Description
本発明は、国立衛生研究所助成金第CA57345号および第CA43460号による支援を受けてなされたものである。したがって、米国政府は本発明において一定の権利を有する。
本発明は、癌の診断および治療の分野に関する。具体的には、転移を含む、より進行した段階への癌の進行に有意に関連しているように思われる遺伝子に関するものである。
転移は、原発腫瘍が通常外科的に除去され得る場合の、癌による大部分の死の原因である腫瘍過程である。転移細胞では、細胞骨格が変化し、接着が損なわれ、運動性が増し、基底膜を分解するタンパク質分解酵素が発現される(1〜3)。しかし、この致死的過程については多くが依然として不明であり、さらなる進歩には、転移病巣において一貫してかつ特異的に変化する新規の遺伝子および経路の同定が条件である。

進行癌と比較しておよび早期癌と比較して、ならびにそれらの由来である正常細胞と比較して、一連の遺伝子が転移癌においてより高いレベルで特異的に発現されることは、本発明者らの発見である。そのような遺伝子を表1に明らかにしている。1つの遺伝子、転移性結腸直腸癌において一貫して過剰発現されるチロシンホスファターゼ(タンパク質チロシンホスファターゼIVA型メンバー3、PRL-3としても知られる)が特に有用であると思われる。表1の遺伝子は、診断および治療に使用するための優れた標的、ならびに薬剤スクリーニングプログラムの標的を表す。

実施例1:肝臓転移のSAGE解析
どの遺伝子がこの過程に関与しているのかを知るため、遺伝子発現の連続解析(Serial Analysis of Gene Expression)(SAGE)技術により、肝臓転移の包括的な遺伝子発現プロファイルを行った(5)。まず始めに、顕微解剖した転移からSAGEライブラリーを調製した(6)。驚いたことに、これらのライブラリーで同定された転写産物の多くが、正常肝細胞または炎症細胞に特有であることが認められ、定量的解析が妨げられた(7)。より特異的な転移性上皮細胞のプロファイルを作製するため、本発明者らはイムノアフィニティー分画手順を開発し、混入する間質細胞および肝細胞から結腸直腸上皮細胞を精製した(8)。この方法で精製した細胞からSAGEライブラリーを調製し、少なくとも17,324個の転写産物を表す約95,000個のタグが得られた(6)。これらのタグを、特に正常および悪性(しかし非転移性の)結腸直腸上皮のものである、多種多様なSAGEライブラリー由来の約400万個のタグと比較した(9)。144個の転写産物が他のライブラリーよりも転移ライブラリーにおいて有意に高いレベルで表され、一方79個の転写産物が転移ライブラリーにおいて有意に低いレベルで表された(10)。
結腸上皮細胞は、以前に記載された上皮細胞を精製するためのイムノアフィニティー精製スキームを改変したものを用いて精製した(St Croixら、Science 289, 1197 (2000))。外科的除去後直ちに組織を入手し、コラゲナーゼを用いて37℃で1時間消化した。400 mm、50 mm、および25 mmのナイロンメッシュを通す一連の連続的ろ過を行い、単一細胞懸濁液を得た。抗CD14および抗CD45の免疫磁気ビーズ(Dynal)混合物を用いて、造血細胞および磁気ビーズに非特異的に結合する細胞を除去した(負の選択)。続いて、細胞懸濁液中に残存する上皮細胞を、抗BerEP4免疫磁気ビーズ(Dynal)への結合によって単離した(正の選択)。BerEP4抗体は、正常結腸上皮上および腫瘍結腸上皮上には存在するが肝細胞または間質細胞上には存在しない全上皮抗原を認識する(U. Latza, G. Niedobitek, R. Schwarting, H. Nekarda, H. Stein, J. Clin. Pathol. 43, 213 (1990))。上皮に結合したBerEP4は、血球計数器で定量した。
RNAgents(Promega、ウィスコンシン州マディソン)を用いて全RNAを単離し、メッセージメーカー・リージェンツ・アッセンブリー(MessageMaker Reagent Assembly)(Gibco BRL)を用いてmRNAを選択した。スーパースクリプトII逆転写酵素(Superscript II Reverse Transcriptase)(Gibco BRL)を用いて、製造業者の説明に従って一本鎖cDNAを作製した。同時に、逆転写酵素を添加せずに擬鋳型調製物を調製した。ピコグリーン(Pico Green)色素(Molecular Probes、オレゴン州ユージーン)を用いてiCycler(Bio-Rad、カリフォルニア州ヘラクレス)により定量的PCRを行い、iCyclerソフトウェアv2.3を用いて閾値サイクル数を求めた。解析する各配列用のプライマーセットを、補足の表2に記載する。増幅の条件は、95℃で2分間の1サイクル、その後、95℃で15秒間、58℃で15秒間、72℃で15秒間の35サイクルとした。定量的PCR反応は3通り行い、閾値サイクル数を平均化した。RT-PCR産物は、この研究に含まれる2つの基準を満たす必要があった。第一に、逆転写酵素(RT)由来cDNAによるシグナルは、逆転写酵素なしで行った対照反応のシグナルの少なくとも100倍高い必要があった。第二に、RTを用いた反応によるPCR産物は、ゲル電気泳動において予測した大きさである必要があった。遺伝子発現は、SAGEによって評価される場合にすべての結腸直腸組織において均一に発現される遺伝子である、b-アミロイド前駆タンパク質(APP)の遺伝子発現に対して標準化した。相対的発現は、以下の式を用いて算出した。式2(Rt-Et)/2(Rn-En)、式中、Rtは実験試料においてAPPについて観察された閾値サイクル数であり、Etは実験試料においてRPL-3について観察された閾値サイクル数であり、Rnは6つの腺腫で観察されたAPPについての平均閾値サイクル数であり、Enは6つの腺腫で観察されたRPL-3についての平均閾値サイクル数である。
Qiagen DNA簡便精製キット(Qiagen DNA Easy Purification Kit)(Qiagen)を用いて、精製した上皮細胞からゲノムDNAを調製した。遺伝子のコピー数を評価するため、各DNA試料に対して6回の独立した反復を行った。ゲノムDNA鋳型なしで対照反応を行ったことを除いては、発現解析について上記したようにリアルタイムPCRを行った。

(Celeraスカフォールド:GA_x5HB7VCY1B9)であった。増幅の条件は、95℃で2分間の1サイクル、その後、95℃で15秒間、58℃で15秒間、72℃で15秒間の35サイクルとした。
転移ライブラリーにおいて欠失しているよりも濃縮されている転写産物の方が、診断および治療の明らかな可能性を有する。したがって、本発明者らは、さらなる解析のために最も興味深い濃縮転写産物38個を選択した(表1)(11)。SAGEデータを確認するため、定量的リアルタイムPCRにより、いくつかの顕微解剖した転移、原発性癌、前悪性腺腫、および正常上皮におけるこれらの転写産物の発現を比較した(8)。これらの転写産物38個はすべて、試験した転移病巣の少なくとも一部において増加していることが見出されたが、1つの遺伝子PRL-3のみが一貫して過剰発現していることが見出された。
PRL-3(PTP4A3としても知られる)は小さな22 kDチロシンホスファターゼをコードしているが、これはカルボキシル末端がプレニル化された場合には細胞膜に位置し、この脂質に複合化しなかった場合は核に存在する(12)。正常なヒト成人組織中で、PRL-3は主に筋肉および心臓で発現されている(13)。PRL-3はこれまでヒトの癌と関係づけられていなかったが、PRL-3の過剰発現がヒト胎児腎臓線維芽細胞の増殖を増強することが見出され(13)、PRL-3に近縁であるPRL-1またはPRL-2の過剰発現が、培養においてマウス線維芽細胞およびハムスター膵上皮細胞を形質転換すること、およびヌードマウスにおいて腫瘍増殖を促進することが見出された(14, 15)。この情報、本発明者らの予備実験データ、および細胞のシグナル伝達および異常増殖における他のホスファターゼの重要性に基づき、本発明者らはPRL-3をさらに詳細に調べた。まず、上記の手順により、様々な段階の結腸直腸腫瘍の結腸直腸組織から精製した上皮細胞におけるPRL-3の発現を調べた(8)。この精製は、その発現の正確な定量化には必須であることが判明した。PRL-3は正常結腸直腸上皮および良性腫瘍(腺腫)由来の上皮において低レベルで発現し、悪性I期またはII期の癌の一部において中レベルで発現していた(図2)。それに対し、12の結腸直腸癌転移のそれぞれにおいて比較的高レベルで発現していた(図2の例)。
図2で評価した転移は、初期の病変を入手した同じ患者由来ではなかった。以前の解析から、多くの遺伝子の発現パターンは個人間で異なり、別の患者由来の組織の比較により誤解を招くことになり兼ねないことが明らかになっている(16)。この事柄に対処するため、6人の別々の患者それぞれの肝臓転移、転移を起こす進行性(III期)結腸直腸癌、および正常結腸直腸上皮から上皮細胞を精製した。これらの場合の正常上皮では発現はほとんど見られず、進行性原発性癌において中程度の発現が見られ、対応する転移病巣それぞれにおいて有意に高い発現が見られた(図3)。
転移においてPRL-3が比較的高レベルで発現されるという知見は、それが転移において原因となる役割を果たすことと一致している。しかし、ヒトの癌において遺伝子を意味づける最も決定的な方法は、その遺伝子の遺伝子改変を同定することである(17)。したがって、ヒトの癌において増殖制御遺伝子の発現を増加させるよく知られた機構である遺伝子増幅を通じて、PRL-3が遺伝子改変されているか否かを判定した(18)。放射線ハイブリッドマッピングおよびシンテニーマッピングにより、PRL-3遺伝子は、染色体バンド8q24.3に相当する位置である、染色体8qのテロメアから約3 Mbに位置することが見出された(19)。それぞれ別の患者由来の12の転移病巣の精製上皮細胞から調製したゲノムDNAのリアルタイムPCR解析を行い、PRL-3遺伝子の含有量を測定した。発現解析と同様に、そのような精製が信頼のおける定量化に重要であることが明らかになった。それぞれの場合において、染色体8qのセントロメア近傍の配列のゲノム含有量に対する、PRL-3配列のゲノム含有量を測定した(8)。この比較により、真の増幅を異数性による染色体数の単なる増加と識別することが可能となった(18)。試験した12の転移のうち3つが、二倍体ゲノム当たり約25コピー、26コピー、および37コピーレベルの増幅を示した。
次に、染色体8qにわたって分配される配列のゲノム含有量を比較し、これら3つの転移において増幅された配列を限定した(8)。ほとんど完全なヒトのゲノム配列が入手可能であることから、このマッピングへの試みが大幅に促進された(20, 21)。PRL-3遺伝子はヒトゲノム解析計画で作成された公的データベース中には認められなかったが、染色体8pのテロメアから約145 Mbおよび染色体8qのテロメアから約3 Mbに位置するCeleraスカフォールドに見出され、本発明者らの放射線ハイブリッドマッピングのデータと一致した(19, 22)(図4)。すべての場合において、増幅は、PRL-3を含む染色体8qの非常に小さな領域に制限されていた(図4)(22)。この小さな単位複製配列中に存在する既知遺伝子または予測遺伝子は、PRL-3およびTATA結合タンパク質と相同的な推定遺伝子のみであった。後者の遺伝子の発現は、RT-PCRにより評価したところ、転移病巣において検出されなかった。しかし、PRL-3が位置するCeleraスカフォールドの領域は(22)、大きさ<20 kbと推定されるギャップを含んでいる。これらのギャップが実際には予想を上回るとすれば、単位複製配列の全体の大きさは図4で示す100 kbよりも大きいことになり、さらなる遺伝子を含む可能性が出てくる。
Claims (44)
- 人体において結腸直腸癌または結腸直腸癌の肝臓転移の腫瘍増殖の部位を同定するための組成物の製造における、タンパク質チロシンホスファターゼIVA型メンバー3(PRL-3)に特異的に結合する抗体の使用。
- 抗体が検出可能な程度に標識されている、請求項1記載の使用。
- 組成物が、検出可能な程度に標識され、かつ抗体に特異的に結合する薬剤をさらに含む、請求項1記載の使用。
- インビトロまたはエクスビボで、結腸直腸癌患者の組織試料において、タンパク質チロシンホスファターゼIVA型メンバー3(PRL-3)をコードする遺伝子のすべてまたは一部にハイブリダイズするハイブリダイゼーションプローブを用いて、または前記遺伝子のすべてまたは一部を増幅するプライマーを用いて、タンパク質チロシンホスファターゼIVA型メンバー3(PRL-3)をコードする第一の染色体セグメントの増幅を検出する段階を含み、増幅の検出により結腸直腸癌の肝臓転移傾向が示唆される、
転移の予測に役立つ方法。 - 第一の染色体セグメントの量の、8qに位置する第二の染色体セグメントの量に対する比を測定することにより増幅が検出される、請求項4記載の方法。
