Description
それ故、本発明の目的は、手術者が多量の血液の利用を必要としない場合に使用でき、血液の最適で安全な処理及び手術者による便利な管理を保証できる、身体外循環において血液を処理する装置を提供することである。
装置は気泡捕獲器2を有する。気泡捕獲器2は患者から血液を受け取るための静脈ラインに接続するようになった入口コネクタ2aを具備する。
気泡捕獲器2の出口コネクタ2bはダクト3の一端で接続される。ダクト3の他端は遠心ポンプ4の入口に接続される。遠心ポンプ4は、遠心ポンプ4から熱交換器5の入口コネクタ5aへ血液を提供するように接続された送給ダクト4aに接続された出口コネクタ4bを画定する透明な囲いを有する。熱交換器5は酸素付加器6の入口6aへ血液を運搬するように形状づけられた出口コネクタ5bを具備する。
酸素付加器6の出口6bから、血液は動脈フィルタ7に達し、このフィルタは血液を患者へ戻すための動脈ラインに接続するようになった出口コネクタ7aを具備する。
遠心ポンプ4の透明な囲いは熱交換器5及び酸素付加器6で構成された構造体の端面に一体的に接続される。遠心ポンプ4は、ポンプの軸線が水平になり、出口コネクタ4bがポンプの頂部に位置するように、方位決めされる。この構成はいくつかの理由で有利である。第1に、充填中にポンプ内に収容されたすべての空気の完全な除去を本質的に保証する。その理由は、装置内のすべての空気気泡はその浮力のために上昇し、ポンプの頂部の出口コネクタを通って排出されるからである。第2に、遠心ポンプの軸線が水平であるため、操作者は透明な囲いを通してロータ及びポンプ室の全体を見ることができる。このことは、操作者が、ポンプ内へ又はポンプを通って進むことのあるいかなる空気気泡をも目で位置確認するのを許容し、それに応答して適当な較正行動を行うのを許容する。
本発明に係る装置は極めて機能的である。身体外回路内で血液循環を生じさせるための組立体の素子として遠心ポンプを採用する選択は、静脈ラインを通って吸入部に流入し、患者から出発する血液の部分真空が、そこから血液を吸引する患者の血管を劣化させない値に制限されることを保証する。ちなみに、容積ポンプでは、このような劣化が生じることがある。
空気気泡が遠心ポンプに到達するのを阻止することが望ましい。その理由は、ポンプを通過中に空気気泡が微粒化されて、動脈フィルタ7により捕獲できなくなるからである。回路内でポンプの上流側で接続された気泡捕獲器2はこの機能を遂行するために設けられる。
本発明の装置は、伝統的な身体外血液回路内での種々の個々の素子の使用に対する迅速で有効かつ便利な代替として使用することができる。装置の構造がこれらの種々の個々の素子を予め接続された一体の構造体として組み込んでいるので、極めて迅速に設置することができ、使用態勢に入れる。更に、本発明は、いくつかの理由で、心臓鼓動間式の手術に使用するのに有利である。第1に、血液の一部が身体外回路内へ逸らされるので、心臓は一層少ない血液を送り出し、それ故、鼓動が一層少なく、手術者にとって心臓を処置するのが一層容易になる。第2に、手術中に完全なバイパス状態に変更する必要がある場合、本発明の一体の装置はこの変更を行うのに必要な能力及び機能を提供する酸素付加器及び他の素子を有する。本発明はまた心臓バイパス式手術中又は心臓鼓動間式の手術中血液を処理する装置を使用する方法を含む。
ここに開示した実施の形態は本発明の現在好ましい実施の形態を示すことを理解すべきである。本発明の要旨を逸脱することなく、これらの実施の形態に対して種々の修正及び付加を行うことができる。

Claims (3)
- 身体外の血液回路を通って流れる血液を酸素付加及びろ過処理するための一体化した装置において、
静脈血を受け取るための入口(2a)と、静脈血を供給するための出口(2b)とを有する気泡捕獲器(2);
気泡捕獲器(2)の出口から血液を受け取るように接続された入口と、出口(4b)とを有する血液ポンプ(4);
上記ポンプ(4)の上記出口(4b)から静脈血を受け取るように接続された血液入口(5a)と、温度制御された静脈血を供給するための血液出口(5b)とを有する熱交換器(5);
上記熱交換器(5)の上記血液出口(5b)から静脈血を受け取るように接続された入口(6a)と、酸素付加された血液を供給するための出口(7a)とを有する酸素付加器(6);
上記酸素付加器(6)の上記出口(6b)から酸素付加された血液を受け取るように接続された入口と、ろ過され酸素付加された血液を供給するための出口(7a)とを有する動脈血フィルタ(7);及び
上記気泡捕獲器(2)を画定する第1の部分と、上記血液ポンプ(4)を画定する第2の部分と、上記熱交換器(5)を画定する第3の部分と、上記酸素付加器(6)を画定する第4の部分と、上記動脈血フィルタ(7)を画定する第5の部分とを有する一体のハウジング;
を有し、
上記気泡捕獲器(2)は、上記熱交換器(5)、上記酸素付加器(6)及び上記動脈血フィルタ(7)の上方に配置され、上記ポンプ(4)は、上記熱交換器(5)、上記酸素付加器(6)及び上記動脈血フィルタ(7)の下方に配置されることを特徴とする一体化した血液処理装置。 - 上記血液ポンプ(4)が遠心ポンプであることを特徴とする請求項1に記載の一体化した血液処理装置。
- 上記遠心ポンプが軸線を有し、当該遠心ポンプは、同遠心ポンプの上記軸線が水平となるように上記一体のハウジング内に位置することを特徴とする請求項2に記載の一体化した血液処理装置。
