Description
図1Aは、対象血管のスキャン撮影の実施に適したバルーンカテーテルシステム10の特徴を示す。図示の膨張体12は、血管壁の一部を撮影する時に光学的歪みを低減させる目的で、血管内体液を遮断するために使用される。上記したように、膨張体は典型的にはノンコンプライアントバルーンであり、対象血管の直径よりも大きい最大限度膨張径を有する。典型的なバルーンカテーテルシステム10はまた、回転光ファイバー部を内蔵する撮影プローブ14と、膨張体12を膨張/収縮させるための圧力計付き膨張/収縮装置15とを備えている。膨張体12は、典型的には、生理食塩水や放射性不透明染料で着色された生理食塩水溶液等の液体を利用して膨張する。また、撮影プローブ14の先端周辺のカテーテル材料(図示せず)は、光ファイバー部によって送受信されるOCT波長を送信し、また散乱を最小限に抑えるために、典型的に選択されるものである。
Claims (18)
- 血管壁の一部を撮影する間に、血管壁を有する血管中で血液を遮断するシステムであって:
該血管中に導入されるように適合された膨張可能要素であって、膨張状態にある該膨張可能要素が、撮影される該血管の直径より大きい直径を有し;該膨張可能要素が非膨張状態における使用のために適合され、該血管壁が該膨張可能要素によって実質的に変形されず、該膨張可能要素が該血管を実質的に遮断し、血管流体から生じる撮影歪みを低減する、膨張可能要素;
該膨張可能要素の遠位方向に延びる撮影管腔であって、該撮影管腔に沿って複数の噴出孔を規定し、該複数の噴出孔が該膨張可能要素に対して配向されて洗浄溶液を方向付け、該洗浄溶液が該血管を通常の血液流れの方向とは逆の方向に、該流体が該膨張可能要素と接触し、そして該血管中に乱流域を形成し、光学コヒーレンス断層撮影スキャンが実施され得る血管壁の長さを十分に清澄にするように洗浄する、撮影管腔;および
光ファイバーが中に配置された光学断層撮影プローブであって、該プローブ中に配置された光ファイバーを回転する間、該プローブが後退され得るように、該プローブが該撮影管腔内の配置され、そして該血管の清澄にされた長さを通って該膨張可能要素に対して位置決めされる、光学断層撮影プローブ、を備える、システム。 - 前記膨張可能要素が、拡大可能部材を含む、請求項1に記載のシステム。
- 前記拡大可能部材の表面の一部分が、前記血管壁と接触するとき襞を備える、請求項1に記載のシステム。
- 前記膨張可能要素がバルーンを含む、請求項1に記載のシステム。
- 光学コヒーレンス断層撮影システムであって:
血管内流体を実質的に遮断することにより撮影品質を改善するための膨張可能カテーテルおよび撮影システム;
回転可能光ファイバーを備える光学断層撮影プローブ;
以下を備える膨張可能カテーテル、
血管接触表面を有するバルーン部分であって、対象血管よりも大きく、そして非膨張状態における使用のために適合されたバルーン部分;および
該バルーン部分の遠位方向に延びる撮影管腔であって、該撮影管腔に沿って複数の噴出孔を規定し、該複数の噴出孔が該バルーン部分に対して配向されて洗浄溶液を方向付け、該洗浄溶液が該バルーン部分に、乱流域が、光学コヒーレンス断層撮影スキャンが実施され得るように血管から血液を形成かつ除去するように衝突し、該光学断層撮影プローブが、該プローブが該血管に沿って退却され、該回転可能光ファイバーを用いる光学コヒーレンス断層撮影スキャンを実施し得るように該撮影管腔内に配置され、かつ該バルーン部分に対して位置決めされる撮影管腔、を備える、光学コヒーレンス断層撮影システム。 - 前記バルーン部分が、約2mm〜約4mmの範囲の直径を有する、請求項5に記載の光学コヒーレンス断層撮影システム。
- 前記バルーン部分の膨張圧力が、約150mbar〜約750mbarの範囲である、請求項5に記載の光学コヒーレンス断層撮影システム。
- 前記血管接触表面が、非膨張状態にあるときに襞を備える、請求項5に記載の光学コヒーレンス断層撮影システム。
- 前記膨張可能カテーテルが、前記血管接触表面に付与された疎水性コーティングを備える、請求項5に記載の光学コヒーレンス断層撮影システム。
- バルーンカテーテルシステムであって:
膨張管腔に連結されるバルーン、
回転可能光ファイバーを備える光学断層撮影プローブ、
該バルーンの遠位方向に延びる、洗浄および撮影に併用される管腔、
該洗浄および撮影に併用される管腔に沿った複数の噴出孔であって、洗浄溶液が該バルーンと接触し、そして該バルーンから離れて方向付けられて乱流域を形成し、そして光学コヒーレンス断層撮影法スキャンが実施され得るに十分、血管から血液を除去するように該バルーンに対して配向されて該洗浄溶液を方向付ける複数の噴出孔、を備え、該光学単層撮影法撮影プローブが、該プローブが該血管に沿って退却され、該回転可能光ファイバーを用いる光学コヒーレンス断層撮影法スキャンが実施され得るように、該洗浄および撮影に併用される管腔内の配置され、そして該バルーンに対して位置決めされる、バルーンカテーテルシステム。 - 前記複数の噴出孔が、通常の血液流れに逆流する角度で洗浄流れを方向付ける、請求項10に記載のバルーンカテーテルシステム。
- 前記バルーンの遠位方向にある前記洗浄および撮影に併用される管腔の一部分が、血管に対して非外傷性であるように適合される、請求項10に記載のバルーンカテーテルシステム。
- 前記噴出孔の少なくともいくつかが、噴出された洗浄溶液が通常の血液流れに対して十分逆流するように配向される、請求項10に記載のバルーンカテーテルシステム。
- 前記洗浄および撮影に併用される管腔が、該管腔の長手方向および周縁方向の両方に配列されるいくつかの噴出孔を有する、請求項10に記載のバルーンカテーテルシステム。
- 前記バルーンが、前記洗浄および撮影に併用される管腔が前記バルーンの近位方向に血管部分で少なくとも光学的に実質的に透明に維持するよう適合されている、請求項10に記載のバルーンカテーテルシステム。
- 前記洗浄および撮影に併用される管腔に沿った前記噴出孔が、膨張された前記バルーンの壁に対して洗浄溶液を方向付け、該洗浄溶液の乱流をさらに増加し、残存動脈血液との混合および残存動脈血液の除去を改善する、請求項10に記載のバルーンカテーテルシステム。
- 前記洗浄および撮影に併用される管腔が、前記回転可能光ファイバーのための安定化する支持を提供する、請求項10に記載のバルーンカテーテルシステム。
- OCTチャネルが、前記バルーン、カテーテルの少なくとも1つ、もしくは両方に含まれる、請求項10に記載のバルーンカテーテルシステム。
