Description
このPCT国際出願は、発明の名称が「神経再生ペプチドの合成類似体」である2007年10月17日提出の米国仮特許出願第60/999,292号、及び発明の名称が「神経再生ペプチドの合成類似体」である2007年10月18日提出の米国仮特許出願第60/999,503号の優先権を主張する。これらの両仮特許出願は、個々に参照によって本明細書に完全に組み込まれたかのように、参照によって本明細書に完全に組み込まれる。
(発明の分野)
この発明は、神経の再生、移動、増殖、分化及び/又は軸索伸長特性を有するペプチドの合成類似体に関する。これらのペプチドは「神経再生ペプチド」又は「NRP」と呼ばれる。特に、この発明は、NRPの1つ以上の生物学的特性を有する比較的小さいペプチドの類似体に関する。
神経再生ペプチド(NRP)は、哺乳動物における神経機能を促進するために望ましい特性を示すことが分かっている分類のペプチドである。これらの機能には、神経生存、神経増殖、神経細胞伸長、神経移動及び神経細胞分化が含まれる。数種のNRPが以前に記載されており、米国特許出願第10/225,838号及び第10/976,699号、PCT/US02/026782、PCT/US2004/036203、PCT/US2006017534及びPCT/US2006026994で開示されているものが挙げられる。上記各特許出願は、個々に参照によって完全に本明細書に明白に組み込まれたかのように、参照によって完全に本明細書に明白に組み込まれる。
これまでに開示されたNRPは望ましい薬力学的特性を有し、神経の再生、移動、増殖、分化及び/又は軸索伸長を促進する。我々は最近、改良された薬物動態学的特性をも有する合成NRP類似体を発見した。当技術分野では、NRPの薬力学的特性と同様の望ましい薬力学的特性を有するのみならず、改良された薬物動態学的特性をも有し、及び/又は化学的に安定な合成分子若しくは修飾ペプチドが要望されている。
本発明の分子を同定するため、用語「NRP化合物」、「NRPの類似体」、「配列番号:」及び簡単にするためその他の用語を使用するが、それらの完全な特徴を提供するものでないことを理解すべきである。従って、「NRPの類似体」は特定のアミノ酸配列、特定の二次元表現の構造を有するとして本明細書では特徴づけてあるが、請求した実際の分子は、三次元構造、一定の結合についての移動度及び全体として分子の他の特性などの他の特徴を有することが分かる。この発明の主題は、分子自体及び全体としてのその特性である。
また当然のことながら、ペプチドの「NRP」との命名は、それが神経作用のみを有することを意味しない。むしろ、用語NRPは上記特許出願に記載されているのと同様の構造成分を有するペプチドを含む意図であるが、他の細胞型、組織、及び/又は器官にも作用しうる。特定の実施形態では、比較的短いNRPの類似体を提供する。該類似体は、少なくとも部分的に酵素分解の減少のため、安定性が高くなりうる。他の実施形態では、アミノ酸が修飾されたNRP類似体を提供する。なおさらなる実施形態では、アミノ酸の代わりに非アミノ酸置換基を有するNRP類似体を提供する。
いくつかの実施形態では、天然ペプチドの配列を有するNRP化合物を提供する。例えば、1つの該NRPは下記配列を有する11アミノ酸長(11-mer)ペプチドである。

当然のことながら、NRPの合成化合物又は類似体はアミド化したC-末端を有してよく、或いはC-末端ヒドロキシル残基(OH)を有しうる。また当然のことながら、用語「NRP化合物」、「NRP類似体」及び同様の用語は、この発明の化合物又は以前に開示されたNRPペプチド若しくはNRPタンパク質を指す。
下記タイプの修飾の1つ以上を有しうるNRPの合成類似体を提供する:(1)βターンの安定化、(2)グリシン残基の置換、(3)N-末端グリシン残基の置換及び/又は(4)環化。
βターン予測のチョウ・ファスマン(Chou and Fasman)確率は、配列番号:1における高い可能性のβターンが以下の太字で示すドメインAPGR(配列番号:2)及びRAGG(配列番号:3)内で見つけられることを明らかにする。
アルキル化アミノ酸のような立体的障害を導入することによってβターンを安定化することができる。使用できる容易に入手可能なアミノ酸としては、アラニン及びグリシン残基のどちらか又は両方の置換として使用できるアミノイソ酪酸(Aib、アラニンのα-Hがメチルで置換されている)が挙げられる。
配列APGR(配列番号:2)において、アラニン又はグリシンをアミノイソ酪酸(Aib)で置換することができる。アラニンのAibによる置換は下記類似体を生じさせる。

別のβターン配列、RAGG(配列番号:3)では、アラニンをアミノイソ酪酸(Aib)で置換して下記配列を有する類似体を生じさせうる。

実験は、配列番号:5が神経保護性であり(図1〜4参照)、貯蔵条件下で安定性であり(図2及び3)、無置換NRPより高い神経保護性であり(図1及び4)、末梢神経障害(図6及び7)及びALS(図8)に有効であることを示した。
アスパラギン(N)で内部グリシン残基を置換すると、アスパラギンはグリシンより高いβターン傾向を有するためβターンを誘導することができる。従って、内部グリシン残基をアミノ酸位置10にてアスパラギンで置換して、下記配列を有するペプチドを生じさせうる。
このNRP類似体は3-NPで誘導された神経毒性のin vitroモデルで神経保護性であることが分かった。
N末端でG1を切断すると、生物学的活性の損失をもたらしうる。アセチル基でG1を置換すると、生物学的活性を修復することができる。結果として生じるNRP類似体は、アセチル化されて下記配列を有するペプチドをもたらす。

このNRP類似体は3-NPで誘導された毒性に逆らって神経保護性であることが分かった。
1つ以上のL-アミノ酸の代わりにD-アミノ酸が存在することによって、ペプチドの二次構造が影響を受けうる。N-末端から3番目のアミノ酸の置換は下記配列を有する化合物を生じさせる。

このNRP類似体は、3-NPによって誘発された神経毒性のin vitroモデルで神経保護性であることが分かった。
配列番号:1の環状ペプチド模倣薬の合成を実行することができる。ある方法は、配列の各末端にシステイン残基を付加してから、結果として生じた生成物を酸化して下記配列を有する環状ジスルフィドを生じさせる工程を含む。
これらの化合物の調製で使用する出発原料及び試薬は、Aldrich Chemical Company(Milwaukee, Wis.)、Bachem(Torrance, California)、Sigma(St. Louis, Mo.)等の商用供給業者から入手可能であり、又はFieser and Fieser's Reagents for Organic Synthesis, vols. 1-17, John Wiley and Sons, New York, N.Y., 1991;Rodd's Chemistry of Carbon Compounds, vols. 1-5 and supplements, Elsevier Science Publishers, 1989;Organic Reactions, vols. 1-40, John Wiley and Sons, New York, N.Y., 1991;March J; Advanced Organic Chemistry, 4th ed. John Wiley and Sons, New York, N.Y., 1992;及びLarock: Comprehensive Organic Transformations, VCH Publishers, 1989のような参考文献に記載の手順に従い、当業者に周知の方法で調製される。多くの場合、アミノ酸とそのエステル又はアミド、及び保護されたアミノ酸は広く市販されており;かつ修飾アミンとそのアミド又はエステルの調製については化学及び生化学文献で広く記載さされているので当業者には周知である。例えば、N-ピロリジン酢酸は、Dega-Szafran Z and Pryzbylak R.「双性イオンα-(1-ピロリジン)アルカノカルボン酸とそのN-メチル誘導体の合成、IR、及びNMR研究(Synthesis, IR, and NMR studies of zwitterionic α-(1-pyrrolidine)alkanocarboxylic acids and their N-methyl derivatives.)」J. Mol. Struct.: 436-7, 107-121, 1997に記載され;かつN-ピペリジン酢酸はMatsuda O, Ito S, and Sekiya Mに記載されている。各論文は、参照によって明白に本明細書に完全に組み込まれる。
ペプチド合成の代替手法は、Bodanszky et al, Peptide Synthesis, 2nd ed, John Wiley and Sons, New York, 1976(参照によって明白に本明細書に完全に組み込まれる)に記載されている。例えば、アミド結合形成の化学的又は酵素的方法を利用するアミノ酸又はペプチドフラグメントの段階的又はブロックカップリングのどちらかを含む、標準的な溶液ペプチド合成方法論を用いて本発明のペプチドを合成することもできる(例えば、酵素的ペプチド合成方法を記載しているH. D. Jakubke in The Peptides, Analysis, Synthesis, Biology, Academic Press, New York, 1987, p. 103-165;J. D. Glass, ibid., pp. 167-184;及び欧州特許第0324659(A2)号を参照されたい)。これらの溶液合成方法は技術上周知である。
市販のペプチド合成機、例えばApplied Biosystems Model 430Aをこれらの方法の実施に利用できる。
この発明のNRP類似体を用いて神経障害を治療することができる。NRPは、予想外に、脳の自己免疫障害に付随する神経変性、例えばEAE及び多発性硬化症(MS)、筋萎縮性側索硬化症(ALS)及び神経細胞への毒性傷害などの治療に有効だった。
この発明のNRP化合物が有益でありうる障害及び状態として以下のものが挙げられる。
NRP類似体で治療可能な神経系状態として、細菌、真菌、スピロヘータ及びサルコイド感染などの中枢神経系の感染症、例えば発熱性感染症、急性細菌性髄膜炎又は軟膜炎が挙げられる。
脳血管疾患としては、脳卒中、虚血性脳卒中、低酸素/虚血、アテローム硬化性血栓症、ラクナ(lacune)、塞栓症、高血圧性出血、破裂動脈瘤、血管奇形、一過性脳虚血発作、頭蓋内出血、自然発症くも膜下出血、高血圧性脳症、脳動脈の炎症性疾患、例えば、心不全(おそらく冠動脈バイパス形成手術に起因する)及び他の形態の脳血管疾患によって起こる灌流の減少が挙げられる。
頭蓋脳損傷としては、頭蓋底骨折及び脳神経損傷、頸動脈海綿洞瘻、気頭症、気瘤及び鼻漏、脳挫傷、外傷性脳内出血、外傷性脳損傷、穿通性外傷性脳損傷及び子供の急性脳腫脹が挙げられる。
末梢神経障害は末梢神経細胞への損傷又は末梢神経細胞の損失によって特徴づけられる一般的な身障状態である。100種を超える末梢神経障害があり、それぞれそれ自体に特徴的な症状、発症パターン、及び予後のセットがある。末梢神経障害は遺伝的又は後天的のいずれもありうる。遺伝型の末梢神経障害は、遺伝子の突然変異によって引き起こされうる。いくつかのタイプの末梢神経障害とそれらに共通する特徴を下表1に示す。表1は、ピリドキシ誘発、ストレプトゾトシン(streptozotin)誘発及び化学療法誘発末梢神経障害並びに糖尿病性末梢神経障害の間の比較を示す。
高用量のピリドキシン(ビタミンB6)によって誘発されたヒト感覚神経障害の最初の報告はSchaumberg et al., New Eng. J. Medicine 309:445-448, 1983に由来する。2〜14カ月の期間にわたって2000〜6000mg/日の範囲で毎日摂取した。全ての患者が手足の麻痺による「靴下・手袋型(stocking-glove)」知覚損失及び不安定な歩行を示した。
末梢神経障害のラットモデルを用いて、ピリドキシンによって誘発された神経異常の検査が可能である。例えば、1,200又は800mg/kg/日のピリドキシンをラットに5〜10日間投与すると、感覚神経の壊死をもたらし、特に坐骨神経及び後根神経節内の大径神経細胞を冒す(Xu et al., Neurology 39:1077-1083, 1989)。
動物におけるピリドキシン誘発末梢神経障害は、治療薬の効果を研究するために認められているシステムである。特に、このシステムは該薬剤がヒトの末梢神経障害に及ぼす効果の予測システムである。
神経系の後天性代謝障害として、錯乱、昏迷又は昏睡-虚血-低酸素症、低血糖症、高血糖症、肝不全及びライ症候群の1つ以上を含む症候群として現れる代謝疾患、進行性錐体外路症候群として現れる代謝疾患、小脳失調症、高熱症、セリアックスプルー(celiac-sprue)病として現れる代謝疾患、クッシング病及びステロイド性脳症、甲状腺性精神障害及び甲状腺機能低下症並びに膵性脳症といった精神障害又は認知症を引き起こす代謝疾患が挙げられる。神経障害をもたらしうる代謝障害の例は、さらに完全に後述するピリドキシン過剰である。
薬物及び他の化学薬品に起因する神経系の障害としては、アヘン剤及び合成鎮痛剤、鎮静催眠薬、刺激薬、精神賦活薬、細菌毒素、植物毒、有毒性咬傷及び刺傷、重金属、工業毒、抗腫瘍及び免疫抑制薬、サリドマイド、アミノグリコシド抗生物質(聴器毒性)及びペニシリン誘導体(てんかん)、心保護薬(β-ブロッカー、ジギタリス誘導体及びアミオダロン)によるものが挙げられる。
なおさらに一般的に、本発明は、外傷、毒素曝露、窒息又は低酸素性虚血の形態の傷害後の損傷領域への神経細胞及び神経芽細胞移動の誘導に適用できる。
NRP類似体を患者に直接投与して使用することができる。NRP類似体を薬物又は医薬製剤の一部として投与することができる。これはNRP類似体をいずれの医薬的に適切な担体、アジュバント又は賦形剤と併用することをも含みうる。さらにNRP類似体を他の非NRP神経保護薬、増殖薬、又は他の薬剤と共に使用することができる。担体、アジュバント又は賦形剤の選択は、使用する予定の投与経路によって決まりうる。
投与経路は個々の状態に合わせて広く変化しうる。NRP類似体を異なる経路で:腹腔内、静脈内又は脳室内投与することができる。末梢適用は中枢神経系と直接干渉しないので1つの選択経路でありうる。
投与経路として、皮下注射(例えば、0.9%塩化ナトリウムに溶解)及び経口投与(例えば、カプセルで)が挙げられる。
当然のことながら、時にはいずれかの適切な投与経路で患者のCNSにNRP類似体を直接投与することが望ましいこともある。例として、側脳室注射によるか又は患者の脳の側脳室へ外科的に挿入したシャントを介して、脳脊髄液へ又は患者の脳の患部に直接投与することが挙げられる。
この発明の一部の実施形態では、脳損傷を治療するための方法は、1種以上のNRP類似体を約0.01μg/kg(体重)〜約100μg/kg(体重)の用量範囲で投与する工程を含む。他の実施形態では、1μg/kg(体重)〜約10μg/kg(体重)の用量を利用しうる。さらなる実施形態では、NRPの用量は約0.01μg/kg(体重)〜約0.1mg/kgの範囲であってよい。
他の実施形態では、投与すべきNRP類似体の有効量の決定は当業者のスキルの範囲内であり、当業者にとっては日常的であろう。特定の実施形態では、本明細書に記載のアッセイシステムを利用するin vitro研究によって、使用すべきNRP類似体の量を見積もることができる。投与すべきNRP類似体の最終量は、投与経路、使用するNRP類似体並びに治療する予定の神経障害又は状態の性質によって決まるだろう。適切な用量範囲は、例えば約0.01μg/kg(体重)〜約15μg/kg(体重)であってよく、又は他の実施形態では、約20μg/kg(体重)〜約30μg/kg(体重)でありうる。
非PEG化ペプチドより寿命が長いPEG化ペプチドをも、例えば、1995年11月30日に公開されたWO95/32003に記載の接合技術に基づいて利用することができる。
担体は適宜少量の添加剤、例えば等張性及び化学的安定性を高める物質を含む。該材料は望ましくは使用する投与量及び濃度でレシピエントに無毒であり、例としてであるが、緩衝液、例えばリン酸、クエン酸、コハク酸、酢酸、及び他の有機酸又はその塩;抗酸化剤、例えばアスコルビン酸;低分子量(約10未満の残基)ポリペプチド、例えば、ポリアルギニン又はトリペプチド;タンパク質、例えば血清アルブミン、ゼラチン、又は免疫グロブリン;親水性ポリマー、例えばポリビニルピロリドン;グリシン;アミノ酸、例えばグルタミン酸、アスパラギン酸、ヒスチジン、又はアルギニン;単糖類、二糖類、及び他の炭水化物、例えばセルロース又はその誘導体、グルコース、マンノース、トレハロース、又はデキストリン;キレート化剤、例えばEDTA;糖アルコール、例えばマンニトール又はソルビトール;対イオン、例えばナトリウム;非イオン性界面活性剤、例えばポリソルベート、ポロキサマー、又はポリエチレングリコール(PEG);及び/又は中性塩、例えば、NaCl、KCl、MgCl2、CaCl2などが挙げられる。特定の実施形態では、0.5Mスクロース又は0.5Mトレハロースを用いて本発明のペプチドを安定化することができる。このような糖の使用によって、ペプチドを長期間貯蔵することができる。
他の実施形態では、NRP類似体を単回又は複数回投与容器、例えば、封止アンプル又はバイアル内で再構成用水溶液又は凍結乾燥製剤として貯蔵することができる。凍結乾燥製剤の例として、10mLバイアルを、化合物の滅菌ろ過した0.01%(v/v)水溶液5mlで満たし、結果として生じる混合物を凍結乾燥する。静菌水又は他の適切な溶媒を用いて凍結乾燥化合物を再構成することによって注入液を調製することができる。
なおさらなる実施形態では、キットは所定量の凍結乾燥NRP化合物、剤形の調製のための生理学的に適合性の溶液、混合バイアル、混合装置、及び使用説明書を含む。当業界の普通のプラクティスに従って該キットを製造かつ貯蔵することができる。
NRP化合物含有組成物を1つ以上の種々の経路で投与してよい。例として、静脈内、腹腔内、脳内、脳室内、吸入、洗浄、直腸、膣、経皮、皮下投与を使用できる。
(NRP化合物の調製)
NRP化合物はAuspep(Australia)によって提供された。標準的固相合成を利用してペプチドを合成した。ペプチドはアミド化C-末端を有して供給され、かつMALDI-MSスペクトル解析で分析すると95%より純度が高かった。ペプチドは使用するまで、0.5Mスクロース又は0.5Mトレハロース内アルゴン下で-80℃にて凍結乾燥した。それらを0.5Mスクロース又は0.5Mトレハロース内で、PBS、或いは100μ/mlヒトトランスフェリン/PBS又は他の実施形態では、100μg/mlのBSA/PBS内で再構成した。
2つの半球の積層した小脳皮質をP3、P4、P7又はP8ウィスターラットから外植し、GBSSと0.65%のD(+)グルコース溶液内で小片にカットし、0.4mmゲージ針で粉砕してから125μmの孔径ふるいに通した。得られた微小外植片を2回遠心分離(60Xg)して培地を血清フリーBSA補充START V-培地(Biochrom)に交換した。最後に、微小外植片を500μlのSTARTV-培地で再構成した。培養のため、空気中5%CO2を含み、34℃で100%の湿度の雰囲気下にて35mmのペトリ皿内ポリ-D-リジン被覆カバーガラス上で1時間38μlの細胞懸濁液をインキュベートした。引き続き、損傷毒素(後述する通り)、NRP類似体及び1mlのSTARTV培地を加え、2〜3日の培養後に培養を評価した。
免疫組織化学及び神経細胞移動実験のため、下記措置後の培養内で2〜3日後に小脳微小外植片を固定した:微小外植片を2分の連続処置(それぞれ0.4%;1.2%;3%のパラホルムアルデヒド)後、0.1Mリン酸ナトリウム中4%パラホルムアルデヒド/0.25%のグルタルアルデヒド(pH 7.4)内で5分のインキュベーションで固定した。
酸化ストレスは神経変性をもたらしうる。これはハンチントン舞踏病によるヒト障害で観察される症状についての1つの考えられるメカニズムである。酸化ストレス産生性毒素3-ニトロプロピオン酸(3-NP)は、ハンチントン舞踏病のヒトで見られる当該作用を模倣する実験動物で作用を生じさせることが以前に示されている。従って、3-NP誘発神経毒性を有する実験動物における治療薬の研究は、ハンチントン舞踏病又は酸化ストレスを特徴とする他の障害のあるヒトの治療における当該薬物の効果を予測できる。
上記毒素を用いて、培養の初めに小脳微小外植片を24時間NRPの希釈物(生存アッセイ)又はNRPと0.1μM BrdU(増殖アッセイ)にさらした。引き続き、新しい毒素とNRPを添加せずに80%の培地を交換した。3日後、上述したように小脳培養をin vitroで固定した。組み込まれたBrdUレベルの検出を前述したように行った。
生存の統計解析のため、最高の細胞密度を有する各固定化小脳培養の4つのフィールド(各フィールドの面積は0.65mm2)を選択し、神経突起伸長を示す細胞を数えた(生存アッセイ)。
神経保護
NRP類似体は、3-NPで処置した外植片内での神経細胞生存増加を向上させた(実施例2、3及び4参照)。
異なる濃度の配列番号:1、及びこの発明の配列番号:5の、実施例1に従って調製した細胞培養に及ぼす効果を研究した。配列番号:1と配列番号:5は、配列番号:1のアミノ酸配列の位置9のアラニン(A)をアミノイソ酪酸アミノ酸類似体(Aib)で置換して、配列番号:5に示す配列を有する化合物を生じさせたこと以外、同じアミノ酸配列を有する。アラニンのAib置換はペプチドの線形構造の変化に加え、βターンを安定化し、ひいては非置換ペプチドと比べて異なる三次元構造及び特定結合について異なる移動度を有するペプチドを生じさせうることが分かる。
図1は、これらの研究結果のグラフを示す。3-NPのみでは、ビヒクル処置コントロールに比べて神経突起を示す細胞の深刻な損失をもたらし、この化合物が実際に神経毒性であることを示唆している。10fM又は1pMの濃度で配列番号:1の配列を有するペプチドは、3-NPの神経毒作用を有意に減らした(平均±SEM;p<0.001;n=4)。同様に、配列番号:5は、濃度依存様式で3-NP-誘発神経毒性を低減し、閾値が約1fM未満であり、約100fMの濃度で最大効果を示した(平均±SEM;p<0.001;それぞれn=4)。
この研究から、配列番号:5が神経保護性であると結論づける。この結果は、配列番号:5が、神経障害を患うヒトの神経変性を治療するために役立つ治療的NRP化合物でありうることを意味する。さらに、酸化ストレスはハンチントン舞踏病のヒトで観察される神経変性と同様の神経変性を誘発することが分かっているので、この発明の合成NRP化合物を用いて、酸化ストレスによって生じた神経変性のあるヒトを治療することができる。
この発明のNRPの貯蔵中の安定性を決定するため、配列番号:1を用いて一連の研究を行った。この研究では、配列番号:1を合成してから該ペプチドを9週間-20℃又は-4℃のどちらかの温度で貯蔵した。次に上記3-NPの神経毒作用に対して小脳神経細胞を保護するときの効力について該NRPを試験した。小脳外植片をビヒクルのみ(白抜き柱)、神経毒薬3-NPのみ(薄い点描柱)又は3-NP+9週間-20℃又は-4℃で貯蔵した4種の濃度の配列番号:1で処置した。
対照的に、-4℃の温度で貯蔵した配列番号:1は3-NPの神経毒作用をほとんど低減しなかった。この研究から非置換NRPは-4℃での貯蔵期間にわたって活性を失い、配列番号:1を-20℃で貯蔵するとその効力を保護できると結論づけた。
この研究では、上記実施例3と同様に異なる貯蔵条件と異なる濃度における置換NRP、配列番号:5の安定性を決定した。配列番号:5を合成してから-20℃又は-4℃のどちらかの温度で9週間貯蔵した。次に上記実施例2及び3で述べたように、3-NPの神経毒作用を低減する配列番号:5の効力を試験した。
驚くべきことに、-4℃で貯蔵した場合でさえ、配列番号:5はその神経保護効果を保持することを見出した。実際に、-4℃での貯蔵後の配列番号:5によってもたらされる神経保護の度合は、-4℃での貯蔵後の配列番号:5によってもたらされる神経保護の度合と統計的に有意には異ならなかった。この結果は上記配列番号:1の研究に基づいては全く予想外だった。配列番号:5を有するNRPの高い安定性は、この化合物が普通に使用される条件下での貯蔵及び輸送に適切であることを意味する。
実施例1の方法を用いて得た大量のデータにおいて、図1に示す結果を確証した。図4は、神経毒3-NPで処置した小脳微小外植片の研究結果のグラフを示し、各グループの6つの研究で配列番号:5と配列番号:1による神経保護を実証する。配列番号:1及び配列番号:5は、神経毒3-NPへの曝露後の死から神経細胞を保護すると結論づける。また、配列番号:5及び配列番号:1は、神経毒性に苦しむヒトを治療する際に有用な治療薬でありうると結論づける。
重症の軸索損傷及びその後の病変(例えば多発性硬化症;MSで)につながるCNSにおける慢性炎症に対してNRPが影響を及ぼすかを決定するため、免疫原としてミエリンオリゴデンドロサイト糖タンパク質(MOG)を用いて、MSの重症度の進行状態を模倣する実験的自己免疫性脳脊髄炎(EAE)のマウスモデルでNRPを試験した。
動物
それぞれ平均体重が24gの6〜8週齢の雌マウス、株C57Bl/6Jを使用した。
NRPの調製
配列:NH2-REGRRDAPGRAGG-NH2(配列番号:12)
(米国特許出願第10/976,699号で開示された配列番号:30としても知られる)を有するペプチドはAuspep(Australia)によって供給された。ペプチド配列番号:12は、C-末端がアミド化されて供給され、MALDI-MS分光法、HPLCで決定した場合、純度が95%より高かった。質量分析で配列を確認した。ペプチドは使用するまでアルゴンガス下-80℃の温度で凍結乾燥して貯蔵した。使用する日にPBS内でペプチドを再構成した。
800μugのヒト型結核菌(Mycobacterium tuberculosis)(Difco, Detroit, USA)を含有する完全フロイントアジュバント(Difco, Detroit, USA)中200μugの脳炎惹起性ペプチドMOG35-55 MEVGWYRSPFSRVVHLYRNGK(配列番号:13;C S Bio Co. USAから得た)を含有する200μulLのエマルションを一方の側腹部に皮下注射した。即座に400ngの百日咳毒素(List Biological Laboratories, USA)をマウスに腹腔内注射器し、48時間後に再び注射した。
治療処置
疾患のピーク時(MOG免疫化後17日)に、0.1μgのペプチド/動物(4.16μg/kg)の1日用量で14日間動物に配列番号:12を腹腔内(i.p.)処置した。
(神経障害の評価)
毎日マウスをモニターし、以下の任意の臨床スコアに基づいて神経障害を採点した:0、臨床徴候なし;1、弛緩性尾部;2、後肢脱力;3、後肢麻痺;4、後肢脱力及び前肢脱力;5、対麻痺;6、死亡。
マウスのEAEに対するNRPの治療効果
最初のNRP処置後の37日の成果を図5に示す。末梢投与した場合、配列番号:12の有意な治療効果がある。同様の薬物効果が、神経再生化合物EPOについてメチルプレドニソロンとの併用療法で示されている。EPOの欠点はその大きいサイズであり、容易には合成又は投与することができない。NRPは、MSのEAE疾患モデルで起こる運動機能障害のピーク時に治療薬として投与した場合、かなり長期間該疾患の重症度を低減できると結論づける。スコア1は最低点であり、尾部の弛緩のみを意味するが、より高いスコアは後肢の脱力(スコア2)又は完全な麻痺(スコア3)を意味する。**p<0.01対処置日1のスコア。
この発明のNRP類似体が末梢神経障害の治療のために役立つ治療薬であるかを決定するため、ピリドキシンで誘発された末梢神経障害のあるラットで一連の研究を行った。
1日当たり単回ボーラスとして非常に低用量で投与したNH2-G1RRAAPGR-Aib-GG11-NH2(配列番号:5)は、毒性用量のピリドキシンで処置した動物の運動障害を減弱できることを実証した。
雄のスプラーグドーリーラットを使用し、塩化ピリドキシンによる中毒の最初に秤量すると278〜349gだった。中毒の前、1週間1日ごとに光線を歩いて渡ることにラットを慣らした。
実験I:中性pHに調整した無菌蒸留水に溶解して400mg/kgの塩化ピリドキシンをラットに8週間1日2回腹腔内(i.p)投与し、同時に配列番号:5の配列を有するペプチドを全部で10日間腹腔内投与した。全部で29日間ラットを観察した。
実験II:より短期間にわたって、より高用量のピリドキシン(1200mg/kg/日)をラットに投与した。中性pHに調整した無菌蒸留水に溶解した塩化ピリドキシンをラットに4日間1日2回600mg/kgの用量で腹腔内投与した。同時に配列番号:5の配列を有するペプチドを全部で4日間投与した。日5に、ラットを運動障害について試験した。
ピリドキシン中毒後の運動障害及び配列番号:5の効果を正確な光線歩行を利用して分析した。1.5mの長さの光線を横切る7つの連続した足踏みをビデオテープで録画し、足根の位置に従ってこれらの足踏みを1〜4で採点した(1−光線の中線上に後肢足根;2−光線の中線の上半分に足根が触れていた;3−光線の中線の下半分に足根が触れていた;4−光線の中線より足根が下)。全ての7つの足踏みのスコアの結果を一緒に加えた。光線上に立てるが、歩けない場合に30というスコアを与えた。光線上に立つことさえできない場合には32というスコアを与えた。
実験I:配列番号:5について試験した2種の濃度は、40ng/kg/日及び4μg/kg/日だった。
実験II:食塩水と配列番号:5を4μg/kg/日の用量で試験した。
(統計解析)
二元分散分析及びBonferroniのpost-hocテストで運動障害データを評価した。薬物処置コホートとビヒクル処置との間でp<0.05の場合に統計的に有意と結論づけた。
実験I:
ピリドキシン中毒の休止4日後、配列番号:5で処置した両ラットコホートはビヒクル処置群に比べて減弱した運動障害を示す(低投与量:p<0.05;高投与量:p<0.01)。ピリドキシン中毒開始後16日では、高用量の配列番号:5群は、コントロール群よりずっと有意に少ない運動障害を有した(p<0.05)(図6)。
実験II:
高用量のピリドキシン中毒の次の4日は、コントロール群のラットでかなりの運動障害をもたらす。日5では、配列番号:5で処置したラット群(投与量:4μg/kg/日)は運動障害の非常に有意な減弱を示した(p<0.001)(図7)。
配列番号:5は、運動障害、ピリドキシン誘発末梢神経障害の非常に有意かつ臨床的に相当な減弱を示したと結論づける。また、4μg/kg/日の用量で、配列番号:5は良く耐えられたと結論づける。4μg/kg/日の濃度は、ピリドキシン中毒のさらに慢性モデル(8日間800mg/kg/日)及び4日間投与した1200mg/kg/日で誘発した急性ピリドキシン中毒でも同様に良く作用した。
さらに、これらの研究から、配列番号:5はヒトの末梢神経障害に有効な処置でありうると結論づける。
この系列の研究では、ALSのヒトの運動神経疾患と同様の運動神経疾患をもたらす遺伝子欠陥(SOD-1)を有するマウスに及ぼす配列番号:5の効果を調べた。この障害のある動物は、経時的に運動協調性の進行性損失を示し、疾患の早期に究極的に死に至る。この動物系は、ヒトのALSを治療するのに役立つ可能性のある薬剤の効果を研究するのに有用である。従って、得られた結果はALSのヒトで観察される結果を高度に予測できる。
疾患発症点以降にビヒクル又は配列番号:5のどちらかを受けるようにマウスをランダムに割り当てた。各処置割当群の疾患発症は有意には異ならなかった:92〜93日。
配列番号:5の処置(40μg/kgを1日1回腹腔内投与(図8)又は0.4μg/kgを1日1回腹腔内投与(図8))は疾患の発症日に開始した。
2つの研究では、配列番号:5処置はALS様障害に苦しむマウスの寿命の有意な延長をもたらした(図8A及び8B)。40μg/kgの1日用量の腹腔内投与の配列番号:5は、疾患発症後の長寿を有意に促した。図8A及び8Bは、SOD-1変異体(ALS)トランスジェニックマウスについてのカプラン・マイヤー生存確率曲線を示す。
ビヒクル処置動物は日120で死亡し始め、日143で全て死亡した(研究1;図8A;実線)。同様に、研究2では(図8B;実線)、ビヒクル処置動物は日120で死亡し始め、日154で全て死亡した(図8B;実線)。
これらの研究から、配列番号:5はその非置換対応物に比べて予想外に改善された安定性を有する有効な薬剤であると結論づける。また、NRPのβターンを安定化すると、治療効果を改善することができ、かつ安定性を高めることができ、両者がNRPの類似体の治療的有用性を改善しうると結論づける。
従って、この発明のNRP類似体は、急性及び慢性神経変性障害、例えばALS、神経毒性、酸化ストレスに伴う神経変性、自己免疫障害、外傷性脳損傷並びに他の神経疾患及び状態の治療で役立ちうる。さらに、NRP類似体は、神経学的機能の損失が症状である状況において有益な治療効果を有しうると結論づける。
後述する走触性移動アッセイにおいて、マウス神経幹細胞に及ぼす移動誘導性/化学誘引的活性についてNRP類似体を試験する。
初期NRPコーティング
孔径が12μmのTranswellプレート(Corning)のコントロールウェルを1.5mlのBSA/PBSビヒクル内でコーティングする。残りのプレートを0.1ng/mlのNRP類似体(10ug/mlのBSAを含有するPBS内で調製)を用いてコーティングする。
細胞外マトリックスコーティング
マウス初代幹細胞用の細胞外マトリックス(ECM)コーティングとしてラミニン(7μg/ml)を使用する。マトリックスを37℃;5%C02で2時間室温にてインキュベートする。細胞をインサート上に播く(30,000細胞/ウェル)。プレートを1〜2日でin vitro固定する(DIV)。
5ug/mLのPDL/PLL混合物(PBS中)を用いてインサートをコーティングする。引き続き、インサートをMilliQ水ですすぐ。
細胞固定化
インサートを捨て、ウェルを段階希釈のPFA(0.4、1.2、3及び4%)内で3〜5分間各希釈にて固定する。ウェルをすすぎ、段階希釈のPFA(0.4、1.2、3及び4%)内で3〜5分間各希釈にて貯蔵する。ウェルをすすぎ、数えるまでPBS内で貯蔵する。神経突起伸長を示し、下部チャンバーに移動した全ての細胞を移動性細胞として数える。
NRP類似体で処置したプレート内では、NRPのないプレート内で移動する細胞より多くの細胞が移動する。NRP類似体は神経細胞の移動を誘発することができ、かつNRP類似体を用いて、神経損傷又は神経疾患に付随する神経変性をそれぞれ治療することができる。
後述するように、損傷状態の走触性移動アッセイにおいて、マウス神経幹細胞に及ぼす移動誘導性/化学誘引的活性についてNRP類似体を試験する。
アストロサイトの単層の生成
P1(出生後の日1)ウィスターラット又はスプラーグドーリーラットを頭切除術で犠牲にする。皮質半球を取り出し、1管当たり4mlのDMEM-1皮質を含有する個別管に収集する。組織を機械的に粉砕する。無菌ピペットを用いて細胞を培地に移し、100umの細胞ストレーナーを介してろ過して50mlの遠心管に入れる。各管をDMEMで50mlまで満たす。管を5分間350xgで22℃にて遠心分離する。細胞を40mlのDMEM+10%FBSに再懸濁させる。次に細胞を12-ウェルプレート+5nMのオカダ酸(ocadaic acid)(アポトーシス細胞死を誘発することで神経細胞を除去するため)中に播き、Boyden Chamber内で37℃/10%CO2にて24時間インキュベートする。1日後に培地+FBSを新鮮なDMEM+10%FBSと交換する。培養密度まで細胞の成長をモニターする(14〜18日)。
アストロサイトを活性化するため、薬理学的薬剤形質転換増殖因子β1(TGFβ1)と同時に単層の機械的引っ掻きを利用してアストロサイト単層の損傷の誘発を行う。10ng/mlのTGFβ1を24時間アストロサイト単層に投与する。さらに、メスでアストロサイト培養を機械的に傷つける(ウェルの底全体をひと掻き)。
予め標識した幹細胞の播種
未分化フルオレッセイン二酢酸標識した胚性マウス神経幹細胞(NSC)をポリ-D-リジン(PDL-5μg/ml)被覆インサート中に播く。Boydenチャンバーの下部区画が100fMのNRP類似体を受ける。
インサートを捨て、段階希釈のPFA(0.4、1.2、3及び4%)で各希釈にて3〜5分間ウェルを固定する。ウェルをすすぎ、段階希釈のPFA(0.4、1.2、3及び4%)内で3〜5分間各希釈にて貯蔵する。ウェルをすすぎ、数えるまでPBS内で貯蔵する。神経突起伸長を示し、下部チャンバーに移動した全ての細胞を移動性細胞として数える。
標識細胞の移動した幹細胞数を24時間後に蛍光ベースのコンピュータ処理画像システム(Discovery-1)で分析する。
NRP類似体は、ビヒクル処置コントロールより多くの幹細胞を刺激する。NRP類似体は、神経幹細胞の移動を誘発し、かつNRP類似体は、神経損傷又は神経疾患に付随する神経変性の治療するために役立ちうると結論づける。
以下、上記実施形態から把握できる技術的思想を付記する。

- 前記神経障害が神経細胞の障害である、請求項3に記載の使用。
- 前記神経障害が、筋萎縮性側索硬化症、神経毒傷害、酸化傷害、多発性硬化症、末梢神経障害、低酸素/虚血、外傷性脳損傷、又は冠動脈バイパス移植手術である、請求項3又は4に記載の使用。
- 前記神経障害が糖尿病性末梢神経障害である、請求項3〜5のいずれか1項に記載の使用。
