Description
本発明は、活性化されたタンパク質C(APC)およびタンパク質C阻害剤(PCI)の複合体(以降、APC-PCIと呼ぶ)を、血漿中での測定によりヒト疾患を診断およびモニターする方法に関する。この濃度は、全身性炎症性応答症(SIRS)を増大させる状態では、タンパク質C(PC)の不適切に低い活性化またはPCの高レベルの活性化のいずれかを示す。これは、特に敗血症、重症敗血症または敗血症ショックを有する患者に関係があり、ここで不適切に低いPCの活性化は、APCの静脈投与によるか、または他の抗炎症剤および/または抗凝固剤による治療的介入のための指標を提供し得る。そのようなものとして、本発明は、国内医薬の分野、特に救命用または集中治療用医薬、感染症疾患医薬、および敗血症後の手術または外傷に関連する手術と関係がある。
全身炎症性応答症(SIRS):2以上の下記兆候をもたらす炎症系の全身活性化の影響:心拍数>90拍/分、体温<36℃または>38℃、呼吸速度>20呼気/分またはPaCO2<32mmHg、白血球計測値<4x109または>12x109細胞/Lまたは>10%未熟好中球の存在。
近年、敗血症における治療的介入の可能性は、炎症および凝固系統の全身活性化を調節する生物学的治療剤の開発により拡張されてきた。2つのかかる治療剤は、組換えDNA技術により調製されたヒトAPCであり、またヒト血漿から精製されたアンチトロンビンである。両方の薬剤は、イン・ビトロ試験系または動物モデルにおける抗凝固性および抗炎症活性を示す。しかしながら、非選択的敗血症患者をこれらの薬剤により処置した結果は、生存率の増加に関して不十分(APC)であるか、または検出不可能(アンチトロンビン)であった。APCの場合において、生存率に対する有利な効果は、重症敗血症かつ25以上のAPACHEIIスコアを有する患者群のみに見られた(Bernard et al.、2001)。APACHEIIは、疾患の重症度の分類体系であり、これは12の所定の生理学的測定値の初期値、年齢および以前の健康状態を基にしたポイントスコアを使用して一般的な測定単位を提供するもので、院内死亡のリスクと相関する0〜71の範囲のスコアとする(Knaus et al.、1985)。
我々は、上記したとおりAPACHEIIスコア単独を用いるよりも不適切な処置を回避する際により効果的であると考えられる、APC処置のために敗血症患者を選択する方法を発明した。この新規方法は、内因性PCの高い活性化を既に有する患者にはAPC投与が有益ではないようであるという考察に基づいており、またPC活性化において増加を示さない敗血症患者は、敗血症患者よりも全体として死亡率が高いという新たな所見に基づいている。
i)サンプル採取後にさらなるAPC-PCI複合体の形成を防止する薬剤を含むチューブに、患者から血液サンプルを採取する工程、
ii)該血漿を分離する工程、
iii)該血漿中のAPC-PCI複合体の濃度を測定する工程、
iv)該濃度を、0.75〜1.5の間の修正係数を乗じた健康な個体由来の血漿サンプル中で見出されたAPC-PCI濃度の上限に相当するカットオフ値と比較する工程、
により、敗血症患者における本来のPCの活性化状態を決定することを含むもので、
ここで該カットオフ値より低い濃度は、内因性PCの適切な活性化がないことを示し、また外因性APCの投与による処置に対する指標を提供するものである。
本発明の好ましい実施形態において、我々は、血漿中のAPC-PCI複合体の濃度を決定するための免疫化学的方法を開発し、病院の集中治療施設に入院した135人の継続患者のAPC-PCI複合体を測定するために本方法を用いた。実施例1に提示したこの試験の結果は以下のとおりである。驚くべきことに、入院中に、通常の上限に近い値と等しいかまたは高い血漿中のAPC-PCI複合体の濃度を生成するに十分なPCの活性化を示さなかった敗血症患者のサブグループは、全敗血症患者よりも高い死亡率を示したことが判った。高い死亡率および低い死亡率の患者間の適切な区別を提供するカットオフ値は、通常の上限付近であって、また通常濃度の上限に関して1付近の係数を乗じて規定できた。入院中にPCの中程度の活性化を示した患者のサブグループは、全敗血症患者の死亡率よりも低い死亡率を示した。一方で、入院中にPCの高い活性化を示した患者のサブグループはまた全敗血症患者よりも高い死亡率を示した。
集中治療に入院した非選択患者の臨床試験
本発明の好ましい実施形態において、我々は、血漿中のAPC-PCI複合体の濃度を決定するためのサンドイッチ酵素結合免疫吸着剤アッセイ法(ELISA)(実施例2)を開発し、これを用いて、入院期間中病院の集中治療室に入院した135人の患者中のAPC-PCI複合体を測定した。53人の患者は、入院期間中(敗血症群)、ある段階にて敗血症を示し、また82人は示さなかった(非敗血症群)。入院期間中に通常の上限に近い値と同等またはよりも高い血漿中のAPC-PCI複合体の濃度となるように十分なPCの活性化を示さなかった敗血症患者のサブグループは、全敗血症患者の死亡率(32%)よりも高い死亡率(44%)を示し、相対死亡率136%を示した。この場合において、最適な識別を示すカットオフ値は、0.26ng/mLである通常値の上限に近い0.25ng/mLであることが判った。通常の上限が最良のカットオフを見出すために乗じられるべき係数は0.25/0.26または0.96であった。血漿中のAPC-PCI複合体の最高記録濃度を0.26-0.72ng/mLの範囲とするに十分なPCの中程度の活性化を入院中に示した患者のサブグループは、相対死亡率41%を示す全ての敗血症患者よりも低い死亡率を示した(13%)。一方で、入院中に、0.72ng/mLを超える血漿中のAPC-PCI複合体の最高記録濃度をもたらすに十分なPCの高い活性化を示した患者のサブグループは、全ての敗血症患者の死亡率よりも高い死亡率(50%)を示し、156%の相対死亡率を示した。
サンドイッチELISAによる血漿中のAPC-PCI複合体の決定
標準的かつ計測材料として使用するためのAPC-PCI複合体をStrandbergら(2001)に記載した方法の改変により調製した。ヒト組換え活性化タンパク質C(Xigris; Ely Lilly)を、Strandbergら(2001)に記載の緩衝液中に、マルモ大学病院の臨床化学部(Clinical Chemistry、University Hospital of Malmoe、Sweden)から入手した精製ヒトPCIの1.5倍モル過剰を用いて、室温でインキュベートした。指定時間での、インターバル時点で、GruberおよびGriffin(1992)の免疫酵素アッセイを改変して残存APC活性を決定した。全ての検出可能なAPC活性は、1時間までに消失した。グリセロールを、終濃度50%v/vまで調製液に加え、該溶液のアリコートを-135℃で保存した。

Claims (12)
- 敗血症患者が、活性化されたタンパク質Cまたは他の抗凝固剤または抗炎症剤の投与により、処置されるべきかまたは処置されるべきでないかを決定する方法であって、
下記工程:
i)該患者からの血漿または全血液サンプル中の、活性化されたタンパク質Cおよび活性化されたタンパク質Cに結合するセルピンの複合体の濃度を決定する工程、ここで該セルピンは、タンパク質C阻害剤、α1-抗トリプシン、α2-抗プラスミンおよびC1-エステラーゼ阻害剤から選択される、
ii)該濃度を、健康な個体からの血漿または全血液のサンプル中の上記複合体の通常濃度の上限に相当する予め決定された第一のカットオフ値と比較する工程、
を含み、
該カットオフ値より低い濃度は、
a)敗血症患者についての平均よりも高い死亡率を示す、および
b)該処置のための指標を提供する、
方法。 - 活性化されたタンパク質Cに結合するセルピンが、タンパク質C阻害剤である、請求項1の方法。
- 血漿中のカットオフ値が、0.20ng/mLまたは0.20ng/mL〜0.41ng/mLの値である、請求項2の方法。
- 患者のAPACHEIIスコアを決定する工程をさらに含む請求項1〜3のいずれか一項に記載の方法であって、ここでカットオフ値より低いバイオマーカーの濃度と25と等しいかまたは25よりも大きいAPACHEIIスコアとの両方の存在は、
a)敗血症患者にとっての平均よりも高い死亡率、および
b)外因性の活性化されたタンパク質Cまたは他の抗凝固剤または抗炎症剤の投与により患者を処置するための強力な適応指標、
に関する指標となる、方法。 - 工程i)およびii)を1回以上繰り返す工程をさらに含んでいる、請求項1の方法。
- 24時間以内で、工程i)およびii)を繰り返す工程をさらに含む、請求項5の方法。
- 敗血症患者の処置が開始されたかまたは完了された後に工程i)およびii)を繰り返す工程をさらに含む、請求項5または6の方法。
- 活性化されたタンパク質Cに結合するタンパク質が、タンパク質C阻害剤である、請求項5〜7のいずれかの方法。
- 敗血症患者が、敗血症患者についての平均よりも高い死亡率を有するかまたは有さないかを決定する方法であって、
該方法は、下記工程:
i)患者からの血漿または全血液のサンプル中のタンパク質C活性化の指標となる、活性化されたタンパク質Cおよび活性化されたタンパク質Cに結合するセルピンの複合体の濃度を決定する工程、ここで該セルピンは、タンパク質C阻害剤、α1-抗トリプシン、α2-抗プラスミンおよびC1-エステラーゼ阻害剤から選択される、
ii)該濃度を第二のカットオフ値と比較する工程、を含む方法、
ここで、該第二のカットオフ値は、敗血症患者にとっての平均よりも低い死亡率に関連のある内因性タンパク質Cの中程度の活性化を排除するために選択されており、該カットオフ値を超える濃度は、敗血症患者にとっての平均よりも高い死亡率の指標となる。 - 活性化されたタンパク質Cと結合するセルピンがタンパク質C阻害剤である、請求項9の方法。
- 第二のカットオフ値が、0.70ng/mLまたは0.70ng/mL〜1.05ng/mLの値である、請求項10の方法。
- 活性化されたタンパク質Cおよびその結合タンパク質の複合体が、複合体上のネオエピトープ、結合タンパク質、該結合タンパク質上のネオエピトープ、タンパク質Cまたは活性化されたタンパク質Cに特異的に結合する1以上の分子により測定される、請求項1または9の方法。
