Description
図1、2、18及び34から36は、確認されるべき本発明の髄核インプラント1の配置及び構造の様々な実施形態を表している。
図3から12及び19から29は、脊柱の脊髄分節Srの二つの隣接する椎骨Va、Vbを示し、その椎間板Diは椎弓根アプローチによって髄核摘出される。
椎弓根経路は従来の輪の経路に対して以下の利点:
髄核摘出の器具による輪の分解を回避すること、
垂直軸を有するリングの形状に椎間支持具を形成するために、フィラメント50、51の導入を可能にするために必要な椎骨の板に対してほぼ中央及び垂直のアプローチを髄核空間Esにおいて構成すること、
閉塞器又は別の手段を用いて椎弓根の骨のチャンネルCaを閉じることによって、髄核空間Esを閉じる可能性を生じること、を有する。
こうして、椎弓根のターゲッティング、椎間板Diの髄核摘出、及び髄核インプラント1の導入は執刀医によって以下のように行われる。
例えば中央内部空間52の範囲を規定するフィラメント50のリングによって充填材2が形成される場合、リング又は球のいずれかとして配置されている別のフィラメント51によって前記空間は満たされる。充填材2は、例えば髄核空間Esを部分的に埋めており且つ擬似輪を形成することができるフィラメント50のリングによって形成されており、擬似輪の機能は、「線維輪(AF)」を保護及び補強し、「線維輪(AF)」のその後の形成又は分解のリスクを回避することである。
執刀医は、椎骨の板に到達するまで別の柔軟なフィラメントのガイドシース19を導入することで、髄核空間Esの内側に出現させることができる(図34)。
充填材2は環状の単一のフィラメント50から形成されていてもよく、フィラメント50の中央内部空間52及び隙間30がゲル若しくはペースト状の生成物、又は線維若しくは注入可能な粘弾性材料からなる生成物53で充填されることによって、「擬似核」を形成する(図18)。
Claims (12)
- 脊髄分節の二つの隣接する椎骨の間に配置されており、脊柱の前記椎骨によって形成される機能ユニットの可動性を確保し且つ衝撃を吸収する椎間板支持具からなる髄核インプラントであって、
少なくとも一つの連続するフィラメントを有する少なくとも一つの充填材からなり、
前記フィラメントは、椎間板の髄核摘出後に形成される髄核空間の内部に環状に配置されており、
前記フィラメントのらせんは中央内部空間の範囲を定め、
椎間板の髄核摘出後に形成される髄核空間の内部に第1のリングを形成する第1のフィラメントを一方に有し、第1のリングによって規定される中央内部空間内にリングを形成するよう配置される第2のフィラメントを他方に有する充填材からなる髄核インプラント。 - 椎間板の髄核摘出後に形成される髄核空間の内部に第1のリングを形成する第1のフィラメントを一方に有し、第1のリングによって形成される中央内部空間内にリングを形成するよう配置される第2のフィラメントを他方に有する充填材からなる請求項1に記載の髄核インプラント。
- フィラメントの配置によって得られるリングによって形成される中央内部空間が、ゲル若しくはペースト状の生成物又は線維若しくは注入可能な粘弾性材料からなる生成物によって充填されている請求項1又は2に記載の髄核インプラント。
- フィラメントは、充填材の内部に線維の形成を促進する有効成分を含む請求項1から3のいずれかに記載の髄核インプラント。
- フィラメントは放射線不透過性であり、髄核空間においてフィラメントの充填及びリング状の配置を観察することができる請求項1から4のいずれかに記載の髄核インプラント。
- フィラメントは、複合の又は組み合わされた生体吸収性材料で作られている請求項1から5のいずれかに記載の髄核インプラント。
- リング状に配置されているフィラメントは、脊髄分節の椎骨の板に対してほぼ垂直である巻き軸を有する請求項1又は2に記載の髄核インプラント。
- フィラメントは、組織への損傷を回避するために細く又は先細になっている自由端を有する請求項1から6のいずれかに記載の髄核インプラント。
- フィラメントは、閉塞器又はフィラメントの移動を防止するための別の手段を用いて椎骨上に保持されている請求項1に記載の髄核インプラント。
- 同一の髄核空間内において相互に隣接して配置される少なくとも二つのフィラメントのリングを有する請求項1に記載の髄核インプラント。
- 請求項1に記載の髄核インプラント用の導入装置であって、
ネジ付きカニューレ又は湾曲したガイドチューブと、
第1又は第2の柔軟なフィラメントのガイドシースと、
を有し、ガイドシースは前記ネジ付きカニューレ又は前記湾曲したガイドチューブの内部に導入され、柔軟なシースの一端が髄核空間内に現れ、他端は、前記髄核空間内にフィラメントを導入可能な前進又は器具手段に接続されていることを特徴とする導入装置。 - 請求項1に記載の髄核インプラント用の導入装置であって、
内部チャンネル及びヘッドを備えるネジ付きカニューレを有し、
ネジ付きカニューレは対応する椎骨Vbに骨固定をするねじ山を一方の端部に備えており、
前記内部チャンネルは、下側の椎骨の上の板又は上側の椎骨の下の板に到達するための側部出口まで延びており、
前記ヘッドは、上側の椎骨又は下側の椎骨の内部において内部チャンネルの側部出口の位置を執刀医に視認可能にする位置指標と、前記カニューレの接続を確保する延長部とを有することを特徴とする導入装置。
