手術での典型的な使用
オペレータは、次に、気体圧力が外科手術の間にカフ10中に維持されるように、基準圧力レベルを選ぶ。オペレータは、表示されたRTPを前記基準圧力レベルとして認めることを選択することができ、あるいは前記オペレータは、各自の判断、経験または制度上の規約に基づいた別の基準圧力レベルを手動で設定することができる。選ばれた前記基準圧力レベル近傍の圧力へのその後のカフ10の膨張は、次に、キーパッド28上の「インフレート」キーをオペレータが押下することにより、開始される。機器6の前記圧力調整器は、その時、選ばれた前記基準圧力レベルの近くにカフ10の気体圧力を維持するように作用する。前記基準圧力レベルは、機器6のオペレータにより、いつでも調整し、また新しいレベルに設定することができる。外科手術の完了時に、オペレータは、キーパッド30の前記デフレートキーを作動させてカフ10の空気抜きを開始する。次に、カフ10は、収縮後、直ちに患者の四肢4から取り外される。カフ10は、両コネクタ16および20を解除し、また両コネクタ18および22を解除することにより、機器6から切り離すことができる。

Claims (8)
- 患者の四肢閉塞圧力を迅速かつ正確に測定するためのタニケット装置であって、
一箇所で四肢を取り巻くための膨張可能なタニケットカフと、該タニケットカフに取り外し可能に接続されるタニケット器とを含み、
該タニケット器は、
前記タニケットカフ中のカフ圧力レベルを示すカフ圧力信号を生じるための圧力感知手段と、
前記タニケットカフに接続され、該タニケットカフ中の前記カフ圧力レベルを基準圧力レベルの近傍に調整するために前記カフ圧力信号に応答する圧力調節手段と、
前記四肢の前記タニケットカフより端末側の部分に適用され該部分を通して光を伝えこれにより前記四肢の前記部分の血流信号を生じる血流変換手段と、
前記四肢の部分の静脈血の容量の増加に起因する光伝達の減少を補償するために、予め決められた最小の強度を超える血流信号の強度を維持するに必要とされる透過光の強度を連続的に再計算するための補償手段と、
前記血流信号および前記カフ圧力信号に応答する四肢閉塞圧迫手段とを含み、
該四肢閉塞圧迫手段は、血流の動脈の拍動の強度を検出し、動脈の拍動に同期して前記基準圧力レベルを増大し、また動脈の拍動の強度が最小レベルより小さな最も低いカフ圧力を示す四肢閉塞圧力値を生じるように、動作可能である、タニケット装置。 - 前記四肢閉塞圧迫手段は、さらに、第1のレベルから、選択された動脈拍動時の前記カフ圧力信号によって示される前記カフ圧力レベルと増分圧力レベルとの合計に等しい第2のレベルに前記選択された動脈の拍動に同期して前記基準圧力レベルを増大させる、請求項1に記載のタニケット装置。
- 前記増分圧力レベルは、前記選択された動脈の拍動の強度と基準強度との差に比例している、請求項2に記載のタニケット装置。
- 前記四肢閉塞圧迫手段は、さらに、前記基準圧力レベルを初期基準圧力レベルに設定し、また前記動脈の拍動の強度が初期設定時間中に最小の初期強度より大きくならないときにオペレータに知覚可能な指示を与えるように、前記初期設定時間中に動作可能な手段である、請求項1に記載のタニケット装置。
- 前記四肢閉塞圧迫手段は、さらに、前記指示が前記初期設定時間中に生じると該初期設定時間後に動作不能になり、また前記タニケット器が、さらに、前記指示が前記初期設定時間中に生じると該初期設定時間後にオペレータによって前記基準圧力レベルを前記タニケット器に設定することを可能にするための手段を含む、請求項4に記載のタニケット装置。
- 前記補償手段は、さらに、不応期の間、動作可能であり、該不応期の間、前記四肢閉塞手段は動脈の拍動検出のための血流信号に反応しない、請求項1に記載のタニケット装置。
- 前記補償手段は、さらに、前記不応期の間、前記四肢の部分を通る光のレベルを増大すべく前記血流変換手段を制御するように動作可能である、請求項6に記載のタニケット装置。
- 前記タニケット器は、前記タニケットカフと前記タニケット器との間にそれぞれ第1及び第2の空気通路を形成すべく第1のカフポートおよび第2のカフポートに取り外し可能に接続され、前記圧力感知手段は、前記カフ圧力信号を生じるために前記第1の空気通路に連通し、
また、前記圧力調整手段は、前記タニケットカフ中の前記カフ圧力のレベルを基準圧力レベルの近傍に調整するために前記第2の空気通路に連通する、請求項1に記載のタニケット装置。
