Description
本願は、2009年12月28日出願の米国特許仮出願第61/290,341号、及び2010年1月27日出願の米国特許仮出願第61/298,800号に基づく利益を主張するものであり、これらの内容を援用するものである。
本発明は、一般的には、医療装置、システム及び方法に関し、より詳細には、副鼻腔内の治療を行うための低侵襲性装置、システム及び方法に関する。
1.制御された量の陰圧又は真空を吸引装置に供給するための吸引ポンプ。
2.生理食塩水、抗生物質溶液、又は他の適当な灌流媒質を供給する灌流ポンプ。
3.電力をドリル又は他の電気的装置に供給する電力モジュール。

6.各種の診断又は治療処置中に用いられる装置に接続されるか、又はこれらと相互作用するX線透視装置、MRI、CT、ビデオ、内視鏡若しくはカメラ、又は他のイメージングモジュール。

(1) 患者の副鼻腔を治療するための方法であって、
拡張装置をガイド装置に沿って前記患者の頭部内に進めることと、
前記拡張装置を使用して、前記副鼻腔につながる開口部を拡張することと、
前記ガイドを前記患者の頭部内に留置したまま、前記拡張装置を前記頭部から抜去することと、
前記ガイドに沿って前記副鼻腔内に組織除去装置を進めることと、
前記組織除去装置を使用して、前記患者の前記副鼻腔、異なる副鼻腔、又は鼻腔の少なくとも1つから組織を除去することと、を含む、方法。
(2) 前記副鼻腔が、上顎洞、蝶形骨洞、前頭洞、及び篩骨洞からなる群から選択される、実施態様1に記載の方法。
(3) 前記ガイド装置が、管腔を有するガイドカテーテル、ガイドワイア、及びガイドカテーテルとガイドワイアとの組み合わせからなる群から選択される、実施態様1に記載の方法。
(4) 前記拡張装置がバルーンカテーテルを含む、実施態様1に記載の方法。

(13) 前記組織を除去することが、ポリープ、粘膜組織、嚢胞、骨破片、骨、及び粘液胞からなる群から選択される種類の組織を除去することを含む、実施態様1に記載の方法。
(14) 前記組織を除去することが、モルセレータ、スネア、複合スネア/カッター、フィルター、高周波切断及び/又は凝固装置、収縮可能なメッシュ装置、ブレードを備えたバルーン、骨カッターアセンブリ、カッターと組み合わされた開口部を有するチューブ、高圧スプレー装置、並びにピンセット把持器アセンブリからなる群から選択される装置を使用することを含む、実施態様1に記載の方法。
(15) 患者の副鼻腔を治療するための方法であって、
前記患者の頭部内にガイドカテーテルを進めることと、
前記ガイドカテーテルを通じ、前記副鼻腔につながる開口部を通じてガイドワイアを進めることと、
前記ガイドワイア上で前記ガイドを通じてバルーン拡張カテーテルを進めて、前記開口部内に前記カテーテルのバルーンを配置することと、
前記バルーンを膨張させることによって前記開口部を拡張することと、
前記バルーンを抜去して、少なくとも前記ガイドワイアを定位置に留置することと、
組織除去装置を前記ガイドワイア上で前記患者の頭部内に進めることと、
前記組織除去装置によって、前記患者の前記副鼻腔、異なる副鼻腔、及び鼻腔の少なくとも1つから組織を除去することと、
前記組織除去装置及び前記ガイドワイアを前記患者の頭部から抜去することと、を含む、方法。
(16) 患者の副鼻腔を治療するための方法であって、
前記患者の頭部内にガイド装置を進めることと、
組織拡張及び除去装置を前記ガイド装置に沿って進めて、前記装置の拡張要素を前記副鼻腔につながる開口部内に配置することと、
前記拡張要素を膨張させることによって前記開口部を拡張することと、
前記組織拡張及び除去装置を使用して、前記副鼻腔から組織を除去することと、
前記組織拡張及び除去装置と前記ガイド装置とを前記患者の頭部から抜去することと、を含む、方法。
(17) 患者の副鼻腔を治療するためのシステムであって、
前記副鼻腔を治療するために、前記患者の頭部内に1又は2以上の装置を案内するためのガイド装置と、
前記副鼻腔につながる開口部を拡張するための、前記ガイド装置に沿って通されるように構成された拡張装置と、
前記副鼻腔の内部から組織を除去するための、前記ガイド装置に沿って通されるように構成された組織除去装置と、を含む、システム。
(18) 前記副鼻腔が、上顎洞、蝶形骨洞、前頭洞、及び篩骨洞からなる群から選択される、実施態様17に記載のシステム。
(19) 前記ガイド装置が、管腔を有するガイドカテーテルを含む、実施態様18に記載のシステム。
(20) 前記ガイド装置がガイドワイアを更に含む、実施態様19に記載のシステム。
(22) 前記開口部が副鼻腔の小口である、実施態様20に記載のシステム。
(23) 前記開口部が人工の開口部である、実施態様20に記載のシステム。
(24) 前記組織除去装置が吸引サブ構造を備える、実施態様20に記載のシステム。
(25) 前記組織除去装置がカッターを備える、実施態様20に記載のシステム。
(26) 前記組織除去装置が、ポリープ、粘膜組織、嚢胞、骨破片、骨、及び粘液胞からなる群から選択される組織を除去するように構成されている、実施態様20に記載のシステム。
(27) 前記組織除去装置がスネアを備える、実施態様20に記載のシステム。
(28) 前記組織除去装置が、無線周波エネルギーを供給するように構成されている、実施態様20に記載のシステム。
(29) 患者の副鼻腔を治療するためのシステムであって、
前記副鼻腔を治療するために、前記患者の頭部内に1又は2以上の装置を案内するためのガイド装置と、
前記副鼻腔につながる開口部を拡張し、更に前記副鼻腔から組織を除去するための複合組織拡張及び除去装置であって、前記ガイド装置に沿って通されるように構成されている、組織拡張及び除去装置と、を含む、システム。
Claims (11)
- 患者の副鼻腔を治療するためのシステムであって、
前記副鼻腔を治療するために、前記患者の頭部内に1又は2以上の装置を案内するためのガイド装置と、
前記副鼻腔につながる開口部を拡張するための、前記ガイド装置に沿って通されるように構成された拡張装置と、
前記副鼻腔の内部から組織を除去するための、前記ガイド装置に沿って通されるように構成された組織除去装置と、を含み、
前記組織除去装置は、細長い管状部材、及び、組織除去部を含み、
前記細長い管状部材は、負の圧力勾配を形成する圧力勾配装置に取り付けられた内部管腔を有し、
前記組織除去部は、前記細長い管状部材の前記内部管腔内を移動して、前記細長い管状部材の遠位端を超えて進み、かつ前記細長い管状部材の前記内部管腔内に引き込むことができ、
前記圧力勾配装置により前記細長い管状部材の前記内部管腔内に負の圧力勾配を形成して、前記組織除去部により除去された組織を、前記細長い管状部材の前記内部管腔内に吸引することができ、
前記組織除去装置が、前記細長い管状部材に取り付けられた吸引サブ構造を備え、前記吸引サブ構造は、前記圧力勾配装置により前記細長い管状部材の前記内部管腔内に引き込まれた組織を保持するフィルターを含む、システム。 - 前記副鼻腔が、上顎洞、蝶形骨洞、前頭洞、及び篩骨洞からなる群から選択される、請求項1に記載のシステム。
- 前記ガイド装置が、管腔を有するガイドカテーテルを含む、請求項2に記載のシステム。
- 前記ガイド装置がガイドワイアを更に含む、請求項3に記載のシステム。
- 前記拡張装置がバルーンカテーテルである、請求項3に記載のシステム。
- 前記開口部が副鼻腔の小口である、請求項4に記載のシステム。
- 前記開口部が人工の開口部である、請求項4に記載のシステム。
- 前記組織除去装置がカッターを備える、請求項4に記載のシステム。
- 前記組織除去装置が、ポリープ、粘膜組織、嚢胞、骨破片、骨、及び粘液胞からなる群から選択される組織を除去するように構成されている、請求項4に記載のシステム。
- 前記組織除去装置がスネアを備える、請求項4に記載のシステム。
- 前記組織除去装置が、無線周波エネルギーを供給するように構成されている、請求項4に記載のシステム。
