Description
本発明は、ムチン糖タンパク質(MUC-1)ベースの製剤による処置に適した癌を有する個体を特定するための方法に関する。そのような癌の例は、非小細胞肺癌および前立腺癌である。いくつかの例では、MUC-1ベースの製剤はBLP25ワクチンである。
本国際出願は、2005年6月28日に出願された米国特許仮出願第60/694,233号に対する優先権を主張し、これはその全体が参照により本明細書に組み入れられる。
肺癌は、北米では男女両方にとって主要な癌関連死亡原因である。2004年には、米国内で約174,000人(男性54%、女性46%)が新たに肺癌と診断された。さらに、2004年には、米国のみで、約160,000人がこの疾患により死亡した。
本発明は、MUC-1ベースの製剤、およびMUC-1製剤による処置が可能な癌に罹患している個体を処置するための方法を提供する。そのような癌の例は、NSCLCおよび前立腺癌である。本発明はまた、記載するMUC-1ベースの製剤による、NSCLCおよび前立腺癌に加えてその他の癌の処置も包含する。
一つの態様において、MUC-1ベースの製剤は、一定量のMUC-1リポペプチドBLP25および一定量のアジュバントを含む。このような製剤は、本明細書ではBLPリポソームワクチン(「L-BLP25」)と呼び、これは液剤であっても凍結乾燥製剤であってもよい。例えば、製剤またはワクチンは、単一投与量に約1000μgのMUC-1リポペプチドBLP25と約500μgのリピドAとを含有できる。
SEQ ID NO:1および2に示すMUC-1コアリピート配列に替わるものとして、本発明の製剤はこれらMUC-1コアリピートの免疫学的に活性な相同体または変異体を組み入れてもよい。したがって本発明は、SEQ ID NO:1またはSEQ ID NO:2のいずれかに示すアミノ酸配列に類似するが同一ではない配列を有するMUC-1コアリピートの使用を包含する。したがって、本発明は、SEQ ID NO:1またはSEQ ID NO:2の配列と比較して、99%、98%、97%、96%、95%、94%、93%、92%、91%、90%、89%、88%、87%、86%、85%、84%、83%、82%、81%、または80%の配列同一性を有し、免疫学的に活性であるMUC-1コアリピートの使用を意図する。
SEQ ID NO:1または2の完全長配列、あるいはその変異体を有するMUC-1コアリピートペプチドは、通常は結合していない別のポリペプチドに結合することもできる。そうすることでMUC-1コアリピートペプチドは、そのN末端またはC末端にて、MUC-1コアリピートと実質的に相同でないアミノ酸配列を有する非相同的ポリペプチドと動作可能に結合できる。「動作可能に結合」とは、MUC-1コアリピートペプチドと非相同的ポリペプチドの両方がインフレームであることを示している。
MUC-1コアリピート変異体にはまた、誘導体または類似物が含まれ、その場合、(i)あるアミノ酸が、遺伝コードによってコードされないアミノ酸のアミノ酸残基で置換されている、(ii)成熟ポリペプチドがポリエチレングリコールのような別の化合物に融合している、あるいは(iii)リーダーもしくは分泌配列またはポリペプチド精製のための配列といった追加のアミノ酸がMUC-1ポリペプチドに融合している。
MUC-1コアペプチド、BLP25ポリペプチド、またはBLP25リポソームワクチンを含むMUC-1ベースの製剤を被験体に与える場合、当業者は、投与量が被験体の体重、腫瘍の大きさ、または腫瘍の進行度を含むが、これらに限定されるものではない複数の要因に依存することを了解している。一般的に、本明細書で用いる場合、MUC-1ベースの製剤を投与される被験体は単一の生物である。ある態様において、被験体は哺乳動物であろう。特に、被験体は男または女を含むヒトであってよい。多くの態様において、被験体は患者、あるいは診療および処置を待っているか、または受けている個体であろう。
多くの態様において、MUC-1ベースの製剤はリポソームと共に用いられるであろう。リポソームは、水性区画を取り囲む、1つまたは複数の脂質二重層からなる顕微鏡レベルの小胞である。例えば、Bakker-Woudenbergら、Eur. J. Clin. Microbiol. Infect. Dis. 12 (Suppl. 1): S61 (1993)、およびKim, Drugs, 46: 618 (1993)を参照されたい。リポソームは天然の細胞膜にも存在する巨大脂質分子から作ることができるため、リポソームは一般的に安全に投与でき、また生分解性である。ゆえに、リポソームは薬物送達に多く用いられている。
多くの態様においてMUC-1ベースの製剤は、SEQ ID NO:2のように脂質付加されていてもよい。本明細書では、「脂質」とは官能性の酸素、窒素、またはイオウ基を有するアミノ酸に結合できるミリスチル、パルミトイル、またはラウリル分子でよい。このようなアミノ酸としては、スレオニン、セリン、リジン、アルギニン、およびシステインが挙げられるが、これらに限定されるものではない。
本MUC-1ベースの製剤は、1つまたは複数のアジュバントを含んでもよい。または、1つもしくは複数のアジュバントを、本発明のMUC-1ベースの製剤を投与する前、同時、または後に投与してもよい。
MUC-1ベースの製剤がワクチンである場合、ワクチンは1つまたは複数の薬学的に許容される賦形剤と共に製剤化してもよい。このような賦形剤の特性は当技術分野では周知であるが、典型的には生理学上許容でき、かつ不活性である賦形剤、または本発明の組成物のワクチン特性を高める賦形剤が含まれる。薬学的に許容される賦形剤の非限定例としては、無菌生理食塩水のような液体媒質が挙げられる。賦形剤はリポソームワクチン配合時のいかなる時点で加えてもよく、または完成したワクチン組成物に混合してもよい。賦形剤を加える時点および本発明のワクチンとの使用に適した賦形剤は共に容易に決定できる。
MUC-1ベースの製剤による処置の前に、被験体をシクロホスファミドで「前処置」してもよい。多くの態様において、シクロホスファミドの投与量は、約300mg/m2、約400mg/m2、約500mg/m2、または約600mg/m2であろう。約300mg/m2の範囲のシクロホスファミド投与量は、低投与量と考える。ある態様において、シクロホスファミドは単回投与で与えられるであろう。別の態様において、シクロホスファミドは、ある期間にわたって複数回の投与で与えられるであろう。
本発明のMUC-1ベースの製剤は、ワクチンを含め複数の投与経路に合わせて製剤化してもよい。具体的な経路には、静脈内、筋肉内、皮下、もしくは皮内注射、エアゾール、経皮、肺内、経鼻、経口、膣内、直腸内、眼内、局部(粉末、軟膏、もしくは滴剤)、頬側、嚢内、腹腔内、または局所投与などを介して、またはこれら経路の組み合わせによってなどの任意の薬学的に適した投与方法が含まれ、一回もしくは複数回の単位投与量で投与する。ワクチンまたはリポソーム結合MUC-1コアリピートペプチドはまた、例えば経皮パッチを介するような、経皮送達に適した製剤を介して投与してもよい。
NSCLCおよび前立腺癌などMUC-1ベースの製剤による処置が可能な癌であると診断されたいかなる被験体も、本明細書に記載のMUC-1ベースの製剤による処置を受けることができる。または、任意のステージのNSCLCまたは任意のステージの前立腺癌の症状など、MUC-1ベースの製剤による処置が可能な癌の任意のステージの症状を示すが、正式にはその癌を有すると診断されていない個体もMUC-1ベースの製剤による処置を受けることができる。さらにまた、上記したように、被験体をNSCLCまたは前立腺癌などMUC-1ベースの製剤による処置が可能な癌に罹らないようにするために、MUC-1ベースの製剤を予防的に与えることができる。
CA27.29は、MUC-1タンパク質を同定するために、特に被験体の血流、または血液、尿、もしくは血清試料などの適した生体試料内を循環するMUC-1タンパク質断片を同定するために、特に有用な標的である抗原の名称である。CA27.29抗原は、MUCl産物のタンパク質コアに特異的なモノクローナル抗体によって検出することができる。重複する合成ペプチドと共にマッピングされる最小エピトープは、適したモノクローナル抗体を同定するために用いることができる。そのような抗体の1つはB27.29である。
用語HLAは、ヒト白血球抗原系を意味し、これは第六染色体の短腕上の遺伝子によって制御される。HLA遺伝子座は、主要組織適合遺伝子複合体(MHC)として公知の遺伝領域の一部である。MHCは免疫応答の正常機能に不可欠な遺伝子(HLAを含む)を有する。HLA抗原の重要な役割は、自己認識の制御およびしたがって微生物に対する防御にある。いくつかのHLA抗原は、血液細胞だけでなく身体の組織のすべてで認識される。
NSCLCを有する被験体が本発明のMUC-1ベースの製剤による処置を受ける場合は、ステージIIIB局所領域性(LR)、癌性胸水を伴うステージIIIB、またはステージIVのNSCLCであると診断された被験体が特に処置され得る。しかしながら、本発明はまた、ステージIII局所領域性、ステージIII胸水、およびステージIVの疾患以外の個体の処置も包含する。したがって本発明は、ステージIA、ステージIB、ステージIIA、ステージIIB、ステージIIIA、ステージIIIB、ステージIIIB局所領域性、ステージIIIB胸水、およびステージIVのNSCLCと診断された患者の処置を意図している。参照として本明細書に組み入れられる、Mountain C. F., Chest.; 111(6):1710-7 (1997)を参照されたい。
一般的に、MUC-1ベースの製剤をNSCLCを有する被験体に用いる場合には、個体のNSCLCのステージを処置の前、後、または最中に決定できる。肺癌のステージ分類の概要を以下に記載する。
T分類は小分類T1〜T4からできており、これにより数字が1から4に増加するに従い原発性腫瘍のサイズおよび局所浸潤は大きくなる。T1およびT2は主にサイズで区別され、例えばT1は3cm未満であるのに対し、T2は3cmより大きい。T3腫瘍は、一般的に胸壁を含み、上部肺溝、横隔膜、縦隔胸膜、心膜、または近位気管支幹を含むが、これらに限定されるものではなく、また切除可能である。T4腫瘍は、縦隔膜を侵しているために外科的切除はできず、また心臓、大血管、気管、竜骨または食道を含むこともあり、また癌性胸水がある場合には、胸膜も関係することがある。
結節ステージは、N1、N2およびN3に分けられる。N1結節は一般的に気管支周辺または同側性の肺門結節を含む。これら結節は胸膜内に位置する。N2結節は、一般的に同側性の縦隔または竜骨下結節を含む。N3結節は、一般的には対側性の肺門、または縦隔、斜角筋結節、または鎖骨上結節を含む。
したがってNSCLCの「ステージ」は、T、NおよびM値の様々な順列に基づいた明瞭なNSCLCの分類である。認められているNSCLCのステージは次の通りである。
このカテゴリーでは、患者はTX N0 MOと分類され、患者は気管支肺分泌物には悪性細胞が検出されるが、気管支強または放射線撮影法による腫瘍の証拠はないことを意味する。
ステージIAは、ステージIBの疾患患者(T2 N0 M0)よりも5年生存率が有意に良好であることに基づき、T1 N0 M0と分類される。これらの患者には手術が好ましい処置である。1997年に、ステージIAに外科的に分類された患者の5年生存率は67%であり、またステージIBの同生存率は57%であった。
ステージIIAの疾患は、T1 N1 M0と定義され、外科的分類ベースの5年生存率は55%である。ステージIIBの疾患は、T2 N1 M0およびT3 N0 M0から構成されている。T3 N0 M0の判定は、リンパ節が関与しない腫瘍の肺外への拡張を表している。T3 N0 M0の分類にはT2 N1 M0も加えられているが、それはその外科的に分類された疾患の5年生存率がそれぞれ38%対39%であり、有意な差がないからである。これらの個体にとっても、手術が第一の処置法である。
ステージIIIA患者は、切除可能と考えられるが、一方ステージIIIB患者は切除できない。ステージIIIA患者は、肺外への拡張(T3)を伴い、かつリンパ節の関与は限定的な(N1またはN2)病変により定義される。結節の関与は、同側性縦隔および/または竜骨下リンパ節にまで広がることがある。これらの患者はT3 N1 M0またはT1-3 N2 M0のいずれかに分類される。1997年時点のステージIIIA疾患の5年生存率は23%であった。
ステージIIIB分類は、対側性縦隔または肺門リンパ節;同側性または対側性の鎖骨上または斜角筋結節;遠位部転移を示さない広範囲の縦隔結節;または細胞学的に陽性である癌性胸水を含むが、これらに限定されない肺外関与を示す患者を指す。これらの患者はT1-3 N3 M0またはT4 N0-3 M0のいずれかに分類できる。1997年では、臨床的に分類された疾患の5年生存率は、多様な処置を受けた状態で5%である。
ステージIVは任意の転移の関与により定義される。これらの患者はいずれかのTおよびいずれかのNを持つM1によって分類される。1997年の時点では、NSCLC患者の4分の1を超える患者が臨床上ステージIVであった。
多様な処置を受ける進行ステージの肺癌を患う生存率の低い個体と同様に、前立腺切除術後に生化学的障害を経験している前立腺癌を有する男性には殆ど処置選択肢がない。彼らの一つの処置選択は、アンドロゲン剥奪処置(ADT)である。残念なことにこの処置法は、特に長期間用いた場合には、顕著な病的状態をもたらす。
本発明は、本発明のMUC-1ベースの製剤を用いた、NSCLC全ステージのNSCLC個体の処置、およびPSA障害を有する、根治的前立腺切除術後の前立腺癌個体を含む、前立腺癌を有する個体の処置を包含する。「処置する」という語句は、製剤またはワクチンが、病的状態、疾患の進行、疾患病原因子、またはその他異常な状態の悪影響を予防、治癒、逆転、緩和、変更、最小化、抑制、または阻止するために有用であることを意味するのに用いる。
別の態様において、処置対象に選ばれたNSCLL個体は、Eastern Cooperative Oncology Group(ECOG)のパフォーマンス ステータス<2、好中球数>1.5×109/L;血小板数>100×109/L;WBC>2.5×109/Lおよびヘモグロビン90g/Lである。ECOGの数値を処置に関する個人の評価に用いてもよいが、処置前、処置中、または処置後に特定のECOG数は必須ではない。
NSCLCを有する個体と同様に、前立腺癌を有する個体もまた試験対象基準および試験除外基準にかけることができる。この場合も、これら基準は単なる指針であり、試験対象基準のいずれかを満たさないか、または試験除外基準のいずれか、またはすべてを満たす前立腺癌罹患個体を、それでも本発明の方法で処置してもよい。前立腺癌を患う個体では、試験対象基準としては次の事項を挙げることができる:(a)根治的前立腺切除術が処置の少なくとも6ヶ月前である、(b)前立腺切除術後の最低値から少なくとも50%の増加を伴い、根治的前立腺切除術後に血清PSA値が3回連続して上昇している、(c)骨盤CTおよび骨スキャンの陰性結果により証明されるように、処置前評価時に悪性疾患の証拠が見られない、(d)ECOGパフォーマンス ステータスが0、1である、(e)血液学検査、肝臓および腎臓機能検査が正常である、(f)書面による同意書を理解し署名している;および(g)前立腺癌のためにホルモン治療(すなわちRP前ネオアジュバント療法)を受けたことがある個体については、血清テストステロンが正常範囲内になければならない。上記したように、これらの基準は単なる指針であり、別の基準を示す多くの個体が本発明の方法を用いて処置を受けることができる。例えば、根治的前立腺切除術を受けたことがない前立腺癌を罹患している個体を処置してもよい。さらにまた、増加した血清PSAを示していないか、またはその個体の血清PSAが連続しては増加しないかもしくは前立腺切除術後の最低値に比べて50%を超える増加を示さなかった個体を処置してもよい。
本明細書に記載するMUC-1ベースの製剤を用いた処置は、様々な効果をもたらすことができる。NSCLCと診断された個体、特にステージIIIBのNSCLCと診断された個体を、MUC-1ベースの製剤を用いて処置することの一つの効果は、生存期間の延長である。同様に、記載のMUC-1ベースの製剤を個体に投与することは、その個体の「生活の質」または「健康に関係する生活の質」に影響を与え得る。生存期間の延長、ならびに生活の質への影響は、処置を受けた前立腺癌を有する個体についても見ることができる。さらに、前立腺癌を有するある個体では、MUC-1ベースの製剤による処置は、PSAの低下、PSAの安定化、またはPSA倍加速度の減少をもたらすだろう。
本発明のMUC-1ベースの製剤を用いてNSCLCまたは前立腺癌を有する個体を処置することの利点の一つは、個体が、本発明の組成物を用いた処置を受けていない個体に比べ生存時間が長くなり得ることである。生存率は、生存者数とMUC-1ベースの製剤による処置を開始した時の個体数とを比較することによって決定できる。別の態様において、生存率は特定のタイプの癌について公表されている生存率と比較してもよい。一般に、生存率は処置開始後の任意の時点で測定してよい。
上記したように、NSCLCまたは前立腺癌を患う個体を、本発明のMUC-1ベースの製剤を用いて処置する別の利点は、個体の生活の質の維持または向上である。医師および監督機関は、個体の「生活の質」(「QoL」)が癌の臨床試験の重要なエンドポイントであることを認識している。例えば、参照として本明細書に組み入れられる、Plunkettら、Clin. Lung Cancer, 5(1):28-32 (2003)、およびCellaら、J. Clin. Epidemiol., 55(3):285-95 (2002)を参照されたい。
いくつかの態様において、本発明のMUC-1ベースの製剤による前立腺癌を有する個体の処置は、PSA濃度の低下、PSA濃度の安定化、またはPSA倍加時間の短縮をもたらすだろう。一般的に、PSA濃度またはPSA倍加時間へのMUC-1ベースの製剤の作用は、任意の時点で測定してよい。例えば、処置後のPSA濃度はベースライン値と比較することができるが、PSA濃度は処置ポイント間、または特定の処置ポイントと処置終了時との間で比較してもよい。ある態様において、PSAの反応は処置中に確認されるだろう。
いくつかの態様において、MUC-1ベースの製剤に対する個体の反応は、T細胞増殖反応アッセイのような免疫機能試験を用いて測定されるであろう。いくつかの態様において、T細胞増殖反応アッセイの結果を用いて、MUC-1ベースの製剤処置が個体に有効であるか判定するであろう。これらアッセイの結果はまた、処置経過中の様々な時点での製剤に対する個体の反応を判定するのに用いてもよい。
本発明は、本明細書に記載のMUC-1ベースの製剤によるNSCLCおよび前立腺癌に加えて、別の癌の処置も包含する。
NSCLC処置に関するリポソームMUC1ワクチンの第II相試験
本実施例は、ステージIIIB局所領域性またはステージIV NSCLCを有する被験体の処置に及ぼすL-BLP25ワクチンの効果を証明する。
対照オープンラベル第IIb相試験には171名の患者を登録した。171名の登録患者のうち65名がIIIBの局所領域性疾患を患っていた。そのうち35名を処置群に、30名を最善な標準的療法に無作為に割り振った。グループは、年齢および人種に関してよくバランスが取られていた。最善の標準療法(BSC)群に比べて、より多くの女性患者およびECOG 0の患者が処置群に割り振られ(51.4%および36.7%、ならびに40.0%および26.7%)、また処置群には、試験登録前の癌処置として化学療法に加えて放射線処置を受けている患者の割合が高かった(91.4対76.7%)。
図2に示すように、ステージIIIB局所領域性患者に観察された2年生存率は、ワクチン群で60%であったのに対し、対照群では36.7%であり、平均余命が23.3%の有意義な延長をすることが証明された。患者集団全体では、2年生存率はワクチン群の43.2%に対し対照群の28.9%であり、平均余命が14.3%延長したことを証明している。図1を参照されたい。最善の標準療法のみを受けたステージIIIB局所領域性患者の生存期間中央値は、最善の標準療法を受けた全グループの全生存期間中央値、13.3ヶ月に近かった。これに対し、L-BLP25による処置を受けたステージIIIB局所領域性患者の全生存期間平均値は、最低平均期間24ヶ月で、まだ最終結論はでていないが、平均余命を少なくとも10.7ヶ月延長することが証明された。このことは、本発明のMUC-1組成物が導入される前は、このカテゴリーの患者にこのような結果をもたらすことができる実行可能な治療選択肢がなかったことから、驚くべきことであると同時に予想外である。
処置患者とBSC単独処置患者の間の、ステージIIIB局所領域性(LR)疾患および癌性胸水(PE)を伴うステージIIIB/IVサブグループの比較分析を、FACT-L全スコア、各種サブスケール、およびTOIについてT検定を用いて行った。全/TOI変化スコアがマイナスであることは、QoLの悪化を示し、一方、全/TOI変化スコアがプラスであることは改善を示す。サブグループ分析は、L-BLP25で処置したステージIIIBのLR患者についてより良いQoLを示した。このことは、BLP25で処置したステージIIIBのLR患者で生存率が臨床上有意に改善することを証明した先のデータと一致する(p=0.0692)。
2週間前:FACT-L QoLアンケート調査の配布。
2週間前:患者を、L-BLP25に最善の標準的療法を加えたものか、最善の標準的療法単独のいずれかに無作為に割り振った(最善の標準的療法としては、対症放射線処置および/または現在の標準的臨床基準に従った第二選択化学療法が挙げられ、また心理学的支援、鎮痛剤および栄養補給もこれに含めてよい)。
2週間前:処置前評価(病歴調査、身体検査、および臨床検査)。臨床的に正当な根拠がある場合には、その他潜在的疾患部位の評価も実施し、他の領域への疾患の進行を除外した。妊娠の可能性のある女性は、処置前に妊娠検査(HCG)が陰性であることを要求した。
3日前:処置群の患者にシクロホスファミドを300mg/m2単回静脈投与した。
第0〜7週:L-BLP25ワクチン接種#1〜#8(第一処置期間)。L-BLP25群の患者について生命兆候を評価し、さらに各L-BLP25処置の前に前回の注射部位を検査した。各L-BLP25処置の1時間後に生命兆候もモニタリングした。患者には、各ワクチン接種後にダイアリーカードを渡して有害事象を記録してもらい、次回来訪時毎に前回の注射部位を評価した。毒性はCALGB拡大基準(CALGB Expanded Criteria)にしたがって等級付けした。
第4週:処置群患者に対する処置および安全性の評価、ならびに免疫血液学検査。患者全員にFACT-L QoLアンケート調査。
第8週:処置評価(身体検査、ECOGステータス、生命兆候、L-BLP25群を対象とした処置部位検査、および有害事象評価)。血液試料を採取して標準的な安全性(血液学および化学)ならびに免疫反応についても分析した。患者全員にFACT-L QoLアンケート調査。
第19週+:維持ワクチン接種(6週間隔)および処置評価(12週間隔)。L-BLP25群の患者については、維持ワクチン接種毎に処置の評価および安全性血液検査を実施したのに加え、初回維持ワクチン接種後1週目に免疫学プロフィール試験を実施した。患者全員にFACT-L QoLアンケート調査。
試験対象基準
1. 年齢が18歳を超える、NSCLCを有する男女で、第一選択(first line)標準化学療法終了後にその疾患が安定しているか、または処置に反応した男女。
2. Eastern Cooperative Oncology Group(ECOG)パフォーマンス ステータス<2、好中球数>1.5×109/L;血小板数>100×109/L;WBC>2.5×109/Lおよびヘモグロビン90g/L。
3. 予想生存期間4ヶ月。
4. 同意書の内容を理解し、署名している。
1. 試験登録前4週間以内の手術または免疫療法。
2. 試験登録前3週間以内の、全身性コルチコステロイドを含む免疫抑制薬投与。
3. 肺癌以外の癌の過去および現在の病歴。
4. 自己免疫疾患または認定されている免疫不全性疾患。
6. 臨床上有意な肝臓または腎臓の機能不全。
7. 顕著な心臓疾患または活動性感染症、または脾臓摘出手術を受けた患者。
T細胞増殖反応アッセイ
本実施例は、本発明のMUC-1製剤が実施例1に示した中央生存値の増加に直接関与することを立証するものである。
根治的前立腺切除術(RP)後のPSA障害例を対象とするリポソームMUC1ワクチンの第II相試験
本実施例は、根治的前立腺切除術後にPSAの増加を示した男性での、PSAレベルに対するL-BLP25ワクチンの免疫療法効果を示すものである。
前立腺切除術後にPSAが3回上昇したことで生化学的障害が証明された男性を登録した。これには16名の患者が含まれ、その年齢中央値は60歳であり、ECOGスコアは0または1、そしてグリーソンスコアの中央値は7であった。一次エンドポイントはMUC1リポソームワクチン(L-BLP25)の効力(PSA反応で測定した)および安全性とした。PSA倍加時間(PSADT)の変化も評価した。患者には、300mg/m2のシクロホスファミド(CTX)を単回、静脈投与し、その後8週間、毎週1回、1,000μgの抗原を含むL-BLP25を皮下ワクチン接種した(処置)。その後、ワクチン接種は6週間隔で49週目まで行った(維持)。処置および維持期間中PSA濃度を測定し、これら期間についてPSADTを計算して登録前のPSADTと比較した。
導入後、評価可能な15名中8名が、PSAの安定化または減少を示した(PSA作業グループの定義による)。試験時の最終PSA測定では、一人の患者は安定したPSAを維持した。15名中6名の患者は試験前のPSADTに比べ、PSADTが>50%延長した。
−16名中8名の個体が第一次処置期間後にPSAの安定を得た;
−16名中1名の個体が、維持期間終了までPSAの安定を維持した;および
−PSADTは、15名中の14名の被験者においてワクチン使用により延長した;16名中の6名の個体が>50%のPSADT延長を得た。
2週間前:処置前評価(身体検査、PSA濃度測定、骨盤CT、および骨スキャン)。
3日前:シクロホスファミド前処置。
第0〜7週:L-BLP25ワクチン接種#1〜#8(第一次処置期間)。
第8週:PSA反応を含む第一次処置期間の評価。

試験対象基準
1. 根治的前立腺切除術が試験登録より少なくとも6ヶ月前である。
2. 根治的前立腺切除術後の最低値から少なくとも50%の増加を伴い根治的前立腺切除術後に血清PSA値が3回連続して上昇している。
3. 骨盤CTおよび骨スキャンの陰性結果により証明されるように、処置前評価時に悪性疾患の証拠が見られない。
4. ECOGパフォーマンス ステータスが0、1である。
5. 血液学、肝臓および腎臓機能検査が正常である。
6. 書面による同意書を理解し、署名している。
7. 前立腺癌のためにホルモン治療(すなわちRP前ネオアジュバント療法)を受けたことのある全患者については、血清テストステロンが正常範囲内である。
1. 試験登録前6ヶ月以内のホルモン治療。

3. 試験登録前1年以内の前立腺床への放射線治療。

図1は、回診、治療群(BLP25または対照)、および正常/異常レベルによる、患者のCA27.29値の度数分布を列挙する。MUCl(CA27.29)の血清レベルを、166名の患者のベースライン試料(BLP25群の患者4名(3−試料が得られなかった;1−不適切な標識)および対照群の患者1名(1−入力のためのデータが得られなかった)に対してデータが生成されなかった)、および153名患者の免疫処置後8の試料に対して決定した。CA27.29レベルによる治療群間の有意差は見られない。
HLAタイピング
HLAタイピングは、治療群A(BLP25)および治療群B(対照)におけるすべての患者に対して実施された。本タイピングは、患者が発現するHLAクラスIおよびIIアレルがワクチンの効果に対して重要であり得る可能性を考察するために行われた。HLAタイピングは、血清学ではなくDNA分析によって実施し、用いた命名法はDNAに対してのものである。各治療群について、所与のアレルに対する%頻度およびnを決定した。生存率分析のために、いずれの治療群においてもn>19を有するHLAアレルを選択した。さらに、このハプロタイプに対して公知のMUCI CTLエピトープがあることから、分析はHLA 11に対して実施した。
Claims (24)
- (i)リポソーム;および
(ii)SEQ ID NO:1のアミノ酸配列、およびSEQ ID NO:2のアミノ酸配列からなる群より選択されるアミノ酸配列を含む少なくとも一つのポリペプチド;
を含む、MUC-1ベースの製剤であって、
以下の段階によって、非小細胞肺癌(NSCLC)を有する被験体を処置するための製剤:
(a)処置のために、MUC-1を発現する癌細胞を有する被験体を選択する段階であって、ここで、被験体が被験体の血清において37.7U/mlまたはより少ない循環するMUC-1レベルを有している段階;および
(b)ある期間にわたって、MUC-1ベースの製剤を被験体に投与する段階。 - 製剤が、リピドA、ムラミルジペプチド、ミョウバン、サイトカイン、およびそれらの組み合わせからなる群より選択される少なくとも一つのアジュバントをさらに含む、請求項1記載のMUC-1ベースの製剤。

- MUC-1ペプチドがSEQ ID NO:2に記載された配列を含む、請求項1〜10記載のMUC-1ベースの製剤。
- (i)リポソーム;および
(ii)SEQ ID NO:1のアミノ酸配列、およびSEQ ID NO:2のアミノ酸配列からなる群より選択されるアミノ酸配列を含む少なくとも一つのポリペプチド;
を含む、MUC-1ベースの製剤の使用であって、
