Description
本発明は、脆弱な生理活性物質、例えば、幹細胞または筋芽細胞に対する外傷および/または宿主組織に付随的に生じる損害を軽減する受動的または制御された注入配置技術を用いて、上記のような生理活性物質を標的組織内へと送達する送達システムおよび方法に関する。
循環器疾患は、現在先進工業国における主な死亡原因である。疾患過程では、動脈硬化性プラークが血管内の種々の位置にて発達して、罹患血管を通る血流が制限される。このようなプラークが、心臓の筋肉およびその他の組織に栄養を供給する血管内で発達すると、心臓組織への血流が減少し心筋梗塞および虚血を起こし得る。
大動脈または内乳動脈茎から狭窄した冠状動脈まで導管を作ることによって、バイパスされる。このような処置は一般的に、著しい痛み、長期間のリハビリ、および高い罹患率を伴い、多大な時間を必要とし、実施費がかさむ。
前述を鑑み、本発明の目的は、送達の際の生理活性物質または標的組織に気圧性外傷が生じる危険を少なくして生理活性物質を送達するための方法および装置を提供することである。
心室の特定領域にカテーテルを適合させるための可撓カテーテル内へと選択的に注入されるように構成される、比較的可撓なカテーテルおよび複数の予成形されたスタイレットを備え得る。
本発明は、針跡を形成した後に、標的組織塊内に形成された針跡内に生理活性物質を非侵襲的に沈着させることにより、生理活性物質(特に、脆弱な生理活性物質)を送達するための方法および装置に関する。
Claims (36)
- 組織塊内に生理活性物質を堆積させる装置であって、
近位端と遠位領域とそれらの間に延在する管腔とを有するカテーテルと、
近位端と組織貫通遠位端とそれらの間に延在する管腔とを有し、該管腔を通って組織塊内へと生理活性物質を非侵襲的に送達するよう構成される、該カテーテルの該管腔内に配置された細長部材と、
針跡を形成するため該組織塊内へと該細長部材の該組織貫通遠位端を選択的に伸長および引き込みをするアクチュエータと、を備え、
該生理活性物質が、該細長部材の該組織貫通遠位端が引き込まれる間に、該針跡へと非侵襲的に送達され、そして、
該装置が、該細長部材が引き込まれるときに、該組織塊に対して該生物活性物質を固定するように保持することによって、該生物活性物質を該針跡内に送達するように構成されている、該組織塊内に該生理活性物質を堆積させる装置。 - 或る量の生理活性物質を収容する容器をさらに備え、該容器が、前記細長部材の前記管腔と、該容器内の陰圧を軽減する一方向バルブとに連結される、請求項1に記載の装置。

該装置が、該細長部材が引き込まれるときに、該組織塊に対して該生物活性物質を固定するように保持することによって、該生物活性物質を該針跡内に送達するように構成されている、該中空器官の該組織塊内に生理活性物質を堆積させる装置。 - 針跡を形成するため前記組織塊内へと前記細長部材の前記組織貫通遠位端を選択的に伸長および引き込みをするアクチュエータをさらに備える、請求項15に記載の装置。

- 前記少なくとも1つの部材が、展開された状態のらせん構造に移行する部分をさらに備える、請求項21に記載の装置。
- 前記ガイドシステムが、収縮された送達用状態および展開された状態を有する複数のレールをさらに備え、該複数のレールが、前記中空器官の前記内面と接触する、請求項21に記載の装置。
- 前記少なくとも1つの部材が、複数のレールを備え、前記ガイド部材が、前記複数のレールのうち所望のいずれか1つに沿って平行移動されるよう構成される、請求項21に記載の装置。
- 前記アクチュエータが、近位方向および遠位方向に前記細長部材を平行移動させるレバーを備える、請求項16に記載の装置。
- 前記アクチュエータが、前記細長部材の伸長および引き込みを起こすよう回転されるネジ部を備える、請求項16に記載の装置。
- 前記カテーテルが、遠位端を有し、前記カテーテルの前記管腔が、前記近位端と前記遠位端との間に延在し、前記細長部材の前記組織貫通遠位端が、該カテーテルの該遠位端を越えて選択的に伸長されるよう構成される、請求項15に記載の装置。
- 組織塊内へと生理活性物質を堆積させるためのシステムであって、
近位端と遠位領域とそれらの間に延在する管腔とを有するカテーテルであって、該カテーテルの該遠位領域が組織塊に隣接して配置されるように構成された、カテーテル;
該カテーテルの該管腔内に配置された細長部材であって、近位端と組織貫通遠位端とそれらの間に延在する管腔とを有する細長部材;
該細長部材の該組織貫通遠位端を選択的に伸長および引き込みをするアクチュエータ;ならびに
針跡を形成するため該組織塊内へと該細長部材を伸長させるよう該アクチュエータを操作するための手段;
を備え、
該生理活性物質が、該針跡内に該管腔を通して非侵襲的に送達され、そして、
該システムが、該細長部材が引き込まれるときに、該組織塊に対して該生物活性物質を固定するように保持することによって、該生物活性物質を該針跡内に送達するように構成されている、システム。 - 容器から前記管腔へと前記生理活性物質を提供するための手段、および該容器内からの陰圧を軽減するための手段をさらに含む、請求項29に記載のシステム。
- 前記細長部材の前記組織貫通遠位端が、前記生理活性物質を受け取るための該遠位端に隣接する空間を生み出す手段、を備えることにより、該生理活性物質が該空間内へと送達される、請求項29に記載のシステム。
- 前記カテーテルが、中空器官の内面に対して該カテーテルを推進させるよう構成される少なくとも1つの部材を備えるガイドシステム、および該カテーテルの遠位領域に取り付けられたガイド部材、をさらに備え、該ガイド部材は、前記組織塊に隣接するよう該カテーテルの該遠位領域を配置させるよう、該少なくとも1つの部材に沿って平行移動するように構成されている、請求項29に記載のシステム。
- 組織塊内に生理活性物質を堆積させる装置であって、
近位端と遠位領域とそれらの間に延在する管腔とを有するカテーテルと、
近位端と組織貫通遠位端とそれらの間に延在する管腔とを有する、該カテーテルの該管腔内に配置された細長部材と、
針跡を形成するため組織塊内へと該細長部材の該組織貫通遠位端を選択的に伸長および引き込みをするアクチュエータと、を備え、
該装置が、該細長部材の該組織貫通遠位端が引き込まれるときに、該生物活性物質を、該針跡内に送達するように構成されており、そして、
該装置が、該細長部材が引き込まれるときに、該組織塊に対して該生物活性物質を固定するように保持することによって、該生物活性物質を該針跡内に送達するように構成されている、装置。 - 前記細長部材の前記組織貫通遠位端が、前記生理活性物質を受け取るための該遠位端に隣接する空間を生み出す手段、および該生物活性物質を該空間内に送達するための手段を備える、請求項33に記載の装置。
- 前記遠位端に隣接する空間を生み出す前記手段が、ワイヤーを備え、該ワイヤーが、該遠位端を越えて配置され、該空間を形成するよう拡張するように構成されている、請求項34に記載の装置。
- 前記カテーテルが、中空器官の内面に対して該カテーテルを推進させるよう構成される少なくとも1つの部材を備えるガイドシステムと、該カテーテルの遠位領域に取り付けられたガイド部材とをさらに備える、請求項33に記載の装置。
