Description
本発明は、請求項1の特徴を有する締付け装置で上記課題を解決する。
本発明によって達成される利点は、締付け装置が締付けリングとともに、キルシュナーワイヤーをその通路穴において軸方向に移動可能に受入れることができる点にある。次いで、キルシュナーワイヤーは所望の位置に配置されうるとともに、その前方範囲で圧縮可能に構成された締付け装置のネジ込みによって締付けリングへ締付けられうる。また、これは―従来技術による装置とは対照的に―キルシュナーワイヤーの位置が未だ不十分と思われる場合には何回も繰返されうる。また、別個の小さなグラブネジで横方向の締付けが行われず、追加の構成要素なしに軸方向の締付けが行われ、これがさらに可逆的である点にも利点がある。
従来技術と比べた別の利点は、締付けがキルシュナーワイヤーの軸に沿って行われうることである。従来技術においては、キルシュナーワイヤーに対して横方向にネジ込むことが必要であり、これは手術法および操作を複雑にする。
しかし、締付け装置の応用分野は小児科に限定されず、普遍的に、例えば、近位上腕にも応用される。
特定の実施形態においては、ネジのネジ山は二条ネジで構成されている。これは、円錐形の穴およびその補完的な雌ネジに一条ネジ山の場合よりも迅速に連結するため、ネジのネジ込みを容易にする。
別の実施形態においては、ネジの前端は3つもしくは4つの細長い裂け目が備えられており、これらは、好ましくは、均一の間隔で配置されている。
別の実施形態においては、締付け装置は、中央軸と中央軸に対して同軸に走る円錐形の穴とを含んで成る締付けリングを含んで成り、これはネジを受入れるとともに、少なくとも前端の範囲で半径方向に圧縮するのに適している。
特定の実施形態においては、細長い裂け目の長さLはネジの全高Hよりも小さく、好ましくは、L<0.8Hである。
別の実施形態においては、ネジの通路穴の内壁はざらざらであり、または肉眼的に見える構造を持っている。それによって改善された締付け効果が得られる。
締付け装置には、複数のプレート穴を含んで成る骨プレートもいっしょに備えられうるが、ここでプレート穴の少なくとも1つは円錐形の穴を有し、これは締付け装置のネジを受入れるとともに、少なくとも前端の範囲で半径方向に圧縮するのに適している。したがって、骨ネジの代わりに、キルシュナーワイヤーもプレートの穴を通じて導入され、そこに固定されうる。
本発明および本発明の発展形態を以下、複数の実施例の部分的概略図を用いてさらに詳細に述べる。
図1は、締付け前の締付けリングとともに締付け装置を示す斜視図である。
図2は、図1による締付けリングとともに締付け装置を示す縦断面図である。
図3は、締付けリングにおける締付け後の中に挿入されたキルシュナーワイヤーとともに締付け装置を示す縦断面図である。
図4は、骨プレートおよびキルシュナーワイヤーとともに締付け装置を示す縦断面図である。
さらに、ネジ3は後端8において円筒形の部分を含んで成り、ここにはネジ3をネジ込む手段9が取付けられており、これは円錐軸4に対して同軸にネジ3の後端8から貫入する。ネジ3をネジ込む手段9は、ここでは六角筒として構成されているが、それぞれ任意の適した形を有しうる。
図1に示されているように、ネジ3とともに使用される締付けリング11は外側にその中央軸12に対して同軸に円筒形に構成されており、かつ円錐形の穴13を含んで成り、これはネジ3のネジ山6に補完的な雌ネジ16を有する。締付けリング11へのネジ3のネジ込みに際して、これはネジ3およびそれに補完的に構成された円錐形の穴13の円錐形の構成によって、長さLのその半径方向に弾性の部分で半径方向に圧縮され、ここで通路穴7の円錐軸4に対して直交の断面も固定され、通路7に締付けられた固定要素2(図3)、例えば、キルシュナーワイヤーがネジ3で固定される。
さまざまな実施形態のためのネジ3およびそれに対応する締付けリング11、もしくは骨プレート14における構造物に対する寸法の適切な範囲は以下の通り実施されている。すなわち、
−ネジの高さH:3mm〜7mm、
−細長い裂け目の長さ:2.5mm〜4.5mm、
−細長い裂け目の幅0.3mm−1.2mm、
−ネジカバーの厚さ:円錐状に0.8mmないし4.0mmに増大
−通路穴の直径1.0mm〜3.0mm、
−通路穴とキルシュナーワイヤーとの間の遊び0ないし0.1mm、
−コーニング角15°〜25°、
−二条ネジ山のピッチ0.6〜1.1mm、通常0.8mm。

Claims (5)
- 長手方向の固定要素(2)を堅く固定する締付け装置であって、
前記締付け装置は、
円錐軸(4)を有する全高Hの少なくとも一部の先端が切られている円錐形の形をしたネジ(3)であって、
ネジ山(6)を持つ円錐形のカバー面(5)と、
前記長手方向の固定要素(2)を受入れる前記円錐軸(4)に対して同軸に走る通路穴(7)と、
前記ネジ(3)をネジ込む手段(9)を持つ後端(8)と、
前端(1)と
を有する前記ネジ(3)を備え、
前記ネジ(3)が少なくとも部分的に半径方向に圧縮可能であるために前記ネジ(3)の前記前端(1)には長さLで半径方向には前記通路穴(7)まで達する少なくとも1つの細長い裂け目(10)が備えられている締付け装置において、
A)前記締付け装置が複数のプレート穴(15)を有する骨プレート(14)を備え、
B)前記複数のプレート穴(15)の少なくとも1つが、円錐形の穴(13)であって、前記ネジ(3)を受入れかつ少なくとも前記前端(1)の範囲で半径方向に圧縮するのに適しており、
C)前記プレート穴(15)の少なくとも1つが、前記ネジ(3)の前記カバー面(5)の前記ネジ山(6)に補完的に構成されている円錐状の雌ネジ(16;20)を有する
ことを特徴とする締付け装置。 - 前記ネジ(3)の前記前端(1)は3つもしくは4つの細長い裂け目(10)が備えられていることを特徴とする請求項1に記載の締付け装置。
- 前記細長い裂け目(10)は互いに均一の間隔で配置されていることを特徴とする請求項2に記載の締付け装置。
- 前記細長い裂け目(10)の前記長さLは前記ネジ(3)の前記全高Hよりも小さいことを特徴とする請求項1乃至3のいずれか一項に記載の締付け装置。
- 前記長さLと前記全高HはL<0.8Hであることを特徴とする請求項4に記載の締付け装置。
