本出願は、2006年7月25日に出願された米国仮出願第60/833,324号の特典を主張するものである。

本発明の好ましい実施形態は、精製済み、複製不能な、偽型組換えアデノ随伴ウイルス(rAAV)粒子を利用する。アデノ随伴ウイルス(AAV)は、デペンドウイルス属に属するパルボウイルスである。それらは、複製するためにヘルパーウイルスを必要とする、小さな、非エンベロープ型の、一本鎖DNAウイルスである。ヘルパーウイルス(例えば、アデノウイルス、ヘルペスウイルス、またはワクシニアウイルス)との重感染は、機能的に完全なAAVビリオンを形成するのに必要である。In vitroでは、ヘルパーウイルスとの重感染の不在下において、AAVは潜伏状態を確立し、その間ウイルスゲノムはエピソーム形で存在するが、感染性ビリオンは生成されない。ヘルパーウイルスによる後の感染はゲノムを「レスキュー」し、それを複製しウイルスカプシドにパッケージすることができ、それによって感染性ビリオンを再構築することができる。近年のデータは、in vivoでは野生型AAVと組換えAAVの両方が大型エピソームコンカテマーとして主に存在することを示す。
遺伝子治療用のポリヌクレオチドを送達するための他の方法は、アデノウイルス発現ベクターの使用を含む。「アデノウイルス発現ベクター」は、(a)構築体のパッケージをサポートするおよび/または(b)その中でクローニングされた組織および/または細胞特異的構築体を最終的に発現させるのに十分なアデノウイルス配列を含む構築体を含むことを意味する。
それらの遺伝子を宿主ゲノムに組込み、多量の外来遺伝物質を送達し、広範囲の種および細胞型に感染し、特定の細胞系にパッケージされる能力のため、遺伝子送達ベクターとしてレトロウイルスを選択することができる。
単純ヘルペスウイルス(HSV)は神経向性なので、それは神経系障害を治療する際に相当な興味を引き起こしている。さらに、宿主細胞染色体中への組込みあるいは他の場合宿主細胞の代謝の変化なしでの、非分裂状態のニューロン細胞中の潜伏感染、および潜伏期中活性があるプロモーターの存在を確定するHSVの能力は、HSVを魅力的なベクターにする。さらにHSVの神経向性用途に多大な関心が集中しているが、このベクターはその広い宿主範囲を考慮して、他の組織にも利用することができる。
レンチウイルスは、一般的なレトロウイルス遺伝子gag、pol、およびenv以外に、制御または構造機能を有する他の遺伝子を含む複雑なレトロウイルスである。高度な複雑性によって、ウイルスは潜伏感染の過程と同様にそのライフサイクルを調節することができる。レンチウイルスのいくつかの例には、ヒト免疫不全ウイルス(HIV-1、HIV-2)およびサル免疫不全ウイルス(SIV)がある。レンチウイルスベクターはHIV毒性遺伝子を複合的に抑制することによって作製されており、例えば遺伝子env、vif、vpr、vpuおよびnefが欠失し、ベクターは生物学上安全となる。
ワクシニアウイルスベクターは、それらの構築の容易さ、得られる比較的高レベルの発現、広い宿主範囲およびDNAを保持する大容量のために広く使用されてきている。ワクシニアは、顕著な「A-T」優先性を示す約186kbの線状の二本鎖DNAゲノムを含む。約10.5kbの逆方向末端反復がゲノムと隣接している。大部分の必須遺伝子は、ポックスウイルス間で最も高度に保存されている中心領域内に位置するようである。ワクシニアウイルス中の推定オープンリーディングフレームは150〜200に達する。両方の鎖がコード鎖であるが、リーディングフレームの広範囲の重複は一般的ではない。
マウスポリオーマウイルスなどのパポバウイルスの中空カプシドは、遺伝子導入の考えられるベクターとして注目を得ている。中空ポリオーマの使用は、ポリオーマDNAと精製中空カプシドを無細胞系でインキュベートしたとき最初に記載された。新たな粒子のDNAは膵臓DNaseの作用から保護された。再構築粒子は、ラットFIII細胞への形質転換ポリオーマDNA断片の導入に使用された。中空カプシドおよび再構築粒子は全3個のポリオーマカプシド抗原VP1、VP2およびVP3からなる。米国特許第6,046,173号は、遺伝子導入用に外来物質を取り込んだ、パポバウイルスの主要カプシド抗原から形成され少量のカプシド抗原が除去された偽型カプシドの使用を開示する。
他のウイルスベクター、シンドビスウイルスまたはサイトメガロウイルスなどのウイルス由来のベクターなどを、本発明における発現構築体として利用することができる。それらは様々な哺乳動物細胞に関するいくつかの魅力的な特徴を与える(例えば、Friedmann、Science、1989;244:1275〜1281;Horwich et al.、J.Virol.、1990;64:642〜650を参照)。
本発明のさらに他の実施形態では、特異的結合リガンドを発現するように工学的に作製された感染性ウイルス内に、送達する核酸を収容する。ウイルス粒子はしたがって標的細胞の同族受容体と特異的に結合するはずであり、中身を細胞に送達するはずである。レトロウイルスベクターを特異的に標的化することができるように設計した新規の手法は、ウイルスエンベロープへのラクトース残基の化学的付加によるレトロウイルスの化学修飾に基づいて開発された。この修飾は、シアロ糖タンパク質受容体を介した肝細胞の特異的感染を可能にし得る。
本発明のDNA構築体は一般に細胞に送達する。いくつかの実施形態では、しかしながら、導入する核酸は非感染性であり、非ウイルス的方法を使用して導入することができる。

好ましい実施形態では、本明細書で開示する治療作用物質の注入を使用して、心臓疾患に罹患する患者において治療効果を得る。治療する個体は、心臓障害を伴う臨床的特徴に関してモニタリングすることができる。例えば、心臓血管疾患と関係がある悪影響および症状の低下に関して、対象をモニタリングすることができる。例えば、本発明の方法を使用して対象における鬱血性心不全を治療した後、増大した側脳室の短縮率、細胞および無傷動物レベルで増大した心筋収縮性、心臓リモデリングの反転、および異常に高い心臓拡張レベルの細胞質カルシウムの正規化だけには限られないが、これらを含めた、いくつかの臨床パラメータの改善に関して対象を評価することができる。本発明を用いて治療する対象においてモニタリングすることができる他の臨床的特徴には、生存、心臓代謝、心筋収縮性、心拍数、心室機能(例えば、左心室拡張末期圧(LVEDP)、左心室収縮末期圧(LVSP))、Ca2+代謝(例えば、細胞内Ca2+濃度、ピークまたは休止状態[Ca2+]、SRのCa2+ATPase活性、ホスホランバンのリン酸化状態)、力発生、心臓の弛緩および圧力、力収縮頻度関係、心臓細胞の生存またはアポトーシスまたはイオンチャンネル活性(例えば、ナトリウムとカルシウムの交換、ナトリウムチャンネル活性、カルシウムチャンネル活性、ナトリウムカリウムATPaseポンプ活性)、ミオシン重鎖、トロポニンI、トロポニンC、トロポニンT、トロポミオシンアクチン、ミオシン軽鎖キナーゼ、ミオシン軽鎖1、ミオシン軽鎖2またはミオシン軽鎖3、IGF-1受容体、PI3キナーゼ、AKTキナーゼ、ナトリウム-カルシウム交換体、カルシウムチャンネル(LおよびT)、カルセクエストリンまたはカルレチクリンの活性があるが、これらだけには限られない。評価は治療の前、後、または最中に実施することができる。モニタリングすることができる心臓疾患の他の測定値は、短縮率、心拍出量、駆出率、Tau、逆流量、短期間の入院、改善された生活の質、および増大したトレッドミル時間を含む。
正常コブタ中のAAV2/1/SERCA2aの順行性の心外膜冠動脈注入後のベクター特異的DNAの定量化によって、心筋の形質導入を評価するために試験を実施した。
この試験の対照群(1匹の動物)は治療対照ではなかった。この動物には、AAV2/1/SERCA2aの投与以外は他の群とすべて同じ手順を施した。実験動物には、プログラム可能なシリンジポンプを使用した冠動脈注入カテーテルによって左冠動脈(群3〜5中の5匹の動物)に、あるいは直接筋肉内(IM)注射(群2中の1匹の動物)によって、1×1012個のDRPAAV2/1/SERCA2aを投与した。群には2つのベクター濃度および様々な注入速度でAAV2/1/SERCA2aを与えた。ベクターを与えたすべての群を、ベクター投与後に管およびカテーテルの死容積の血液のみで洗浄した。動物は、少なくとも4カ月齢で8〜12kgのGottingenのコブタであった。動物は表1中に示す5群の1つに割り当てた。
第0日に、標準的な手順を使用して動物に麻酔をかけた。後脚の筋肉に抗生物質を予防的に投与した。生存兆候(心拍数、呼吸数、およびO2パルス酸素濃度計)をモニタリングした。ECGはモニタリング目的で得た。
AAV2/1/SERCA2aの投与、直接IM注射:群2
第0日に、標準的な筋肉内用シリンジおよびニードルを使用してAAV2/1/SERCA2aを背面筋肉の四半部に注射した。投与した容積および濃度は以下の表2中に示す。AAV2/1/SERCA2aは使用するまで-70±10℃以下で凍結保存した。使用日に、AAV2/1/SERCA2aを室温で解凍し、動物に投与する準備ができるまで氷上に保持した。反転により軽く混合した後、0.72mLのAAV2/1/SERCA2aストック溶液を1.4×1012DRP/mLで、標準的な筋肉内用シリンジおよびニードルに無菌状態で移した。1回合計投与用量の1×1012個のDRPAAV2/1/SERCA2aを、次いで背面筋肉の四半部の筋肉内に注射した。
直接注入システムは、従来のガイドシース、0.014"のガイドワイヤー、5F注入(ガイド)カテーテルおよび2つのプログラム可能なシリンジポンプを含む、標準的な(市販の)構成要素から構成される。この方法の重要な態様は、ベクター投与に使用した注入時間である。正確な容積は、開始ベクター濃度、管およびカテーテル単位の死容積などに基づいて変わる可能性がある。
反転により軽く混合した後、0.72mLのAAV2/1/SERCA2aストック溶液を1.4×1012DRP/mLで、および1.3mLの製剤バッファー(130mMのNaCl、20mMのHEPESおよび1mMのMgCl2、pH7.4)を、滅菌済みポリプロピレン容器に無菌状態で移し、5.0×1011DRP/mLの濃度にした。動物に投与する直前に、2.0mLの希釈AAV2/1/SERCA2a(5.0×1011DRP/mL)を室温にし、投与する動物由来の1.0mLの元の全血と混合し、3.0mLの最終容積中で3.3×1011DRP/mLのシリンジの最終希釈を得た。注入経路を血液で処理し、次いでベクターを左主冠動脈に0.5分間注射した。この後カテーテルの死容積に血液のみを流した。
反転により軽く混合した後、0.72mLのAAV2/1/SERCA2aストック溶液を1.4×1012DRP/mLで、および7.3mLの製剤バッファーを、滅菌済みポリプロピレン容器に無菌状態で移し、1.25×1011DRP/mLの濃度をもたらした。動物に投与する直前に、8.0mLの希釈AAV2/1/SERCA2a(1.25×1011DRP/mL)を室温にし、投与する動物由来の4.0mLの元の全血と混合した。注入経路を血液で処理し、次いでプログラム可能なシリンジポンプを使用してベクターを左主冠動脈に10分間注射し、次にカテーテルの死容積および第2のプログラム可能なシリンジポンプに血液のみを流した。
反転により軽く混合した後、0.72mLのAAV2/1/SERCA2aストック溶液を1.4×1012DRP/mLで、および1.3mLの製剤バッファーを、滅菌済みポリプロピレン容器に無菌状態で移し、5.0×1011DRP/mLの濃度をもたらした。動物に投与する直前に、2.0mLの希釈AAV2/1/SERCA2a(5.0×1011DRP/mL)を室温にし、投与する動物由来の1.0mLの元の全血と混合した。次いでプログラム可能なシリンジポンプを使用してベクターを左主冠動脈に15分間注射し、次にカテーテルの死容積および第2のプログラム可能なシリンジポンプに血液のみを流した。
麻酔薬の投与後、臨床化学試験用に血液サンプルを回収した。次いで動物を30mg/kgの塩化カリウムの静脈内投与によって安楽死させ、PCR用組織サンプルを図3中に示す心臓の以下の領域:左心室の基底層および中間層中の前壁、後壁、中隔および自由壁、左心室の先端層中の前壁および後壁、右心室の基底層および先端層由来の自由壁から、標準的な技法を使用して回収した。
定量ポリメラーゼ連鎖反応(qPCR)アッセイを使用して、回収した組織サンプル中のAAV2/1/SERCA2aを検出し定量化した。qPCRアッセイは、ABI Prism 7700 Sequence Detection Systemを使用してAAV2/1/SERCA2aに特異的な107塩基対配列を検出する。qPCRプライマーは、図4の略図中に示すようにエクソン14および15に広がる。各組織サンプルから抽出した1マイクログラムのゲノムDNA中で検出するAAV2/1/SERCA2aのコピー数は、標準として標的配列を含むプラスミド(pcDNA3.1_huSERCA2)の連続希釈を使用して定量化した。アッセイの検出の下限は20コピーのAAV2/1/SERCA2a/DNA1μgであり、定量化の下限は200コピー/DNA1μgであった。
実験の結果は図5中に示し、この場合AAV2/1/SERCA2a/DNA1μgのコピー、およびボーラス注射後に得られたレベルの割合として表す相対量を報告する(30秒、群3)。12の異なる心臓サンプル(図3)からの値を各動物用に1つの値に平均化し、その値は動物単位当たりで報告する。ボーラス注射(0.5分)と比較して、10分の注入時間は平均51.6%高いコピー(32%〜69%)をもたらし、15分の注入時間はボーラス注射より76%高いコピーをもたらした。本出願人は、直接IM注射は相当数のコピーをもたらすが、これは第2日に殺傷した動物および細網内皮系(単球/マクロファージ)によるAAVベクターの継続的除去の結果であることを記す(Brain J.D.、et al.、Am J Physiol.Lung Cell Mol.Physiol.、1999;276:146〜154)。AAV1ベクターのIM注射は、心臓の有意な長期間の形質導入をもたらさない(Rip J、et al.、Hum.Gene Ther、2005;16(11):1276〜86)。対照的に、順行性の心外膜冠動脈注入は実施例3中に実証するように組織の安定した形質導入をもたらす。これらの結果は、同量のAAV2/l/SERCA2a(1×1012個のDRP)を各群に投与した場合でも、長い注入時間はAAV2/1/SERCA2a/DNA1μgのより多くのコピーをもたらすことを実証する。
正常ヒツジ中の静脈内注射と比較した左冠動脈への注入による1回投与後の、第2日における3つの異なる用量のAAV2/1/SERCA2aの短時間生体内分布を評価するためにパイロット試験を実施した。
プログラム可能なシリンジポンプを使用して左冠動脈に冠動脈注入カテーテルによって、1匹の動物に1×1013DRPのAAV2/1/SERCA2aを与えた。第2群の3匹の動物には3×1012DRPのAAV2/1/SERCA2a、および第3群の2匹の動物には1×1012DRPのAAV2/1/SERCA2aを、いずれもプログラム可能なシリンジポンプを使用して左冠動脈に冠動脈注入カテーテルによって与えた。すべての注入群に2.5/mLの定速で8分間AAV2/1/SERCA2aを与え、次にカテーテルの容積を2分間血液のみで洗浄した。対照群中の1匹の動物に、2mLの静脈内注射で1×1012DRPのAAV2/1/SERCA2aを投与した。これらの群は表5中で以下に示し、投与スケジュールは表6中に示す。
一般的な手順:順行性の心外膜冠動脈注入によるAAV2/1/SERCA2aの投与(群1〜3)
第0日に、すべての動物群に十分量のヘパリンを投与してACT>300を得た。冠動脈注入カテーテルを使用して、プログラム可能なシリンジポンプを使用して左冠動脈にAAV2/1/SERCA2aを注入した。カテーテル移植用材料および手順は前の実施例1および表3および4中に詳細に記載する。ほぼ同じ手順に従った。

麻酔薬の投与後、動物を30mg/kgの塩化カリウムの静脈内投与によって安楽死させ、PCR用組織サンプルを標準的な技法を使用して回収した。サンプルは心臓の以下の領域:左心室の前壁、左心室の下壁、心室中隔、および右心室の自由壁から回収した。
実施例1中に記載した定量PCRアッセイを使用して、回収した組織サンプル中のAAV2/1/SERCA2aを検出し定量化した。
実験の結果は図6中に示し、この場合心臓組織の各サンプル中のAAV2/1/SERCA2a/DNA1μgのコピーを各動物に関して報告する。非常に低い、定量不能なレベルの(20〜200コピー)のAAV2/1/SERCA2aをもたらした(0.5分までの)静脈内注射と異なり、8分間の注入時間は1×1012、3×1012、および1×1013の濃度で相当数のコピーをもたらした。図6は、1×1013DRPの合計用量は3×1012DRPの合計用量より多数のコピーをもたらし、したがってそれは1×1012DRPの合計用量より多数のコピーをもたらしたことを示す。重要なことにAAV2/1/SERCA2aのコピーは、ベクターを左冠動脈のみに投与しても右心室サンプル中に見られた。
重度の僧帽弁逆流(MR)によって作製した心不全のブタモデルにおける賦形剤対照と比較した、心臓機能に対するAAV2/1/SERCA2aの1回投与の影響を評価するためにパイロット試験を実施した。僧帽弁閉鎖不全症としても知られる僧帽逆流(MR)は、心臓の左心室から左心房への僧帽弁を介した血液の異常な漏出である。逆流量、MRの重度の指標は、左心房へ逆流する血液の量である。
Yorkshire-Landraceブタ、3カ月齢、および30〜40kgをこの試験で使用した。実験群の4匹の動物には、Harvard Clinical Technology(HCT)注入ポンプを使用して左冠動脈に冠動脈注入カテーテルによって、1×1012DRPのAAV2/1/SERCA2aを与えた。AAV2/1/SERCA2aは2.5/mLの一定流速で8分の間送達した。注入の終了後、ガイドカテーテルの端とポンプを結び付ける管から残りの溶液を回収した。次いでこの残りの溶液を約2分の間手作業で注入し、次に10mLの生理食塩水を用いてゆっくりと手作業で洗浄した。対照群は、直接注入(DI)またはV-Focusデバイスのいずれかによって、AAV2/1/SERCA2aの代わりに製剤バッファー(130mMのNaCl、20mMのHEPESおよび1mMのMgCl2、pH7.4)を与えた5匹の動物であった(Preovolos AC、et a1.、J.Extra Corpor.Technol.2006;38(1):51〜2)。
第0日-MRの生成および回復
麻酔、挿管および抗凝固
テラゾール6.0mg/kgIMおよびアトロピン2.0mLIMを麻酔薬の導入のために注射した。吸入イソフルラン(1〜2%)を用いて一般的な麻酔を保ち、周期的な動脈血液ガスをモニタリングした。無菌技法を使用して耳周縁静脈に静脈ラインを配置し、必要に応じて生理食塩水IVを施した。漏出を妨げるよう膨張したカフを有する場所に結び付けた適切なサイズの気管内チューブを動物に挿管した。ECGパッドおよび電極を剃髪した中手骨および中足骨領域に置いて、レコード上のリードI、II、およびIIIをモニタリングおよび記録する。生存兆候(心拍数、呼吸数、O2パルス酸素濃度計、血圧)は、手順全体中約5分間間隔でモニタリングした。ヘパリンを投与して300秒を超えるACTを維持した。
適切な深さの麻酔を得た後、頸部をプロビドン-ヨウ素、次に70%エタノールで調製した。一般的な滅菌技法を使用して切り口を作製し、8Frシースを頚動脈と静脈の両方に挿入した。
ベースラインの心エコー図検査を実施した。
頚動脈シースを介して、鉗子を左心室(LV)に入れた。透視下において、鉗子を使用して後乳頭筋の索条組織を切断して、重度の僧帽弁逆流(MR)を生成した。心不全は、第O日に血管造影法により重度のMRおよび左心室X線写真を確認した後に、左心室腔の膨張およびBPの低下およびEDPの上昇として定義した。
ひとたび生存兆候が手順の終了後に10分間安定状態になった後、切り口を閉じ、イソフルランを中止し、急性心不全を予防するために薬剤を投与した。任意の考えられる痛みを、ブプレノルフィン0.005〜0.01mg/kgをIMで与えて通常通り軽減した。動物にはさらにセファゾリンを与えて感染を予防した(1gのIV)。
MR生成後の最初の3日間、1日当たり3回動物を調べた。動物は急性心不全の兆候、頸部切開部位の創傷感染あるいは任意の痛みまたは不快感の兆候に関して毎日調べた。急性心不全のための薬剤を投与し記録した。
心臓機能の試験
画像は長軸、短軸、乳頭筋の挿入のレベルでの短軸M-モード、および僧帽弁輪のカラードップラー画像を含んでいた。心拍出量、Tau、駆出率、LV短縮率、LV自由壁および中隔の寸法、およびMR評価値を計算し記録した。
(心不全の定義を満たして)第56日まで生存した動物を、2つの群:群1、1×1012DRPの用量でAAV2/1/SERCA2a(n=4)、または群2、賦形剤対照(n=5)の1つに割り当てた。
第56日には、すべての動物群に十分量のヘパリンを投与してACT>300を得た。冠動脈注入カテーテルを使用して、HCT注入ポンプを使用して左冠動脈にAAV2/1/SERCA2aまたは賦形剤を注入した。カテーテル移植用材料および手順は前の実施例2および表3および4中に詳細に記載する。ほぼ同じ手順に従った。
軽く混合した後、0.56mLのAAV2/1/SERCA2aストック溶液を滅菌済みポリプロピレンチューブに無菌状態で移し、10mLの製剤バッファー(130mMのNaCl、20mMのHEPESおよび1mMのMgCl2、pH7.4)で希釈し、1×1011DRP/mLの濃度にした。動物に投与する直前に、10mLの希釈AAV2/1/SERCA2a(1×1011DRP/mL)を室温にし、投与する動物由来の10mLの元の全血と混合し、20.0mLの最終容積中0.5×1011DRP/mLのシリンジ中で最終希釈をもたらした。注入経路を血液で処理し、次いでHCT注入ポンプを使用してベクターを左主冠動脈に8分間送達した。注入の終了後、ガイドカテーテルの端とポンプを結び付ける管から残りの溶液を回収した。次いでこの残りの溶液を約2分の間手作業で注入し、次に10mLの生理食塩水を用いてゆっくりと手作業で洗浄した。
前に記載した群1に使用したのと同じ手順を、賦形剤(製剤バッファー、130mMのNaCl、20mMのHEPESおよび1mMのMgCl2、pH7.4)対照群中の数匹の動物に使用した、ただしAAV2/1/SERCA2aは投与しなかった。他の対照群動物には、V-Focusデバイス(Preovolos AC、et al.、J.Extra Corpor.Technol.2006;38(1):51〜2)を使用して左冠動脈に10分間、製剤バッファー(130mMのNaCl、20mMのHEPESおよび1mMのMgCl2、pH7.4)を投与した。順行性の心外膜冠動脈注入法と以前に試験したV-Focus心臓系送達の両方が、10分間で冠動脈に試験物質または賦形剤のいずれかを送達する。未治療対照とV-Kardia心臓系送達によって投与した賦形剤を得た対照動物の間に差異はない。
麻酔、挿管および抗凝固
第112日に、生存動物に麻酔をかけ、挿管し抗凝固処理した。
画像は長軸、短軸、乳頭筋の挿入のレベルでの短軸M-モード、および僧帽弁輪のカラードップラー画像を含んでいた。心拍出量、Tau、駆出率、LV短縮率、LV自由壁および中隔の寸法、およびMR評価値を計算し記録した。組織ドップラー法も実施した。
心臓停止液を使用することによって動物を安楽死させ、タンパク質およびmRNAの発現分析用に組織サンプルを回収した。
SR小胞を含むミクロソーム分画を、以下の方法によって凍結ブタ心臓から調製した。脂肪および結合組織を除去した約5〜10gの心筋を液体窒素中で粉砕し、Potterホモジェナイザーを用いて5mMのTris-HCl、pH7.4、2mMのEDTA、および8.5%のスクロースを含むバッファー溶液中で均質化した。ホモジェネートは10分間1000×gで遠心分離にかけた。次いで上清を15分間9000×gで遠心分離にかけ、生成した上清を再度2回15分間20000×gで遠心分離にかけた。この後者の20000×g上清中に存在した筋小胞体の小胞を、1時間の110000×gスピンによって後にペレット状にした。ペレットは500μlの均質化バッファー中に再懸濁させた。すべての遠心分離段階は0〜4℃で実施した。
正常未治療非実験対照動物(対照)、AAV-SERCA2a形質導入動物(SERCA2a)、および製剤バッファー(生理食塩水)治療動物由来のタンパク質サンプルを、単離ブタミクロソーム分画から調製し、(ブラッドフォード法を使用して)タンパク質濃度を適合させ、SDS-PAGEによって分離し、ニトロセルロース膜に移した。膜ブロットはSERCA2aに対する抗体(Affinity Bioreagents、1:400希釈)と共に4℃で一晩インキュベートした。反応性バンドを化学発光物質(PE Life Sciences)によって目に見える状態にし、少なくとも3つの独立した実験からのフィルムをスキャニングし、免疫反応性バンドの密度はNIH Imageソフトウェアを使用して評価した。GAPDHのタンパク質レベルは内対照として使用した。SERAC2aのバンドの密度値はGAPDH値に対して正規化した。
SERCA2aのmRNAレベルをRT-PCRを使用して測定した。すべてのRNAは、TRIzol試薬(Invitrogen)を使用して、正常未治療非実験対照心臓(対照)、AAV-SERCA2a形質導入心臓(SERCA2a)、および製剤バッファー(生理食塩水)治療心臓から単離した。組織を完全に破壊した後、クロロホルムを加え、室温での軽いインキュベーション前にサンプルを完全に攪拌した。次いでサンプルを遠心分離にかけ、(RNAを含む)上清は、その下部の細胞残骸を破壊せずに注意深く除去した。上清溶液中のRNAは等体積の氷冷イソプロパノールを加えることによって沈殿させ、4℃において10分間12,000×gで遠心分離によってペレット状にし、75%エタノールで洗浄した。RNAペレットはRnaseを含まない水(Invitrogen)中に再懸濁させた。cDNAは20μlの最終体積でiScript逆転写酵素(Bio Rad)を使用して1μgの全RNAから合成した。GAPDHのmRNAのレベルは内対照として評価した。PCR反応サイクルのアニーリング温度は、それぞれの特異的プライマーセットに関する最適アニーリング温度に従い調節した。
実験の結果は図7〜13中に示す。図7Aは、3匹の非実験対照動物、3匹の製剤注入動物(生理食塩水)および3匹のAAV2/1/SERCA2a治療動物における、SERCA2aタンパク質(上部)およびmRNA(下部)の発現を示すポリアクリルアミドゲルである。図7Bは、タンパク質発現のレベルをGAPDHの発現に正規化した、3つの治療群間のSERCA2aのタンパク質発現を比較するグラフである。この実験はAAV2/1/SERCA2a群が、非実験対照動物または製剤バッファー注入対照群のいずれかより高い、SERCA2aのmRNAおよびタンパク質発現のレベルを有していたことを示す。さらに図7Bは、SERCA2aのタンパク質発現の正規化レベルは、実験生理食塩水注入対照よりSERCA2a注入群中で統計上有意に高かったことを示す。
鬱血性心不全を有する患者における3用量レベルのAAV2/1/SERCA2aの1回冠動脈内投与の安全性、実現性および有効性を、第1相、ランダム化、二重盲検、プラセボ対照用量漸増試験において試験する。
対象集団はNYHAクラスIII/IV慢性心不全を有する成人患者である。対象は4群に分け、3×1011DRPのAAV2/1/SERCA2a、3×1012DRPのAAV2/1/SERCA2a、1×1013DRPのAAV2/1/SERCA2a、3×1012DRPのAAV2/1/SERCA2a、またはプラセボのいずれかを与える。AAV2/1/SERCA2aを与えた対象は12カ月追跡する。プラセボ対象は6カ月後には非盲検状態でありAAV2/1/SERCA2a治療を与える。
注入の目的は、順行性の、心外膜冠動脈注入による、AAV2/1/SERCA2aへの広範、均質な心筋の露出である。同時側性化パターン、閉塞性疾患、および解剖的変異(例えば、バイパス手術後の解剖学的構造)に基づく多数の注入シナリオが存在するが、臨床医の目的はAAV2/1/SERCA2aの1/3を前外側に送達し、1/3を後外側に送達し、かつ1/3を下部/下外側心筋に送達することである。
・ 15mLの血液
・ 45mlの注射用正常生理食塩水
・ 0.3mlの1×1012AAV2/1/SERCA2ADRP/mL溶液
・ 15mLの血液
・ 42mlの注射用正常生理食塩水
・ 3.0mlの1×1012AAV2/1/SERCA2aDRP/mL溶液
・ 15mLの血液
・ 35mlの注射用正常生理食塩水
・ 10.0mlの1×1012AAV2/1/SERCA2aDRP/mL各溶液)
・ 15mLの血液
・ 45mlの注射用正常生理食塩水
60mLの溶液を6mL/分の一定流速で注入する。最終血管造影法を実施して、注入による一時的な解剖学的変化を評価する。ガイドカテーテルは回収する。大腿鞘の除去および/または閉鎖はオペレーターの裁量に従い実施する。
2本の動脈注入した対象/グラフでは、第1の注入は6mL/分の一定流速で40mLの用量である。目的はAAV2/1/SERCA2A生成物の2/3を大きな心筋領域に注入することである。最初の部分の注入後、最後の血管造影法を実施して、注入による一時的な解剖学的変化を評価する。ガイドカテーテルは回収する。
多数の解剖学的シナリオが存在する可能性があり、これらは注入に関する動脈の選択肢を決定して、「主要」灌流心筋領域への生成物の2/3の送達および「小さな」領域への1/3の注入を実施する。
(1)非閉塞性動脈疾患/正常冠動脈、事前の冠動脈バイパス手術(CABG)なし-標準的な左主冠動脈用ガイドカテーテルは左冠動脈(左前下および左回旋枝)系への2/3の注入に関与し、標準的な右冠動脈(RCA)ガイドカテーテルはRCA系への1/3の注入に関与する。
(2)事前のCABG、RCAの完全閉塞なし、左から右への冠動脈の同時側性化-左主用ガイドカテーテルの関与、100%の生成物を送達してすべての(LAD、LCx、およびRCA)領域に注入する。
(3)事前のCABG、LADおよび/またはLCxの完全閉塞なし、RCAからの右から左への同時側性化-RCAはこの状況では「主要」領域になり、2/3の生成物はRCAに送達し、1/3は左冠動脈系に送達する。
(4)多枝閉塞性疾患、事前のCABGなし-2/3の生成物は心筋の大部分として働く領域に、1/3は小さな領域に送達する。
(5)事前のCABG、親の移植手術-2/3はLADの移植片に送達し、1/3はRCAの移植片に送達する。
(6)事前のCABG、混合型閉塞性バイパス手術および原型冠動脈疾患-原型またはバイパス手術、2/3は最大の心筋領域を与える血管に注入し、1/3は小さな領域に注入する。
AAV2/1/SERCA2a投与後の3、6および12カ月と比較したベースラインからの変化に基づいて治療群内および間で評価し比較する一次活性/有効性の終点には、以下の:心肺運動試験によって評価したVO2最大値、左心室駆出率、LV寸法、局所壁運動、拡張機能、および僧帽弁逆流を含めた心エコー評価、6分間歩行試験中に歩いた距離、NYHA分類、およびB型ナトリウム利尿ペプチド(BNP)レベルの1つまたは複数がある。
前述の活性/有効性の終点の1つまたは複数の実質的および有意な改善は、プラセボ群と比較して治療群中で見られる。
Claims (14)
- 体循環から冠循環を分離または実質的に分離せずに、冠循環の一つ以上の血管の血流にポリヌクレオチドを少なくとも8分間にわたって直接注入する段階によって、大型哺乳動物における心臓疾患の治療または予防において使用するための治療用ポリヌクレオチド組成物であって、
前記ポリヌクレオチドをアデノ随伴ウイルス(AAV)ベクターのDNase耐性粒子(DRP)にパッケージし、注入するDRPの合計数が1×10 14 以下であり、
前記治療用ポリヌクレオチドを心臓細胞にトランスフェクトして、前記心臓疾患の治療、予防、又は寛解を生じさせる、治療用ポリヌクレオチド組成物。 - 冠循環の流出が不自然に制限されない、請求項1に記載の組成物。
- 前記疾患が鬱血性心不全である、請求項1または2のいずれかに記載の組成物。
- 前記一つ以上の血管が左冠動脈、右冠動脈、または左冠動脈と右冠動脈の両方である、請求項1から3のいずれかに記載の組成物。
- 前記一つ以上の血管中への前記注入が1.0〜8.0mL/分の流速である、請求項1から4のいずれかに記載の組成物。
- 治療用ポリヌクレオチドによる側脳室前部、側脳室下部、中隔および右心室の心臓細胞のトランスフェクションがPCRを使用して検出可能である、請求項1から5のいずれかに記載の組成物。
- 前記ウイルスベクターがAAV6ベクターである、請求項1に記載の組成物。
- 前記AAVウイルスベクターが、カプシドタンパク質VP1、VP2およびVP3がすべて同じ血清型のAAVのタンパク質ではないように、異種カプシドタンパク質を含む、請求項1に記載の組成物。
- 前記ウイルスベクターがAAV2/1ベクターである、請求項1に記載の組成物。
- 前記ポリヌクレオチドがCMV系プロモーターと作動可能に連結しており、前記ウイルスベクターにパッケージされている、請求項1から9のいずれかに記載の組成物。
- 前記ポリヌクレオチドがSERCA2aコード配列を含む、請求項1から10のいずれかに記載の組成物。
- 前記ウイルスベクターがAAV2/1であり、前記ポリヌクレオチドがSERCA2aコード配列を含み、前記一つ以上の血管が左、右、または左および右冠動脈であり、前記ポリヌクレオチドの前記注入が少なくとも8分間続き、前記注入の前記流速が1.0〜8.0mL/分であり、前記哺乳動物がヒトである、請求項1に記載の組成物。
- 前記心臓細胞の前記トランスフェクションが、ポリヌクレオチド注入前の側脳室の短縮率と比較して、前記注入後4カ月で測定すると側脳室の短縮率を増大させる、請求項1から12のいずれかに記載の組成物。
- 前記流速が5.0mL/分である、請求項1から13のいずれかに記載の組成物。
