Description
本発明は蛍光性コバラミンおよびこれらの化合物の使用に関する。より詳細には、本発明は、コバラミンに共有結合する蛍光性、燐光性、ルミネセンス、または発光の化合物よりなる蛍光性コバラミンに関する。これらの蛍光性コバラミンは、診断および予後マーカーとして用いられ、(a)癌の細胞または組織と健康な細胞および組織とを区別し、(b)個体が、コバラミンに基づいた治療剤バイオコンジュゲートを用いて化学療法にポジティブに反応するかを決定できる。
本発明は、蛍光性コバラミンおよびこれらの化合物の使用に関する。より詳細には、本発明は、コバラミンに共有結合する蛍光性、燐光性、ルミネセンスのまたは発光の化合物よりなる蛍光性コバラミンに関する。これらの蛍光性コバラミンは、診断および予後マーカーとして用いて、(a)癌の細胞または組織と健康な細胞および組織とを区別し、(b)個体が、コバラミンに基づいた治療剤バイオコンジュゲートを用いて化学療法に対してポジティブに反応するかを決定できる。本発明の蛍光性コバラミンは、(1)癌細胞による急速な輸送および貯蔵(最大取込みは、4〜6時間にて生じる)、(2)非常に低濃度にて視覚的に検出できる明るいフルオロフォア(fluorophore)、および(3)非毒性化合物の必要な特性を提供する。
本発明は、蛍光性コバラミンおよびこれらの化合物の使用に関する。より詳細には、本発明は、コバラミン(ビタミンB12)に共有結合する蛍光性化合物(フルオロフォア)、燐光性化合物(ホスホロフォア)、ルミネセンス化合物(化学ルミネセンス発色団)または発光化合物よりなる蛍光性コバラミンに関する。これらの蛍光性コバラミンは、診断および予後マーカーとして用いて、(a)癌細胞または癌組織と健康な細胞および組織とを区別し、(b)個体が、コバラミン−治療剤バイオコンジュゲートを用いて化学療法に対してポジティブに反応するかを決定できる。

予後、予測またはモニターし;
・ in vivo、ex vivoまたはin situにて転移性の癌を同
定し;
・ 癌の進行段階を決定し;
・ 癌を経皮的に同定し;
・ 転移性の癌を経皮的に同定し;
・ 前哨リンパの節もしくは複数の節または腋窩リンパの節もしくは複数の節
中を含めたりンパ節中の癌を同定し;
・ 乳癌、卵巣癌、肺癌、前立腺癌、上皮癌(腺癌)、肝癌、黒色腫およびリ
ンパ腫のごとき癌の治療、検出、予後、予測またはモニタリングにおける
転移性疾患を同定し;
・ 白血病またはリンパ腫を診断、予測、予後、モニタリングまたは特徴付け
するための骨髄穿刺液または末梢血試料のフローサイトメトリー試験を行
い;
・ 患者が、コバラミン−治療剤バイオコンジュゲートの使用に基づく化学療
法にポジティブに応答するかを予測し;
・ 生体組織検査または乳腺腫瘤摘出における腫瘍マイクロマージン
(micromargin)の定義を改善し;
・ 生体組織検査、乳腺腫瘤摘出または腫瘍摘出において、癌細胞を置き去り
にする機会を減少させ、それによって、残っている癌細胞を取り除くため
のフォローアップ手術の必要性を低下させる。
・ 蛍光性マーカーは、結節が潮泊渠(tidal basin)をドレインしていること
を単に示すというよりはむしろ、リンパ管および節中の癌細胞に特異的で
あろう。また、該蛍光性マーカーは、癌細胞と健康な細胞とを区別するで
あろう。
・ 該マーカーは、蛍光検出によって得られた固有の感度のために低濃度にて
用いることができる。現在使用中の青色色素は、活性な節を不明瞭にする
傾向があり、病理学者による組織の手術後の検査を複雑にする。また、青
色色素は出血している血管を不明瞭にする傾向があり、それによって、結
節の外科的切除および後の傷を閉じることを厄介にする。蛍光性マーカー
の使用はこれらの問題を回避するであろう。
・ 現在実践されるごとき手順で見られるごとく、癌細胞に特異的な蛍光性マ
ーカーは、5〜10%の偽陰性率を改善するであろう。
・ 減少した偽陰性率は、患者と外科医によるこの技術の受入を改善するであ
ろう。これは外科医がこの手順を学習するのに必要なトレーニング時間(
完全な軸結節切開を持つ典型的には30例以上の事例)を減少できる。
本発明は、以下の実施例を引用してさらに記載され、それらは例示的に提供され、何ら本発明を限定することを意図するものではない。当業者によく知られた標準的技術または特に後記された技術を利用する。
フルオロフォアのコバルトへの結合による蛍光性コバラミンの合成
コバラミン局在性の視覚的指標として、フルオレセインをコバラミンに共有結合することによってコバラミンの5つの蛍光性アナログを調製した。緑色光の照明下、フルオレセイン分子は、0.1ppm未満の濃度まで暗順応眼(dark-adapted eye)によって検出できる黄色光を発光する。この発光は、表面蛍光顕微鏡を介して、ならびに視覚検査によって癌細胞の敏感な検出を可能とする。5つの各蛍光性コバラミンは、固有の蛍光性を示した。これらの全化合物は、公開された技術に従って、塩化アミノプロピルとcob(I)alaminとを反応させることによって合成して、アミノプロピルcob(III)alaminを生成した。引き続いての工程において、アミノプロピルcob(III)alaminを様々なフルオロフォアイソチオシアネート(すなわち、フルオレセインイソチオシアネート、「FITC」)と反応させて、アミノプロピルリンカーを介してコバラミンに結合する対応するフルオロフォア(すなわち、フルオレセイン−アミノプロピル−cob(III)alamin)を生成した。この後者の反応を図1に示す。
癌細胞によるコバラミンアナログの取込み
白血病の骨髄芽球標本を急性骨髄白血病(AML)M1(FAB分類における最小限の成熟骨髄芽球)を有している61歳の患者の骨髄穿刺液から調製した。細胞は、実施例1に記載のごとく調製した蛍光性コバラミンで採取3日後に処理した。蛍光性コバラミンアナログの異なる取込みは、蛍光顕微鏡または蛍光フローサイトメトリーによって決定されるごとく、正常および白血病のヒト骨髄細胞で判明した。正常な骨髄細胞と白血病の骨髄芽球(癌細胞)の間との差は特に注目すべきものであり、正常細胞によって検出可能なコバラミンは、吸収されなかった。健康な個体からの骨髄試料は、蛍光性ラベリングを示さなかった。潜在的な化学療法化合物として元来合成されたドキソルビシン(doxorubicin)−コバラミンコンジュゲートの取込みは、P−388ネズミ白血病細胞およびHCT−116のヒト大腸腫瘍細胞において見られた。これらの結果は、白血病および充実性腫瘍の細胞系におけるコバラミンの蛍光性誘導体の取込みを示す。
シアノコバラミンモノカルボン酸の調製
b−、d−およびe−モノカルボン酸をシアノコバラミンの酸−触媒の加水分解によって調製した。図2を参照されたし。要約すると、シアノコバラミン(527.0mg、0.389ミリモル)を100mlの丸底フラスコに入れ、40mlの0.5 MHClに溶解した。フラスコを50℃の水浴に置き、4時間攪拌した。その反応は、表1に作表された勾配を用いて、HPLC(Waters社、3.9×300mm DeltaPak 100C−18カラム)によってモニターした。
e−モノカルボン酸を単離し、全収率14%の約78mgであった。
CNCbl酸と1,12 ジアミノドデカンとのコンジュゲート
b−およびd−アミンを、図3に示すごとく調製した。CBC−195(55.4mg、0.0408ミリモル)を、小さなガラスバイアルに加え、約2.5mlのDMSOに溶解し、続いてEDCl・HCl(12mg、0.0626ミリモル)およびN−ヒドロキシスクシンイミド(NHS)(25mg、0.217ミリモル)を添加した。反応物を室温で一晩撹拌した。従前の試みから、いくつかの当量のEDClおよびNHS(合計6当量)を反応を完了させるのに必要とした。24時間後、一つのさらなる当量のEDClを添加し、反応を26時間内で完了した。反応を表3の勾配を用いてHPLCを介してモニターした。CBC−195は、9.07分の保持時間を有し、CBC−195のNHS−エステルは、10.55分の保持時間を有する。
CBC−208およびフルオレセイン−5EX−NHSのコンジュゲーション
CBC−208を図4に従って、フルオレセイン誘導体フルオレセイン−5EX(Molecular Probes社から入手可能)にカップリングした。CBC−208(6.0mg、3.87マイクロモル)を小さなガラスバイアルに加え、約0.5mlのDMSOに溶解し、続いて、フルオレセイン−5EX−NHS(2.5mg、4.23モル)を添加した。反応物は、室温にて一晩攪拌させた。反応は表5中の方法を用いて、HPLCを介してモニターした。
顕微鏡を介する乳房腫瘍組織のex vivo評価
乳房の腫瘍を含めた悪性および良性の腫瘍の試料を、正常縁の組織を伴って、患者から摘出する。これらの試料は、Utah大学施設内倫理委員会(IRB)およびHuntsman癌研究所臨床癌調査委員会(CCIC)の承認をもって得る。生きている組織試料を前記の調製された蛍光性コバラミンの一つと4〜6時間インキュベートする。各試料の薄い組織切片をクリオミクロトーム(cryomicrotome)で調製し、蛍光性マーカー量を表面蛍光顕微鏡によって正常および癌の組織中で定量する。対応する組織切片を解剖病理学者による評価のためにヘマトキシロン/エオシン(H&E)で染色する。正常および癌の細胞間の鏡界面は、注意深く評価する。また、腫瘍の低酸素症の領域内の細胞が代謝をしばしば低下させているので、腫瘍内部からの細胞を蛍光性マーカーの取込みにつき評価する。
リンパ節における癌細胞のex vivo評価
転移性疾患を持つ摘出されたリンパ節は、患者から取り除かれ、前記に調製された蛍光性コバラミン誘導体のうちの1つで4〜8時間インキュベートした。各リンパ節を薄切し、癌細胞への蛍光性コバラミンの輸送につき顕微鏡的に評価した。この実験は、画像化および視覚化のために十分な蛍光性コバラミンを吸収するリンパ節内の転移性細胞の能力を示した。
患者がコバラミンに基づいた治療剤バイオコンジュゲートでの化学療法に
有利に応答するかを決定するための蛍光性コバラミンの使用
白血病を持つ患者からの骨髄穿刺液または末梢血試料を蛍光性コバラミンコンジュゲートとインキュベートする。4〜8時間後、骨髄穿刺液または末梢血試料を洗浄して、組込まれていない蛍光性標識を除去し、次いで、該細胞試料を表面蛍光顕微鏡またはフローサイトメトリーによって定性的および質量的な蛍光分析に付した。かなりの量の蛍光性コバラミンを吸収した細胞をより明るい蛍光性を示す。かなりの量の蛍光性コバラミンの取込みは、患者が有する白血病のタイプがコバラミンに基づいた治療剤での処置に有利に応答するであろうことを示す。蛍光性コバラミンコンジュゲートでの処置後、大きな蛍光性を示さない骨髄穿刺液または末梢血試料は、患者がコバラミンに基づいた治療剤コンジュゲートに有利には応答しないことを示す。同様なアプローチを充実性腫瘍に適用できる。この場合、摘出した腫瘍組織の一部分は蛍光性コバラミンコンジュゲートとインキュベートし、約4〜8時間後、腫瘍組織中の蛍光性を定量する。腫瘍組織によって示された蛍光性が大きいほど、癌がコバラミンに基づいた化学療法での処置に有利に応答する見込みもより大きくなる。

Claims (24)
- 請求項1記載のコバラミンと、癌であるか、あるいは癌細胞を含むことが疑われる組織とを接触させ、光で該組織を照射し、次いで、発光した蛍光を検出することにより、個体の癌組織または癌細胞を含んでいる組織の同定用の診断剤を調製するための請求項1記載のコバラミンの使用。
- 癌細胞を含むことが疑われる該組織がリンパ節であることを特徴とする請求項2記載の使用。
- 該リンパ節が前哨リンパ節または腋窩リンパ節であることを特徴とする請求項3記載の使用。
- 該同定が顕微鏡でまたは視覚的に行なわれることを特徴とする請求項2記載の使用。
- 癌であるか、あるいは癌細胞を含むことが疑われる該組織が生体組織検査によって該個体から得られることを特徴とする請求項2記載の使用。
- 請求項1記載のコバラミンと個体からの組織とを接触させ、該組織を照射し、次いで、蛍光を視覚的に検出し、それによって、該癌組織が蛍光を発し、健康な組織がほとんど蛍光を示さないことにより、該個体の癌組織と健康な組織とを視覚的に区別する診断剤を調製するための請求項1記載のコバラミンの使用。
- in vitroまたはex vivoにて腫瘍の縁を規定する方法であって、請求項1記載のコバラミンと腫瘍を含んでいることが疑われる個体からの組織とを接触させ、該組織を照射し、次いで蛍光を検出し、それによって、該腫瘍組織が蛍光を発し、腫瘍の縁を規定することを特徴とする該方法。
- 請求項1記載のコバラミンと個体からの転移性の癌であることが疑われる組織または細胞とを接触させ、該組織を照射し、次いで、蛍光を検出し、それによって、該転移性の癌の組織または細胞が蛍光を発することにより、該個体における転移性の癌を同定する診断剤を調製するための請求項1記載のコバラミンの使用。
- 該同定が視覚的にまたは顕微鏡で行なわれることを特徴とする請求項9記載の使用。
- 請求項1記載のコバラミンと、個体からの組織または細胞とを接触させ、該組織を照射し、次いで、蛍光を検出し、それによって、該癌の組織または細胞が蛍光を発し、健康な組織がほとんど蛍光を示さないことにより、in vivo、ex vivoまたはin situにて癌を診断、検出、予測またはモニターする診断剤を調製するための請求項1記載のコバラミンの使用。
- 該接触が、組織および細胞の臨床病理学的評価中に行なわれることを特徴とする請求項11記載の使用。
- 請求項1記載のコバラミンと、個体からの組織または細胞とを接触させ、該組織を照射し、次いで、蛍光を検出し、それによって、該癌の組織または細胞が蛍光を発し、健康な組織がほとんど蛍光を示さないことにより、該個体における癌の治療、診断、検出、予測、予後またはモニタリングにおける転移性疾患を同定する診断剤を調製するための請求項1記載のコバラミンの使用。
- 該癌が乳癌、大腸癌、卵巣癌、肺癌、前立腺癌、肝癌、黒色腫、またはリンパ系を介して広がった癌であることを特徴とする請求項13記載の使用。
- 該癌がリンパ腫または白血病であることを特徴とする請求項13記載の使用。
- 該組織が骨髄穿刺液または末梢血であることを特徴とする請求項15記載の使用。
- フローサイトメトリーまたは体液の自動分析を利用することを特徴とする請求項13記載の使用。
- 請求項1記載のコバラミンと癌細胞とを接触させ、該細胞を照射し、次いで、蛍光を検出し、それによって、癌細胞でない細胞より大きな蛍光を示す癌細胞がコバラミンに基づいた化学療法剤での治療に対して有利に応答することを予測する診断剤を調製するための請求項1記載のコバラミンの使用。
- 該治療が化学療法であることを特徴とする請求項18記載の使用。
- 該化学療法がコバラミン−治療剤コンジュゲートを利用することを特徴とする請求項19記載の使用。
- 該治療がホルモン療法であることを特徴とする請求項18記載の方法。
- 試料中のコバラミン量をアッセイする方法であって、請求項1記載のコバラミンを用いて該試料につき競合的結合アッセイを行い、次いで、該試料中に存在するコバラミン量を決定することを特徴とする該方法。
- in vitroにて試料中の不飽和のコバラミン結合能の量をアッセイする方法であって、請求項1記載のコバラミンを用いて該試料から単離されたコバラミン結合蛋白質につき競合的結合アッセイを行い、次いで、該試料中の不飽和のコバラミン結合能の量を決定することを特徴とする該方法。
- in vitroにて試料中のコバラミン結合蛋白質に結合したコバラミン量をアッセイする方法であって、請求項1記載のコバラミンを用いて、該試料から単離されたコバラミン結合蛋白質から分離されたコバラミンの競合的結合アッセイを行い、次いで、該試料中の該蛋白質に結合したコバラミン量を決定することを特徴とする該方法。
