Description
血管内膜炎用の拡張可能なプロテーゼまたは体内埋入物、たとえばステントが配置されるとき、患者の管腔内における目的とする位置に正確に位置決めすることが非常に重要である。従来技術のステント配置システムでは、被覆シースがその下にあるステントを露出させるために引き抜かれるとすぐに、ステントの遠位側端が急速かつ不規則に拡張して、一つ以上のステントのストラットが不規則に、たとえば後方に曲げられるように、変形する危険性がある。ステントが取り付けられている送達装置の遠位側端が管の中央にない場合に、これが発生する危険性が高まる。
米国特許第5019085号明細書は、被術者の管内の所望の位置までステントを送るステント送達装置および方法を開示する。デリバリーワイヤは、デリバリーカテーテルの開口から送られ、ステントの一部分を超えて輪状にされ、その後デリバリーカテーテルの内部に戻される。ステントの最遠位側端において、ワイヤはデリバリーカテーテルから再度抜け出て、ステントと再係合し、カテーテルの中央通路内に押し戻される。これにより、ステントを位置決めするために管を通ってデリバリーカテーテルが操作され得る形状に、ステントが圧縮される。デリバリーカテーテルからステントを解放するために、デリバリーワイヤは、その遠位側端がデリバリーカテーテルの一対の開口の両方を通り過ぎ、ステントが拡張して管の壁と係合できるように、引き込まれる。
本発明は、改良された体内埋入物解放機構を提供することを目的とする。
好ましい実施の形態では、近位側および遠位側の体内埋入物拘束位置は、ワイヤ保持要素を含む。典型的には、ワイヤ保持要素は、単数または複数の拘束ワイヤが通り得る閉じられた経路または穴を含む。有利には、遠位側拘束位置は、配置装置の先端において、解
放動作によって引き抜かれるまでワイヤの端部を保持するための、ぴったりとした態様または別の解放可能に固定された態様でワイヤの端部を収容する、穴を含む。
図1および図2を参照して、公知の送達装置の一例を示すが、これは本明細書中に教示される解放機構の原理を理解するのに有用である。以下では、送達装置10は、導入器と称される。導入器は、外科医または臨床医によって操作される外部操作部12と、患者の管腔内へ導入される遠位側端とを含む。遠位側端は、遠位側連結領域14と、近位側連結領域16とを含む。遠位側連結領域14および近位側連結領域16は、体内埋入物18の遠位側および近位側の端部に、それぞれ固着する。
その経路は、たとえば医療処置の配置段階において行なわれる血管造影を可能とするための、造影剤を供給するために好適であり得る。
位側ワイヤ解放機構46と近位側ワイヤ解放機構50とは、スライド移動可能に本体48に装着されている。
内埋入物18の近位側および遠位側端を解放するための別々の解放ワイヤ機構を含み、また解放ワイヤを操作するための特定のロック52と解放機構50とを含む。
イヤ62の端部がそれぞれの穴74を通過してその後穴72へ入るときに、典型的に起きる。このようにして、ステント60の近位側端が最初に解放されて拡張する。
Claims (18)
- 配置装置に搭載されて移送された体内埋入物を解放するための体内埋入物解放機構であって、前記体内埋入物は、近位側端と、遠位側端と、前記近位側端と前記遠位側端との間の中央部とを含み、前記体内埋入物解放機構は、近位側および遠位側の体内埋入物拘束位置を備える細長い体内埋入物支持部と、近位側および遠位側のワイヤ保持機構と、少なくとも一つの拘束ワイヤとを含み、前記少なくとも一つの拘束ワイヤは、前記近位側および遠位側の位置の両方において体内埋入物を拘束するために、前記近位側および遠位側のワイヤ保持要素によって拘束され、前記拘束ワイヤは、前記体内埋入物の前記中央部を拡張
させ、一方、近位側端および遠位側端は、前記近位側および遠位側の位置において拘束される、体内埋入物解放機構。 - 複数の拘束ワイヤを含み、その各々は体内埋入物の前記近位側端および遠位側端の両方の少なくとも一部を保持するように配置される、請求項1に記載の機構。

- 配置装置と体内埋入物とを含む組立品であって、前記配置装置は、配置装置に搭載されて移送された体内埋入物を解放するための体内埋入物解放機構を含み、前記体内埋入物は、近位側端と、遠位側端と、前記近位側端と前記遠位側端との間の中央部とを含み、前記体内埋入物解放機構は、近位側および遠位側の体内埋入物拘束位置を備える細長い体内埋入物支持部と、近位側および遠位側のワイヤ保持要素と、少なくとも一つの拘束ワイヤと、を含み、拘束ワイヤは、前記近位側および遠位側の位置の両方において前記体内埋入物を拘束するために、前記近位側および遠位側のワイヤ保持要素によって拘束され、前記拘束ワイヤは、前記体内埋入物の前記中央部を拡張させ、一方、近位側端および遠位側端は、前記近位側および遠位側の位置において拘束される、組立品。
- 前記体内埋入物は、ステントまたはステントグラフトである、請求項7に記載の組立品。
