Description
本願は、2006年2月9日に出願した米国仮特許出願第60/771,678号の利益を主張する。
本発明は、EO9調合物および方法を用いた膀胱癌の処置に関連する。本発明は、プロピレングリコール濃縮物および/またはヒトの膀胱の移行上皮癌におけるNAD(P)H:キノンオキシドレダクターゼ−1(NQO1)、シトクロムP450オキシドレダクターゼ(P450R)およびグルコーストランスポーター1(Glut−1)タンパク質の発現を利用して、個々に標的化した膀胱癌処置を提供し得る。
膀胱癌は、世界中で7番目に多い癌である。2000年に、英国の男性において4番目に多い癌であり、その年9000例の新規症例が診断された(非特許文献1)。2002年、ヨーロッパにおいて推定280,000例の膀胱癌が存在し、そして2004年には米国において60,000例を超える新規症例が予測された。

表在性および浸潤性腫瘍の間で、NQO1発現における有意な差が見出され、筋肉浸潤性腫瘍において低いレベルで観察された。対照的に、P450RおよびGlut−1は、発現は腫瘍ステージと共に増加するが(特にGlut−1の場合)、TCCの全てのステージおよびグレードにおいて発現していた。それに加えて、Glut−1の発現は、G3腫瘍において有意に増加しており、一方NQO1の存在は低レベルであった。これらの結果は、NQO1およびGlut−1の発現における著しい差が、表在性および浸潤性膀胱TCCの間に存在することを示した。それに加えて、異なる浸透プロファイルを有する、キノンに基づく生体内還元性薬剤の医薬品調製物が見出された。
キノンに基づく生体内還元性薬剤は、プロドラッグであり、酵素的活性化の後、細胞毒性種を産生する。2電子レダクターゼ酵素である、その酵素NAD(P)H:キノンオキシドレダクターゼ−1(NQO1;DT−ジアホラーゼ(DTD)とも呼ばれる)が、有酸素条件下で、キノンに基づく生体内還元性薬剤の活性化に主な役割を果たす。キノンに基づく生体内還元性薬剤はまた、低いNQO1活性を有する細胞を含む低酸素条件下で細胞毒性である。シトクロムP450レダクターゼのような1電子還元酵素は、低酸素条件下で、キノンに基づく生体内還元性薬剤の活性化により主要な役割を果たし得る。前述のことに基づいて、これらのレダクターゼのレベルおよび低酸素条件が、様々なキノンに基づく生体内還元性薬剤を使用する適切さを含む、異なる癌処置の適切さを示し得る。従って本発明は、様々なグレードおよびステージのTCCにおいて、記載されたレダクターゼのレベルおよび低酸素条件を評価した。
I.材料および方法
A.ヒト組織
まず医学研究会議の規定に従って、地域研究倫理委員会(LREC)から同意を得た後、ヒト膀胱移行上皮癌のホルマリン固定、パラフィン包埋標本(n=52)をこの研究のために使用した。全ての患者の詳細は匿名にして機密性を保証し、そして全ての実験を、LRECによって規定されたガイドラインに従って行った。本研究のために使用した腫瘍は、ヒト膀胱TCCの表在性(19 pTa;19 pT1)および筋肉浸潤性(14 ≧pT2)ステージ両方の、全てのグレード(11 グレード1;26 グレード2;15
グレード3)を代表していた。組織マイクロアレイ(TMA)の構築および続く免疫組織化学的分析のために、全ての腫瘍塊を使用した。
組織マイクロアレイ構築物(TMA)を、ヒト膀胱TCCの様々なグレード(G1−G3)および様々なステージ(pTa、pT1、≧pT2)を代表するために、パラフィン包埋ブロックから構築した。組織マイクロアレイ構築物(TMA)を、本明細書中で参考文献に組み込まれる、Bubendorfら(11)の修飾した方法を用いて、Beecher Instrumentsマイクロアレイヤー(Silver Spring、MD、USA)を用いて達成した。簡単には、パラフィン包埋ドナーブロックそれぞれの切片を、ヘマトキシリンおよびエオジン(H&E)を用いて染色し、顕微鏡で調査し、そして関心のある組織を含む領域を、ワックスブロック上にマークした。円筒形のコア(600μM)を、これらの代表的な領域からパンチ生検し、そしてレシピエントブロックに移した。組織のサンプリングは、各腫瘍ブロックから4つのコアを使用して、各親ブロックについて代表的なデータを提供した。TMAブロックあたり26人の患者を代表する全部で108のコアサンプルが含まれ、そして2つのTMAブロックを構築した。5μMの厚さの切片を、レシピエントTMAブロックから切断し、そしてテープトランスファーシステム(tape−transfer system)(Instrumedics、USA)を用いてスライドグラスに載せた。各マイクロアレイブロックから切断される、最初および続く10個の切片ごとに、組織学およびサンプルの完全性の確認のためにH&E染色を行った。ついでTMAスライドを、免疫組織学的分析にかけた。
使用した抗体は、NQO1に対するマウスモノクローナル抗体(University of Colorado Health Sciences Center、Denver、USAのDrs.Siegel およびRossによって提供された)、P450Rに特異的なヤギポリクローナル抗体(Santa Cruz Biotechnology、USA)、Ki67に対するマウスモノクローナル抗体(BD Biosciences、UK)、およびグルコーストランスポーター−1に特異的なウサギポリクローナル抗体(GLUT−1;Dako、UK)を含んでいた。
NQO1、P450R、GLUT−1およびKi67の免疫局在性を、以前に記載されたように(9、10、12、13)および当業者によって理解されるように、免疫組織化学によって評価した。簡単には、抗原の回収および非特異的免疫グロブリン結合の阻害の後、TMAを適切な一次抗体とインキュベートした:1:1のTBSTM(10mMのTris−HCl、150mMのNaCl、0.2%のTween20、5%の脱脂粉乳)で希釈した抗NQO1抗体と約60分間インキュベートした;PBSで1:100に希釈したP450Rについては約90分間インキュベートした;PBSで1:25に希釈した抗Glut−1抗体と約90分間インキュベートした;またはPBSで1:100に希釈した抗Ki67抗体と、4℃で一晩インキュベートした。一次抗体の代わりに、普通のIgGを用いてコントロールを行った。適切なビオチン化二次抗体(1:200に希釈;Vector Labs.、USA)を用い、続いてVectastain ABCキット(Vector Labs.、USA)を用いてシグナルを増幅し、そして3,3’−ジアミノベンジジン(DAB)(Vector Labs.、USA)で可視化することによって、免疫局在化(immunolocalisation)を達成した。次いで切片を、Harris’ヘマトキシリンで対比染色し、脱水し、透明にし、そしてDPXマウンタント(DPX mountant;Sigma、UK)中で標本を乗せた。
陽性の免疫染色を、3人の独立した観察者が半定量的に得点をつけた。NQO1およびP450Rはどちらも、腫瘍内で細胞質に局在化していた。染色の強度および分布に基づく、各腫瘍コアの上皮部分のスコアを、0(染色なし)から4(最大の染色強度)まで割り当てた。平均スコアリング強度を、独立した観察者の結果から、TMAの各コアおよび各腫瘍に関して計算した。その結果を、臨床病理学的なパラメーターに対するあらゆる関連および相関に関して比較した。
NQO1およびP450Rの発現を、以下の臨床病理学的パラメーターと比較した:腫瘍ステージ、腫瘍グレード、腫瘍低酸素(Glut−1発現)、および増殖。SPSSソフトウェアパッケージ、バージョン11.0(SPSS Inc.、Chicago、IL)を用いて、統計学的分析を行った。免疫組織化学的研究において、発現が正常に分布していないので、各カテゴリーの平均発現値は、四分位範囲を有する中央値として報告された。独立した変数間の差を、Mann−Whitney Uテストによって決定した。両側分析における0.05より小さいPの値を、有意と考えた。
A.NQO1タンパク質レベル、腫瘍ステージおよびグレード間の関係
NQO1は、全ての病理学的グレードおよびステージの膀胱腫瘍の上皮において、細胞質に局在化しており、そしてNQO1の発現は、腫瘍間で変動した(図1、表1)。多くの場合において、同じ腫瘍内でNQO1の不均一な発現パターンが観察され、同じサンプル内でNQO1発現の高いおよび低い領域があった(データは示していない)。NQO1は、全ての病理学的ステージ(pTa、pT1、≧pT2)の腫瘍において発現していたが、NQO1の発現レベルは、様々なステージの間で異なっていた(表1)。NQO1発現における有意な差は、表在性腫瘍(pTa+pT1)および筋肉浸潤性腫瘍(≧pT2)の間で観察され、筋肉浸潤性腫瘍において、発現は有意に低かった(p=0.02)。NQO1発現の腫瘍浸潤可能性(potential)に対する逆の関係が、非浸潤性(pTa)および浸潤性(pT1+≧pT2)腫瘍の間に観察された発現の有意な差によってさらに強化される(p=0.03)。全ての病理学的グレードのTCCがNQO1を発現していた(表1)。NQO1の発現は、グレード1またはグレード3のいずれかと比較して、グレード2の腫瘍において有意に高かった(表1)。高度に分化した(グレード1)およびあまり分化していない(グレード3)腫瘍の間に、有意な差は観察されなかった(表1)。
調査した全ての腫瘍は、細胞質に局在化した、検出可能なレベルのP450Rを発現していた。NQO1と対照的に、P450Rの発現は、一般的に腫瘍内で均一であった。代表的な免疫染色を、図1に描写する。P450Rは、全てのステージのTCCに発現していた(表1)。P450Rのレベルは、表在性(pTa+pT1)腫瘍と比較して、筋肉浸潤性腫瘍(≧pT2)において有意に高かった(P<0.01)。NQO1と対照的に、P450Rの発現は、腫瘍ステージの増加と正の関係を示すが、浸潤性(pT1+≧pT2)および非浸潤性(pTa)腫瘍の間に有意差が観察されないことから明らかであるように、腫瘍の浸潤可能性とは関連しない(表1)。全ての病理学的グレードのTCCが、P450Rを発現していた(表1)。P450Rのレベルおよび腫瘍グレードの増加の間に、正の相関が観察された(表1)。
Glut−1タンパク質の発現は、個々の腫瘍標本内でも、および個々の患者サンプル間でも不均一であった。代表的な免疫染色およびその腫瘍ステージおよびグレードとの関係を、図1および表1にそれぞれ示す。Glut−1タンパク質は、調査した全てのステージおよびグレードにおいて発現していたが、Glut−1のレベルは、≧pT2腫瘍(pTa腫瘍と比較して、p=0.05)、およびグレード3腫瘍(グレード1[P=0.03]およびグレード2[P<0.01]腫瘍の両方と比較して)において有意に高かった。それに加えて、統計学的に有意な差(p=0.02)が、非浸潤性(pTa)および浸潤性(pT1+≧pT2)腫瘍の間に存在し、浸潤性疾患は、より高いGlut−1タンパク質の発現と、そして従ってより高いレベルの低酸素と関連していることを示唆する。
Ki67抗原の発現レベルを、腫瘍増殖指数の指標として使用した(表1)。予測されるように、腫瘍グレードの増加(分化の減少)および増殖指数の間に、有意な相関が観察された(p<0.01)。腫瘍増殖および腫瘍浸潤可能性(pTa対pT1+≧pT2)の間に、関係は観察されなかった。対照的に、おそらく筋肉浸潤およびより高い腫瘍グレードの間の関係の結果として、腫瘍増殖は、表在性腫瘍(pTa+pT1[P<0.01])と比較して、筋肉浸潤性腫瘍(≧pT2)において有意に高かった。興味深いことに、腫瘍増殖指数およびGlut−1発現(P=0.01)およびP450R発現(P<0.01)の両方の間に有意な関係が観察されたが、NQO1発現とは観察されなかった。
mRNA発現および腫瘍ステージの増加の間の逆の関係が報告された(15)、以前に発表された研究と一致している。Glut−1に関して同様に、腫瘍グレード(G1およびG2をG3腫瘍とそれぞれ比較した場合、P=0.03および<0.01)、および腫瘍ステージ(pTa腫瘍を≧pT2腫瘍と比較した場合、P=0.05)と共に増加するタンパク質発現は、以前の報告と一致する(16)。筋肉浸潤性TCCと比較して、表在性TCCにおけるP450R mRNAのより高いレベル示す、以前に発表された報告と対照的に(15)、この研究においては、P450Rタンパク質レベルは、筋肉浸潤性(pTa+pT1と比較した≧pT2)疾患において有意に高かった(P<0.01)。それに加えて、P450Rタンパク質発現は、腫瘍グレードの増加(分化の低下)と正の相関を示す(表1)。興味深いことに、おそらくP450R、Ki67、および腫瘍グレードの増加(分化の低下)の間の強い関連の結果として、P450Rの発現はまた、増殖指数と強い正の相関を示した(P<0.01)。それにもかかわらず、高い増殖指数は、膀胱癌における悪い予後と関連することが示されたので(17、18)、P450Rを含む生体内還元性の処置を評価する場合にはこれを心にとめておくべきである。まとめると、免疫組織化学によるタンパク質発現の分析は、Glut−1発現によって示されるような低酸素は、腫瘍ステージ、グレード、および腫瘍浸潤の増加と関連することを示唆する。腫瘍酵素学に関して、この研究は、NQO1レベルは、腫瘍ステージ(および浸潤可能性)の増加の関数として有意に減少し、一方P450Rレベルは腫瘍グレードおよび浸潤可能性と共に増加することを示唆する。
I.材料および方法
A.装置および一般的なアッセイ原理
図2に示すように、記載した実験において使用する装置は、24穴培養プレートの1つのウェルに挿入された、トランスウェルインサート(transwell insert;Costar)を含んでいた。そのインサートは、その底部にコラーゲンでコートした膜を有し、そして従って上部および下部容器の間のバリアおよび細胞が接着および増殖し得る表面の両方を形成した。この研究で使用した細胞系統は、DLD−1ヒト結腸腺癌細胞であり、それが細胞間に強力な接合部を形成し、それによって薬剤が越さなければならない連続的な「バリア」を形成する能力のためにそれを選んだ。薬剤の浸透を評価するために、薬剤を上部容器に加え、そして下部容器における薬剤の濃度を、ある範囲の時間間隔にわたって決定した。
DLD−1細胞は、日常的に10%胎児ウシ血清、ピルビン酸ナトリウム(1mM)、L−グルタミン(2mM)、ペニシリン/ストレプトマイシン(50IU/ml、50μg/ml)を加え、そしてHEPES(25mM)で緩衝化したRPMI1640培地中で維持した。DLD−1細胞(200μlの培地中2.5×105)を、上部容器に加え、そしてCO2を高めた(5%)雰囲気中で、37℃で約3時間、安定させそして膜に接着させた。一旦細胞が接着したら、トランスウェルを24穴プレートの1つのウェルに挿入し、そして600μlの培地を下部容器に加えた。次いで装置を、上部および下部容器両方の培地を毎日替えて、37℃で4日間インキュベートした。以前の研究に基づいて、培養4日後の多細胞層の厚さは、約50μmである。各アッセイに関して、3つのトランスウェルを、組織学的調査および厚さおよび完全性の正確な決定のために除去した(詳細に関しては下記を参照のこと)。
以下の溶液を、下記で記載し、そして図3でまとめたように調製した。
固体EO9を、100%DMSOに溶解して、347mMのストック溶液を作成した。10μlのストック溶液を、10mlの完全RPMI培地(フェノールレッドを含まない)に加えた。EO9の可能性のある沈殿を予防するために、EO9ストック溶液の培地への添加は、連続的に振とうしながら行った。最終的なEO9の濃度は347μMであり、それは4mg/40mlと同等である。
200ミリグラムの炭酸水素ナトリウム(NaHCO3)を、4mlのEDTA溶液(0.5mg/mL、それを0.5Mのストック溶液、Sigmaから新しく調製した)に溶解した。次いでその溶液を6mlのPG溶液(2mlのPG+4mlのH2O)と混合して、20%のPGを含む10mlの最終容量を作成した。この溶液を、EO9(2mg)、炭酸水素ナトリウム(5mg)およびマンニトール(12.5mg)を含む20mlの一般的な試験管に加えた。その溶液を、連続的に振とうしながら、EO9が完全に溶解するまで(約5−6時間)、37℃でインキュベートした。次いで、その溶液を水で1:1に希釈して、10%のPG溶液を産生した。

200ミリグラムの炭酸水素ナトリウム(NaHCO3)を、4mlのEDTA溶液(0.5mg/mL、それを0.5Mのストック溶液、Sigmaから新しく調製した)に溶解した。次いでその溶液を6mlのPG溶液(6mlのPG+0mlのH2O)と混合して、60%のPGを含む10mlの最終容量を作成した。この溶液を、EO9(2mg)、炭酸水素ナトリウム(5mg)およびマンニトール(12.5mg)を含む20mlの一般的な試験管に加えた。その溶液を、連続的に振とうしながら、EO9が完全に溶解するまで(約2時間)、37℃でインキュベートした。次いで、その溶液を水で1:1に希釈して、30%のPG溶液を産生した。
全ての手順を通して、使用した培地は、フェノールレッドを含まない培地を使用したという事実以外は、上記で記載した通りであった(フェノールレッドは、クロマトグラムにおいてEO9と非常に近く溶出する)。EO9を、t=0において、100μlの容量で上部容器に加え、そして下部容器は600μlの培地を含んでいた(常に攪拌していた)。37℃で10分間インキュベートした後、トランスウェルを除去し、そして600μlの新しい培地を含む24穴プレートの新しいウェルに置いた。上部容器の薬剤溶液を除去し、そして100μlの新しい薬剤溶液と置換した(すなわち、上部容器の濃度を、一定の濃度に維持した)。この手順全体を、全部で1時間の期間中、10分間隔で繰り返した。
EO9をIsoluto C18 SPEカートリッジを用いてすぐに抽出した。サンプルを添加する(500μl)前に、カートリッジを1mlのメタノールで満たし、続いて1mlの脱イオン水で洗浄した。1mlの脱イオン水でさらに洗浄した後、EO9を300μlのメタノールで溶出した。サンプルを真空下で(室温で回転エバポレーターにおいて)乾燥し、そして分析に必要になるまで−20℃で保存するか、またはすぐに分析するために移動相(下記を参照のこと)中で再構築した。
EO9のクロマトグラフ分析を、本明細書中で参考文献に組み込まれる、Phillipsら(British Journal of Cancer、65(3):359−64、1992)によって記載されたように行った。簡単には、Hichrom RPBカラム(25cm×4.6mm id、Hichrom Ltd、UK)を、分離のために使用した。Masslynx 3.4ソフトウェア(Micromass Ltd)を有するWaters 996 Photodiode Array Detector(λ1=280nm)を、関心のあるピークのスペクトル分析のために使用した。移動相は、1Mのリン酸緩衝液(1%)、メタノール(42%)、およびHPLCグレード水(57%)から成っていた。流速を、オートサンプラーも組み込んだ、Waters Alliance 2690(Milford、MA、USA)4次ポンプクロマトグラフィーシステムを用いて、1.2ml/mlに設定した。検出限界は、10ng/ml(34.7nM)であった。
各実験に関して、3つのトランスウェルインサートを回収した;1つはコントロールであり、そして2つは実験の終わりであった。各トランスウェルを、10%のホルマリンで1時間固定してから、70%エタノールへ移し、そして一晩保存した。清潔なメスを使用して、膜を注意深くプラスチックのインサートからはがし、そして当業者に公知の標準的な手順を用いて、パラフィンワックスへ包埋するために処理した。Leitz回転ミクロトーン(microtone)を用いて標本を切片にし(5μm)、タンパク質でコートしたスライドグラスに乗せ、そしてヘマトキシリンおよびエオジンを用いて、当業者に公知の標準的な手順も用いて、染色した。多細胞層の厚さを、ステージミクロメーターを用いて較正した接眼レンズの計数線を用いて測定した。各切片に関して5つの測定値を得て、そしてサンプルごとに3つの切片を測定した。
A.代表的なクロマトグラム
図4は、WV14内部標準(保持時間=11.059分)を加えたブランクサンプルのクロマトグラムを示す。6.870分のピークは混入したピークである。図5は、RPMI1640培地中のEO9標準品(1μg/ml(図5A)および20ng/ml(図5B))を示す。図5Aに示すように、EO9およびWV14のピークは、それぞれ8.029分および13.023分に溶出する(7.292分のピークは、上記で記載した混入ピークである)。実験室における温度の変動によって保持時間は動き得るが、相対的な保持時間は一定のままであることに注意するべきである。図5Bは、検出の限界を示す。図6は、0.1%のDMSO(図6A);30%のPG(図6B);20%のPG(図6C);および10%のPG(図6D)中の、EO9標準品のクロマトグラムを示す。
各EO9調製物に関して較正曲線を構築し、そしてその結果を図7に示す。較正曲線は再現性があり、そして図7に示したように、各較正曲線の傾きにわずかな違いが観察された。その違いの理由は明らかではないが、異なる調製物間の抽出効率のわずかな差を反映し得る。0.1%のDMSO、10%のPG、20%のPGおよび30%のPG中のEO9の抽出効率は、それぞれ92.3%、81.7%、79.9%、および81.1%であった。この変動のために、較正曲線は、行った実験それぞれに関して作成した。いずれのクロマトグラムにおいても、明らかな分解産物は見えなかった。
図8で見られるように、PGの濃度が増加するにつれて、EO9の多細胞層浸透速度は減少する。0.1%のDMSO中のEO9に関して、上部容器中の濃度をだいたい一定の値に維持する場合に予測されるように、その動態は直線状である。試験した2つの最も高い濃度のPGにおいて、その動態は全く直線状というわけではない−時間が増加するにつれて速度の累進的な増加が存在することは注目に値する。この効果はおそらく、PGによって誘導される多細胞層の厚さの変化を反映する(図9を参照のこと)。評価したいずれの時点においても、明らかな代謝物または分解産物は観察されなかった。
Claims (8)
- 膀胱癌の処置を選択するのを補助する方法であって、
グルコーストランスポーター1(Glut−1)のレベルまたは炭酸脱水酵素IX(CAIX)のレベルを測定することによって腫瘍内の低酸素レベルを決定する工程;
腫瘍内のNAD(P)H:キノンオキシドレダクターゼ−1(NQO1)およびNADPHシトクロムP450レダクターゼ(P450R)の酵素のレベルを決定する工程;および
該酵素のレベルおよび該低酸素レベルに基づいて処置を選択する工程;
を包含し、ここで、該処置は、(a)該低酸素レベルが高く、該NQO1レベルが低く、かつ、該P450Rレベルが高い場合;または、(b)該低酸素レベルが中程度であり、該NQO1レベルが高く、かつ、該P450Rレベルが低い場合;または、(c)該低酸素レベルが中程度であり、該NQO1レベルが中程度であり、かつ、該P450Rレベルが中程度である場合に、EO9の投与を、単独で、または別の処置と組み合わせてのいずれかで含む、方法。 - 前記処置が、EO9を単独で投与することを含む、請求項1に記載の方法。
- 前記処置が、EO9を別の処置と組み合わせて投与することを含む、請求項1に記載の方法。
- 前記別の処置が、放射線療法および/または少なくとも1つの化学療法剤の投与である、請求項1に記載の方法。
- 膀胱癌の処置を選択するのを補助する方法であって、NAD(P)H:キノンオキシドレダクターゼ−1(NQO1)のレベル、NADPHシトクロムP450レダクターゼ(P450R)のレベル、およびグルコーストランスポーター1(Glut−1)のレベルを含む尺度に基づいて処置を選択する工程を包含し、該処置は、(a)該Glut−1レベルが高く、該NQO1レベルが低く、かつ、該P450Rレベルが高い場合;または、(b)該Glut−1レベルが中程度であり、該NQO1レベルが高く、かつ、該P450Rレベルが低い場合;または、(c)該Glut−1レベルが中程度であり、該NQO1レベルが中程度であり、かつ、該P450Rレベルが中程度である場合に、EO9を単独で、または別の処置と組み合わせてのいずれかで投与することを含む、方法。
- 前記処置が、EO9を単独で投与することを含む、請求項5に記載の方法。
- 前記処置が、EO9を別の処置と組み合わせて投与することを含む、請求項5に記載の方法。
- 前記別の処置が、放射線療法および/または少なくとも1つの化学療法剤の投与である、請求項5に記載の方法。
