Description
本発明による管腔内注入カテーテルは、カテーテル本体と針とを備える。この針は、通常はカテーテル本体の遠位端部近くにある、カテーテル本体の保護凹部内で、半径方向に配置される。「半径方向に配置される」とは、針が配置される地点において、カテーテル本体の軸線に垂直となる全般的な横方向を向いて針が位置することを意味する。これにより、針が保護凹部から半径方向に延びる際に、針は、略真っ直ぐな半径方向に管腔壁に入り、針が貫通可能な管腔壁の距離を最大化すると共に、管腔組織の断裂又は損傷を最小化することになる。特に、本発明は、針が配置される地点において、保護凹部がカテーテル本体の幅の60%を上回る深さを有する状態を提供し、更に、針が配置される位置のカテーテル本体の幅の少なくとも60%に等しい長さを、針が有する状態を提供する。通常、カテーテル本体の幅は、円形の断面を有するカテーテル本体の直径となるが、卵形の断面を有するカテーテル本体の長軸又は短軸にすること、或いは、他の断面形状を有するカテーテル本体のその他の幅にすることも可能である。
本発明の利点は、患者に大きなリスクを与えずに、極めて局部的な治療又は診断用物質の配置を提供することである。本発明は、著しい軸線方向運動構成要素なしで、動脈又は静脈の壁に微小規模の開口部を形成する局部的な力を生成できる。これにより、血管壁の断裂のリスクは、実質的に排除される。本発明は、微小な自己回復性の傷を生成する。本発明により、様々な介入的手技のうち特に、局部的腫瘍治療と硬化した動脈の治療とが可能となる。
本発明は、微細加工外科用デバイスと、カテーテルを用いた介入的手技においてこうしたデバイスを使用する方法とを対象とする。本発明については、いくつかの代表的な実施形態と、治療又は診断用物質の血管壁又は血管周囲組織への介入的送達のために、微細加工針又はマイクロニードル、或いはマクロニードルを製造する処理に関して説明する(血管壁は動脈又は静脈のいずれかの壁である)。例示的な治療物質に含まれるものは、無機薬理学的作用物質と、有機薬理学的作用物質と、未分化、部分的分化、又は完全分化幹細胞、島細胞、及び遺伝子組み換え細胞を一部として含む特別な治療機能を伴う細胞と、ウイルス、バクテリア、菌類、及び寄生虫を一部として含む微生物と、遺伝子、染色体、プラスミド、DNA、RNA、mRNA、rRNA、tRNA、合成RNA、合成DNA、及びこれらの組み合わせを一部として含む有機遺伝物質と、上記の物質の任意の組み合わせとである。例示的な診断用物質に含まれるものは、造影剤と、放射線形成物と、蛍光形成物と、抗体形成物と、酵素形成物とである。
Claims (17)
- 介入的外科手技のためのアクチュエータであって、
遠位端部及び近位端部を有するアクチュエータ本体と、
遠位端部と近位端部との間に位置し、長手方向に沿って形成される開口部を有する中央拡張可能部であり、折り畳み状態である非作動状態と展開状態である作動状態との間で動作可能である中央拡張可能部と、
前記中央拡張可能部に位置し、シャフトと、前記シャフトの先端に鋭端部を有する針であり、前記中央拡張可能部を前記折り畳み状態から前記展開状態へ変化させる時、前記アクチュエータ本体の長手方向中心軸線に対してほぼ垂直な方向に、前記アクチュエータ本体内部の位置から前記アクチュエータ本体外部の位置へ移動可能である針とを備え、
前記中央拡張可能部は、前記折り畳み状態にあるとき、C字状断面を有し、前記鋭端部は前記開口部の径方向内側に配置されている、
アクチュエータ。 - 近位端部が、カテーテルの端部に接合される請求項1記載のアクチュエータ。
- 遠位端部が、カテーテルのチップ端部に接合される請求項2記載のアクチュエータ。
- 前記中央拡張可能部が内側開放領域を含み、前記中央拡張可能部を前記折り畳み状態から前記展開状態へ変化させるために開放領域に始動流体を供給する伝達コンジットを更に備える請求項1記載のアクチュエータ。
- 前記中央拡張可能部が、ポリマにより作成される請求項1記載のアクチュエータ。
- 遠位端部と近位端部に位置する止め輪を更に備える請求項1記載のアクチュエータ。
- 先端部が出口ポートを含み、治療又は診断用物質を出口ポートへ供給する流体経路を更に備える請求項1記載のアクチュエータ。
- 治療物質が、無機薬理学的作用物質と、有機薬理学的作用物質と、未分化、部分的分化、又は完全分化幹細胞、島細胞、又は遺伝子組み換え細胞を含む、治療機能を伴う細胞と、ウイルス、バクテリア、菌類、又は寄生虫を含む微生物と、遺伝子、染色体、プラスミド、DNA、RNA、mRNA、rRNA、tRNA、合成RNA、又は合成DNAを含む有機遺伝物質とにより構成されるグループから選択される請求項7記載のアクチュエータ。
- 診断用物質が、造影剤と、放射性標識と、蛍光標識と、抗体標識と、酵素標識とにより構成されるグループから選択される請求項7記載のアクチュエータ。
- 針が、血管壁を貫通し、血管周囲組織へ到達する微細効果針である請求項1記載のアクチュエータ。

- 介入的外科手技のための微細加工外科用デバイスであって、
血管が発達した生物の体内における挿入及び操作のための遠位端部と、ユーザに対して体内でのカテーテルの遠位端部の操作に関する制御を提供する近位端部とを有するカテーテルと、
カテーテルの遠位端部に接合されるアクチュエータであり、アクチュエータ本体と、アクチュエータ本体の拡張可能部に接合された針とを含み、前記アクチュエータが非作動状態にある時、前記針が前記アクチュエータ本体内に位置し、前記アクチュエータが作動状態にある時、アクチュエータ本体の外部に位置するように構成され、前記アクチュエータが非作動状態から作動状態へ変化する時、前記針が前記アクチュエータ本体の長手方向中心軸線にほぼ垂直な経路に沿って移動可能であるアクチュエータと、
カテーテルの遠位端部と前記アクチュエータの近位端部とに位置する流体接続部であり、治療又は診断用物質を前記針へ供給し、更には始動流体を前記アクチュエータに提供して、前記アクチュエータを非作動状態から作動状態に変化させる流体接続部とを備え、
前記拡張可能部はポリマで作成されると共に長手方向に沿って形成される開口部を有し、
前記針は、シャフトと、前記シャフトの先端に鋭端部を有し、
