した。循環血中suPARと他の変数との重回帰分析は、SPSSで行われた。データは平均±標準誤差(SEM)として表された。全ての統計的検定は両側とし、P < 0.05を有意と見なした。
巣状分節性糸球体硬化症(FSGS)は、罹患個体のおよそ3分の1で腎臓移植後に再発し、同種移植片喪失をもたらしうる。有足細胞の超微細構造変化に及ぼすsuPARの影響を、再発性FSGSまたは新規FSGSの間に調べた。有足細胞構造に及ぼす治療の影響を判定した。
本試験は、腎移植後の再発性FSGSまたは新規FSGSの処置前の循環血中suPARのレベルが、FSGS診断時の腎同種移植片における有足細胞の足突起消失の重度と有意に相関することを実証する。第二に、本発明者らの試験は、腎同種移植片における再発性FSGSおよび新規FSGSの初期病理所見が、電子顕微鏡検査によって検出された、有足細胞の足突起消失であり、これは光学顕微鏡的変化がない場合であっても軽度(25%以下)から重度(75%以上)、場合によっては完全な消失に及びうることを立証した。第三に、本発明者らは、リツキシマブの有無にかかわらず血漿交換に対する完全応答または部分応答が、有足細胞の足突起消失の顕著な改善をもたらすことを実証した。
試験デザインおよび母集団
本発明者らは、2003年1月1日から2011年12月31日までの間に腎移植を受け、かつ単一の三次病院での腎臓移植後に再発性FSGSまたは新規FSGSを発症した全成人の腎移植レシピエントの後ろ向き観察試験を行った。本発明者らは、年齢18歳およびそれ以上の腎移植レシピエント105人を特定した。93人が自然腎の生検により確定診断されたFSGSを有し; 12人が可能性の高いFSGS診断を有していた。個体25人が腎移植後に新規FSGS (n=4)および再発性FSGS (n=21)を発症した。再発性FSGSは、腎移植の前に無尿であった個体(無尿8人および不明2人)でのタンパク尿(1 g超/24時間)の存在または腎移植の前に無尿でなかった個体(n = 15)間でのタンパク尿の悪化によって定義された。新規FSGSは、ESRDの主因がFSGSによるものではなかったレシピエントでの腎移植後のタンパク尿の新たな発症と定義された。診断は、処置開始前または処置開始14日以内に得られた同種移植片生検での有足細胞の足突起消失の存在によって確認された。本試験は、ジョンズ・ホプキンス大学医学部の施設内倫理委員会によって承認された。
社会人口学的および臨床的データは、腎移植時から腎移植後3年または利用可能な最後の臨床追跡調査までダブルデータ入力を用いて患者の医療記録から抽出された。収集されたドナーの臨床的特徴には、ドナーの生命状態、レシピエントとの関連性、およびレシピエントとのABO型適合性が含まれた。レシピエントの臨床的特徴には、FSGS診断時の年齢、ESRDの主因、従前の腎移植数、透析を受けている者の間での期間、血清クレアチニンおよび尿中タンパク質とクレアチニンとの比によって定義されるタンパク尿が含まれた。糸球体ろ過量(eGFR)は、年齢、性別、および人種を調整するCKD-Epi方程式を用いて推定した(Levey et al., Ann. Intern. Med. 150:604-612, 2009)。レシピエントは血漿交換で処置された。持続的に顕著なタンパク尿を有する者には、1回または2回のリツキシマブ注入も行った。完全応答は、処置過程の完了時におよび/または利用可能な最後のタンパク尿定量化により1 g/g未満までのタンパク尿の減少によって定義された。治療に対する部分応答は、処置の終了時に、ピークのタンパク尿レベルから50%だけのタンパク尿の低下、しかし処置の終了時にタンパク尿が1 g/gまたはそれ以上残存している状態と定義された。腎臓の病理組織は、腎臓病理学者により、光学顕微鏡検査、免疫蛍光および電子顕微鏡検査を用いて評価された。有足細胞の足突起消失度は、生検報告、ならびに最初の生検報告に盲目とされた第2の腎臓病理学者による電子顕微鏡検査の二次評価およびレシピエントの転帰に基づいていた。次に、これらの2つの評価を平均化して、平均の有足細胞足突起消失を得て、これを25%未満(軽度)、25%〜74% (中等度)、および75%またはそれ以上(重度)に分類した。ベースライン時の電子顕微鏡検査結果は、治療の開始の前または直後に行われた腎生検から得られた。処置後の足突起消失は、血漿交換セッションの完了後にまたは難治例ではリツキシマブ注入後に行われた腎生検から評価された。処置開始の前に貯蔵血清が利用可能なレシピエントの間で、suPARレベルが、マイアミ大学医学部で製造元のプロトコルにしたがいQuantikineヒトuPAR免疫アッセイ(R&D Systems)を用いて測定された。
記述的分析を行って、レシピエントのベースライン時の社会人口学的および臨床的特徴の分布を評価した。Kruskal-Wallis検定を用いベースライン時の足突起消失の3つのカテゴリーにわたって処置前の平均suPARレベルを比較した。次に、対応のあるスチューデントのt検定を用いてベースライン時から処置後までの平均の有足細胞消失、腎臓機能、タンパク尿、およびsuPARレベルの変化を評価した。これらの比較は、全試験母集団を用いて行われ、その後、処置に対する部分応答または完全応答を達成したレシピエントだけに制限された。全ての統計分析は、Stata/MPバージョン11.2 (StataCorp)を用いて行われた。
Claims (8)
- (i) 注入口;
(ii) 支持体;
(iii) 1種もしくは複数種のsuPAR特異的抗体またはその機能的部分へのsuPARの特異的結合の前、間、および/または後に該支持体に付着する、該1種もしくは複数種のsuPAR特異的抗体またはその機能的部分;
(iv) 排出口;
(v) 内部に該支持体を含有するハウジング; ならびに
(vi) 該注入口および該排出口を接続する、該ハウジングを通る流路
を含み、
可溶性血液成分を構成し、該注入口から入り、該ハウジングを通る該流路をたどり、該排出口から出る、流体相中のsuPARが、該suPARと該抗体またはその機能的部分との間の免疫複合体の形成および該支持体へのその付着を通じて固定化され;かつ
アフェレーシス用の装置に挿入されるように構成されている、
可溶性ウロキナーゼ受容体(suPAR)に特異的な免疫吸着カートリッジ。 - 2 μg〜10 μgのsuPARを結合することが可能である、請求項1記載のカートリッジ。
- 前記支持体が、少なくとも1つの流体透過膜、1種もしくは複数種の多孔質繊維、および複数の粒子からなる群より選択される、請求項1記載のカートリッジ。
- 前記ハウジングが少なくとも膜ろ過またはカラムクロマトグラフィー用に構成されている、請求項1記載のカートリッジ。
- 前記抗体またはその機能的部分が、少なくとも前記支持体に可逆的に付着する、請求項1記載のカートリッジ。
- 前記抗体またはその機能的部分が、少なくとも前記支持体に不可逆的に付着する、請求項1記載のカートリッジ。
- 前記ハウジング、前記注入口、および前記排出口を滅菌状態および発熱性物質なしの状態に保持するために該ハウジングを取り囲む無菌包装をさらに含む、請求項1記載のカートリッジ。
- 対象の循環血液からの可溶性ウロキナーゼ受容体(suPAR)の除去のために用いられる、suPARに特異的な免疫吸着カートリッジ。
