Claims (6)
- 心臓の右心室内のヒス束近傍に電極を送達し右心室および左心室のペースを調整し、固有活動に対する少なくとも1つの心室の同期を向上させる心室ペーシング信号を生成するように構成されるペーシング出力回路を備える装置であって、ペーシング出力回路は、
第1の信号成分を送達するように構成される第1の端子と、
第2の信号成分を送達するように構成される第2の端子と、
基準信号成分を送達するように構成される第3の端子とを備え、
第1および第2の信号成分は基準信号成分に対して互いに反対の極性で提供され、
ペーシング出力回路が、第1極性態様から第2極性態様へと第1および第2の信号成分の極性を繰り返し反転させるように構成され、
第1および第2の信号成分の極性は、N回の拍動又はN回の拍動に対応するN時間後に、第1極性態様から第2極性態様へと反転し、
第1および第2の信号成分の極性は、M回の拍動又はM回の拍動に対応するM時間後に、第2極性態様から第1極性態様へと再度反転し、
NはMよりも大きく、Nは2回以上の拍動を含む装置。 - 請求項1記載の装置において、
ペーシング出力回路が、少なくとも部分的に時間が重複する反極性の第1および第2の信号成分を提供するように構成される装置。 - 請求項1又は2記載の装置において、
心房収縮を感知するように構成される心房感知回路と、
ペーシングAV遅延を判定し、心房収縮からのペーシングAV遅延を同調させるように構成されるコントローラ回路とを備え、
コントローラは、ペーシング出力回路が、心臓の右心室内のヒス束近傍に電極を送達し右心室および左心室のペースを調整し、固有活動に対する少なくとも1つの心室の同期を向上させる心室ペーシング信号を提供する間、ペーシングAV遅延が最大心出力に対して低減された心出力を提供するように規定される値に設定される一時モードを含む装置。 - 心臓の右心室内のヒス束近傍に配置した電極にペーシング信号を供給するため、自動でペーシング装置のペーシング出力回路を作動する方法であって、
前記ペーシング装置のペーシング出力回路を用いて、第1の信号成分と、基準信号に対して第1の信号成分と反対の極性を有する第2の信号成分とを備えるペーシング信号を生成する工程と、
前記ペーシング装置のペーシング出力回路を用いて、第1極性態様から第2極性態様へと第1および第2の信号成分の極性を繰り返し反転させる工程とを備え、
ペーシング出力回路を用いて第1および第2の信号成分の極性を反転させる工程は、N回の拍動又はN回の拍動に対応する時間後に第1極性態様から第2極性態様へと反転させることと、M回の拍動又はM回の拍動に対応する時間後に第2極性態様から第1極性態様へと再度反転させることとを含み、
NはMよりも大きく、Nは2回以上の拍動を含む方法。 - 請求項4記載の方法は、更に、
右心室及び左心室のペースを調整して固有活動に対する少なくとも1つの心室の同期を向上させるため、ペーシング信号が心臓の右心室内のヒス束近傍に配置した電極に心室ペーシング信号を供給する間、最大心出力に対して低減された心出力を提供するため、前記ペーシング出力回路を用いて、自動でペーシングAV遅延を低減する方法。 - 請求項5記載の方法において、
心臓を低減された心出力で回復させるため、所定の期間後に、前記ペーシング出力回路を用いて、自動でペーシングAV遅延を増大する方法。
