Description
本発明は、全体的に、切除を実施するためのシステムおよび方法に関する。より具体的には、本発明は、例えば、以前に形成された組織腔において、バルーンを使用して切除を実施するためのシステムおよび方法に関する。
装置は、カテーテル、伝導性要素およびバルーンを備える。このカテーテルは管腔を有する。伝導性要素は、カテーテルに沿って配置される。バルーンは、そのカテーテルの管腔と流体連絡した内部を有する。このバルーンは、その伝導性要素と電導性に繋がれた伝導性物質から形成される。バルーンは、折り畳まれた構成および膨張した構成を有する。
一旦腫瘍が除去されたら、組織腔が残存する。この腔を囲む組織は、患者において、腫瘍の再発が最も高い可能性で生じ得る位置である。従って、腫瘍が除去された後、周囲の組織(本明細書中で、「マージン組織」とも言われる)を破壊することが望ましい。本明細書中に記載される種々の実施形態は、バルーンカテーテルデバイスに関し、そして例えば、腫瘍除去により形成される組織腔と関係するマージン組織を切除するための方法に関する。
本発明の種々の実施形態が上記に説明されているが、これらは例示のみの目的のために提示されており、そして限定のためではないことが理解されるべきである。従って、本発明の広がりおよび範囲は、上記の実施形態にいずれによっても限定されるべきでなく、添付の特許請求の範囲およびそれらの等価物に従ってのみ規定されるべきである。
Claims (19)
- 以下;
管腔を有するカテーテル;
該カテーテルに沿って配置される伝導性要素;および
該カテーテルの該管腔と流体連絡した内部を有するバルーンであって、該バルーンは伝導性要素と伝導性に繋がれた伝導性物質から形成される第一層を有し、折り畳まれた構成と膨張された構成とを有する、バルーンを備え、該バルーンがさらに第二層および第三層を備え、該バルーンの第二層が絶縁体物質から形成され、該バルーンの該第三層が第二の伝導性物質から形成され、該バルーンの第一層および第三層が二極性の電極構成を備える、装置。 - 前記バルーンの前記伝導性物質が複数の伝導性部分を備え、該複数の伝導性部分由来の2つの隣り合う伝導性部分が、複数の絶縁体部分由来の1つの絶縁体部分によって分離される、請求項1に記載の装置。
- 請求項1に記載の装置であって、前記バルーンの第一層が複数の伝導性部分を備え、該バルーンの第一層のうちの複数の伝導性部分由来の2つの隣り合う伝導性部分が、複数の絶縁体部分由来の1つの絶縁体部分によって分離され、
該バルーンの第三層が複数の伝導性部分を備え、該バルーンの第三層のうちの複数の伝導性部分由来の2つの隣り合う伝導性部分が、複数の絶縁体部分由来の1つの絶縁体部分によって分離され、
該バルーンの第一層のうちの複数の伝導性部分由来の各々の伝導性部分が、該バルーンの第三層のうちの複数の伝導性部分由来の対応する伝導性部分からオフセットされる、装置。 - 請求項1に記載の装置であって、前記カテーテルの前記管腔が第一管腔であり、ここで:
該カテーテルが第一末端部分、第二末端部分および第二管腔を有し、該カテーテルの該第一末端部分が前記バルーン内に配置され、該カテーテルの第一末端部分が、該第一管腔と繋げられた入り口および該第二管腔と繋げられた出口を有し、
流体レギュレーターが該カテーテルの該第二末端部分と連結され、該流体レギュレーターが、該バルーンの伝導性物質の温度より低い該バルーン内温度で流体を循環させるように構成される、装置。 - 請求項1に記載の装置であって、前記バルーンが第一バルーンであり、前記カテーテルの前記管腔が第一管腔であり、以下:
該第一バルーンの外側に配置された第二バルーンであって、該第二バルーンは流体透過性である、第二バルーン、
該第二バルーンと流体連絡した第二管腔を備える、該カテーテル
をさらに備える、装置。 - 請求項1に記載の装置であって、前記バルーンが第一バルーンであり、前記カテーテルの前記管腔が第一管腔であり、以下:
該第一バルーンの外側に配置される第二バルーンであって、該第二バルーンが流体透過可能である、第二バルーン
該第二バルーンと流体連絡した第二管腔を備える該カテーテル;および
該カテーテルの該第二管腔と繋がれた流体調節器であって、該流体調節器が該第二バルーンと関係したインピーダンスに基づき、該第二バルーンから灌流する流体の速度を制御するよう構成される、流体調節器
をさらに備える、装置。 - 前記バルーンの遠位末端に配置される、非外傷性先端をさらに備える、請求項1に記載の装置。
- 前記カテーテルの管腔および前記バルーンの内部の中に配置される、ガイドワイヤをさらに備える、請求項1に記載の装置。
- 請求項1に記載の装置であって、以下:
前記カテーテルが第一非伝導性層、第二非伝導性層および伝導性層を有し、該伝導性層が該第一非伝導性層と該第二非伝導性層との間に配置され、該伝導性層が前記バルーンの伝導性物質と電気的に繋がれる、カテーテル
をさらに備える、装置。 - バルーンを有するカテーテルを操作するためのシステムであって、該バルーンは該カテーテルと流体連絡状態にあり、該システムは、
折り畳まれた状態で、以前に形成された組織腔の中に経皮的に配置されるように構成されているバルーンであって、該バルーンが伝導性物質から形成されている第一層を有し、該バルーンが折り畳まれた構成と膨張された構成とを有し、該バルーンがさらに第二層および第三層を備え、該バルーンの第二層が絶縁体物質から形成され、該バルーンの該第三層が第二の伝導性物質から形成され、該バルーンの第一層および第三層が二極性の電極構成を備える、バルーン;
該膨張された構成に該バルーンを膨張させるための手段であって、該膨張された構成における該バルーンが以前に形成された組織腔の形状と関係した形状を有する、手段;および
該バルーンの伝導性部分に対して高周波シグナルを適用するための手段、
を備える、システム。 - 請求項10に記載のシステムであって、以下:
前記バルーン内に流体を循環させるための手段であって、該バルーン内の該流体が、該バルーンの前記伝導性物質の温度より低い温度を有する、手段
をさらに備える、システム。 - 請求項10に記載のシステムであって、前記バルーンが第一バルーンであり、前記カテーテルが該第一バルーンの外側に配置されそして流体透過可能である第二バルーンを備え、以下:
第二バルーン内に流体を提供するための手段
をさらに備える、システム。 - 請求項10に記載のシステムであって、前記バルーンが第一バルーンであり、前記カテーテルが、該第一バルーンの外側に配置されかつ流体透過可能である第二バルーンを備え、以下:
前記組織腔と関係したインピーダンスに基づき、該第二バルーン内に流体を提供するための手段
をさらに備える、システム。 - 請求項10に記載のシステムであって、前記バルーンの膨張が、前記以前に形成された組織腔の形状を、実質的に球面の形状に変更させるように、該バルーンが構成されており、該システムが、
該変更された組織腔から該バルーンを取り除くための手段;および
該変更された組織腔の中に挿入されるように構成された放射線治療デバイスであって、該放射線治療デバイスに基づき放射線治療が実施され得るようになっている、放射線治療デバイス
をさらに備えるシステム。 - 組織塊の除去後の組織腔と関係したマージン組織を処置するための装置であって、以下:
管腔を規定する管状部材;および
バルーンであって、少なくとも1つの電極を有し、そして該管状部材の管腔と流体連絡した内部を規定し、該バルーンが伝導性物質から形成される第一層を有し、該バルーンが該組織腔に対応する膨張した構成および折り畳まれた構成を含む構成の範囲を有し、そして該バルーンがさらに第二層および第三層を備え、該バルーンの第二層が絶縁体物質から形成され、該バルーンの該第三層が第二の伝導性物質から形成され、該バルーンの第一層および第三層が二極性の電極構成を備える、バルーン
を備える、装置。 - 前記バルーンの前記少なくとも1つの電極が該バルーンと共に形成される、請求項15に記載の装置。
- 請求項15に記載の装置であって、ここで:
前記バルーンの前記少なくとも1つの電極が複数の伝導性部分を備え、該複数の伝導性部分由来の2つの隣り合う伝導性部分が、複数の絶縁体部分由来の1つの絶縁体部分によって分離される、装置。 - 請求項15に記載の装置であって、ここで:
前記少なくとも1つの電極がバルーンの第一層の中に配置され、該バルーンの該第三層が少なくとも1つの電極から形成される、装置。 - 請求項18に記載の装置であって:
前記バルーンの前記第一層の少なくとも1つの電極、および該バルーンの前記第三層の少なくとも1つの電極に繋がれた高周波発生装置をさらに備える、装置。
