【発明の属する技術分野】
本発明は、電気手術装置、更に詳しくは高周波電流の出力制御部分に特徴のある電気手術装置に関する。

【従来の技術】
一般に、電気メス等の電気手術装置は、外科手術あるいは内科手術で生体組織の切開や凝固、止血等の処置を行う際に用いられる。この様な電気手術装置には、高周波焼灼電源と、この高周波焼灼電源に接続される処置具が設けられており、処置具を患者に接触させて高周波焼灼電源から高周波電流を供給することで上記処置を行う。

上述した電気手術装置は従来より種々提案されており、例えば特開平8−98845号公報では、凝固する組織の炭化を防止し、組織の電極への付着を防止するため、凝固の終了を組織インピーダンスより判定し、高周波出力を停止する技術が示されている。

また、特開平10−225462号公報の電気手術装置では、特開平8−98845号公報と同様の目的を達成するため高周波出力を低下させる技術が示されている。

【発明が解決しようとする課題】
上記特開平8−98845号公報、及び特開平10−225462号公報の電気手術装置では、凝固する組織の体積が極端に大きい場合、十分な凝固力を得るために高周波出力を上げる必要があり、完全に組織の炭化を防止し、組織の電極への付着を防止することは出来なかった。

そこで、本発明は上記の問題に鑑み、高周波電流を制御することよって、生体組織の確実な凝固を行い、炭化を防止し、組織の電極への付着を軽減できる電気手術装置を提供することを目的とするものである。

【課題を解決するための手段】
本発明の第1の電気手術装置は、手術具を介して生体組織に供給するための、出力されている間は時間経過にかかわらず常に一定な所定定常高周波電力を発生する高周波電力発生手段と、前記高周波電力発生手段から出力される定常高周波電力の出力を変更可能とする可変手段と、前記高周波電力発生手段から出力される定常高周波電力を前記手術具を介して生体組織に供給した際における当該生体組織に係る組織インピーダンス値または高周波電流値を検出する第1の検出手段と、前記第1の検出手段において検出した前記組織インピーダンス値または高周波電流値に基づいて、所定期間における当該組織インピーダンス値の最小値または高周波電流値の最大値を検出する第2の検出手段と、前記高周波電力発生手段からの前記定常高周波電力の出力が開始された後に、前記第2の検出手段において前記組織インピーダンス値の最小値または高周波電流値の最大値が検出された後は、前記可変手段を制御して、前記組織インピーダンス値または高周波電流値に係る所定の出力停止用閾値と所定停止期間とに基づいて前記高周波電力発生手段から出力される前記定常高周波電力の出力と停止とを繰り返し制御する制御手段と、を具備し、前記制御手段は、前記出力と停止とが繰り返される定常高周波電力の現在の出力回数をNとするとき、前記定常高周波電力の出力が開始された後に最初に前記第2の検出手段において検出された前記組織インピーダンス値の最小値より大きな値であってかつ、当該最小値に対して前記出力回数Nに所定の正の係数を乗じた値と所定の正の定数との和を乗じて得られる値であって当該出力回数毎に増加する値、または、前記定常高周波電力の出力が開始された後に最初に前記第2の検出手段において検出された前記高周波電流値の最大値より小さな値であってかつ、当該最大値に対して前記出力回数Nに所定の正の係数を乗じた値と所定の正の定数との差を乗じて得られる値であって当該出力回数毎に減少する値を、定常高周波電力の出力回数N回目の出力期間中における出力停止用閾値として設定し、かつ、前記定常高周波電力の所定の出力期間中に前記第1の検出手段において検出される前記組織インピーダンス値または高周波電流値が現在設定されている出力停止用閾値に達した際には前記定常高周波電力の出力を停止し、当該出力停止後所定停止期間の経過後に次回の出力として前記定常高周波電力の出力を再開するよう前記可変手段を制御することを特徴とする。

また、本発明の第2の電気手術装置は、手術具を介して生体組織に供給するための、出力されている間は時間経過にかかわらず常に一定な所定定常高周波電力を発生する高周波電力発生手段と、前記高周波電力発生手段から出力される定常高周波電力の出力を変更可能とする可変手段と、前記高周波電力発生手段から出力される定常高周波電力を前記手術具を介して生体組織に供給した際における当該生体組織に係る組織インピーダンス値または高周波電流値を検出する第1の検出手段と、前記第1の検出手段において検出した前記組織インピーダンス値または高周波電流値に基づいて、所定期間における当該組織インピーダンス値の最小値または高周波電流値の最大値を検出する第2の検出手段と、前記高周波電力発生手段からの前記定常高周波電力の出力が開始された後に、前記第2の検出手段において前記組織インピーダンス値の最小値または高周波電流値の最大値が検出された後は、前記可変手段を制御して、前記組織インピーダンス値または高周波電流値に係る所定の出力停止用閾値と所定停止期間とに基づいて前記高周波電力発生手段から出力される前記定常高周波電力の出力と停止とを繰り返し制御する制御手段と、を具備し、前記制御手段は、前記出力と停止とが繰り返される定常高周波電力の現在の出力回数をNとするとき、所定の出力回数N回目における前記定常高周波電力の出力期間中に前記第2の検出手段において検出された前記組織インピーダンス値の最小値に所定の正の係数を乗じた値、または、所定の出力回数N回目における前記定常高周波電力の出力期間中に前記第2の検出手段において検出された前記高周波電流値の最大値に所定の正の係数を乗じた値を、現在の出力期間中における出力停止用閾値として設定し、かつ、前記定常高周波電力の所定の出力期間中に前記第1の検出手段において検出される前記組織インピーダンス値または高周波電流値が現在設定されている出力停止用閾値に達した際には前記定常高周波電力の出力を停止し、当該出力停止後所定停止期間の経過後に次回の出力として前記定常高周波電力の出力を再開するよう前記可変手段を制御することを特徴とする。
さらに本発明の第3の電気手術装置は、手術具を介して生体組織に供給するための、出力されている間は時間経過にかかわらず常に一定な所定定常高周波電力を発生する高周波電力発生手段と、前記高周波電力発生手段から出力される定常高周波電力の出力を変更可能とする可変手段と、前記高周波電力発生手段から出力される定常高周波電力を前記手術具を介して生体組織に供給した際における当該生体組織に係る組織インピーダンス値または高周波電流値を検出する検出手段と、前記高周波電力発生手段からの前記定常高周波電力の出力が開始された後に、前記可変手段を制御して、前記組織インピーダンス値または高周波電流値に係る所定の出力停止用閾値と所定停止期間とに基づいて前記高周波電力発生手段から出力される前記定常高周波電力の出力と停止とを繰り返し制御する制御手段と、を具備し、前記制御手段は、前記出力と停止とが繰り返される定常高周波電力の現在の出力回数をNとするとき、前記定常高周波電力の出力が開始された際の前記組織インピーダンス値を初期値とし当該インピーダンス初期値より大きな値であってかつ、当該インピーダンス初期値に対して前記出力回数Nに所定の正の係数を乗じた値と所定の正の定数との和を乗じて得られる値であって当該出力回数毎に増加する値、または、前記定常高周波電力の出力が開始された際の前記高周波電流値を初期値とし当該電流値初期値より小さな値であってかつ、当該電流値初期値に対して前記出力回数Nに所定の正の係数を乗じた値と所定の正の定数との差を乗じて得られる値であって当該出力回数毎に減少する値を、現在の出力期間中における出力停止用閾値として設定し、かつ、前記定常高周波電力の所定の出力期間中に前記検出手段において検出される前記組織インピーダンス値または高周波電流値が現在設定されている出力停止用閾値に達した際には前記定常高周波電力の出力を停止し、当該出力停止後所定停止期間の経過後に次回の出力として前記定常高周波電力の出力を再開するよう前記可変手段を制御することを特徴とする。

【発明の実施の形態】
発明の実施の形態について図面を参照して説明する。
〔第1の実施の形態〕

(構成)
図1に示すように、本実施の形態の電気手術装置1は、高周波焼灼電源2を備え、高周波焼灼電源2は処置具(手術具)の一部としての電極3を介して患者4に接続される。また、高周波焼灼電源2にはフットスイッチ5が接続されている。なお、図1に示す電極3は一対の電極で構成されているが、処置用電極3としては、単極、多極、いづれの電極を用いても良い。

図2に示すように、高周波焼灼電源2には、直流電流を供給する電源回路6と、電源回路6からの直流電流を高周波電流に変換する高周波発生回路7と、高周波発生回路7に対して高周波電流の波形を指示する波形回路8と、高周波発生回路7からの高周波電流を電極3に出力する出力トランス9と、出力トランス9より出力される出力電流を検出する電流センサー10と、出力トランス9より出力される出力電圧を検出する電圧センサー11と、電流センサー10及び電圧センサー11の信号をデジタルデータに変換するA/Dコンバータ12と、A/Dコンパータ12からのデジタル化されたデータに基づいて電源回路6及び波形回路8を制御する制御回路13と、電流センサー10及び電圧センサー11からの検出信号に基づいて得られた生体情報(組織インピーダンスや、組織温度など)を表示する表示回路(図示せず)とを備えて構成される。

上記構成で、高周波発生回路7,波形回路8及び出力トランス9は、高周波電流を発生するための高周波電流発生手段を構成している。制御回路13は、電源回路6による直流電流の供給のオン/オフを制御できる一方、波形回路8による高周波電流の波形を制御できる。従って、電源回路6は、直流電流の供給のオン/オフを制御されることで、高周波電流の出力を変更するための出力変更手段を構成している。電流センサー10と電圧センサー11とA/Dコンバータ12と制御回路13の一部とは、生体組織4aの生体情報を検出(測定)し、その検出結果に基づいて生体組織の凝固状態を判断するための凝固状態判断手段を構成している。また、制御回路13の一部は、高周波電流の出力を可変させるように前記出力変更手段を制御するもので、高周波電流が出力/一時停止を繰り返す様に前記出力変更手段を制御し、処置用電極3に高周波電流を供給するための制御手段を構成している。そして、この制御手段は、前記出力変更手段による高周波電流の一時停止を、前記凝固状態判断手段からの生体情報(組織インピーダンスや、組織温度など)に基づいて決定する機能を有している。

なお、本実施の形態では、生体情報としての組織インピーダンスとしては、電流センサ10からの電流検出データと電圧センサ11からの電圧検出データに基づいて制御回路13が一対の電極3間の生体組織4aのインピーダンスを測定することによって取得するようになっている。制御回路13では、測定した組織インピーダンスに基づいて生体組織4aの凝固状態を判断することが可能である。そして、制御回路13における組織インピーダンスの測定動作は、電流センサ10からの電流検出データと電圧センサ11からの電圧検出データに基づいて処置用電極3に高周波電流を出力中に行なっても良いし、高周波電流の一時停止中に行なっても良い。

(作用)
生体組織4aに高周波電力を投与すると、加熱により組織4aはタンパク変性し、その後組織4a内の水分が蒸発することで乾燥して行く。この過程で組織4aは凝固される。組織4aが乾燥した後も高周波電力を投与しつづけると、組織4aの炭化が発生し、組織4aの電極3への付着が生じる。組織4aの電極3への付着を防止するには、乾燥が発生した時点で高周波電力の供給を停止するべきである。

図3(a)に示すように、生体組織4aに対して投与する高周波電力は時間経過に関わらず常に一定とする。生体組織4aに一定高周波電力を投与し続けると、図3(b)に示すように、組織温度は組織の変性、乾燥に伴い序々に上昇してゆく。一方、図3(c)に示すように、組織インピーダンスは、一旦減少した後にほぼ一定の状態を経て組織の乾燥に伴い急激に上昇する。従来は、組織インピーダンスまたは組織温度から乾燥が生じたことが分かった時点で、高周波出力を停止する等の制御を行っていた。

ここで、高周波電力の供給を図4(a)に示した様に断続的に行うと、図4(b),(c) に示すように時間経過に伴い一旦上昇した組織温度,組織インピーダンスは、高周波電力の停止に伴い低下する。ここで再度高周電力を供給すると、再び組織温度,組織インピーダンスは上昇する。この過程を繰り返すことにより、組織の状態を変性,乾燥に止め、炭化を防止しながら、多くの高周波電力が投入できる。この結果、前述の従来の方法に比較し、より広範囲の組織を凝固することが可能となる。

更に、それぞれの出力で組織温度,組織インピーダンスにより凝固状態を判定し出力の一時停止を決定すれば、次回出力開始時に組織が過度に凝固されていて電力が有効に伝達できないといったことが無く、また組織の電極への付着を防止することができる。

次に、以上の生体組織の性質を利用した本実施の形態の作用について、図5を参照しながら説明する。

フットスイッチ5が踏まれると、制御回路13は図5に示すフローチャートに従って制御を開始する。

フットスイッチ5が踏まれると、制御回路13は、ステップS1で患者4の組織インピーダンスの最小値Zminを∞に、高周波の出力回数Nを0に設定する。次にステップS2で、出力回数Nをカウントアップし、ステップS3で高周波の出力を開始する。ステップS4で電流センサー10、電圧センサー11の信号をA/Dコンバータ12を介して取り込み、組織のインピーダンスZを計算する。次にステップS5で、S4で計算した組織インピーダンスZが最小値Zminより小さい場合は、ステップS6で最小値Z minを更新する。

ステップS7で、S4で計算した組織インピーダンスZがZmin×(1.2+0.1×N)より小さい場合S4から同様の処理を繰り返す。ここで最小値Zminは、フットスイッチ5が踏まれた後の組織インピーダンスの最小値である。S7で、S4で計算した組織インピーダンスが、Zmin×(1.2+0.1×N)より大きければステップS8で出力を一時停止する。これは、組織インピーダンスの上昇より凝固状態を判定し、組織の過度の凝固、電極への付着を防止する為である。出力回数の増加に伴い、徐々に凝固の程度を強くしたいため、出力回数に従い閾値を上げていく。この後、一時停止時間が例えば一秒等予め定められた所定時間が経過したかをステップS9で判断する。S9で所定時間経過後、ステップS10で出力回数が予め定められた所定値を超えたか判断し、所定回数以内の場合は上記ステップS2から同様の処理を繰り返す。S10で所定回数を超えた場合は、ステップS11で出力を停止する。

このように本実施例の形態では高周波電流の出力/一時停止を繰り返し、更に高周波出力を生体組織状態によって一時停止するため、組織の温度を炭化が発生しない範囲に保ちつつ繰り返して高周波電流を投与できる。この結果確実に凝固を行い、炭化、組織の電極への付着を防止できる。

図13の構成で、高周波発生回路7,波形回路8及び出力トランス9は、高周波電流を発生するための高周波電流発生手段を構成している。制御回路13は、電源回路6による直流電流の供給のオン/オフを制御できる一方、波形回路8による高周波電流の波形を制御できる。従って、電源回路6は、直流電流の供給のオン/オフを制御されることで、高周波電流の出力を変更するための出力変更手段を構成している。電流センサー10とA/Dコンバータ12と制御回路13の一部とは、生体組織4aの生体情報を検出(測定)し、その検出結果に基づいて生体組織の凝固状態を判断するための凝固状態判断手段を構成している。また、制御回路13の一部は、高周波電流の出力を可変させるように前記出力変更手段6を制御するもので、第1の出力と、前記第1の出力より小さい第2の出力を、交互に出力する様に前記出力変更手段6を制御し、処置用電極3に高周波電流を供給するための制御手段を構成している。そして、この制御手段は、前記出力変更手段6における高周波電流の前記第1,第2の出力の切り替えを、前記凝固状態判断手段からの生体情報(組織インピーダンスや、組織温度など)に基づいて決定する機能を有している。

なお、本実施の形態では、生体情報としては、電流センサ10からの電流検出データに基づいて制御回路13が一対の電極3における高周波電流値を測定することによって取得するようになっている。制御回路13では、測定した高周波電流値に基づいて生体組織4aの凝固状態を判断することが可能である。そして、制御回路13における高周波電流の測定動作は、電流センサ10からの電流検出データに基づいて行なうが、処置用電極3に第1の高周波電流を出力中に行なっても良いし、処置用電極3に第2の高周波電流を出力中に行なっても良い。

(作用)
第1の実施の形態で述べた様に、組織4aの凝固が進むと、組織インピーダンスはそれに伴い変化する。組織インピーダンスが大きくなると高周波電流は減少するため、高周波電流は図14(c) に示した様に組織インピーダンス(図3(c)参照 )とは逆の挙動を示す。図14(a)は、生体組織4aに対して投与する一定の高周波電力を示す。これは時間経過に関わらず常に一定とする。生体組織4aに一定高周波電力を投与し続けると、図14(b)に示すように、組織温度は組織の変性、乾燥に伴い序々に上昇してゆく。一方、図14(c)に示すように、高周波電流は、一旦上昇した後にほぼ一定の状態を経て組織の乾燥に伴い急激に下降する。
