Description
本発明は、好ましくは第VIIa因子形の第VII因子を含み、マンニトールおよびスクロースを含まない液体形または固体形の医薬組成物を提供する。
本出願人は、室温で物理的および化学的に安定で、特に血友病または先天性第VII因子不全に罹患した患者が使用しやすい第VII因子組成物を提供する。
好ましくは第VIIa因子形の第VII因子;
所望により塩酸形のアルギニン;
イソロイシン;
リジン;
グリシン;
クエン酸三ナトリウムまたは塩化カルシウム;および
適切であれば、ポリソルベート80またはポリソルベート20。
好ましくは第VIIa因子形の第VII因子;
10〜40g/lの所望により塩酸形のアルギニン;
4.2〜6.6g/lのイソロイシン;
0.6〜1.8g/lのリジン;
0.6〜1.8g/lのグリシン;
1〜2g/lのクエン酸三ナトリウム二水和物または0〜0.2g/lの塩化カルシウム二水和物;および
適切であれば、0〜0.5g/lのポリソルベート80。
第VII因子(好ましくは第VIIa因子の形の)を0.2〜2g/l、
アルギニン塩酸塩を24g/l、
イソロイシンを6g/l、
クエン酸三ナトリウム二水和物を1.5g/l、
グリシンを1.2g/l、
リジン塩酸塩を1.2g/l、および/または
ポリソルベート80を0.07g/l。
第VII因子(好ましくは第VIIa因子の形の)を0.2〜2g/l、
アルギニン塩酸塩を34g/l、
塩化カルシウム二水和物を0.15g/l、
イソロイシンを6g/l。
1.1:液体製剤F1〜F7の製造
精製し、活性化した第VII因子(10〜20ml)(それぞれ濃度約0.6mg/ml(製剤F1〜F5)および0.4mg/ml(製剤F6およびF7))を、表1の対応する列に記載したように2Lの緩衝溶液に12時間透析した。pH(6.0±0.2)に1M NaOHまたは1M HClを用いて調整した。製剤化したFVIIa溶液をろ過し、ボトルに0.5mL/ボトルの割合で分配した。次に、ボトルをブロモブチル栓で予備的に栓をした。
1.2:上記1.1項で製造した液体製剤F1〜F7からの凍結乾燥製剤の製造
1.3:液体製剤F8の製造
1.4:上記1.3項で製造した液体製剤F8からの凍結乾燥製剤の製造
1.5:液体製剤F9の製造および凍結乾燥
液体で5℃で144時間、次いで25℃で6時間、
凍結および溶解サイクル(4連続サイクル)、
凍結乾燥し、25℃および40℃で6月間保存、
<-70℃で6月間凍結。
液体で5℃で72時間、さらに25℃で6時間、
凍結乾燥し、25℃および40℃で保存、
緩衝液中で<-70℃で凍結し、次いで透析工程:Trometamol(2.42 mg/ml)、NaCl(8.77 mg/ml)、およびマンニトール(30 mg/ml)。
材料および方法
2.1. 動的光散乱(以後「DLS」という)の測定
500マイクロリットルの凍結乾燥から得た各試料をマイクロキュベット(Plastibrand(登録商標)、Wertheim、Germany)に入れ、Zetasizer Nano装置(Malvern Instruments、Worcestershire、UK)に移した。この装置を、633nmの4mW He-Neレーザーおよび非侵襲性後方散乱もしくはNIBS技術を用いて操作する。該タンパク質のモノマーポピュレーションのサイズ分布(強度および容量による)をDispersion Technology Software(Malvern、バージョン4.00)を用いて計算した。該物質および分散物の屈折率は、それぞれ1.33および1.45と定義した。測定時の温度を調節し、20℃に固定した。試験した製剤の品質パラメーターには、モノマー形のタンパク質ポピュレーションの直径およびタンパク質ポピュレーションの散乱強度が含まれる。
2.2. 再構成製剤の視覚的検査
- = 視認可能粒子なし
ε= 視認可能粒子がごくわずか
+ = 視認可能粒子が少量
++ = 視認可能粒子が大量
+++ = 視認可能粒子が非常に大量
全ての試験は独立して3人の異なるオペレーターにより行った。
2.3. 凝集物を検出するためのフィルター試験
- = 検出されるタンパク質由来の視認可能粒子なし
ε= 検出されるタンパク質由来の視認可能粒子がごくわずか
+ = 検出されるタンパク質由来の視認可能粒子が少量
++ = 検出されるタンパク質由来の視認可能粒子が大量
+++ = 検出されるタンパク質由来の視認可能粒子が非常に大量。
2.4. ガラス転移温度(Tg)の測定
2.5. FVIIa/FVII:Ag比の測定
第VII因子(以後「第VII因子抗原」および「FVIIAg」という)のアッセイ:

結果:
1. 製剤F1〜F8
これら製剤について行った試験結果を下記表2〜4に示す。
製剤F9は凍結乾燥形で40℃で6月間後も安定のままである。さらに、この結果は、FVIIは40℃で6日間後もいかなる明らかな分解の兆候も示さないことを示す。
製剤F10を用いて行った試験(表3)は、FVIIaが液体形で5℃で48時間安定であることを示す。
表2から解るように、製剤F10が4回の凍結/溶解サイクル後も安定であり、得られた結果は、FVIIの分解がないことを証明する。
製剤F10は、凍結乾燥形では25℃または40℃で6月間後も安定である。本試験の結果は、FVIIがいかなる分解も受けなかったことを示す。製剤F10(非凍結乾燥)は、-70℃以下の温度で6月間凍結後も安定である。上記結果はFVIIがいかなる分解も受けなかったことを示す。
製剤F11(表4)は、凍結乾燥形では25℃または40℃で6月間後に安定である。本試験の結果は、FVIIがいかなる分解も受けなかったことを示す。製剤F11は、凍結乾燥形では-70℃以下の温度で6月間凍結後も安定である。上記結果はFVIIはいかなる分解も受けなかったことを示す。液体形の製剤F11の安定性試験は、FVIIが5℃で72時間+25℃で6時間後に安定であることを示す。
Claims (16)
- 第VII因子;
アルギニン;
イソロイシン;
リジン;
グリシン;および
クエン酸三ナトリウムまたは塩化カルシウムを含み、
マンニトール、スクロース、およびグリシルグリシンを含まない液体形または固体形の医薬組成物。 - 第VII因子が活性化形(FVIIa)である請求項1に記載の組成物。
- あらゆる糖、ポリオール、およびメチオニンを含まない請求項1または2に記載の組成物。
- さらにポリソルベート80またはポリソルベート20を含む請求項1または2に記載の組成物。
- 以下のものを含む請求項1または2に記載の組成物:
第VII因子;
10〜40g/lのアルギニン;
4.2〜6.6g/lのイソロイシン;
0.6〜1.8g/lのリジン;
0.6〜1.8g/lのグリシン;および
1〜2g/lのクエン酸三ナトリウム二水和物または0〜0.2g/lの塩化カルシウム二水和物
。 - 以下のものを含む請求項3に記載の組成物:
第VII因子;
10〜40g/lのアルギニン;
4.2〜6.6g/lのイソロイシン;
0.6〜1.8g/lのリジン;
0.6〜1.8g/lのグリシン;および
1〜2g/lのクエン酸三ナトリウム二水和物または0〜0.2g/lの塩化カルシウム二水和物
。 - さらに0.5g/l以下のポリソルベート80を含む請求項5記載の組成物。
- 以下のものを含む請求項7記載の組成物:
第VII因子を0.2〜2g/l、
アルギニン塩酸塩を24g/l、
イソロイシンを6g/l、
クエン酸三ナトリウム二水和物を1.5g/l、
グリシンを1.2g/l、
リジン塩酸塩を1.2g/l、および
ポリソルベート80を0.07g/l。 - 以下のものを含む請求項1〜3のいずれかに記載の組成物:
第VII因子を0.2〜2g/l、
アルギニン塩酸塩を34g/l、
塩化カルシウム二水和物を0.15g/l、および
イソロイシンを6g/l。 - アルギニンが塩酸塩形である請求項1〜7のいずれかに記載の組成物。
- リジンが塩酸塩形である請求項1〜7のいずれかに記載の組成物。
- 請求項1〜11のいずれかに記載の組成物から得ることができる凍結乾燥固体組成物。
- 第VII因子と緩衝溶液を混合し、必要であればpHを調整し、ろ過し、次いで必要であれば、乾燥させて固体形を得ることを含む、
請求項1〜11のいずれかに記載の医薬組成物、の製造方法。 - 請求項1〜11のいずれかに記載の固体組成物を注射用水中に溶解させることを含む、治療に用いるための注射用製剤の製造方法。
- 請求項14記載の方法により得ることができる注射用製剤。
- 血友病または先天性第VII因子不全の治療に用いるための、請求項1〜11のいずれかに記載の医薬組成物または請求項15に記載の注射用製剤。
