Description

図４〜図７を参照して、ハンドルアセンブリ１２は、筐体３８を含み、筐体３８は好ましくは、プラスチックモールド筐体ハーフセクション３８ａおよび３８ｂから形成される。あるいは、金属、例えばステンレススチールを含む他の材料が、筐体を形成するために使用され得る。筐体３８は、ハンドルアセンブリ１２の静止ハンドル２６およびバレル部分３０を形成する（図１参照）。移動可能ハンドル２８は、円筒状部材４０の周りで筐体ハーフセクション３８ａと３８ｂとの間で回転可能に支持され、円筒状部材４０は、移動可能ハンドル２８の中の開口部４１内で受けられる。付勢部材４２は好ましくは、トーションばねであり、移動可能ハンドル２８を静止ハンドル２６から遠ざけて、圧縮されていない位置へと促す。移動可能ハンドル２８は、ピボット部材４７を受けるように寸法が取られた一対の通し穴４６を含む。つめ４８は、ピボット部材４７上で回転可能に支持され、ばね５０によって作動シャフト５２に向かって付勢される。
作動シャフト５２は、筐体３８のバレル部分３０内で引っ込められた位置と進められた位置との間でスライド可能に支持され、発射ロッド５８の近位端５６を回転可能に受けるように構成されたリセス５４を画定する遠位端を含む。ばねで付勢されたリトラクトアーム５７は、筐体ハーフセクション３８ａと３８ｂとの間で回転可能に取り付けられ、延長５７ａを含む。延長５７ａは、作動シャフト５２に形成されたスロット５９（図４）内に配置され、作動シャフトを完全に引っ込められた位置に促す。作動シャフト５２は、歯つきラック６０を含む。つめ４８は、ばね５０によって作動シャフト５２の歯つきラック６０の方に付勢される係合フィンガ６２を有している。移動可能ハンドル２８が作動させられるとき、すなわちばね４２の付勢に反して静止ハンドル２６の方に圧縮されるとき、つめ４８の係合フィンガ６２は、作動シャフト５２の歯つきラック６０と係合し、作動シャフト５２および発射ロッド５８を遠位方向に進める。ローディングユニット１６の近位端が、外科用ステープリングデバイス１０の細長い本体１４と係合しているとき、発射ロッド５８の遠位端は、ローディングユニット１６の駆動アセンブリ２１２の近位端と係合する。
外科用ステープリングデバイスは、使い捨てローディングユニット、すなわち「ＤＬＵ」を含む。所望のステープルサイズまたはサイズ（複数）を有するローディングユニット、および所望のステープル列長さは、該デバイスと共に組み立てられる。ローディングユニットは、近位本体部分に関してアーティキュレートし得るツールアセンブリ、またはアーティキュレーションを提供しないローディングユニットを含み得る。ローディングユニットは、直線状の列のステープルまたは他のステープル構成を有するツールアセンブリを含み得る。ローディングユニットからステープルを発射した後、ローディングユニットは、デバイスから除去され得、新しいローディングユニットがデバイスと共に組み立てられ得る。
図１および図７〜図９を参照して、アーティキュレートするツールアセンブリを有するローディングユニットが示されている。ローディングユニット１６は、ツールアセンブリ２０、近位本体部分１８、および取り付けアセンブリ２０２（図１０）を含む。本体部分１８は、以下に詳細に論議される態様で細長い本体１４の遠位端と解放可能に係合するように適合されている近位端を有している。取り付けアセンブリ２０２は、本体部分１８の遠位端に旋回可能に取り付けられ、ツールアセンブリ２０の近位端に動かないように固定されている。取り付けアセンブリ２０２のピボット運動は、ツールアセンブリ２０を旋回させ、その結果ツールアセンブリ２０の長手方向の軸は、細長い本体１４の長手方向の軸に関して角度を持つ。細長い本体１４の長手方向の軸に対して実質的に垂直な軸を中心とする取り付けアセンブリ２０２のピボット運動は、ツールアセンブリ２０の長手方向の軸が細長い本体１４の長手方向の軸と一直線になっているアーティキュレートされていない位置と、ツールアセンブリ２０の長手方向の軸が細長い本体１４の長手方向の軸とある角度で配置されるアーティキュレートされた位置との間のツールアセンブリ２０のアーティキュレーションをもたらす。
図７〜図９を参照して、ツールアセンブリ２０は、カートリッジアセンブリ２２およびアンビルアセンブリ２４を含む。アンビルアセンブリ２０は、複数のステープル変形凹面３０（図９）を有するアンビル部分２８およびアンビル部分２８の上面に固定されているカバープレート３２を含む。カバープレート３２およびアンビル部分２８は、その間に空洞３４を画定する。カバープレート３２は、ローディングユニット１６の作動、およびローディングユニット１６の中を通る駆動アセンブリ２１２の前進の間、組織が挟まれることを防ぐ。長手方向のスロット３８は、アンビル部分２８の中を通って延び、駆動アセンブリ２１２の保持フランジ４０の通過を容易にする。アンビル部分２８上に形成されたカム表面４２は、駆動アセンブリ２１２の保持フランジ４０上で支持された一対のカム部材４０ａによって係合され、アンビルとカートリッジアセンブリとの接近をもたらす。一対のピボット部材２１１および一対の安定部材２１５は、アンビル部分２８上に形成されている。
カートリッジアセンブリ２２は、ステープルカートリッジ２２０を受けるような大きさとされ、かつ構成された細長いサポートチャンネル２１８を画定するキャリア２１６を含む。キャリア２１６は、キャリア２１６において画定された一対のショルダ２１７および一対のスロット２１３を有する。一対のスロット２１３は、一対のピボット部材２１１を受け、アンビル部分２８がカートリッジアセンブリ２２に対して旋回運動することを可能にする。一対の安定部材２１５の各々は、それぞれのショルダ２１７と係合し、アンビル部分２８がピボット部材２１１を中心として旋回されるとき、アンビル部分２８がステープルカートリッジ２２０に対して軸方向にスライドすることを妨げる。ステープルカートリッジ２２０および細長いサポートチャンネル２１８に沿って形成された対応するタブ２２２およびスロット２２４はそれぞれ、サポートチャンネル２１８内の固定された位置にステープルカートリッジ２２０を保持するように機能する。ステープルカートリッジ２２０上に形成された一対のサポートストラット２２３は、キャリア２１６の側壁上に載るように配置され、サポートチャンネル２１８内でステープルカートリッジ２２０をさらに安定させる。
ステープルカートリッジ２２０は、複数のステープルすなわち締め具２２６およびプッシャ２２８を受けるための保持スロット２２５（図７）を含む。横方向に間隔を置かれた複数の長手方向のスロット２３０は、ステープルカートリッジ２２０の中を通って延び、作動スレッド２３４（図７）の直立したカムウェッジ２３２を収容する。中央の長手方向のスロット２８２は、実質的にステープルカートリッジ２２０の全長にわたって延び、駆動アセンブリ２１２（図９）の通過を容易にする。外科用ステープルデバイス１０の動作の間、駆動アセンブリ２１２は、発射ロッド５８によって進められる。駆動アセンブリ２１２は、作動スレッド２３４に接し、ステープルカートリッジ２２０の長手方向のスロット２３０の中を通って作動スレッド２３４を押し、カムウェッジ２３２を進めてプッシャ２２８と順次接触させる。プッシャ２２８は、締め具保持スロット２２５内でカムウェッジ２３２に沿って垂直に移動し、締め具２２６を保持スロット２２５から促してアンビルアセンブリ２４のステープル変形空洞２０６（図８）の中に入れる。
駆動アセンブリ２１２は、ワーキングヘッド２６８を有する駆動ビーム２６６を含む。駆動ビーム２６６のワーキングヘッド２６８の遠位端は、メスレード２８０を支持する垂直サポートストラット２７８（図９）、およびステープリング処置の間作動スレッド２３４の一部分と係合する接面２８３によって画定される。メスブレード２８０は、ステープルカートリッジ２２０における中央の長手方向のスロット２８２の中を通って、作動スレッド２３４のわずかに背後で移動するように配置され、ステープルされた身体組織の列の間で切開を形成する。保持フランジ４０は、垂直ストラット２７８から遠位方向に突出し、その遠位端において円筒状カムローラ４０ａを支持する。カムローラ４０ａは、アンビル部分２８上のカム表面４２と係合し、身体組織に対して、アンビル部分２８を締め付けるような大きさとされて構成される。
使用に際して、ユーザは、組織を締め付けてステープルを発射するようにハンドルアセンブリ１２を操作する。カートリッジならびにアンビルアセンブリ２２および２４を接近させ、組織を締め付けるために、移動可能ハンドル２８は、静止ハンドル部材２６に向かう方向に動かされる。移動可能ハンドル２８は、トーションばね４２の付勢に対して静止ハンドル２６に向かって圧縮され、作動シャフト５２と係合する。つめ４８の係合フィンガ６２は、作動シャフト５２の歯つきラック６０と係合し、作動シャフト５２および発射ロッド５８を遠位方向に進める。
発射ロッド５８は、その遠位端において、駆動ビーム２６６を含む軸方向駆動アセンブリ２１２に接続され、その結果発射ロッド５８の前進は、駆動ビーム２６６の前進をもたらす。駆動ビーム２６６が進められるとき、カムローラ４０ａは移動してアンビル部分２８のカム表面４２と係合し、カートリッジ２２０の方向にアンビル部分２８を促し、カートリッジならびにアンビルアセンブリ２２および２４を接近させ、それらの間で組織を締め付ける。
移動可能ハンドル２８が作動されて、カートリッジならびにアンビルアセンブリ２２および２４接近させた後、付勢部材４２は、ハンドルを静止ハンドル２６から離れた圧縮されていない位置へ戻す。
一旦組織が締め付けられたとき、ステープリングデバイス１０を発射するためには、移動可能ハンドル２８は、作動ストロークの終わりまで静止ハンドル部材２６に向かって動かされ、その間、つめ４８の係合フィンガ６２は、作動シャフト５２の歯つきラック６０と係合し、作動シャフト５２および発射ロッド５８を遠位方向にさらに進める。ローディングユニット１６からすべてのステープルを発射するためには、２回以上の作動ストロークが必要とされる。発射ロッド５８が、上に論議された態様で進められるとき、駆動ビーム２６６は、ステープルカートリッジ２２を通って遠位方向に進められ、作動スレッド２３４と係合し、同時にメス２８０で組織を切断し、プッシャ２２８を駆動して順次カートリッジからステープル２２６を排出する。様々な長さのステープル列を有するローディングユニットが使用され得、作動ストロークの数は当然変化する。ツールアセンブリの構造および動作は、その開示が参照として本明細書中に援用されている米国特許第５，８６５，３６１号に開示された実施形態に従い得る。
細長い本体１４は、図５に示されるように回転可能な部材３２の中に取り付けられ、回転可能な部材３２は、細長い本体１４およびツールアセンブリ２０を含むローディングユニット１６が長手方向の軸の周りに回転可能となるように、ハンドルアセンブリ１２に取り付けられる。回転可能部材３２は、１つ以上の管状または円錐形の部材から形成され、デバイス１０の長手方向の軸に関してツールアセンブリ２０をアーティキュレートするためのアーティキュレーション作動メカニズムを収納する。アーティキュレーション作動メカニズムは、アーティキュレーションレバー１２２（図６）を含む。アーティキュレーションレバー１２２は、細長い本体１４の中を通って延びるアーティキュレーションアームに動作可能に接続されている。アーティキュレーションレバー１２２は、ツールアセンブリ２０のアーティキュレーションの所定の角度を画定するためのメカニズムに接続され得る。アーティキュレーションレバー１２２の動作および構造は、米国特許出願公開第２００４／０２３２２０１号に開示されているとおりであり得、その開示は本明細書中に参考として援用されている。アーティキュレーションレバー１２２は、ピボットピンの周りで回転可能な部材３２に取り付けられ、かつアーティキュレーションアームに取り付けられ、その結果レバー１２２の、ピボットピンの周りの回転はアーティキュレーションアームの長手方向の動きを生じる。アーティキュレーションアームは、細長い本体１４の中を通って延び、ローディングユニット１６が、細長い本体１４に取り付けられるときに、ローディングユニット１６のアーティキュレーションリンク２５６（図９）に取り付けられる。アーティキュレーションレバー１２２は、ツールアセンブリ２０をアーティキュレートするために、外科用ステープリングデバイス１０のユーザによって回転され得る。アーティキュレーションレバー１２２が第１の方向に回転されると、レバーに取り付けられたアーティキュレーションアームは、遠位方向に進められる。アーティキュレーションアームは、ローディングユニット１６のアーティキュレーションリンク２５６を進め、取り付けアセンブリ２０２をピボット２４４の周りで旋回させ、ツールアセンブリ２０を第１の方向にアーティキュレートする。アーティキュレーションレバー１２２が第２の方向に回転されると、レバーに取り付けられたアーティキュレーションアームは、近位方向に引っ込められる。アーティキュレーションアームは、ローディングユニット１６のアーティキュレーションリンク２５６を引っ込め、ピボット２４４の周りで取り付けアセンブリ２０２を旋回させ、第２の方向にツールアセンブリ２０をアーティキュレートする。
図６に示されるように、ローディングユニット１６は、細長い本体１４の遠位端に除去可能に取り付けられている。ローディングユニット１６の本体部分１８は、軸方向の駆動アセンブリ２１２の前進を可能にするためにチャンネル２５３を画定する、第１の筐体２５０および第２の筐体２５２を含む（図９）。筐体２５０および２５２は、アーティキュレーションリンク２５６のためのスロットも画定する。筐体２５０および２５２は、外側チューブ２５１の中に受けられる。筐体２５０および２５２の近位端は挿入チップ１９３を画定し、挿入チップ１９３の上では一対のラグ２５４が形成される。ラグ２５４は、細長い本体１４との開放可能な接続を形成し、その結果ローディングユニット１６は細長い本体１４に取り付けられ得、かつ細長い本体１４から除去され得る。一対のブロウアウトプレート２５５は、近位本体部分１８の遠位端に隣接して、かつ取り付けアセンブリ２０２に隣接して配置される。ブロウアウトプレート２５５は、ツールアセンブリ２０のアーティキュレーションおよび発射の間、駆動アセンブリ２１２を支持する。ブロウアウトプレート２５５の構造および動作は、その開示が参考として本明細書中に援用されている米国特許出願公開第２００４／０２３２２０１号に、さらに完全に記述されている。
細長い本体１４の遠位端は、それにローディングユニット１６を取り付けるためのチップアセンブリ３０１を画定する。図１０〜図１５および図１８は、本開示に従って、チップアセンブリ３０１を示す。チップアセンブリは、回転するように固定された細長い本体１４の遠位端に取り付けられたリング３００と移動可能にリング３００に取り付けられるヨーク４００とを含む。（図１０および図１４参照）。リング３００は、通路３０３を画定し、通路３０３の中では２つの螺旋状の案内ランプ３０２が形成される。案内ランプの各々は、遠位端３０４および近位端３０６、ならびに近位端３０６に隣接したレッジ３１０を有する。溝３１２は、ヨーク４００を取り付けるため、リング３００の内面に画定される。（図１０参照）
移動可能ヨーク４００は、ローディングユニット１６上のラグ２５４と相互に作用するため、少なくとも１つの突起を有している。図１１に示されるように、突起は、対で配列された２つのタブ４０２および２つのストップ４０４を含み、これによって各タブ４０２および各ストップ４０４は受け空間５０１を画定する。ヨーク４００の遠位端は、リッジ４０６も有し、リッジ４０６は、リング３００の溝３１２と協働し、それによってヨーク４００はリング３００に関して、第１の、最初の位置から第２の位置へ、回転可能となる。ヨーク４００が回転するとき、案内ランプ３０２の近位端３０６に対する、タブおよびストップの位置は、変化する。図１１に示されるように、ヨーク４００の第１の位置において、タブ４０２とストップ４０４のペアは、案内ランプ３０２のうちの１つの近位端３０６のうちの１つに隣接して配置され、これによって受け空間５０１は、ローディングユニット１６のラグ２５４のうちの１つを受けるように配置される。図１２に示されるように、ヨーク４００の第２の位置において、タブ４０２とストップ４０４のペアは、受け空間５０１、およびその中に配置されているラグ２５４がレッジ３１０の下に配置されるように、配置される。
ローディングユニット１６は、チップアセンブリ３０１の中に挿入され、その結果挿入チップ１９３は、通路３０３の中に挿入される。ラグ２５４は、リング３００の通路３０３の中を進められ、案内ランプ３０２の遠位端３０４と接触する。（図１１参照）。ローディングユニット１６が、方向Ａに回転されるとき（図１２参照）、ラグ２５４は、案内ランプ３０２の近位端３０６に向かって案内ランプ３０２上を案内され、ヨーク４００の受け空間５０１の中に嵌る。図１４は、受け空間５０１の中のラグ２５４を示す。チップアセンブリ３０１はまだ第１の位置の中にあり、ストップ４０４およびタブ４０８は、ラグ２５４の両側にあり、かつ案内ランプ３０２のうちの１つの近位端３０６に隣接して配置される。この位置において、ローディングユニット１６は、遠位方向に動かされ得、かつ細長い本体１４から除去され得る。ユーザは、方向Ａにローディングユニット１６を回転し続け、この結果ラグ２５４はタブ４０２を押し、これによって、図１２に示されるように移動可能なヨーク４００を第２の位置へ回転させる。ラグ２５４は、レッジ３１０の下に位置する。図１５は、レッジ３１０の下に配置されたラグ２５４を示す。ストップ４０４は、ローディングユニット１６が、ヨーク４００に対して回転することを防ぐ。従って、ローディングユニット１６は、チップアセンブリ３０１の中に捕捉され、ヨーク４００を回転させずに動かされることはあり得ない。
ローディングユニット１６が、細長い本体１４の遠位端に取り付けられるとき、発射ロッド５８の遠位端は、駆動アセンブリ２１２の近位端に接続される。駆動アセンブリ２１２の近位端は、発射ロッド５８の遠位端を受けるためのポートホール（ｐｏｒｔｈｏｌｅ）を有する駆動部材２７２を含む。ローディングユニット１６が、ローディングユニットの取り付け中に回転されるとき、アーティキュレーションリンク２５６は、移動してアーティキュレーションアームの遠位端の係合構造と係合する。
デバイスからローディングユニット１６を除去するためには、ローディングユニット１６は、方向Ａとは反対の方向に回転され、それと一緒にヨーク４００を回転させる。ラグ２５４はそれによって、レッジ３１０から遠ざかるように動かされる。ローディングユニット１６が回転されるとき、アーティキュレーションリンク２５６およびアーティキュレーションアームは、互いとの係合から遠ざかるように動かされる。ローディングユニット１６は、ローディングユニット１６を回転し続け、その結果ラグ２５４は、遠位端３０４に向かって案内ランプ３０２に従い、ＤＬＵを遠位方向に移動させることによってデバイスから除去され得る。細長い本体１４からＤＬＵを除去する際に、発射ロッド５８は、駆動アセンブリ２１２から係合を解かれる。
図１６〜図２３に示されるように、本開示による外科用ステープリングデバイス１０は、センサメカニズム５１０およびロッキング構造５１３を含む。センサメカニズム５１０およびロッキング構造５１３は、チップアセンブリ３０１（図１７）と相互に作用し、ローディングユニット１６を細長い本体１４に固定する。センサメカニズム５１０およびロッキング構造５１３は、ローディングユニット１６を細長い本体１４から解放する。ロッキング構造５１３は、ローディングユニット１６が細長い本体１４にロードされるまで、発射ロッド５８を正しい位置にロックする。
図１８に示されるように、センサメカニズム５１０は、溝５０４を備えた遠位端を有するセンサチューブ５０２を含む。チップアセンブリ３０１のヨーク４００は、溝５０４と係合し、ヨーク４００の動きをセンサチューブ５０２に合わせる突起４０７を有する。ローディングユニット１６のローディングの間、ヨーク４００は回転され、センサチューブ５０２は同じ方向に回転される。センサチューブ５０２の近位端は、ロッキング構造５１３に接続されている。ロッキング構造５１３は、細長い本体１４の近位端において、またはハンドルアセンブリ１２上に、ボタン５１４または他の操作可能なアクチュエータを含み、それによってロッキング構造５１３はデバイス１０のユーザに利用可能となる。例えば、ボタン５１４は、図１９において回転可能な部材３２上に示されている。ボタン５１４は、センサチューブ５０２の方に延びているボタンタング５１２を有している。リリースフランジ５０８は、センサチューブ５０２に取り付けられ、ボタンタング５１２（図２０）から離れた第１の位置から、リリースフランジ５０８の動きがロッキング構造５１３（図１９）のボタンタング５１２によって遮られる第２の位置へ、センサチューブ５０２と共に回転する。ボタン５１４は、ばねによって遠位方向に付勢される。
プランジャ５１６は、発射ロッド５８と相互に作用する。図２０〜図２３に最も良くみられるように、発射ロッド５８の近位端５２４は、そこに画定されているノッチ５２６を有する。プランジャ５１６は、ノッチ５２６において発射ロッド５８と係合するための第１の端、およびボタン５１４（図２１および図２３）と連係するために配置された傾斜した表面５２２を有する第２の端を有する。プランジャ５１６は、発射ロッド５８から離れる方向に付勢される。
ローディングユニット１６がデバイス１０に取り付けられる前の最初の位置において、ロッキング構造５１０はノッチ５２６において発射ロッド５８と係合し、発射ロッド５８の前進を妨げる。リリースフランジ５０８は、ボタン５１４が遠位方向に動くことを妨げ、それによってボタン５１４は、プランジャ５１６の、ノッチ５２６との係合を維持する。ローディングユニット１６がデバイスに取り付けられるとき、ヨーク４００は、回され、それによってセンサチューブ５０２を回す。リリースフランジ５０８は、ボタンタング５１２から離れるように動き、ボタン５１４が遠位方向に動くことを可能にする。図２３に示されるように、ボタン５１４は、プランジャ５１６がノッチ５２６から離れるように動くことを可能にする。移動可能なハンドル２８が操作され、デバイス１０が作動させられ、組織を締め付け、そしてステープルを発射するとき、ロッキング構造５１０は、発射ロッド５８から係合が外され、発射ロッド５８が移動することを可能にする。リリースレバー５０８がボタンタング５１２によって遮られ、センサチューブ５０２の回転を妨げるので、ローディングユニット１６は、デバイス１０にロックされもする。センサチューブ５０２の回転が妨げられると、ローディングユニット１６をデバイス１０に保持するヨーク４００が、回転を妨げられる。
ローディングユニット１６が、デバイス１０から除去されるべきとき、ロッキング構造５１０のボタン５１４は、図２１に示されるように、ボタンのばねの付勢に逆らって動かされ、ボタンタング５１２を、図２０に示されるように、リリースフランジ５０８から離れるように移動させる。ローディングユニット１６は回転され得、細長い本体１４のチップアセンブリ３０１から除去される。さらに、図２１に示されるように、ボタン５１４がプランジャ５１６を下方にカムで押し下げ、ノッチ５２６の中に入れるので、ロッキング構造５１０は、発射ロッド５８と係合する。
本開示によるロッキング構造および／またはセンサメカニズムは、任意の外科用ローディングユニット、例えばステープルカートリッジ、取替え可能なツールアセンブリ、または他のエンドエフェクターを固定するために使用され、一方、外科用器具からそれらの解放を提供する。望ましくは、外科用ローディングユニットを解放し、かつ／またはロックする操作可能なアクチュエータは、ハンドルアセンブリに隣接して配置される。内視鏡器具において、操作可能なアクチュエータは、内視鏡シャフトまたは細長い本体の近位端に、またはそれに隣接して配置される。
発射した後、かつローディングユニットを除去する前、撤収メカニズムが使用される。引っ込みメカニズムは、リターンノブ３６（図１）を含み、リターンノブ３６は、結合ロッド８２（図４）によって作動シャフト５２の近位端に接続されている。結合ロッド８２は、左右の係合部分８２ａおよび８２ｂを有し、左右の係合部分８２ａおよび８２ｂは、筐体ハーフセクション３８ａおよび３８ｂに形成された細長いスロット８３（図１）の中を貫通して延び、リターンノブ３６を受けるように構成されている。結合ロッド８２の中央部分８２ｃは、作動シャフト５２の近位端に形成されたスロット８４内でスライド可能に受けられるように寸法が取られている。解放プレート８６は、一対のピン８８（図４）によって作動シャフト５２の１つの側で支持される。ピン８８は、解放プレート８６を貫通して形成された角度の付いたカムスロット９０内に配置される。結合ロッド８２は、解放プレート８６の近位端に形成された開口部９２の中を通って延びる。
使用に際して、ノブ３６が外科医によって後方に引かれると、ロッド８２は作動シャフト５２のスロット８４の中でスライドするので、結合ロッド８２は最初、作動シャフト５２に対して後方に解放プレート８６を動かす。これが起きると、ピン８８は、解放プレート８６を、作動シャフト５２の歯つきラック６０を覆う位置までカムで押し下げ、つめ４８のフィンガ６２を歯つきラック６０から外す。結合ロッド８２が、それがスロット８４のバックエンド８４ａと係合する位置まで後方に引き下げられると、ノブ３６のさらなる後方への動きが、作動シャフト５２および発射ロッド５８の近位方向への動きをもたらす。
フック９６は、作動シャフト５２の上面に形成されるスロット９８において支持される。フック９６は、結合ロッド８２を受けるような大きさの通し穴９６ａを含む。フック９６の前方端は、ばね１００の１つのループ端１００ａを受けるように構成されている上向き部分９８を含む。ばね１００の反対端は、作動シャフト５２上に形成されたポスト１０２を受けるような大きさのループ１００ｂを含む。ばね１００は張力を維持され、結合ロッド８２を、作動シャフト５２におけるスロット８４の前方端に向かって促す。結合ロッド８２が、作動シャフト５２のスロット８４の前方端に配置されるとき、解放プレート８６は、作動シャフト５２の歯つきラック６０の上の高い位置に保持される（すなわちカムで上へ動かされる）。

Claims (22)

  1. 外科用器具であって、
    ツールアセンブリを有するローディングユニットと、
    近位端および遠位端を有する細長い本体であって、該細長い本体の該遠位端は、該ローディングユニットを該細長い本体に除去可能に固定する固定構造を含み、該固定構造は、少なくとも１つの案内ランプ、ならびに第１の位置および第２の位置を有する移動可能なヨークを含み、該ローディングユニットは、該ヨークが該第２の位置にあるとき、該細長い本体上に固定されている、細長い本体と、
    該ローディングユニットを該細長い本体の該遠位端上に取り付ける際に、該ローディングユニットと該ローディングユニットとの動きによる係合のために配置されたセンサチューブを含む回転可能なセンサメカニズムと、
    を備え、
    該少なくとも１つの案内ランプは、該ヨークとの係合に向かう所定の方向に該ローディングユニットの動きを案内するように配置され、該ヨークは、該ローディングユニットが、該所定の方向に動かされるとき、該第２の位置に向かって移動可能であり、
    さらに、該外科用器具が、該センサメカニズムの動きを遮るための第１の位置と、該センサメカニズムの動きを可能にするための第２の位置とを有するロッキング構造を含むハンドルアセンブリであって、該ローディングユニットが、該ロッキング構造が該第２の位置にあるときに該細長い本体から除去可能である、ハンドルアセンブリを備え、
    ここで、該ローディングユニットは、該ヨークと係合するための突出する部分を有し、該少なくとも１つの案内ランプは、弓状に曲がった経路を画定する、外科用器具。
  2. 前記固定構造は、通路を画定するリング、該リングによって画定され、該通路の中を延びる２つの案内ランプを含む、請求項１に記載の外科用器具。
  3. 前記案内ランプは、形状が螺旋形である、請求項２に記載の外科用器具。
  4. 前記案内ランプは、ほぼ１８０度離れて配置される、請求項２に記載の外科用器具。
  5. 前記少なくとも１つの案内ランプの各々は、前記細長い本体の前記近位端に向かって延びる、請求項１に記載の外科用器具。
  6. 前記リングは、前記細長い本体の前記遠位端に取り付けられ、前記ヨークは該リングに対して近位に、移動可能に取り付けられる、請求項１に記載の外科用器具。
  7. 前記ヨークは、前記リングに回転可能に取り付けられる、請求項６に記載の外科用器具。
  8. 前記ヨークおよび前記リングは、回転を可能にするための相互係合する溝とリッジの構造を有する、請求項７に記載の外科用器具。
  9. 前記ヨークは、前記突出部分による係合のための少なくとも１つの突起を有する、請求項１に記載の外科用器具。
  10. 前記ヨークは、一対の突起を有する、請求項９に記載の外科用器具。
  11. 前記一対の突起は、前記ヨークが前記第１の位置にあるとき、前記案内ランプの近位端に隣接して配置される、請求項１０に記載の外科用器具。
  12. 前記一対の突起は、前記突出部分に対する受け空間を画定する、請求項１１に記載の外科用器具。
  13. 前記固定構造はレッジを有し、前記突出部分は、前記ヨークが前記第２の位置にあるとき、該レッジに対して近位であり、前記ローディングユニットを前記細長い本体に固定する、請求項１に記載の外科用器具。
