Claims (16)
- 患者の迷走神経に迷走神経刺激治療(VST)を行うためのシステムであって、
前記患者の頸部に埋め込まれるように構成された複数の電極と、
該複数の電極に対する複数の可能な電極構成から少なくとも1つの電極構成を使用して前記VSTを行うように構成された神経刺激器と、
VSTに対する生理反応の指標を受け入れるように構成されたフィードバック回路であって、少なくとも1つの生理的パラメータを感知するように構成されたセンサ、又は患者入力式又は臨床医入力式フィードバックを受け入れるように構成されたユーザ入力デバイスのうちの少なくとも一方を含む前記フィードバック回路と、
前記神経刺激器にかつ前記フィードバック回路に接続されたコントローラであって、該コントローラが、
患者の頸部の神経ターゲットを刺激するための神経刺激試験ルーチンを実行し、複数の頭部位置の各々に対して、該神経刺激試験ルーチンの実行が、複数の電極構成の試験を含み、
前記複数の頭部位置に対する前記試験された電極構成の各々に対して閾値データを記録し、かつ
前記記録された閾値データに基づいて電極構成を推奨する、
ように構成される前記コントローラと、
を含むことを特徴とするシステム。 - 前記複数の電極は、前記頸部の血管内に埋め込まれるように構成され、前記神経刺激器は、前記迷走神経に神経刺激を経血管的に送出するように構成されることを特徴とする請求項1に記載のシステム。
- 前記複数の電極は、前記患者の頸動脈鞘内に埋め込まれるように構成され、前記神経刺激器は、該頸動脈鞘内の該電極を使用して前記迷走神経を刺激するように構成されることを特徴とする請求項2に記載のシステム。

ことを特徴とする請求項1から請求項3のいずれか1項に記載のシステム。 - 前記コントローラは、前記頭部位置の各々に対して記録された閾値データを使用して前記可能な電極構成の各々に対する最高喉頭振動閾値を判断し、かつ該最高喉頭振動閾値に対して最低値を有する電極構成に基づいて前記複数の可能な電極構成から望ましい電極構成を判断するように構成されることを特徴とする請求項4に記載のシステム。
- 前記閾値データは、閾値電流振幅を含み、
前記コントローラは、前記推奨された電極構成に対して刺激電流を推奨するように更に構成され、
前記推奨された刺激電流は、安全係数を有する前記閾値電流振幅に対応する振幅を有する、
ことを特徴とする請求項5に記載のシステム。 - 前記フィードバック回路は、咳を感知するように構成されたセンサを含み、
前記コントローラは、前記複数の可能な電極構成に対して咳閾値を試験するように構成される、
ことを特徴とする請求項1から請求項6のいずれか1項に記載のシステム。 - 前記コントローラは、前記頭部位置に対して記録された閾値データを使用して前記可能な電極構成の各々に対して最低咳閾値を判断し、かつ該最低咳閾値に対して最高値を有する電極構成に基づいて前記複数の可能な電極構成から望ましい電極構成を判断するように構成されることを特徴とする請求項7に記載のシステム。
- 前記コントローラは、
前記頭部位置の各々に対して記録された閾値データを使用して各電極構成に対する最高の望ましい反応閾値及び最低の望ましくない反応閾値の両方を判断し、
前記最高の望ましい反応閾値に対する最低値を判断し、かつ
前記最低の望ましくない閾値に対する最高値を判断する、
ように構成され、
前記電極構成を推奨する際に、前記コントローラは、前記最高の望ましい反応閾値に対する前記最低値及び前記最低の望ましくない反応閾値に対する前記最高値の両方に応じて該電極構成を推奨するように構成される、
ことを特徴とする請求項1に記載のシステム。 - 前記センサは、圧力センサを含むことを特徴とする請求項1から請求項9のいずれか1項に記載のシステム。
- 前記センサは、加速度計を含むことを特徴とする請求項1から請求項10のいずれか1項に記載のシステム。
- 前記センサは、分時換気センサを含むことを特徴とする請求項1から請求項11のいずれか1項に記載のシステム。
- 前記センサは、インピーダンスセンサを含むことを特徴とする請求項1から請求項12のいずれか1項に記載のシステム。
- 前記センサは、心電図(EKG)を検出するように構成されたセンサを含むことを特徴とする請求項1から請求項13のいずれか1項に記載のシステム。
- 前記センサは、筋電図(EMG)を含むことを特徴とする請求項1から請求項14のいずれか1項に記載のシステム。
- 前記センサは、血圧センサを含むことを特徴とする請求項1から請求項15のいずれか1項に記載のシステム。
