Description
本発明は、野生型p53の異なる多形性形態に関連した疾患を発症する危険を有する個体を同定するためのスクリーニング方法に関する。
細胞性腫瘍抑圧蛋白質p53は細胞増殖の主要な制御因子のひとつである。刺激のコンテクストに依存して、p53は細胞成長停止またはプログラムされた細胞死(アポトーシス)を誘発する。結果として、これはDNA変異を獲得した細胞の引き続く増殖を妨害し、そしてこれは癌に対する生物の鍵となる防御を代表する。多くのヒト腫瘍において、p53の不活性化は腫瘍の発生における主要な因子の一つである。ヒトパピローマウイルス(HPV)関連の癌においては、しかしながら、p53はほとんどいつも野生型である。これは、ユビキチン媒介性分解のためにp53を標識するウイルス性E6蛋白質の活性のためであり、即ち、正常なp53機能に打ち勝つ。このため、HPVsは女性における頸部癌(cervical cancer)の発生のための主要な発癌物質であり、全世界の癌のもっとも共通の形態の一つである。
a)生物学上のサンプルを患者から得て;そして
b)上記サンプル中のp53proまたはp53argの存在を決定するが、その際p53pro/p53pro,p53arg/p53arg,p53pro/p53argからなる群から選択される患者の対立遺伝子パターンが疾患を発症する危険因子の指標であること
からなる。
a)ポリメラーゼ鎖反応により増幅すること;
b)DNAまたは蛋白質を配列決定すること;
c)p53proまたはp53argのDNAの何れかに特異的な少なくとも一つのプローブとハイブリダイズすること;および
d)p53proまたはp53argの対立遺伝子を同定するためにp53pro蛋白質またはp53arg蛋白質の何れかに特異的な少なくとも一つの抗体を用いて免疫検出すること。
a)患者から生物学上のサンプルを得て;そして
b)上記中のp53proまたはp53arg野生型対立遺伝子の存在を測定するが、その際p53arg/p53argの患者が優先的に予防接種されるべきであること
からなる。
本発明に従い、我々は、2つの形態のp53がヒトパピローマウイルスE6蛋白質により別々に標的化されるか否かを測定することに興味を注いだ。我々は、p53のアルギニン形態が、p53のプロリン形態よりも、E6媒介性分解に対してより顕著に感受性であることを示した。これは、ウイルス感染またはウイルス形質転換においては、アルギニンp53のみを発現する個体がウイルスの作用に対してより感受性であることを示すはずである。これによる予測は、p53アルギニンのみの個体はHPV関連腫瘍を発症する可能性がより高いはずであることである。この仮説を試験するため、我々は、頸部癌を有する一連の患者並びに皮膚癌を有する一連の腎移植レシピエント(RTRs)をスクリーンした。頸部癌の後、皮膚癌はHPV関連腫瘍の第2の主要な群を代表する。このスクリーンの結果は、正常な対照集団に比して癌患者中のp53アルギニンのみの個体の圧倒的優勢を証明する。この結果は、p53アルギニンのみの個体が、同型接合患者またはp53プロリンのみの個体に比してHPV関連主要を発症する可能性がより高いことを証明する。
野生型p53の遺伝子型(p53proまたはp53arg)の測定方法
腫瘍生検は侵入性および皮膚偏平細胞の両方を含んだ。全ての病巣は組織学上確認されて生検切除または外科的切除の時に回収して、スナップ凍結して−70℃に保存した。7つの頸部SCCsを、ホルマリン固定してパラフィンを埋め込んだ公的材料から得た。対照群に関するサンプルは、健常ボランティアから採血した全血から抽出されたDNAからなった。
DNA抽出
凍結した組織からのDNAをプロテイナーゼK消化およびフェノールクロロホルム抽出により抽出し、パラフィン埋め込み組織からのDNAを脱パラフィン後に同じようにして抽出した。Nucleon DNA抽出キット(Scotlab)を用いて対照の個体からの全血からDNAを抽出した。
p53多形性配列のPCR増幅
p53Pro配列は、PCRによりプライマー対p53Pro+/p53−(p53Pro+:5'GCCAGAGGCTGCTCCCCC(配列番号:1)、p53−:5'CGTGCAAGTCACAGACTT(配列番号:2)により検出し、そしてp53Argはプライマー対p53+/Arg−(p53+:5'TCCCCCTTGCCGTCCCAA(配列番号:3)、Arg−:5'CTGGTGCAGGGGCCACGC(配列番号:4))を用いて検出した。プライマー対(各2ug)を混合して50uCi[α32P]ATP(アマシャム)を含む50ulの全量にてポリヌクレオチドキナーゼ(プロメガ)により[32p]で末端標識した。1ulの標識されたプライマー混合物をPCR反応あたり用いた。PCR反応(25サイクル)は、0.2ユニットのレッドホットポリメラーゼ(アドバンスドバイオテクノロジーズ)を用いて、全量50ulにて、反応バッファーIV(製造者により供給された)中にて1.5mM Mg++にて実施した。10ulの反応産物を8%ポリアクリルアミドゲル上において1XのTBEバッファー中で分画し、乾燥して、そしてX線フィルム上に露出するかあるいはストーム840ホスホルイメージャー(モリキュラーダイナミックス)およびイメージクワントソフトウエアを用いて定量した。
p53遺伝子型決定の結果
我々は、最初に一連の頸部癌生検中のp53ProおよびArg対立遺伝子の頻度を調査し、そしてこれを対照群において観察された頻度と比較した(表1)。各々の相対頻度を括弧で示す。
p53遺伝子型は、異常なPAPスメアを伴って提示される個体において測定できた。p53Arg対立遺伝子に関して同型接合である(p53Arg/p53Arg)と決定された個体は、従って、HPVに関連した頸部新形成および頸部癌を発症する危険性がより高いとして分類されるはずである。同型接合p53Argの個体は、次に、少なくともp53Pro対立遺伝子上に有する個体よりも攻撃的な様式で臨床医により治療される。
実施例II
かなりの努力が、頸部新形成および頸部癌の範囲を減じるためのHPVに対するワクチンの開発に注がれている最中である。p53Arg対立遺伝子に関して同型接合(p53Arg/p53Arg)であると決定された個体は、従って、HPV関連頸部新形成および頸部癌の発症の危険性が高いとして分類されるはずである。同型接合p53Argの個体は次に優先的に予防接種されるが、なぜならば、それらはHPV関連の病理例えば癌を発症する危険性が高いからである。
実施例III
個体は様々な理由例えば免疫抑圧性薬剤または感染因子例えばHIVのために免疫抑圧性になりうる。p53Arg対立遺伝子に関して同型接合(p53Arg/p53Arg)であると決定された免疫抑圧個体は、従って、HPV関連病理例えば癌を発症する危険性が高いとして分類されるはずである。
Claims (9)
- 野生型p53の異なる多形性形態に関連したヒトパピローマウイルス関連の悪性腫瘍を発症する危険性のあるHPV−感染個体を同定するスクリーニング方法であって、患者の生物学上のサンプル中のp53arg72野生型対立遺伝子の存在を測定する工程を含み、p53arg72/p53arg72のサンプル中の対立遺伝子パターンがヒトパピローマウイルス関連の悪性腫瘍を発症する危険因子の指標であるスクリーニング方法。
- 悪性腫瘍が、新形成、癌、およびウイルス感染からなる群から選択される、請求項1記載のスクリーニング方法。
- 悪性腫瘍が、頸部のいぼ、子宮頸部新形成または子宮頸部癌である、請求項1記載のスクリーニング方法。
- 測定工程が:
a)ポリメラーゼ鎖反応により増幅すること;
b)DNAまたは蛋白質を配列決定すること;および
c)p53arg72のDNAに特異的な少なくとも一つのプローブとハイブリダイズさせること;
の少なくとも一つからなる、請求項1記載のスクリーニング方法。 - 患者が免疫抑圧治療下にあり、そしてウイルス感染およびウイルスによる病状に対してより感受性である、請求項1記載のスクリーニング方法。
- ウイルスによる病状がヒトパピローマウイルスに関連した皮膚癌である、請求項3記載のスクリーニング方法。
- あらゆるHPV予防接種プログラムにおける有力な予防接種候補をスクリーニングする方法であって、患者からの生物学上のサンプル中のp53arg72野生型対立遺伝子の存在を測定する工程を含み、その際p53arg72/p53arg72の患者が優先的に予防接種されるべきであることからなる、スクリーニング方法。
- 患者が、異常PAPスメア、頸部のいぼ、または低級の頸部病巣を有する、請求項1ないし7の何れか1項記載のスクリーニング方法。
- 患者が新生児または生殖器のHPV感染を有する母の新生児である、請求項1ないし7の何れか1項記載のスクリーニング方法。
