実験例1
この実験例において、一側性で継続的に高レベルの耳鳴を患う成人2名について、識別およびテストを行った。対象AS1は耳鳴を2.5年間患っており、中度の難聴(8kHzで50dB)である。また、大きさのランクは10段階の7であり、耳鳴は反対側の耳の純音(8kHz、70dB SPL)に整合する。このため、AS1はカテゴリ1の一側性高度である(図1を参照)。対象AS2は耳鳴を1.5年間患っており、聴力損失は4kHz〜8kHzで25dB〜40dBである。また、ランクは10段階の5であり、耳鳴は反対側の耳の純音(7kHz、69dB SPL)に整合する。AS2はカテゴリ1の一側性中度である。

テスト刺激
音響抑制を行うために、タッカーデイビステクノロジー(Tucker−Davis Technologies)(TDT)システムIII(タッカーデイビステクノロジー社、フロリダ州アラチュア)を用いて音をデジタル的に生成し、ヘッドホンによって伝達した。16ビットD/A変換器を、サンプリングレート44,100Hzとして使用する。スペクトルのスプラッタを回避するために、2.5msのランプをすべての刺激に与える。この刺激には、純音(20Hz〜10kHz)と、中心周波数を100Hz〜10kHzとし、変調周波数を0.5Hz〜中心周波数の25%とする正弦波振幅変調された(sinusoidal amplitude modulated(SAM))トーンと、様々な遮断周波数のバンドパスノイズとが含まれる。僅かに聴き取れる大きさから、マスクを完全に使用するのに十分な大きさまでのレベルが使用される。
対象の聴覚が正常である場合、または幾分残聴がある場合、大きさとピッチについて、耳鳴は外部音に整合される。これは低ピッチ(100Hz)または高ピッチ(10000Hz)を、一側性耳鳴においては反対側の耳に、または両側性耳鳴においては耳鳴が弱い方の耳に伝達することによって行われる。対象は、ピッチが耳鳴よりも高いか低いかについて識別するように求められる。周波数は、整合していると思われるまで、あるいは非常に近いと思われるまで調節される。整合を保証するために音質が純音とあまりに異なる場合、対象は、広いバンドノイズまたは狭いバンドノイズを用いて音と別の音との整合について説明するように求められ、あるいは、振幅変調または周波数変調が試みられる。次いで、耳鳴と外部音との間で大きさを整合するように調節される。正確な整合が必要とされ、PIは二重段階波の適応手順(double−staircaseadaptive procedure)を開発および使用した(ツェン(Zeng)およびターナー(Turner)、1991年)。
少なくとも幾つかの対象においては、本発明の音響的または電気的な耳鳴抑制音のオフセット後、耳鳴のリバウンド増加が生じ得る。幾つかの場合、サプレッサが8時間まで使用されると、リバウンドは数時間持続し得る。リバウンドを軽減する1つの方法は、サプレッサにおいて約1分間のオフセットランプを生成することである。オフセットランプは、1分間またはそれ以上の長い時間にわたり、耳鳴抑制音の振幅を徐々に低下させる。
Claims (21)
- ヒトである対象の耳鳴を治療するための装置であって、
提示音を提示する音声インタフェースと、
前記対象からのフィードバックを受け取る入力インタフェースとを備え、
該装置は、
前記対象により知覚される耳鳴のピッチに整合させるべく調節される前記提示音の周波数に関する前記フィードバックであって、前記入力インタフェースを介して前記対象から受け取った前記フィードバックに基づき、前記対象により知覚された耳鳴のピッチに整合するピッチを少なくとも有する調節音を第1の生成音として生成すること、
前記第1の生成音のピッチと同一のピッチを有する音を第2の生成音として生成すること、
前記第2の生成音を、振幅変調または周波数変調して該変調音を第3の生成音として生成すること、および
前記音声インタフェースを介して前記第3の生成音を前記対象に適用し、それにより前記対象の耳鳴を抑制すること
を実行する、装置。 - 前記第2の生成音を正弦波で変調して前記第3の生成音を生成する、請求項1に記載の装置。
- 前記第1の生成音は前記対象により知覚された大きさを有し、前記第2の生成音は前記知覚された大きさよりも小さい、請求項1または2に記載の装置。
- 前記変調音は100Hzから10kHzまでの中心周波数を有する、請求項1〜3のいずれか一項に記載の装置。
- 前記変調音は0.5kHzから中心周波数の25%までの変調周波数を有する、請求項4に記載の装置。
- 前記第3の生成音は純音、クリックトレイン、スピーチ、または音楽である、請求項1〜3のいずれか一項に記載の装置。
- 前記変調音は、その振幅が徐々に低下されるオフセットランプを含む、請求項1〜3のいずれか一項に記載の装置。
- 前記オフセットランプの持続時間は1分間またはそれ以上である、請求項7に記載の装置。
- 前記変調音の持続時間は8時間以下である、請求項1〜3のいずれか一項に記載の装置。
- 前記音声インタフェースは、スピーカ、ヘッドホン、ヘッドセット、またはイヤホンである、請求項1〜3のいずれか一項に記載の装置。
- 前記入力インタフェースは、前記対象により知覚された提示音の大きさについて、前記対象により選択された1〜10のランク段階からの選択を受け取るように構成されている、請求項1〜3のいずれか一項に記載の装置。
- 前記提示音は、前記音声インタフェースを介して前記対象の耳に3〜6分間提示される、請求項1〜3のいずれか一項に記載の装置。
- ヒトである対象の耳鳴を治療するための装置の作動方法であって、
前記装置は、
前記対象に提示音を提示する音声インタフェースと、
前記対象からのフィードバックを受け取る入力インタフェースとを備え、
前記装置は、
前記音声インタフェースを介して前記対象の耳に提示される前記提示音を生成する工程と、
前記対象により知覚される耳鳴のピッチに整合させるべく調節される前記提示音の周波数に関する前記フィードバックであって、前記入力インタフェースを介して前記対象から受け取った前記フィードバックに基づき、前記対象により知覚された耳鳴のピッチに整合するピッチを少なくとも有する調節音を第1の生成音として生成する工程と、
前記第1の生成音のピッチと同一のピッチを有する音を第2の生成音として生成する工程と、
前記第2の生成音を、振幅変調または周波数変調して該変調音を第3の生成音として生成する工程と、
前記対象の耳鳴を抑制すべく前記音声インタフェースを介して前記対象に適用される前記第3の生成音を生成する工程と、を実行する、作動方法。 - 前記第2の生成音を正弦波で変調して前記第3の生成音を生成する、請求項13に記載の作動方法。
- 前記第1の生成音は前記対象により知覚された大きさを有し、前記第2の生成音は前記知覚された大きさよりも小さい、請求項13または14の作動方法。
- 前記変調音は100Hzから10kHzまでの中心周波数を有する、請求項13〜15のいずれか一項に記載の作動方法。
- 前記変調音は0.5kHzから中心周波数の25%までの変調周波数を有する、請求項
16に記載の作動方法。 - 前記第3の生成音は純音、クリックトレイン、スピーチ、または音楽である、請求項13〜15のいずれか一項に記載の作動方法。
