Description
本願は、2010年7月15日に出願された米国仮特許出願第61/364,453号の出願日の利得を主張するものであり、その開示内容は、参照することによって、ここに含まれるものとする。
本発明は、人工心臓弁置換に関し、さらに詳細には、折畳み可能な人工心臓弁の経カテーテル送達のための装置、システム、および方法に関する。
弁受容区画28は、シース22の内側に配置されたリテーナ30、リテーナ30に隣接する近位円錐端31、およびリテーナ30から離間した遠位円錐端32を備えている。円錐端32は、弁受容区画28の一端において内側シャフト24に接合されており、円錐端31およびリテーナ30は、弁受容区画28の他端において内側シャフト24に取り付けられた補剛部材25に接合されている。好ましくは、内側シャフト24および補剛部材25は、(図示されていない)ガイドワイヤーを受け入れるように構成された同一の内径を有している。代替的に、本明細書に記載されている送達装置実施形態のいずれにおいても、内側シャフト24および補剛部材25は、単一体のシャフトであってもよい。患者内に送達するために、折畳み可能な弁が、弁受容区間29の円錐端31,32間に内側シャフト24を囲むように装填され、該弁のステント部分が、リテーナ30に連結されるようになっている。
Claims (14)
- 少なくとも1つの保持部材を一端に有する移植可能な医学装置用の送達装置において、前記送達装置は、
長手方向に延在しているシャフトと、
前記シャフトの長手方向部分を囲んでいる細長シースであって、前記シャフトに対して長手方向に摺動可能になっている細長シースと、
前記シースの内側に画定され、組立状態にある前記医学装置を受容するように構成された区画と、
前記区画の一端に配置されたリテーナであって、内側片と、前記内側片の周りに回転可能となるように、かつ前記内側片に対して長手方向における運動が制約されるように、前記内側片上に取り付けられた外側片とを備えているリテーナと、
前記組立状態にある前記医学装置の前記保持部材を受け入れるように構成された、前記リテーナ内の少なくとも1つの受入れ部と、
を備え、
前記外側片は、前記区画と向き合った保持縁を有しており、前記受入れ部は、前記保持部材を受け入れるように寸法決めされた第1の領域を有しており、前記受入れ部は、前記第1の領域と前記保持縁との間に狭小ネックを画定する少なくとも1つの突起を備えていることを特徴とする送達装置。 - 前記医学装置は、複数の支柱を有する自己拡張型ステントを備えており、前記外側片は、前記保持縁における開端から閉端に向かって長手方向に延在する少なくとも1つの凹部を有しており、前記凹部は、前記組立状態にある前記ステントの端における前記複数の支柱の1つを受け入れるように構成されていることを特徴とする、請求項1に記載の送達装置。
- 前記受入れ部は、組立状態において前記複数の支柱の前記1つが前記凹部の前記閉端に接触すると共に前記保持部材が前記受入れ部の閉端から離間するような、長手方向長さを有していることを特徴とする、請求項2に記載の送達装置。
- 請求項1に記載の送達装置を含むシステムであって、前記システムは、
前記区画内に組み込まれた移植可能な医学装置であって、前記医学装置は、その一端に少なくとも1つの保持部材を有しており、前記保持部材は、前記受入れ部内に配置されている医学装置をさらに備えていることを特徴とするシステム。 - 前記外側片および前記内側片の一方は、周方向に延在する溝を備えており、前記外側片および前記内側片の他方は、前記溝に組み込まれて前記外側片が前記内側片の周りを回転することを可能にする環状リングを備えていることを特徴とする、請求項1に記載の送達装置。
- 前記リテーナは、前記内側片上に取り付けられて前記外側片に固定して接続された支持片をさらに備えており、これによって、前記外側片および前記支持片は、前記内側片の周りを一緒に回転可能になっていることを特徴とする、請求項5に記載の送達装置。
- 前記医学装置は、複数の支柱を有する自己拡張型ステントを備えており、前記外側片は、前記保持縁における開端から閉端に向かって長手方向に延在する少なくとも1つの凹部を有しており、前記ステントの端における前記複数の支柱の1つは、前記凹部内に組み込まれていることを特徴とする、請求項4に記載のシステム。
- 前記受入れ部は、前記複数の支柱の前記1つが前記凹部の前記閉端に接触すると共に前記保持部材が前記受入れ部の閉端から離間するような、長手方向長さを有していることを特徴とする、請求項7に記載のシステム。
- 前記少なくとも1つの受入れ部から半径方向外方に延在して前記保持部材の開口に係合された、ピンと、
前記ピンに連結されたアクチュエータであって、前記ピンを長手方向に移動させるように構成されており、これによって、前記受入れ部に対する前記保持部材の長手方向位置を調整するようになっているアクチュエータと、
をさらに備えていることを特徴とする、請求項4に記載のシステム。 - 前記受入れ部は、組立状態において前記受入れ部の長さが前記保持部材の長手方向長さよりも大きくなるような長手方向長さを有していることを特徴とする、請求項1に記載の送達装置。
- 前記医学装置は、複数の支柱を有する自己拡張型ステントを備えており、前記リテーナは、前記保持縁における開端から閉端に向かって長手方向に延在する少なくとも1つの凹部を有しており、前記凹部は、前記組立状態にある前記ステントの端における前記複数の支柱の1つを受け入れるように構成されていることを特徴とする、請求項10に記載の送達装置。
- 前記医学装置は、自己拡張型折畳み可能な人工弁であることを特徴とする、請求項4に記載のシステム。
- 前記少なくとも1つの受入れ部から半径方向外方に延在しているピンであって、前記保持部材の開口に係合するように構成されているピンと、
前記ピンに連結されたアクチュエータであって、前記ピンを長手方向に移動させるように構成されており、これによって、前記受入れ部に対する前記保持部材の長手方向位置を調整するようになっているアクチュエータと、
をさらに備えていることを特徴とする、請求項1または10に記載の送達装置。 - 前記組立状態において、前記複数の支柱の前記1つは、前記凹部の前記閉端に接触しており、前記保持部材は、前記受入れ部の閉端から離間していることを特徴とする、請求項11に記載の送達装置。
