Description
ステントは、開口血管およびその他の身体管腔を支持するために用いられる管状足場構造である。ステントの最も一般的な使用は、閉塞冠動脈を開口し、再狭窄を防止することである。再狭窄の可能性を最小限にするのを助けるために、治療薬で被覆されたステントの使用が提唱された。例えばパクリタキセルで被覆されたステントは、非被覆ステントと比較して、再狭窄率を低減することが示された。
本発明人等は、薬剤被覆ステントの安全性および効力に関する好ましい薬剤用量および薬剤放出プロフィールを確認した。本明細書中に記載された実施態様は、高分子担体中のパクリタキセルで被覆された金属ステントに特有であるが、しかし本発明は、高分子担体を用いた場合も用いない場合も、その他の薬剤で被覆されたステントに適用可能であると考えられる。
薬剤用量および薬剤放出プロフィールが薬剤被覆ステントの安全性および効力に関する有意の因子である、ということを本発明人等は見出した。薬剤被覆ステントのための最適用量および放出動態を、本発明人等は確認した。特に、最低有効薬剤投薬量の送達を可能にし、したがって患者の安全性を強化して、薬剤からの任意の副作用を最小限にする用量および放出動態を、本発明人等は確定した。
パクリタキセルに特有のものとして本発明を説明したが、本発明人等の知見は広範な種々の薬剤系に適用可能であると認識されるべきである。
Claims (25)
- 薬剤被覆ステントであって、
患者の身体管腔中に挿入可能な金属ステントと、
前記ステントの少なくとも一部分の上に被覆されるパクリタキセルとを含み、
前記パクリタキセルが、ステント表面積1平方mm当たり2μgまでの投薬量を有する、
ことを特徴とするステント。 - 前記パクリタキセルが、ステント表面積1平方mm当たり0.4μgから2μgまでの投薬量を有することを特徴とする、請求項1に記載のステント。
- 前記パクリタキセルが、ステント表面積1平方mm当たり1μgの投薬量を有することを特徴とする、請求項1又は2に記載のステント。
- 前記パクリタキセルが、ステント当たり85μgの総投薬量を有することを特徴とする、請求項3に記載のステント。
- 前記パクリタキセルが、前記ステントの少なくとも一部分の上に被覆されたポリマー担体中に混入されることを特徴とする、請求項3に記載のステント。
- 前記ポリマーが、スチレン−イソブチレンベースのブロックコポリマーであることを特徴とする、請求項5に記載のステント。
- 前記ポリマー担体中のパクリタキセルの重量分画が8.8%から35%であることを特徴とする、請求項5又は6に記載のステント。
- 患者への挿入後、少なくとも8日間の期間、前記パクリタキセルが前記ステントから患者に放出されることを特徴とする、請求項3から請求項7のいずれか1項に記載のステント。
- 患者への挿入後、少なくとも10日間の期間、前記パクリタキセルが前記ステントから患者に放出されることを特徴とする、請求項8に記載のステント。
- パクリタキセルの放出速度が、患者への挿入後、1日から3日の間で最大にされることを特徴とする、請求項9に記載のステント。
- 前記パクリタキセルの放出速度が、患者への挿入後、2日で最大にされることを特徴とする、請求項10に記載のステント。
- 60μgまでのパクリタキセルが、水性環境への曝露後10日、前記ステントから放出されることを特徴とする、請求項3から請求項7のいずれか1項に記載のステント。
- 20μgまでのパクリタキセルが、水性環境への曝露後10日、前記ステントから放出されることを特徴とする、請求項12に記載のステント。
- 15μgまでのパクリタキセルが、水性環境への曝露後10日、前記ステントから放出されることを特徴とする、請求項12に記載のステント。
- 10μgまでのパクリタキセルが、水性環境への曝露後10日、前記ステントから放出されることを特徴とする、請求項12に記載のステント。
- 8μgまでのパクリタキセルが、水性環境への曝露後10日、前記ステントから放出されることを特徴とする、請求項12に記載のステント。
- 6μgまでのパクリタキセルが、水性環境への曝露後10日、前記ステントから放出されることを特徴とする、請求項12に記載のステント。
- 4μg未満のパクリタキセルが、水性環境への曝露後10日、前記ステントから放出されることを特徴とする、請求項12に記載のステント。
- 10μg未満のパクリタキセルが、水性環境への曝露後2日、前記ステントから放出されることを特徴とする、請求項3から請求項7のいずれか1項に記載のステント。
- 5μg未満のパクリタキセルが、水性環境への曝露後2日、前記ステントから放出されることを特徴とする、請求項19に記載のステント。
- 2μg未満のパクリタキセルが、水性環境への曝露後2日、前記ステントから放出されることを特徴とする、請求項20に記載のステント。
- 前記パクリタキセルが、ステント表面積1平方mm当たり2μgの投薬量を有することを特徴とする、請求項1又は2に記載のステント。
- 前記パクリタキセルが、ステント当たり175μgの総投薬量を有することを特徴とする、請求項22に記載のステント。
- 前記パクリタキセルが、前記ステントの少なくとも一部分の上に被覆されるポリマー担体中に混入されることを特徴とする、請求項22又は23に記載のステント。
- 前記ポリマーが、スチレン−イソブチレンベースのブロックコポリマーであることを特徴とする、請求項24に記載のステント。
