Description
図1は、ペーシング治療を行うための埋め込み型心臓デバイス100を示す。埋め込み型ペーシングデバイスは、典型的には患者の胸部の皮下又は筋肉下に配置され、リードが静脈内で心臓に通されて、心腔内に配置された電極にデバイスを接続し、該電極は、腔を感知及び/又はペーシングするのに用いられる。電極はまた、様々な手段によって心外膜上に位置決めすることができる。プログラム可能電子コントローラは、経過時間間隔及び/又は感知電気活動に応答してペーシングパルスが出力されるようにする(すなわち、ペーシングパルスの結果としてではない内因性心臓拍動)。デバイスは、1つ又はそれ以上の感知チャネルを通して内因性心臓電気活動を感知し、これらチャネルの各々は電極の1つ又はそれ以上を組み込む。内因性拍動が存在しない状態で心筋組織を興奮させるために、ある特定の閾値を超えるエネルギーを有するペーシングパルスが、1つ又はそれ以上のペーシングチャネルを通して1つ又はそれ以上のペーシング部位に送出され、これらペーシングチャネルの各々は、電極の1つ又はそれ以上を組み込む。図1は、2つのリード200及び300を有する例示的なデバイスを示しており、これらリードの各々は、電極201−203及び301−303をそれぞれ有する多極(すなわち、多電極)リードである。電極201−203は、右心室又は中隔領域を興奮又は感知するために右心室に配置され、一方、電極301−303は、左心室の領域を興奮又は感知するために冠状静脈洞に配置される。他の実施形態では、異なる心筋部位を興奮させるために単極及び/又は多極リードの形態であらゆる数の電極を用いることができる。以下に説明するように、デバイス及びリードが埋め込まれると、デバイスのペーシング及び/又は感知チャネルは、特定の心筋部位を選択的にペーシング又は感知するために複数電極から選択されたもので構成することができる。
心臓による動脈系への血液の不十分なポンピングは、場合によっては「前方不全」と呼ばれ、「後方不全」とは、鬱血につながる肺及び全身静脈において結果として生じる血圧上昇を意味する。後方不全は、肺及び静脈系の血液がポンプ輸送できないときの前方不全からの当然の結果である。前方不全は、例えば、冠動脈疾患に起因する心室の収縮性障害によって、或いは、例えば、全身性高血圧又は弁機能不全に起因する後負荷の増大(すなわち、血液排出に抵抗する力)によって引き起こされる場合がある。この状況で心拍出量を増加させるように作用する1つの生理学的代償機構は、心室の拡張期充満圧を増加させ、これによって前負荷を増大させる(すなわち、拡張期終了時に心室が心室内の血液量によって引き伸ばされる程度)後方不全に起因するものである。前負荷の増大は、収縮期の1回拍出量を増大させ、これはフランク−スターリングの法則として知られる現象であり、これによって拡張期中の心筋線維延長の増大は、より大きな収縮力が収縮期中に発生する結果をもたらすことになる。すなわち、心不全は、この機構により少なくとも部分的に代償することができるが、肺及び/又は全身鬱血を犠牲にする可能性がある。
複数の臨床研究から、規則的な(例えば、20分/日、週に3回)運動レジメに従うHF及びMI後の患者は、殆ど体を動かさない患者と比べて症状が改善されることが分かった。しかしながら、全てのHF及びMI後の患者が、その心臓病又は他の衰弱状態に起因して運動可能であるとは限らない。以下に説明するように、ペーシング治療は、心不全の発症の予防及び/又は心臓病の進行の減衰/好転をもたらすために、運動を模擬するように設計することができる。
通常動作モードからAVDRモードへの切り替えは、幾つかの方法で実装することができる。通常モードがペーシング治療の送出を含まない場合、AVDRモードは、短いAVDを有する心房トリガモードでの心室ペーシングの送出、又は内因性速度を上回る速度の非心房トリガモード(例えば、VVI)での心室ペーシングの送出を含むことができる。通常モードが、指定AVDによる心房トリガペーシングを含む場合、AVDRモードは、正常動作モード又はVVIのような非心房トリガモードで用いるよりも短いAVDを用いた心房トリガモードでの心室ペーシングを含む。AVDRモードで用いられるAVDを指定する目的のために、デバイスは、内因性房室間隔を測定してその指定割合としてAVDを計算するように構成することができる。デバイスはまた、内因性洞調律を測定した後に、AVDRモードでの非心房トリガ心室ペーシングに切り替えて幾分高い速度でペーシングを送出するように構成することができる。AVDRモードでの非心房トリガペーシングはまた、ペーシング頻度が内因性拍動を回避しようと試みるようにして変更されるオーバードライブ・ペーシングモードで送出することができる。ある種のペーシング治療が通常動作モードで送出され、デバイスが異なるペーシング部位を備えた複数ペーシングチャネルを有するときには、AVDRモードは、心室ペーシングを送出するために同じ又は異なるペーシングチャネルのいずれかの使用を含むことができる。AVDRモードはまた、ある定められたスケジュールに応じたモードの動作中に、複数部位における心室ペーシング及び/又は異なるペーシング部位への切り替えを含むことができる。
デバイスは、AVDRへの開始及び/又は終了を制御する上で1つ又はそれ以上の開始及び/又は終了条件を用いるように構成することができる。開始又は終了条件は、例えば、経過時間間隔(例えば、その日の指定された時間)、患者によるスイッチの作動(例えば、デバイスコントローラにインタフェース接続されたスイッチの磁気的に又は触覚的作動)、テレメトリを介して受け取ったコマンド、肺浮腫又は仰臥位のような条件の検出又は非検出、或いは指定された範囲内又は範囲外にある測定変数とすることができる。このような測定変数の実施例は、心拍数、活動レベル、分時換気、心拍出量、心音、及び血圧を含む。開始及び/又は終了条件はまた、複数の開始及び/又は終了条件が、互いに論理和演算又は論理積演算されて、複合開始又は開始条件が満たされるかどうかを判断する複合条件とすることができる。図6は、AVDRモードへの開始及び終了を制御するためのデバイスコントローラにより実行可能な例示的なアルゴリズムを示す。この実施例では、開始条件の1つは、その日の指定された時間であり、その間に、他の開始条件が満たされた場合にはAVDRペーシングを送出することが望ましい。図示のように、デバイスのコントローラは、A1からA6で表した幾つかの異なる状態を通って移行するようにプログラムされる。状態A1において、デバイスは、その通常動作モードで動作する。状態A2において、状態A1で動作を続けている間に、デバイスは、経過時間間隔又はトリガ条件に基づいてAVDRモードに切り替えるべきかどうかを判断する。任意選択的に、デバイスはまた、状態A3によって実装されるシミュレート運動モードに切り替える前に、1つ又はそれ以上の特定の開始条件についての試験を行うように構成することができる。AVDRモードへの切り替え前に満足する必要がある開始条件の実施例は、指定された開始範囲内にある測定運動性レベル(ここで、運動性レベルは、例えば、心拍数、活動レベル、又は分時換気によって測定することができる)、心不整脈の非検出、心虚血の非検出、テレメトリ命令の受け取り、及びAVDRモードに切り替えることができるデバイスに組み込まれた磁気的又は触覚的に作動されるスイッチの患者による作動を含む。状態A3において、デバイスは、1つ又はそれ以上の開始条件が満足されているかどうか確認して、そうでなければA1に戻る。適切な開始条件が満足された場合、デバイスは、状態A4でAVDRモードに切り替える。AVDRモードは、AVDRペーシングを実施するのに必要な範囲まで通常動作モードに優先するが、通常動作モードで実行された特定の機能を継続可能にすることができる。或いは、AVDRモードは、通常動作モードの特定の機能を組み込むと考えることができ、その機能は、必要に応じてAVDRペーシングを送出するように修正される。AVDRモードで実行中、デバイスは、デバイスを通常動作モードに戻すようにする1つ又はそれ以上の終了条件を監視するように構成することができる。このような終了条件は、AVDRモードに入る前に満足する必要がある開始条件と同じ又は異なるものとすることができる。状態A5では、AVDRモードで実行中に、デバイスは、測定運動レベルが指定された許容範囲外にある、測定心拍数が指定された許容範囲外にある、心不整脈が存在する、心虚血が存在する、テレメトリ命令を受け取った、及びAVDRペーシングの送出を停止するように患者によってデバイスに組み込まれた磁気的に又は触覚的作動スイッチの患者による作動など、1つ又はそれ以上の終了条件の発生を監視する。終了条件が生じた場合、デバイスは、状態A1の通常動作モードに戻る。そうでなければ、デバイスは状態A6に進み、AVDRペーシングの規定の量及び/又は期間が送出されたかどうかを確認する。AVDRペーシングの規定量又は期間が送出された場合、デバイスは状態A1に戻り、通常動作モードを再開する。そうでなければ、デバイスは状態A5にループバックして終了条件を監視する。
例示的な実施形態では、デバイスは、AVD低減期間又はAVDRP(例えば、15−60分)と呼ばれるある指定時間期間にAVDRモードに周期的に(例えば、24から72時間毎に)切り換えるようにプログラムされる。デバイスが、その通常モード中にある種の治療(例えば、心臓再同期療法、リモデリング制御療法、又は徐脈に対して)を送出する場合、AVDRモードは、通常モードで用いるよりも短いAV遅延を有する心房トリガ心室ペーシング(例えば、VDD又はDDD)として実装することができる。ペーシングが通常モードで送出されない場合、AVDRモードは、指定された短いAV遅延を有する心房トリガ心室ペーシングとして実装することができる。デバイスが、心房リード及び単一埋め込み心室リードを備えている場合、埋め込み心室部位は、AVDRP全体に対して低減AV遅延でペーシングすることになる。デバイスが、複数心室部位(例えば、複数リードとして或いは1つ又は多極リードとして)において埋め込まれた電極を備えている場合、心室ペーシング部位の全て又は一部の選択サブセットは、AVDRP中にペーシングすることができる。心室部位はまた、指定されたデューティサイクルに応じてAVDRP中に交替することができる。例えば、デバイスが、2つの心室部位において埋め込まれた電極を有する場合、AVDRモードは、AVDRP全体に対して部位の選択された1つに送出されたAVDRペーシング、AVDRPのある割合(例えば、50%)に対して一方の部位に送出されて、AVDRPの残りの部分に対して他方の部位に切り替えられるAVDRペーシング、或いは全AVDRP中に各ペーシングされたサイクルに対して両心室部位に(同時に或いは2つの部位間にオフセットして)に送出されたAVDRペーシングとして実装することができる。同様に、デバイスが、2つよりも多い心室リード又は2つよりも多い心室ペーシング部位(例えば、四極リード)を有する場合、AVDRペーシングは、AVDRP全体に対して単一選択部位に送出することができ、AVDRペーシングは、AVDRP中に各ペーシングサイクル(同時に又は指定オフセットで)に対して埋め込み部位の全てに送出することができ、或いは、AVDRペーシングは、AVDRP中に1つの心室部位から次の心室部位に交替することができる。例えば、4つの心室ペーシング部位がある場合、AVDRペーシングは、AVDRPの最初の25%に対して第1の部位に、AVDRPの次の25%に対して第2の部位に、などで送出することができ、ここで、各部位がペーシングされる時間の割合は、均一に分配してもよく、或いは分配しなくてもよい。AVDRモードへの周期的切り換えは、付随事象(例えば、MI又は心不全の代償不全)の直後に開始することができ、或いはデバイスは、ある一定の時間期間(例えば、MI後30日)を自動的に待機して、モード切り替えを開始するようにプログラムすることができる。
上述のように、AVDRペーシングを用いて、非同期的心室収縮を意図的に引き起こすことによって運動をシミュレートすることができる。2006年11月13日に出願された同時係属の米国特許出願シリアル番号第11/559,131号に記載されるように、非同期的収縮はまた、異なる時間において異なる部位をペーシングして非効率的な方法で心臓を収縮させるようにするなどによって、他の方法で生成することができ、本明細書に記載された実施形態は、当該出願に記載された実施形態のいずれとも組み合わせることができる。同様に、一部の患者は、内因性非同期性を示し、従って、より協働収縮をもたらすある種のペーシング治療(例えば、心臓再同期療法)で治療することができ、シミュレート運動は、ある時間期間の間にこのようなペーシングを単にオフにすることによって生成することができる。このような患者に対しては、本明細書に記載された実施形態のいずれもが、ペーシングが停止されたモードにAVDRモードを置き換えることによって修正することができ、内因性収縮が生じることも認められる。

Claims (10)
- 心臓デバイスであって、
心房及び心室活動を表す電位図信号を受け取る感知回路と、
心房及び心室ペーシングパルスを発生させるためのパルス発生回路と、
通常動作モード又はAV遅延低減(AVDR)モードのいずれかで前記心臓デバイスを動作するようにプログラムされたコントローラと、を備え、通常動作モード及びAVDRモードは、心房センス又はペースに対するAV遅延間隔の後でペーシングが1つ又は2つ以上の心室部位に送出される心房トリガ心室ペーシングモードであり、

肺浮腫を検出するインピーダンスセンサを更に備え、
前記コントローラが、肺浮腫が検出されない場合にのみ前記AVDRモードで動作し且つ肺浮腫が検出されたことの判断を前記AVDRモードを終了させる終了条件として用いるようにプログラムされている、ことを特徴とする心臓デバイス。 - 前記コントローラが、拡張期の間の心室前負荷に対する心房寄与を低減させるために、内因性房室間隔を測定して、AVDRモードで用いられるAV遅延間隔を、内因性房室間隔の指定割合として計算するようにプログラムされる、請求項1に記載の心臓デバイス。
- 前記指定された開始又は終了条件の1つ又はそれ以上が、前記AVDRモードへの周期的切り換えを引き起こす経過時間間隔である、請求項1又は2に記載の心臓デバイス。
- 患者の運動レベルを測定するセンサを更に備え、前記コントローラが、測定された運動レベルが指定範囲内にある場合にのみ前記AVDRモードで動作するようにプログラムされる、請求項1〜3のいずれか1項に記載の心臓デバイス。
- 前記コントローラが、前記AVDRモードにおけるAV遅延間隔を排除するようにプログラムされている、請求項1、3又は4に記載の心臓デバイス。
- 肺動脈圧を測定するセンサを更に備え、前記コントローラが、肺動脈圧が指定範囲内にある場合にのみ前記AVDRモードで動作するようにプログラムされる、請求項1〜5のいずれか1項に記載の心臓デバイス。
- 前記指定された開始又は終了条件の1つ又はそれ以上が、テレメトリ又は患者操作によるスイッチを介して受け取った、前記AVDRモードを開始又は該AVDRモードを終了する命令である、請求項1〜6のいずれか1項に記載の心臓デバイス。
- ペーシング及び感知チャネルを形成するためにパルス発生回路及び感知回路を電極に接続するように前記コントローラによって作動されるスイッチマトリックスを更に備え、
前記コントローラが、前記通常動作モードとは異なるペーシングチャネルを前記AVDRモードにおいて用いるようにプログラムされる、請求項1〜7のいずれか1項に記載の心臓デバイス。 - さらに、ペーシング及び感知チャネルを形成するためにパルス発生回路及び感知回路を電極に接続するように前記コントローラによって作動されるスイッチマトリックスを更に備え、
前記コントローラが、前記AVDRモード中に、各ペーシングサイクルにおいて複数心室ペーシングチャネルを通じて同時に又は1つ又はそれ以上の指定オフセットでペーシングを送出するようにプログラムされる、請求項1〜8のいずれか1項に記載の心臓デバイス。 - さらに、ペーシング及び感知チャネルを形成するためにパルス発生回路及び感知回路を電極に接続するように前記コントローラによって作動されるスイッチマトリックスを更に備え、
前記コントローラが、前記AVDRモード中に、複数心室ペーシングチャネルを通じてペーシングを送出するようにプログラムされ、前記複数心室ペーシングチャネルが、規定スケジュールに応じて交替される、請求項1〜8のいずれか1項に記載の心臓デバイス。
