Description
本出願は、脊椎安定化装置の分野に関する。詳細には、本出願は、脊椎の体節単位(segmental unit)と共に使用されるように構成された、後方安定化ユニット(posterior stabilization unit)に関する。
脊椎手術は、脊椎の機能的体節単位が適切な位置から動かされるか、または別様に損傷を受けた場合に生じる脊椎の状態を治療するために、医業において一般的に用いられる。脊椎の状態を治療するのに使用される処置の例は、椎間板置換術、椎弓切除術、および脊椎融合術を含む。
脊椎安定化システム用の動的ねじの様々な実施形態が本明細書で開示される。一実施形態では、脊椎安定化システム用の動的ねじは、骨係合部材および受容器部材を含む、少なくとも1つの骨アンカー組立体を含む。骨係合部材は、受容器部材内部に保持されたねじ頭部、および受容器部材から延びるねじシャンクを含む、骨ねじを含むことができる。ねじ頭部は、受容器部材の内部に旋回可能に保持されてよい。細長い接続部材が、骨係合部材に旋回可能に接続される。細長い接続部材は、2つ以上の骨アンカー組立体間にまたがるロッドとして設けられることができる。細長い接続部材は、骨アンカー組立体の受容器部材に旋回可能に接続される。
図1および図2を参照すると、例示的な後方動的安定化(PDS)システム22が、脊椎の2つの椎骨20、21間に配列されて示されている。PDSシステム22は、複数の骨アンカー24であって、骨アンカー24間に延びる複数の細長い接続部材26を備えた、複数の骨アンカー24を含む。複数の接続部材26は、ロッド、バー、または他の細長い接続部材を含みうる。各骨アンカー24は、椎骨20または21のうち一方の椎弓根に固定される。それぞれの細長い接続部材26は、上方椎骨20に固着された第1の骨アンカーと、下方椎骨21に固着された第2の骨アンカーとの間に延在する。
図3および図4から、アンカー組立体24内部におけるロッドの中心軸とロッド26の旋回点との間にオフセットが存在していることが分かる。詳細には、図4に示されるように、ロッドの中心軸80(点線80で示されている)が、アンカー組立体24内部のロッドの旋回点(「X」82で示される)から、移されている。このオフセットにより、ロッドの、必須の運動力学および荷重要件を制御するのを助けるために用いられうる一実施形態が提供される。これらの実施形態では、ロッド26は、アンカー組立体に固着され、骨ねじ34を保持する受容器部材40に対して旋回することができない。
図8〜図9を参照すると、PDSシステムのための骨アンカー24の代替的実施形態が示されており、ロッド26は、骨アンカーの受容器部材40に旋回可能に接続されている。骨アンカー24は、受容器部材40内部に保持されたねじ頭部36を有する骨ねじ34を含み、ねじシャンク38が、受容器部材40から延びている。
図16〜図18は、ロッドの旋回点が、ロッドにより定められた中心軸または他の軸からオフセットしている前記のデザイン(例えば図3〜図7)のいずれかと共に用いられるように構成された、代替的実施形態を示す。図16〜図18の実施形態において扱われた利点は、低い外形の動的ねじの利点である。この実施形態では、骨アンカー24の下方部分は、図3および図4のものに類似しており、軸受部材48および骨ねじ34の頭部36を受容するよう構成された、骨ねじ空洞部48を含んでいる。インサート60が、軸受部材54の上に設けられ、インサート60は、軸受部材を空洞部48においてロックする。これも図3および図4と同様に、ロッド空洞部/通路52が、インサートの上に設けられている。しかしながら、図3および図4とは異なり、固着ねじがロッド26の上に設けられていない。代わりに、ロッド26および骨アンカー24は、固着ねじを使用せずに、ロッド26をロッド空洞部52の中にロックする、特徴部を含む。

Claims (8)
- 脊椎安定化システムにおいて、
a)概ね球形状の頭部、およびシャンクを有する骨係合部材と、
b)前記骨係合部材に旋回可能に接続された受容器部材であって、前記受容器部材は、骨ねじ空洞部および接続部材空洞部をその中に規定する側壁を含む、受容器部材と、
c)前記受容器部材の前記接続部材空洞部の中に延びる細長い接続部材と、
d)前記骨係合部材の前記頭部と共に骨ねじ空洞部の中に位置付けられる軸受であって、前記軸受は、前記骨係合部材の前記球形状の頭部に概ね一致する内側表面を有し、前記骨係合部材の前記シャンクが前記受容器部材から延びている状態で、前記軸受の内部に前記骨係合部材の前記頭部を回転可能かつ旋回可能に保持し、前記軸受は、さらに、前記骨ねじ空洞部を規定する前記受容器部材の前記側壁の内側部分に係合するように構成された外側表面を有する、軸受と、
e)前記受容器部材内部で前記細長い接続部材と前記軸受との間に位置付けられた保持部材であって、前記保持部材は、前記骨係合部材の前記頭部および前記受容器部材の前記側壁の前記内側部分と協働し、前記軸受を受容器部材の前記骨ねじ空洞部内部で前記骨係合部材の前記球形状の頭部の周りに固定するように構成されている、保持部材と、
を含み、
前記軸受は、左側部材および右側部材を含む割り軸受である、脊椎安定化システム。 - 請求項1に記載の脊椎安定化システムにおいて、
前記保持部材は、前記軸受、および前記細長い接続部材の双方に係合する、脊椎安定化システム。 - 請求項1に記載の脊椎安定化システムにおいて、
