Description
睡眠時無呼吸とは、一般に睡眠中の呼吸停止を指す。閉塞性睡眠時無呼吸(OSA)と呼ばれる睡眠時無呼吸の一種は、上気道の閉塞および/または圧潰に起因する睡眠中の呼吸の反復的な中断によって特徴付けられ、通常は血中酸素飽和度の減少を伴う。
Claims (15)
- 移植可能な呼吸センサーシステムであって、
末端に配置されたセンサー部を有するリード線本体を含み、前記センサー部の単一指向性を規定するように、前記センサー部が前記リード線本体の単一側に配置された圧力変換器を含むセンサーリード線、および
前記センサー部から基部方向に第1の距離だけ離間して配置されて、前記リード線本体の固定位置に配置された第1の係留具であって、前記リード線本体の前記単一側の前記圧力変換器を肺に向かって対向する配置に維持して、前記圧力変換器を胸膜腔の呼吸圧力に結合するように、前記リード線本体の両側から外方に延在し、かつ前記リード線本体に対して略垂直に延在する一対の要素で構成された第1の係留具、を備え、
前記第1の係留具に近接した前記リード線本体に配置されており、前記第1の係留具から第2の距離だけ離間して配置されるようにリード線本体の長さに沿って選択的に移動可能である第2の係留具を備え、
患者の身体の側面に沿って前記第2の係留具から基部方向に延在する前記リード線本体の近位部分に電気的に結合され、かつ機械的に結合された移植可能なパルス発生器であって、前記近位部分が前記第1の距離より長く、かつ前記第2の距離より長い第3の距離の長さを規定する、移植可能なパルス発生器を備え、
前記リード線本体の前記近位部分の前記第3の距離は、前記第1の係留具と前記センサー部との間に延在する前記リード線本体の末端部分を肋間アクセス点に配置するように決められており、前記リード線本体の前記末端部分が患者の身体の片側で直接隣り合う一対の肋骨間に延在し、前記リード線本体の末端部分を前記隣り合う肋骨に略平行とし、かつ前記センサー部を前記身体の中心線に向かって配置するように決められている、センサーシステム。 - 固定された前記第1の係留具は、前記リード線本体の長さに沿って前記センサー部から3〜9cmの第1の距離だけ離間して配置された、請求項1に記載のセンサーシステム。
- 前記移植可能なパルス発生器は、前記リード線本体を介して前記センサー部に電気的に結合され、被検出圧力を監視し、該被検出圧力の周期的振幅における実質的変化を識別するように構成された、請求項1に記載のセンサーシステム。
- 前記移植可能なパルス発生器は、被検出圧力の周期的振幅における識別された実質的な変化に基づき、
呼吸機能不全の検出と、
前記移植可能なパルス発生器と刺激リード線とを介して適用され、被検出呼吸機能不全を治療するように電気的な刺激治療の同期施療と、
のうちの少なくとも1つを行なうよう構成された、請求項3に記載のセンサーシステム。 - 前記センサー部は、前記圧力変換器に加えて温度変換器を備え、前記圧力変換器と前記温度変換器のそれぞれが互いに電気的に独立した、請求項1に記載のセンサーシステム。
- 前記センサー部は、前記圧力変換器に電気的に結合された温度変換器を備えた、請求項1に記載のセンサーシステム。
- 前記第3の距離は、前記肋間アクセス点が2番目の高さの肋骨から7番目の高さの肋骨の範囲の肋骨群内の前記隣り合う一対の肋骨の間に生じるように決められている、請求項1に記載のセンサーシステム。
- 前記第1の係留具は、前記センサー部を肋骨に略平行に維持するように、肋骨の長手方向の軸に略垂直な方向に前記第1の係留具のアームが向くよう構成された、請求項1に記載のセンサーシステム。
- 前記センサー部が前記胸膜腔の前記呼吸圧力に直接的に結合され得る皮下の胸膜腔内に前記センサー部を配置するように、前記リード線本体の末端部分の前記第1の距離が決められ、かつ前記センサー部が構成された、請求項1に記載のセンサーシステム。
- 前記センサー部が前記胸膜腔の前記呼吸圧力に間接的に結合され得る皮下の胸膜腔外に前記センサー部を配置するように、前記リード線本体の末端部分の前記第1の距離が決められ、かつ前記センサー部が構成された、請求項1に記載のセンサーシステム。
- 移植可能な呼吸センサーシステムであって、
末端に配置されたセンサー部を有するリード線本体を含み、前記センサー部が圧力変換器を含むセンサーリード線、および
前記センサー部から基部方向に第1の距離だけ離間して配置されて、前記リード線本体の固定位置に配置された第1の係留具であって、前記圧力変換器を胸膜腔の呼吸圧力に対して位置及び向きを維持するように、前記リード線本体の両側から外方に延在し、かつ前記リード線本体に対して略垂直に延在する一対の要素で構成された第1の係留具、を備え、
前記第1の係留具に近接した前記リード線本体に配置されており、前記第1の係留具から第2の距離だけ離間して配置されるようにリード線本体の長さに沿って選択的に移動可能である第2の係留具を備え、
患者の身体の側面に沿って前記第2の係留具から基部方向に延在する前記リード線本体の近位部分に電気的に結合され、かつ機械的に結合された移植可能なパルス発生器であって、前記近位部分が前記第1の距離より長く、かつ前記第2の距離より長い第3の距離の長さを規定する、移植可能なパルス発生器を備え、
前記リード線本体の前記近位部分の前記第3の距離は、前記第1の係留具と前記センサー部との間に延在する前記リード線本体の末端部分を肋間筋肉に配置するように決められており、前記リード線本体の前記末端部分が直接隣り合う一対の肋骨間に延在する、センサーシステム。 - 前記第3の距離は、前記肋間アクセス点が1番目の高さの肋骨から7番目の高さの肋骨の範囲の肋骨群内の前記隣り合う一対の肋骨の間に生じるように決められている、請求項11に記載のセンサーシステム。
- 前記第1の係留具は、前記センサー部を肋骨に略平行に維持するように、肋骨の長手方向の軸に略垂直な方向に前記第1の係留具のアームが向くよう構成された、請求項11に記載のセンサーシステム。
- 前記センサー部が前記胸膜腔の前記呼吸圧力に直接的に結合され得る皮下の胸膜腔内に前記センサー部を配置するように、前記リード線本体の末端部分の前記第1の距離が決められ、かつ前記センサー部が構成された、請求項11に記載のセンサーシステム。
- 前記センサー部が前記胸膜腔の前記呼吸圧力に間接的に結合され得る皮下の胸膜腔外に前記センサー部を配置するように、前記リード線本体の末端部分の前記第1の距離が決められ、かつ前記センサー部が構成された、請求項11に記載のセンサーシステム。
