Description
a)固定するための装置が1つの部品のみから成り、
b)最小侵襲性に適用され、大きな切開部を必要とせず、
c)固定を確実にするための装置と骨との間の予張力が不要であり、
d)外固定にも内固定にも使用可能であり、
e)縦通材をすでにあらかじめ装置内に挿入して予備成形しうるとともに、使用者は、骨ネジの位置を確実に知った後初めてその骨ネジを設置すればよく、
f)個々の装置の位置は使用者によって自由に選択可能であり、
g)追加的な装置が必要である場合、それを縦通材の全長にわたって手間をかけて誘導することなく、簡単に縦通材上に差し付けることができ、
h)縦通材は本発明による装置の配置前に挿入され、それによって外科医には、ネジ位置を簡単に決定することができる可能性が与えられ、
i)縦通材を介して骨折を修復する可能性があり、
j)使用者は、縦通材および複数の本発明による装置からなる構成物を介して角度安定性の頭部ロックネジによって個々の骨折部分を最小限の侵襲で結合および固定することができ、かつ
k)本発明による装置は脊柱範囲のみならず、他の骨接合術の適用分野でも使用されうる。
A)縦通材を最初に穿刺切開によって患者の治療すべき範囲へ挿入し、かつ経皮的に所望の位置へ移動し、
B)外科医はここで縦通材の外形をチェックして容易に修正することができ、
C)縦通材の外形が適正である場合は、所望の数のクランプ(本発明による装置)を適切な穿刺切開によって経皮的に挿入し、直接横から棒へ取付けることができ、
D)次に角度安定の頭部ロックネジをクランプの穴へネジ込むが、また締付けず、
E)整復が適正である場合は、角安定の頭部ロックネジを最終的に締付けることができ、その結果、縦通材、クランプ、および骨ネジから形成された構成物が高剛性になって骨折が固定される。
1.B型の本発明による装置(クランプ)を所望の骨部分に相対して位置決めし、
2.頭部ロックネジをB型のクランプの円錐状の穴を通じて所定のねじ込み深度までねじ込んでクランプの予備固定を達成し、
3.解剖学的要件に従って縦通材を成形し、
4.縦通材をすでに組立てられたB型のクランプの開放された通路内へ嵌め込み、
5.次に、頭部ロックネジをB型クランプ内で完全に締付けて縦通材のクランプへの固定を行い、
6.適宜な整復器具を用い縦通材を介して骨折を整復し、
7.前記整復された骨断(骨折)片をA型またはB型のクランプを選択的に用いて固定し、
8.骨固定構成物にA型またはB型のクランプを選択的に追加する。

Claims (11)
- 長手方向の縦通材(3)と骨固定要素(4)を相互固定するための装置(1、2)であって、前記装置を構成する三次元の本体(5)が縦通材(3)を受入れる通路軸(7)を有する片側だけ開放された通路(6)および前記通路軸(7)に対して横断方向に延在し前記本体(5)を完全に貫通する穴(8)を有してなり、前記通路(6)および前記穴(8)が少なくとも部分的に互いに重なり合う装置において、
A)前記穴(8)が、大きめの開口(14)と小さめの開口(15)を有する円錐状に構成されており、かつ
B)前記穴(8)が内側ネジ山(9)を有する、
ことを特徴とする装置(1、2)。 - 前記穴(8)が、5°〜25°の範囲の円錐角度を有することを特徴とする請求項1に記載の装置(1、2)。
- 前記穴(8)が、8°〜15°の範囲の円錐角度を有することを特徴とする請求項2に記載の装置(1、2)。
- A)前記円錐状の穴(8)が前記通路(6)へ向かって先細になり、かつ
B)前記穴(8)の小さめの開口(15)が前記本体(5)の前記通路(6)と同じ側面上に位置している、
ことを特徴とする請求項1〜3のいずれか1つに記載の装置(1、2)。 - A)前記円錐状の穴(8)が前記通路(6)へ向かって広がり、かつ
B)前記穴(8)の大きめの開口(14)が前記本体(5)の前記通路(6)と同じ側面上に位置している、
ことを特徴とする請求項1〜3のいずれか1つに記載の装置(1、2)。 - 骨への導入に適した先端(10)とネジ山軸部(11)とネジの操作に適した頭部(12)を有する骨ネジの形式の骨固定要素(4)をさらに備えることを特徴とする請求項1〜5のいずれか1つに記載の装置(1、2)。
- 前記骨固定要素(4)の前記頭部(12)が、前記内側ネジ山(9)に対応する外側ネジ山(13)を有することを特徴とする請求項6に記載の装置。
- 前記外側ネジ山(13)のピッチが、0.1〜3.0mmであることを特徴とする請求項7に記載の装置。
- 前記骨固定要素(4)の前記頭部(12)が前記先端(10)に向かって円錐状に先細になることを特徴とする請求項6〜8のいずれか1つに記載の装置。
- 前記円錐状に先細になる頭部(12)が、5°〜25°の範囲の円錐角度を有することを特徴とする請求項9に記載の装置。
- 縦通材(3)および少なくとも1つの請求項4に記載の装置ならびに少なくとも1つの請求項5に記載の装置を有する骨固定装置。
