Description

Claims (24)
- 回転して骨組織内に穴を形成しまたは骨組織内へ骨ねじを挿入するように構成される回転可能な工具部であって、長手方向軸線に沿って延び非絶縁先端を有する導電性部材を備える絶縁軸を含み、前記絶縁軸が前記非絶縁先端に対するおよび患者内への電気信号を送る、回転可能な工具部と、
前記回転可能な工具部に取り外し可能に且つ作動可能に結合されたハンドル部であって、前記回転可能な工具部を長手方向軸線の周りで前記ハンドル部に対して回転させるための作動システムを含み、前記ハンドル部は前記回転可能な工具部の使用者を前記非絶縁先端における電気信号から電気的に絶縁するように構成されている、ハンドル部と、
前記ハンドル部に接続され且つ該ハンドル部を介して前記回転可能な工具部に電気的に接続されて電気信号としての電流を前記非絶縁先端へ送る神経監視システムであって、前記電気信号の関数として前記回転可能な工具部に近接している患者の神経要素閾値を検知するように作動可能であり、前記検知は、前記電流に対する患者の筋肉反応の監視に基づいて行われるものであり、前記ハンドル部の作動システムは、前記回転可能な工具部を第1の方向へ前記長手方向軸線の周りで回転させ前記回転可能な工具部を骨組織へ向かって進行させるように作動可能であり、前記神経監視システムは、前記患者の前記神経要素閾値を検知すると、前記回転可能な工具部を前記第1の方向とは反対向きの第2の方向へ回転を逆にして前記患者の神経要素を侵すことのないようにするために前記ハンドル部の前記作動システムに電気信号を提供するように作動可能である神経監視システムと、
を備える装置。 - 前記作動システムは、前記ハンドル部内でモータに作用するように結合される回路を含む、請求項1記載の装置。
- 前記ハンドル部は、前記長手方向軸線に沿って延びる細長い円筒形本体を含む、請求項1または2記載の装置。
- 前記工具部は、骨組織に穴を形成するためにドリルを含む、請求項1から3のいずれかに記載の装置。
- 前記工具部は、骨組織の穴に固定具を挿入するためにねじ回しを含む、請求項1から3のいずれかに記載の装置。
- 前記工具部の前記絶縁軸は、近位端および前記近位端の反対側の遠位端を含み、
前記遠位端は、前記非絶縁先端を含み、
前記絶縁軸は、前記近位端のところに、前記ハンドル部にぴったりと受け入れられる万能コネクタを含む、請求項1から5のいずれかに記載の装置。 - 骨組織の領域に明かりを提供するために、前記ハンドル部に取り外し可能で且つ作動可能に接続される照明組立体をさらに備える、請求項1から6のいずれかに記載の装置。
- 骨組織が脊椎の椎弓根内にある、請求項1から7のいずれかに記載の装置。
- 前記ハンドル部を前記神経監視システムに電気的に結合するための接続リンクをさらに備える、請求項1から8のいずれかに記載の装置。
- 前記ハンドル部は、該ハンドル部から延びるピストル形グリップ部を含む、請求項1から9のいずれかに記載の装置。
- 前記ピストル形グリップ部は、前記ハンドル部に枢動するように結合される、請求項10記載の装置。
- 前記ピストル形グリップ部は、前記ハンドル部に取り外し可能に結合される、請求項10または11記載の装置。
- 前記ハンドル部は円筒形である、請求項1から9のいずれかに記載の装置。
- 前記神経要素閾値が前記先端に対する前記神経要素の近接を含む、請求項1から13のいずれかに記載の装置。
- 回転して骨組織内に穴を形成しまた骨組織内へ骨ねじを挿入するように構成される回転可能な工具部であって、細長い導電性の絶縁軸、および前記軸の遠位端に設けられる非絶縁先端を含み、前記絶縁軸が前記非絶縁先端に対する及び患者内への電気信号を送る、回転可能な工具部と、
前記回転可能な工具部に取り外し可能に且つ作動可能に結合されるハンドル部であって、前記回転可能な工具部の長手方向軸線の周りに前記回転可能な工具部を回転させるためのシステムを含み、前記システムは前記回転可能な工具部を第1の方向へ前記長手方向軸線の周りで回転させて前記回転可能な工具部を骨組織へ向かって進行させるように作動可能であり、前記ハンドル部は前記回転可能な工具部の使用者を前記非絶縁先端における電気信号から電気的に絶縁するように構成されており、前記システムが、前記患者の神経要素を侵すことのないようにするために、電流に対する患者の筋肉反応の監視に基づく、前記電気信号の関数としての前記回転可能な工具部に近接している患者の神経要素閾値の検知を示す信号に応答して自動的に前記回転可能な工具部の回転を前記第1の方向とは反対向きの第2の方向に逆にするように作動可能である、ハンドル部と、
を備える装置。 - 前記システムは、前記工具部を回転させるように作動可能なモータを含む、請求項15記載の装置。
- 前記工具部は、骨固定具に対して係合可能なねじ回しである、請求項15または16記載の装置。
- 前記工具部は、骨組織内に穴を形成するためのドリルである、請求項15または16記載の装置。
- 骨組織の領域を照らすために前記ハンドル部に結合される照明組立体をさらに備える、請求項15から18のいずれかに記載の装置。
- 長手方向軸線に沿って延びる絶縁軸と非絶縁先端とを含み、前記絶縁軸は前記非絶縁先端に対する及び患者内への電気信号を送る、回転可能な工具部と、
前記回転可能な工具部に作動可能に結合されるハンドル部であって、前記回転可能な工具部の使用者を前記非絶縁先端における電気信号から電気的に絶縁するように構成されており、該ハンドル部に対して前記回転可能な工具部を前記長手方向軸線の周りで第1の方向に回転させ、前記回転可能な工具部を骨組織へ向かって進行させるように構成される、ハンドル部と、
前記回転可能な工具部に電気的に結合され、前記回転可能な工具部に近接する患者の神経要素を検知するために前記非絶縁先端に電気信号としての電流を提供する神経監視システムであって、前記検知は、前記電流に対する患者の筋肉反応の監視に基づいて行われる、神経監視システムと、
を備え、前記神経監視システムは、患者の神経要素を検知すると、前記患者の神経要素を侵すことのないようにするために、前記回転可能な工具部を前記第1の方向と反対の第2の方向に回転させるさせるために信号を提供するように作動可能である、システム。 - 前記先端は、骨ねじと係合して回転力を送達するように構成される、請求項20記載のシステム。
- 前記先端は、骨組織内に穴を形成するように構成される、請求項20記載のシステム。
- 神経要素閾値は神経要素に対する前記先端の近接に対応し、前記神経監視システムは、前記神経要素閾値を検知すると、前記工具部の回転を停止させるために前記ハンドル部内の作動システムに信号を送り、又は前記工具部を前記第1の方向と反対の第2の方向に回転させるさせるために信号を送る、請求項20から22のいずれかに記載のシステム。
- 前記作動システムは、前記工具部に作動可能に結合されるモータ、および前記神経監視システムに作動可能に連結される回路を含む、請求項23記載のシステム。
