Description
本発明は、一般的には、特定の薬剤に対する感受性の低下、および/または免疫学的試薬との相互作用の減少を示すウイルス変種に関する。より詳細には、本発明は、ヌクレオシドアナログに対する完全または部分的な耐性、および/または抗体への感受性の低下を含むウイルス表面の成分に対するこれらの抗体との相互作用の減少を示すB型肝炎ウイルス(HBV)変種に関する。本発明はさらに、このようなウイルス変種を検出するためのアッセイ法を意図し、これらのアッセイ法は、抗ウイルス治療養生法をモニタリングすること、ならびにウイルス薬剤および特にHBV変種を指向する新規なまたは修飾したワクチンを開発することにおいて有用である。本発明はまた、ウイルスの感染、複製、および/または放出を阻害することができる薬剤についてスクリーニングするためのウイルス変種の使用を意図する。

本発明は、ETVもしくはLMV、またはETVおよびLMV、ならびに任意で他のヌクレオシドアナログを用いた患者の治療後に、HBVのヌクレオシドアナログ耐性変種を同定および単離することに部分的に基づく。特に、ETV、またはETVおよびLMVで治療した患者は、ETVおよび/またはLMVに対する感受性の低下または減少を示すHBVの変種を生じた。ETVおよび/またはLMVに対する感受性に関連した「低下した」または「減少した」と言うことへの本明細書中における言及は、ヌクレオシドアナログに対する完全または実質的な耐性ならびに部分的な耐性を含み、かつ包含し、かつ、ヌクレオシドアナログの存在下における野生型より高い複製速度または複製効率(収量表現型)を含む。1つの局面において、これは、処理前レベルと同様かまたはより高いレベルにまでウイルス負荷を増加することによって便利に測定される。

感染性HBVゲノムが安定して上記連続した細胞株(例えば、2.2.15またはADであるがこれらに限定されない)に安定して組み込まれるように、複製可能な有効量でHBVのゲノムの感染性コピーを含む連続した細胞株を生成する工程;

上記薬剤に耐性である場合、HBVが遺伝子配列を複製、発現し、かつ/またはウイルスもしくはウイルス様粒子を組み立て、かつ/もしくは放出するのに十分な時間および条件下で、上記細胞を培養する工程;
細胞、細胞溶解物、または培養上清液を、ウイルスまたはウイルス成分検出手段に供して、該薬剤の存在下で、ウイルスが遺伝子物質を複製、発現したか否か、および/またはウイルスが組み立てられ、かつ/もしくは放出されたか否かを決定する工程。
(1)入力Ivsとして、ウイルス薬剤またはウイルス薬剤を含む生物学的試料と関連する少なくとも2つの特徴を受け取るコードであって、該特徴が以下から選択されるコード:
(a)特定の化合物または免疫学的薬剤に対する感受性の低下に対して耐性を示す能力;
(b)野生型HBV由来の変化したDNAポリメラーゼ;
(c)野生型HBV由来の変化した表面抗原;あるいは
(d)患者の罹患率または回復可能性;
(e)複製能力の変化(増加または減少);
(2)上記Ivsを付加し、上記ウイルス変種または生物学的試料に関するPvに対応する合計値を提供するコード;ならびに
(3)コードを保存するコンピュータ読み取り可能な媒体。
(1)機械読み取り可能なデータでコードされたデータ保存材料を含む機械読み取り可能なデータ保存媒体であって、上記機械読み取り可能なデータが、上記ウイルス変種または生物学的試料と関連する少なくとも2つの特徴についてのIvsを含み;上記特徴が以下から選択される、データ保存媒体:
(a)特定の化合物または免疫学的薬剤に対する感受性の低下に対して耐性を示す能力;
(b)野生型HBV由来の変化したDNAポリメラーゼ;
(c)野生型HBV由来の変化した表面抗原;または
(d)患者の罹患率または回復可能性;
(e)複製能力の変化(増加または減少);
(2)上記機械読み取り可能なデータを処理するための命令を保存するための作業メモリ;
(3)上記機械読み取り可能なデータを処理して、上記化合物に関するPvに対応する上記Ivsの合計値を提供するために、上記作業メモリおよび該機械読み取り可能なデータ保存媒体に連結された中央演算処理装置;ならびに
(4)上記Pvを受け取るために、上記中央演算処理装置に連結された出力ハードウェア。
HBVの重複するゲノム
HBVの重複するゲノムを図1に表す。DNAポリメラーゼをコードする遺伝子(P)は、ウイルスエンベロープ遺伝子Pre-S1およびPre-S2と重複し、そしてX遺伝子およびコア遺伝子(C)と部分的に重複する。HBVエンベロープは、小、中、および大HBV表面抗原を含む。大タンパク質成分は、HBV表面抗原(HBsAg)といわれ、S遺伝子配列によって取り囲まれている。Pre-S1遺伝子配列およびPre-S2遺伝子配列は他のエンベロープ成分をコードしている。
患者および治療
患者Aは慢性B型肝炎を有する44歳の男性であり、血清HBV DNAレベルの上昇(>2000pg/ml)を0日目(1999年7月9日)に呈し、すぐにLMV治療を開始した(図2)。患者AはHBsAg陽性であり、かつ抗HBe陽性であった。LMV治療開始後、HBV DNAレベルは治療の54日間にわたって8pg/mlまで落ちた。HBV DNAレベルは199日目まで低いままであり、ここで複製の再発が存在し、その結果HBV DNAレベルが1826pg/mlに達した。241日目までに血清ALTは741IU/lで最高に達した。HBV DNAを配列決定し、LMV耐性ウイルスを検出した。次いで、患者を、382日目(2000年7月24日)に盲検化ETVおよびLMV臨床試験に登録した。HBV DNAレベルは33pg/mlまで減少したのみであり、ALTは784日目までに167IU/Lまで減少した。この患者は、非盲検化(open label)ETVおよびLMVを開始した。しかし、HBV DNAレベルおよびALTの両方は上昇し続け、そしてHBV DNAを894日目で配列決定した(図2)。
ウイルスマーカーの検出
B型肝炎表面抗原(HBsAg)、B型肝炎e抗原(HBeAg)、抗HBeおよびB型肝炎コア抗原(HBcAg)特異的IgGおよびIgMを市販の免疫アッセイ法(Abbott Laboratories, North Chicago, IL, USA)を使用して測定した。B型肝炎ウイルスDNAレベルを、製造業者の指示書(Digene Hybrid Capture II, Digene Diagnostics Inc., Beltsville, MD)に従って捕捉ハイブリダイゼーションアッセイ法を使用して測定した。製造業者は、臨床試料においてHBVウィルス血症を検出するためのカットオフは、0.7×106コピー/mlまたは2.5pg/mlであると述べた(Hendricks DAら、Am J. Clin Pathol 104: 537-46, 1995)。
HBV DNAの配列決定
HBV DNAを、Ayeら、J. Hepatol. 26: 1148-53, 1997によって以前に記載されたように6つの異なる時点(図2)で収集した100μlの血清から抽出した。オリゴヌクレオチドは、Geneworks, Adelaide, Australiaによって合成された。HBVポリメラーゼ遺伝子の増幅は、Ayeら、1997, 前出によって記載された。

HBV DNAの分析
患者A:rtL180MおよびrtM204VでのLMV耐性変異を、199日目までに配列決定することにより決定した(表3)。エンテカビルおよびLMV治療の盲検化段階の間、rtV173Lでの変異もまた検出した。rtI169TでのBドメインにおける固有の変異を、rtL180MおよびrtV173Lでの2つの他のBドメイン変異、ならびにCドメインにおけるrtM204Vでの変異とともに検出した。多数の固有の変化もまた、ポリメラーゼおよび重複するエンベロープ遺伝子において検出した(表4、図4、5、および6)。これらの固有の変化を、固有の変化を決定するために、7つの遺伝子型A-Gの各々からの参照配列、ならびに処理前の試料からのコンセンサス配列と比較した。
エンテカビル耐性のインビトロ分析
エンテカビルに対する耐性HBV変異体の感受性/耐性プロフィールを、組換えHBV/バキュロウイルスを使用してインビトロで試験した。耐性プロフィールを分析するための手順は、以下の実施例7〜14において概説される。
細胞培養
Sf21昆虫細胞を、10%(v/v)熱非働化胎仔ウシ血清(Gibco BRL, Gaithersburg, MD)でさらに補足した補足Grace昆虫培地中で、CO2を含む28℃の加湿インキュベーター内で維持した。HepG2細胞を、10% v/v熱非働化胎仔ウシ血清で補足した最小必須培地(MEM-FBS)中で維持した。HepG2細胞を、加湿した37℃のインキュベーター内で、5% v/v CO2で増殖させた。
特異的点変異を有するHBV/バキュロウイルス移入ベクターの調製
抗ウイルス試験のために使用される組換えHBV/バキュロウイルス系は以前に記載されている(Delaneyら、Antimicrob Agents Chemother 45(6): 1705-1013,2001)。手短に述べると、組換え移入ベクターを、1.3×HBVゲノム構築物を含む断片を切除し、かつそれをバキュロウイルスベクターpBlueBac4.5(Invitrogen, Carlsbad, CA)の多重クローニング領域にクローニングすることによって作製した。点変異を、製造業者の説明書に従い、市販のキットを使用して(QuikChange, Stratagne)、部位特異的変異誘発によって作製した。HBV組換え体は、逆転写酵素変異rtI169T+rtV173L+rtL180M+rtM204Vをコードする。このプラスミドのヌクレオチド配列および部位特異的変異誘発によって生成された点変異を、製造業者の説明書に従って、ABI Prism Big Dye Terminator Cycle Sequencing Ready Reaction Kit(Perkin Elmer, Cetus Norwalk, CT)を使用する配列決定によって確認した。
1.3HBV構築物を含む組換えバキュロウイルスの生成
精製した組換え移入ベクターおよび直鎖状AcMNPVバキュロウイルスDNAを、Invitrogen(Carlsbad, CA)からのBacNBlueトランスフェクションキットを使用して、SF21細胞に同時にトランスフェクトし;組換えウイルスを、製造業者の説明書に従って、プラークアッセイ法によって単離した。SF21細胞の100mmディッシュを感染させることによって、一連の組換えウイルスを単離されたプラークから増幅した。ウイルスDNAを、標準的な手順を使用して、増幅したウイルスから抽出した。精製したウイルスDNAを、制限酵素を用いて消化し、次いで、1% v/v アガロースゲル中の電気泳動によって分画した。どのウイルス単離物が完全な1.3HBV構築物を含んでいるかを決定するために、サザンブロッティングを実行した。Boehringer Mannheim Random Prime DNA Labeling Kit(Indianapolis, IN)を、[P32]-放射性標識プローブを生成するために使用した。全長二本鎖HBVゲノムを、すべての放射性標識プローブの鋳型として使用した。ウイルスDNA配列を、センスプライマー
(HBV Genebankアクセッション番号M38454によるヌクレオチド1408位〜1430位)、およびアンチセンスプライマー
(HBV Genebankアクセッション番号M38454によるヌクレオチド2817位〜2798位)を使用して、ポリメラーゼ触媒領域のPCR増幅によって確認した。以下のプライマーを、内部領域の配列決定のために利用した:
(HBV Genebankアクセッション番号M38454によるヌクレオチド2345位〜2362位)および
(HBV Genebankアクセッション番号M38454によるヌクレオチド1790位〜1810位)。
調製用バキュロウイルス増幅および精製
バキュロウイルスを、0.5pfu/細胞の感染多重度(moi)で、対数期のSf21細胞の懸濁培養を感染させることによって増幅した。感染を、フラスコ中の細胞の大多数が目に見える感染の徴候を示すまで進行させた(4〜5日間)。ビリオンを、感染したSF21培地から、80,000×gの遠心分離、および20-60%(w/v)ショ糖勾配を通しての精製によって濃縮した。精製したウイルスを、終点希釈によってSf21細胞中で4つ組で力価測定した。ポリメラーゼ遺伝子を増幅および配列決定し、実施例9のように部位特異的変異誘発の存在を確認した。
組換えHBV発現バキュロウイルスを用いるHepG2細胞の感染
HepG2を、約20〜40%コンフルエントで播種し、次いで、感染前に16〜24時間増殖させた。感染の日に、3つ組の細胞のプレートをトリプシン処理し、生細胞数を、トリパンブルー排除法により血球計算器で測定した。平均の細胞数を計算し、それを用いて示されたmoiで細胞を感染するのに必要な高力価のウイルスストックの量を決定した。HepG2を無血清MEMで1回洗浄し、微量の血清を取り除いた。バキュロウイルスを、血清を含まないMEMで希釈し、100mm、60mm、および35mmのディッシュを感染するためにそれぞれ1.0、0.5、および0.25mlの量を使用して適切なmoiを達成した。バキュロウイルスを、HepG2細胞に1時間、37℃で、接種物が均等に分布するように15分間毎に穏やかに振動させながら吸着させた。次いで、接種物を吸引し、HepG2細胞をリン酸緩衝生理食塩水で2回洗浄し、種々の濃度の薬剤を有するかまたは有しないMEM-FBSを加えた。
分泌されたHBV抗原の分析
B型肝炎e抗原(HBeAg)の検出を、Abbott Laboratories(Abbott Park, IL, USA)から購入したキットを使用して、放射免疫アッセイ法および微粒子酵素免疫アッセイ法によって実行した。HepG2細胞からの培地を収集し、6,000×gで遠心分離して細胞細片を除去し、清浄なチューブに移し、そして20℃で分析まで保存した。HBeAg値を、陽性対照の倍数として表現する。培地試料を、ラジオ免疫アッセイ法の結果がHBeAgの陽性対照の値より下であるように適切に希釈した。
細胞内複製中間体の検出
HBVコア粒子を、0.5% w/v NP-40中に溶解したHepG2細胞の細胞質画分から単離した。細胞質抽出物を、10mmol/l McC12に調整し、そして保護されていないDNAを、500g/ml プロテイナーゼKとの1.5時間、37℃でのインキュベーションによって除去した。次いで、試料中のHBV DNAを、市販のDNA抽出キット(例えば、Qiagen(DNA extraction))を使用して、または、連続的なフェノールおよびクロロホルム抽出を使用する研究室内の方法を使用して抽出し、核酸をエタノール沈殿によって回収した。核酸を50μl/l TE(10mmol/l Tris, 1mmol/l エチレンジアミン四酢酸)中に再懸濁し、OD260によって標準化し、そして100g/ml RNase(Boehringer Mannheim, Indianapolis, IN)を用いて、1時間、37℃で消化し、その後、リアルタイムPCR、または電気泳動およびサザンブロッティングによる分析を行った。サザンブロット分析後、BioRad GS-670イメージングデンシトメーターおよびMolecular Analystソフトウェア(BioRad, Hecules California)を使用して、サザンブロットの適切な露出を分析した。デンシトメトリーデータを、Jandel ScientificからのTableCurve 2Dソフトウェアパッケージを使用して、ロジスティック用量応答曲線に適合させた。ロジスティック用量応答式を使用して、IC50およびIC90値ならびに変動係数を計算した。
リアルタイムPCR
HBVについてのリアルタイムPCRに基づくアッセイ法のために、HBV DNAを200μlの血清から、製造業者の指示書に従って、QIAamp DNA Mini Kit(QIAGEN GmbH, Hildens, Germany)を使用して抽出した。216ヌクレオチド産物を増幅および検出するために、プライマーおよび分子ビーコンをHBVゲノムのプレコアドメイン内の保存核酸配列に対して設計した(図1)。増幅を、1.0 TaqmanバッファーA(Applied Biosystems, Foster City, CA)、3.0mM MgCl、0.4pmol/μLの各プライマー、正方向プライマー
、および逆方向プライマー
、0.4pmol/μLのHBV特異的分子ビーコン
(ここで、FAMは6-カルボキシフルオレセイン(蛍光体)、およびDABCYLは4-ジメチルアミノフェニルアゾ安息香酸(消光発色団)である)、ならびに1.25UのAmpli Taq Gold DNAポリメラーゼ(Perkin-Elmer)を含む50μl反応混液中で実行した。PCRを、ABI PRISM 7700 分光蛍光分析サーモサイクラー(Applied Biosystems)を使用して実行した。PCRプログラムは、最初のサイクル(95℃、10分間)、続いて45回の増幅サイクル(94℃15秒間、50℃30秒間、72℃30秒間)からなった。機器は、アニーリング段階の間の各反応チューブの蛍光スペクトルを検出および記録した。
ETV治療
ETVを、薬物の凍結融解の反復を避けるために、滅菌水に再懸濁し、アリコートに分け、そして-20℃で凍結させた。ETVを含む培地を、必要に応じて、3TCの新鮮なアリコートを使用して毎日調製した。ETV治療がウイルス感染後に開始される実験において、HepG2細胞を、HBVバキュロウイルスでの感染後直ちに、示されたETVの濃度に露出させた。ETVでの前処理を利用する実験においては、細胞を、HBVバキュロウイルス感染の16時間前にETVを含む培地に加え、HBVバキュロウイルス感染をまた、ETVを含む培地中で実行し、そして感染および洗浄手順の完了後直ちにETVを含む新鮮な培地を細胞に加えた。
野生型およびrtI169T+rtV173L+rtL180M+rtM204VをコードするHBV/バキュロウイルスを用いて実行された抗ウイルス試験
ETVの非存在下(0μM ETV)で増殖した野生型ウイルスに対する定量的リアルタイムPCRの結果を使用した、野生型HBVおよび4つ組の変異を含むHBVに対するETVの用量効果のグラフ分析を図11に示す。ETVは、試験されたすべてのETV濃度で定量的PCRによって検出された、HBV複製中間体の減少によって実証されるように、野生型HBV複製に対して最も明白な効果を有した。対照的に、とりわけ0.5μM ETVまでの濃度で、4つの変異を含む変異体(rtI169T+rtV173L+rtL180M+rtM204V)をコードする組換えHBVによる、ETVに対する感度が減少した。
ETV
ETV(以前にはBMS-200475またはSQ-34676)は、HBV複製の強力な阻害物質である。ETVは、ヘパドナウイルスおよびヘルペスウイルスに対して活性をもつ、生物−経口利用可能な特性を有するシクロペンチルデオキシグアノシンアナログである。ETVの構造は図3に示され、その合成はBisacchiら(Bioorg. Med. Chem. Lit. 7: 127-132, 1997)によって記載されている。前臨床試験により、エンテカビルが、酵素および細胞に基づくアッセイ法においてHBVの高度に強力な阻害物質であることが示されている(Innaimoら、1997、前出;Sieferら、1998、前出;Yamanakaら、1999、前出)。ETVは、以前にはBMS-200475およびSQ-34676として記載された。

Claims (6)
- エンテカビル(ETV)に対する感受性の低下を示すB型肝炎ウイルス(HBV)変種についての潜在能力を決定するための方法であって、該方法が:(a)該HBVからDNAまたは対応するmRNAを単離する工程であって、該DNAまたは対応するmRNAが逆転写酵素(rt)ドメインを有するHBV DNAポリメラーゼをコードするヌクレオチド配列である工程、ならびに、(b)該DNAポリメラーゼのrt中におけるアミノ酸置換を生じるコドンのヌクレオチド配列の変異をスクリーニングする工程を含み、該コドンがメチオニン204(rtM204)、ロイシン180(rtL180)、スレオニン184(rtT184)、メチオニン250(rtM250)およびセリン202(rtS202)からなるリストより選択され、且つ、HBV変種がETVに対して低下した感受性を、(i)rtM204、rtL180およびtrT184、(ii)rtM204、rtL180およびrtM250、(iii)rtM204、rtL180およびrtS202、並びに(iv)rtM204、rtL180、rtT184およびrtS202からなるリストより選択される変異の組み合わせと共に示し、且つ(i)〜(iv)におけるrtM204変異およびrtL180変異がそれぞれrtM204バリン(rtM204V)およびrtL180メチオニン(rtL180M)である、方法。
